Small bowel injuries following blunt abdominal trauma. Early recognition and management.

We reviewed the cases of 15 patients with intestinal perforation following blunt abdominal trauma, which occurred between 1971 and 1988. Twelve patients were treated at The Mary Imogene Bassett Hospital (Cooperstown, NY); three other patients were treated at surrounding area hospitals. These injuries included 11 motor vehicle accidents, three low-velocity impact injuries, and one blast injury. There were four duodenal, ten jejunal, and two ileal injuries. Five patients who were operated on within 12 hours had classic signs of peritonitis or gross blood on diagnostic peritoneal lavage. Four pat

Detection of occult abdominal trauma in patients with severe head injuries.

Among 109 consecutive victims of severe head injury, intra-abdominal injury could not be excluded in 87. These patients were considered to be at risk from occult intra-abdominal haemorrhage. 60 patients underwent diagnostic peritoneal lavage. 10 of these had a positive lavage, correctly indicating intra-abdominal haemorrhage. In each case a lesion requiring surgical repair was identified. 49 patients had a negative lavage, and only 1 of these patients subsequently showed signs of a missed intra-abdominal lesion. 13 patients had low systolic blood-pressure (< 90 mm Hg) on admission, and 4 of th

Principles and practice of antibiotic therapy for post-traumatic abdominal injuries.

Improved surgical techniques, the use of modern antibiotics and better understanding of the microbiologic characteristics of abdominal sepsis have lead to a satisfying clinical outcome in instances of abdominal trauma which once meant almost certain death. Yet many questions remain unanswered. Future development of several newer antibiotics may further improve the rate of complications. A study of host defense mechanisms may provide additional clues to prevent long term morbidity. Hopefully, future studies will encourage the use of a single antibiotic in the treatment of abdominal trauma. Whil

[Blunt abdominal trauma and injuries of the great omentum (author's transl)].

A 81 years old man died four weeks after a traffic accident with blunt abdominal trauma. The cause of death was a mechanical ileus as a result of an incarceration of a loop of the small intestines in a rent of the great omentum. Since no criteria for a traumatic tear of the greaten omentum was found the developmental mechanism of a fissure in the great omentum was explained as an exaggerated process of aging.

[Cryogenic injuries of the rabbit abdominal aorta and the iliac artery].

In rabbits, cryogenic injuries of the walls of abdominal aorta and iliac arteries were histologically examined after application of a cryoprobe ob -160 degrees C for 1 minute. Seven days thereafter the animals were killed. Extended partial and circular necroses and calcifications of the vascular walls were found. The alterations were less expressed when the blood stream was maintaimed during the freezing. Maximal changes occured at additional centripetal pinching the vessels.

Modern management of abdominal aortic injuries: What you need to know.

Abdominal aortic injury is a rare but deadly traumatic insult that occurs in <0.1% of trauma patients but carries a mortality rate of 29% to 70%. Penetrating injuries usually present with shock and require rapid hemorrhage control through open abdominal exploration. Blunt injuries are increasingly being managed nonoperatively and with endovascular techniques. Both mechanisms are commonly associated with other severe intra- and extra-abdominal injuries. The trauma surgeon must be prepared to promptly evaluate for intra-abdominal hemorrhage and choose the appropriate clinical pathway to diagnose

Pelvic fractures: epidemiology and predictors of associated abdominal injuries and outcomes.

BACKGROUND: Pelvic fractures are often associated with major intraabdominal injuries or severe bleeding from the fracture site. OBJECTIVE: To study the epidemiology of pelvic fractures and identify important risk factors for associated abdominal injuries, bleeding, need for angiographic embolization, and death. METHODS: Trauma registry study on pelvic fractures from blunt trauma. Stepwise logistic regression was used to identify risk factors of severe pelvic fractures, associated abdominal injuries, need for major blood transfusion, therapeutic embolization, and death from pelvic fracture. Injuries of the portal vein in patients with blunt abdominal trauma.

Between January 1987 and September 1991, 68 severely traumatized patients underwent emergency laparotomy because of blunt abdominal trauma. Intraoperatively, 54.4% of the patients had a major injury to one organ, 23.5% had injuries to two organs, 16.2% had injuries to three organs and 5.9% to four or more organs. Additionally, in 11.8% of these cases (n = 8) a major vascular injury (portal vein n = 5, vena cava n = 2, mesenteric root n = 1) was found. Injuries to the portal vein were always associated with complete rupture of the pancreas, requiring distal pancreatic resection in four cases an

Thoracic and abdominal injuries in skiers: the role of air evacuation.

