Cognitive Behavioral Therapy and Acceptance and Commitment Therapy for the Discontinuation of Long-Term Benzodiazepine Use in Insomnia and Anxiety Disorders.

Benzodiazepines have proven to be highly effective for treating insomnia and anxiety. Although considered safe when taken for a short period of time, a major risk-benefit dilemma arises in the context of long-term use, relating to addiction, withdrawal symptoms, and potential side effects. For these reasons, benzodiazepines are not recommended for treating chronic sleep disorders, anxiety disorders, nor for people over the age of 65, and withdrawal among long-term users is a public health issue. Indeed, only 5% of patients manage to discontinue using these drugs on their own. Even with the hel

Acceptance and commitment therapy with Huntington's disease: A narrative review and case report of a caregiver-assisted intervention.

Acceptance and commitment therapy (ACT) is an experiential, action-oriented talk therapy that emphasizes acceptance of painful inner experience through increased psychological flexibility. In this narrative review and case report, we first review the extant literature on applications of ACT to patients with a variety of mental and physical health challenges, including neurodegenerative disorders, and their caregivers. We then discuss applications of ACT to Huntington's disease (HD). We provide a case report of a 52-year-old man living with HD who, accompanied by his caregiver, received a virtu

Acceptance and commitment therapy for co-occurring gambling disorder and posttraumatic stress disorder in veterans: a narrative review.

Background: PTSD and gambling disorder (GD) are frequently comorbid. Gambling may provide escape-based coping for the emotions experienced by PTSD sufferers. Military personnel may be at increased risk of PTSD and/or GD. Acceptance and Commitment Therapy (ACT) has been found to improve both PTSD and GD outcomes, yet research into the potential effectiveness of ACT for PTSD and/GD in veterans is scarce. Objective: This review aimed to systematically assess and describe the evidence relating to the use of ACT and acceptance-based therapy for military populations with PTSD and/or GD.Method: Six da

The Effects of Acceptance and Commitment Therapy (ACT) Intervention on Inflammation and Stress Biomarkers: a Randomized Controlled Trial.

BACKGROUND: Psychological processes can be manifested in physiological health. We investigated whether acceptance and commitment therapy (ACT), targeted on psychological flexibility (PF), influences inflammation and stress biomarkers among working-age adults with psychological distress and overweight/obesity. METHOD: Participants were randomized into three parallel groups: (1) ACT-based face-to-face (n = 65; six group sessions led by a psychologist), (2) ACT-based mobile (n = 73; one group session and mobile app), and (3) control (n = 66; only the measurements). Systemic inflammation and stre

Acceptance and commitment therapy as a novel treatment for eating disorders: an initial test of efficacy and mediation.

Eating disorders are among the most challenging disorders to treat, with even state-of-the-art cognitive-behavioral treatments achieving only modest success. One possible reason for the high rate of treatment failure for eating disorders is that existing treatments do not attend sufficiently to critical aspects of the disorder such as high experiential avoidance, poor experiential awareness, and lack of motivation. These variables are explicit targets of Acceptance and Commitment Therapy (ACT). The current study examined the efficacy of an ACT-based group treatment for eating disorders by exam

Acceptance and commitment therapy for people with HIV who drink at unhealthy levels: Protocol for the ACCEPT randomized controlled trial.

BACKGROUND: Alcohol consumption at unhealthy levels is associated with negative consequences at nearly every step of the HIV care continuum. We describe the rationale, aims, and study design for the ACCEPT Trial, a randomized controlled comparative efficacy trial for Acceptance and Commitment Therapy (ACT) compared to a standard brief alcohol intervention (BI) for people living with HIV (PWH) who drink at unhealthy levels. METHODS: A total of 300 PWH who report drinking at unhealthy levels will be randomly assigned 1:1 to the two treatments conditions and assessed, via self-report and an alco

SoundMind Trial: a study protocol for a randomised controlled trial for online acceptance and commitment therapy and sound therapy for tinnitus.

INTRODUCTION: Tinnitus is a common ailment that affects 10%-15% of adults worldwide. Comorbidities associated with tinnitus often include hearing loss, emotional distress and sleep disorders, with insomnia being a common issue among patients with tinnitus. Audiological and psychological approaches are typically used to treat chronic subjective tinnitus, with the combination of sound therapy and cognitive-behavioural therapy-based counselling having the strongest evidence for effectiveness. Acceptance and commitment therapy (ACT) has also shown promising effects in reducing the impact of tinnit

Using an Acceptance and Commitment Therapy Approach When Exposure and Cognitive Therapy Become Rituals in the Treatment of Obsessive-Compulsive Disorder.

