Using telemedicine to facilitate social distancing in otolaryngology: A systematic review.

OBJECTIVES: As the coronavirus 2019 pandemic puts strains on current models of otolaryngology practice, telemedicine is an attractive way for otolaryngologists to reduce in-person appointments while still addressing the health of their patients. This systematic review of the literature aims to identify the evidence basis for using telemedicine in otolaryngology practice to limit person-to-person interactions while achieving comparable quality to in-person services. METHODS: The authors gathered articles from three databases (Embase, PubMed and Web of Science), performed a comprehensive litera

Cytokine production in patients with recurrent acute tonsillitis: analysis of tonsil samples and blood.

The aim of this study was to examine T cell function in tonsils of patients with recurrent acute tonsillitis (RAT) or peritonsillar abscess (PTA) by analyzing the cytokine production following T cell receptor (TCR) and co-receptor stimulation with a combination of anti-CD3 and anti-CD28 antibodies. The release of IFN-γ, TNF-α, IL-2, IL-4, IL-6, IL-10 and IL-17A from isolated, stimulated T cells of 27 palatine tonsils (10 RAT, 7 PTA, 10 tonsils without inflammation) was measured via a bead-based flow cytometric analysis. The results were compared with the cytokine release of isolated peripheral

Low rate of co-infection in complicated infectious mononucleosis.

INTRODUCTION: It remains unclarified if bacterial co-infection is common in patients with infectious mononucleosis (IM) and acute tonsillitis and/or peritonsillar abscess (PTA). Recent studies suggest that Fusobacterium necrophorum is a prevalent pathogen in acute tonsillitis and PTA. We hypothesised that this anaerobe may play a significant role for the aggravated infection and the development of PTA among teenagers and young adults with IM. METHODS: All patients with IM and clinical findings of acute tonsillitis or PTA admitted to our department in the 2001-2015 period were included in the

Group C Streptococcus Causing Rheumatic Heart Disease in a Child.

BACKGROUND: Human infection with group C Streptococcus is extremely rare and a select number of cases have been reported to cause acute pharyngitis, acute glomerulonephritis, skin and soft tissue infections, septic arthritis, osteomyelitis, pneumonitis, and bacteremia. In pediatrics, this bacteria is known to cause epidemic food-borne pharyngitis, pneumonia, endocarditis, and meningitis, and has reportedly been isolated in the blood, meninges, sinuses, fingernail, peritonsillar abscess, and thyroglossal duct cyst, among others. CASE REPORT: Our patient was a 7-year-old previously healthy fema

Current Belgian adenotonsillectomy practice: a survey among Belgian ENT specialists.

OBJECTIVES: Tonsillectomies and adenotonsillectomies (TE and ATE) are among the most frequently performed operations in the western world. Despite the lack of generally accepted clinical guidelines about indications, the beneficial role of the procedure has been established. The aim of this study was to identify current practice relating to indications for tonsillectomies and adenotonsillectomies among Belgian ENT specialists. METHODS: An anonymous questionnaire was sent to all Belgian ENT specialists. Part I of the questionnaire included questions about the ENT specialist himself/herself and

[Clinical relevance of the Streptococcus milleri group in head and neck infections].

Streptococcus constellatus, S. intermedius, and S. anginosus, the 3 species of the S. milleri group, form part of the normal flora commonly found in the mouth, throat, and gastrointestinal and genital tracts. This group has become known as an important pathogen in infections and abscesses, but data on the anatomical distribution of these species is lacking in relation to clinical significance. We obtained 275 strains of the S. milleri group from different departments at our hospital over the last 3 years, including 54 strains from dental surgery, 47 from internal medicine, 44 from otolaryngolo

Peritonsillar and deep neck infections: a review of 330 cases.

INTRODUCTION: Deep neck infections are defined as suppurative infectious processes of deep visceral spaces of the neck. OBJECTIVE: The aim of this study is to review different factors that may influence peritonsillar and deep neck infections and may play a role as bad prognosis predictors. METHODS: We present a retrospective study of 330 patients with deep neck infections and peritonsillar infections who were admitted between January 2005 and December 2015 in a tertiary referral hospital. Statistical analysis of comorbidities, diagnostic and therapeutic aspects was performed with Excel and S

Deep Neck Space Infections: A Study of 76 Cases.

INTRODUCTION: Deep neck space infections (DNSI) are serious diseases that involve several spaces in the neck. The common primary sources of DNSI are dental infections, tonsillar and salivary gland infections, malignancies, and foreign bodies. With widespread use of antibiotics, the prevalence of DNSI has been reduced. Common complications of DNSI include airway obstruction, jugular vein thrombosis, and sepsis. Treatment principally comprises airway management, antibiotic therapy, and surgical intervention. This study was conducted to investigate the age and sex distribution of patients, sympto

Long-Term Outcomes After Intracapsular Tonsillectomy.