The increasing popularity of skiing as a recreational sport has resulted in a greater number of major thoracic and abdominal injuries. These injuries, unlike the more common orthopedic injuries, are often life threatening. This 8-year study reviews 44 thoracic and abdominal ski injuries managed at an urban trauma center since the inception of a helicopter air evacuation program with in-flight resuscitation capabilities. Twenty-five per cent of the injured had signs of hemorrhagic shock, and nearly 60% required a major operation. The injuries were caused by high-speed collisions with stationary

[Detection and emergency management of visceral injuries after abdominal trauma : Hollow organ and mesenteric injuries-keep in mind].

Hollow organ and mesenteric injuries occur in up to 7% of all cases of abdominal trauma and are linked to high morbidity and mortality, especially with delayed diagnosis and treatment. The causes include blunt, penetrating or blast injuries. Hollow organ injuries are more frequent in penetrating trauma, while injuries of the parenchymatous organs dominate in blunt trauma. The clinical diagnosis is difficult as the early symptoms are often nonspecific. Important predictors include abdominal tenderness, the seatbelt sign, leukocytosis, lactatemia and abnormalities in computed tomography (CT). Th

Abdominal pain in a child after blunt abdominal trauma: an unusual injury.

We report a case of perforation of a walled off appendiceal abscess in a 5-year-old boy who sustained blunt abdominal trauma. The past medical history was significant only for a 4-day episode of abdominal pain 1 month prior to this presentation. Initial laboratory studies were unremarkable, and radiographic studies showed free fluid in the pelvis with no evidence of solid organ injury, but inflammation of the right colon. The final diagnosis was made at laparotomy. We emphasize this unique presentation and review the literature on traumatic appendicitis in children.

A meta-analysis of the incidence of intra-abdominal injuries associated with thoracic or lumbar flexion-distraction injuries.

BACKGROUND: Intra-abdominal injuries (IAIs) are often associated with thoracic or lumbar flexion distraction injuries (TLFDIs) or Chance fractures. The incidence ranges from 10 to 50 % in previous literature. AIM: To synthesize data about the incidence of IAIs associated with TLFDIs. METHODS: We searched PubMed, WOS, and Cochrane databases for all studies reporting the incidence of IAIs associated with TL FDIs. The primary outcome was the overall pooled incidence of IAIs, surgical intervention, and specific organ injuries. A subgroup analysis was done for studies that included adults, pediat

[Adrenal injury in blunt abdominal trauma].

10 patients with adrenal damage were observed during 2.5 years. It amounted 0.93% of all patients with closed abdominal injuries. The right adrenal gland was traumatized in all cases evidently due to it's compression between right lobe of liver and vertebral column. Adrenal damage is observed quite often in combination with injuries of right liver lobe, right kidney and retroperitoneal hematoma formation. 5 patients underwent laparotomy on account of intra-abdominal bleeding, but adrenal damage was never revealed. Ultrasound and tomographic semiotics of adrenal damage was worked out, which all

Chyloperitoneum after blunt abdominal injury.

The objective of this study was to evaluate incidence of chylous injury in blunt trauma using a retrospective chart review. We present two patients who sustained chyle duct injury after blunt trauma. The first patient is a pedestrian struck by car. Abdominal CT scan revealed duodenal thickening and a moderate amount of paraduodenal fluid, which prompted surgical exploration. At laparotomy, the patient was found to have a disruption of his lymphatics at the level of the inferior vena cava (IVC) without duodenal injury treated with hemoclips, fibrin sealant and elemental gastrojejunal feeds. [Injury of the pelvic ring and abdominal trauma].

Fractures and disruptions of the pelvic ring are usually found in multiply injured patients and exacerbate the life-threatening character of the concomitant injuries. The main prognostic factors are haemorrhagic complications, abdominal trauma and associated injuries of the urogenital tract. All patients admitted to the Department of Surgery of the University of Münster between July 1985 to December 1989 were analysed with particular reference to pelvic ring instability and the accompanying pelvic and abdominal lesions. Of 27 patients with unstable displaced lesions of the pelvic ring, 9 were