Exposure and response prevention (ERP) for obsessive-compulsive disorder (OCD) is often highly effective, yet some patients experience relapses following a seemingly successful course of treatment. In this article we describe the conceptual basis of ERP, and then present a client who relapsed after seemingly achieving substantial improvement following 11 sessions of ERP. Likely reasons for the relapse and strategies for enhancing ERP to achieve better long-term treatment outcomes are discussed from the perspective of acceptance and commitment therapy (ACT). These strategies mainly apply to the

Acceptance and commitment therapy as an add-on treatment for the management of patients with obsessive-compulsive disorder.

BACKGROUND: Obsessive compulsive disorder (OCD) is a chronic and long-lasting illness affecting approximately 2% of the population. Up to 40%-60% of patients with OCD do not have satisfactory outcomes, thereby leading to disability in performing daily routine activities. Acceptance and commitment therapy (ACT) provides a substitute for old-fashioned psychotherapies, as it involves the client directly experiencing the problematic emotion, that is, anxiety and obsessions in the case of OCD. AIM: To observe the effect of ACT on patients with OCD and compare its effect on obsessive compulsive sym

Development and Feasibility of a Digital Acceptance and Commitment Therapy-Based Intervention for Generalized Anxiety Disorder: Pilot Acceptability Study.

BACKGROUND: Generalized anxiety disorder (GAD) is characterized by excessive worry that is difficult to control and has high comorbidity with mood disorders including depression. Individuals experience long wait times for diagnosis and often face accessibility barriers to treatment. There is a need for a digital solution that is accessible and acceptable to those with GAD. OBJECTIVE: This paper aims to describe the development of a digital intervention prototype of acceptance and commitment therapy (ACT) for GAD that sits within an existing well-being app platform, BioBase. Acceptance and commitment therapy for psychiatric inpatients diagnosed with depression and insomnia: a multiple-baseline single-case study.

INTRODUCTION: The overarching goal of ACT is to increase psychological flexibility, which can be enhanced through mechanisms such as acceptance, valued-driven actions, or mindfulness. Several meta-analyses confirm the effectiveness of Acceptance and Commitment Therapy (ACT) in the treatment of clinical and subclinical populations, including patients diagnosed with depressive disorder and insomnia, largely within outpatient settings. However, there is no scientific research evaluating the efficacy of ACT as an adjunct treatment for psychiatric inpatients with depression. This study aimed to eva

Acceptance and commitment therapy (ACT) for patients with degenerative spinal disorders and maladaptive psychological processes: an observational study.

BACKGROUND: Patients living with chronic pain may experience maladaptive psychological processes that include depression, somatization, kinesiophobia, pain catastrophizing, and anxiety. Spine surgery in the setting of maladaptive psychological processes can lead to poor outcomes, but intervention with acceptance and commitment therapy (ACT), which is a specific form of cognitive-behavioral therapy (CBT), may provide benefits. Our objective was to evaluate the preliminary effectiveness of ACT for patients with degenerative spinal disorders awaiting surgery. METHODS: We performed a retrospectiv

Acceptance and commitment therapy for the treatment of irritable bowel syndrome and inflammatory bowel disease: a narrative review.

BACKGROUND AND OBJECTIVE: Cognitive behavioral therapy (CBT) is a common treatment modality for patients with irritable bowel syndrome (IBS) and inflammatory bowel disease (IBD). CBT may not be a good fit for all patients and some may instead benefit from an acceptance and commitment therapy (ACT) approach. This narrative review presents evidence for the use of ACT in adult patients with IBS or IBD. The authors also suggest instances in which patients with IBS or IBD may benefit from ACT and discuss future directions of research. METHODS: Between August 2023 and January 2024, databases such a

Cognitive-behavioral therapy and acceptance and commitment therapy for anxiety and depression in patients with fibromyalgia: a systematic review and meta-analysis.

INTRODUCTION: Defined by chronic, musculoskeletal pain, fibromyalgia is often comorbid with depression and anxiety. In these cases, the first line medical treatment can be successfully combined with psychological interventions. Cognitive-behavioral therapy and acceptance and commitment therapy are among the most widely studied approaches in relation to chronic pain, including fibromyalgia. The objective of this review is to analyze the efficiency of these psychological treatments for alleviating emotional distress in fibromyalgia. METHOD: The search was conducted on the PubMed, Scopus and Web

Examining Positive and Negative Affect as Outcomes and Moderators of Cognitive-Behavioral Therapy and Acceptance and Commitment Therapy for Social Anxiety Disorder.

Traditional cognitive-behavioral therapy (CBT) for anxiety disorders has been designed to target reductions in negative affect (NA) associated with defense-related processes. However, a subset of anxiety disorders, including social anxiety disorder (SAD), are also characterized by low positive affect (PA) resulting from separate deficits in appetitive-related processes. In contrast to CBT, "third-wave" approaches, such as acceptance and commitment therapy (ACT), align more consistently with motivational processes and, as a result, PA. However, the differential effect of CBT and ACT on PA and N