OBJECTIVE: Research suggests that intracapsular tonsillectomy (ICT) is less morbid than extracapsular tonsillectomy. It is underutilized, in part, because of concerns regarding its effectiveness and the possible need for repeat surgery in the future. We conducted long-term follow-up patient interviews to evaluate symptom control and incidence of revision tonsillectomy in children who had undergone ICT years earlier. METHODS: A computerized collection of office notes and operative reports was queried to identify children undergoing ICT in the years 2010-2023 at an academic pediatric otolaryngo

Peritonsillitis in primary care: patient characteristics and care pathways - a population-based registry study.

BACKGROUND: Peritonsillitis, including peritonsillar abscess and cellulitis, is a common deep neck infection often considered a complication of pharyngotonsillitis. However, population-based data on patient characteristics, clinical presentation, and care pathways are limited. METHODS: We conducted a population-based registry study in Region Kronoberg, Sweden (2012-2016), including all episodes of peritonsillitis and pharyngotonsillitis diagnosed in primary health care (PHC) and hospitals. New episodes were defined as diagnoses separated by ≥ 30 days. Patient characteristics, recent pharyngot

Predictors of pediatric intensive care unit admission in pediatric deep neck infections: A retrospective observational study.

Deep neck infections (DNIs) in children can progress rapidly and lead to life-threatening complications. Early identification of patients at high risk for severe disease is essential for timely intervention and optimal allocation of pediatric intensive care unit (PICU) resources. This study aimed to identify clinical and laboratory predictors associated with PICU admission in children diagnosed with DNIs in the pediatric emergency department. This retrospective, single-center study included pediatric patients diagnosed with DNIs from January 1 to December 31, 2024. Demographic, clinical, labor

Otolaryngology Curriculum During Residency Preparation Course Improves Preparedness for Internship.

OBJECTIVES/HYPOTHESIS: Residency preparation courses (RPCs) have become a widely adopted practice to ease the transition of medical students into residency, but these courses often lack training in skills expected of subspecialty interns. To fill this gap, a simulation-based curriculum in otolaryngology (ORL) was implemented at the University of Michigan Medical School. The curriculum aimed to improve confidence and perceived ability to perform common ORL skills for graduating students prior to internship. STUDY DESIGN: Cross-sectional study. METHODS: Six basic simulations (tracheostomy, fle

Severe neck infections that require wide external drainage: clinical analysis of 17 consecutive cases.

Infections in the neck layers and spaces are potentially life-threatening diseases causing further complications, like mediastinitis, airway obstruction, or sepsis. Despite of the need for a conservative approach, they still regularly require surgical intervention. Records of 17 patients with severe neck infections that were treated by wide external incision and open wound management were retrospectively analyzed. The aim of the study was to clinically characterize these most serious neck infections. The most common presenting symptoms were neck pain and tense neck mass (94-94%) regularly with

Lemierre syndrome: a pediatric case series and review of literature.

BACKGROUND: Lemierre syndrome is a rare disease of the head and neck often affecting adolescents and young adults. Classically, infection begins in the oropharynx with thrombosis of the tonsillar veins followed by involvement of the parapharyngeal space and the internal jugular vein. Septicemia and pulmonary lesions develop as infection spreads via septic emboli. Although a rare entity in modern times, Lemierre syndrome remains a disease of considerable morbidity and potential mortality. METHODS: This was a retrospective review of 3 cases and associated literature. RESULTS: A common 1- to 2-

[Brain damage after tonsillectomy?].

We report on a 47-year old male patient who developed persistent postoperational bleeding after tonsillectomy, which made tamponading of the pharynx necessary. Even though the patient left the hospital after one week with a haemoglobin value of 10.6 g% and without any complaints, he developed personality changes and later severe neurological symptoms which led to the diagnosis of hypoxic brain damage as suggested by a variety of neurologists and psychiatrists. The diagnosis was finally disproved by a brain biopsy revealing the existence of Jakob-Creutzfeldt disease, a degenerative inflammatory

Inflammatory torticollis in children.

Acute torticollis is commonly seen in the pediatric emergency department. It often results from an inflammatory process that irritates the cervical muscles, nerves, or vertebrae. Posturing of the head occurs with unilateral spasm of the sternocleidomastoid muscle such that the child will position the head with the occiput rotated to the affected side and the chin rotated to the contralateral side. We recently treated 26 children who presented to the emergency department with acute nontraumatic torticollis. The most common causes were upper respiratory infection, sinusitis, otomastoiditis, cerv
