Anorectal impalement in a pediatric patient with transanal evisceration of small bowel.

Rectal injuries in the pediatric population are most often attributed to accidental falls on offending objects, sexual abuse, or blunt trauma. Anorectal impalement is a rare injury, and as a result, specific knowledge or experience on the treatment of this type of injury in children is sparse. The extent of injury is often difficult to assess. Mostly, therapeutical decisions are based on general knowledge in adults. We present a case of accidental anorectal impalement in a child by a fall on the handle of a toilet brush. The handle caused an anterior rupture of the intraperitoneal part of the

Rare retinal haemorrhages in translational accidental head trauma in children.

PURPOSE: The characteristic findings in accidental head injury consist of linear skull fracture, epidural haematoma, localized subdural haematoma, or cortical contusion because of a linear or translational impact force. Retinal haemorrhages have been found, although uncommon, in accidental head trauma. METHODS: We performed a retrospective study of 24 consecutive cases of children with severe head injuries caused by falls. Inclusion criteria were skull fractures and/or intracranial haemorrhages documented by computerized tomography. All patients underwent a careful ophthalmic examination incl

Patterns of structural head injury in children younger than 3 years: a ten-year review of 519 patients.

BACKGROUND: Abusive head injury is a major contributor to morbidity and mortality in infants and toddlers, but data comparing patterns of injury in corroborated accidental trauma and confessed child abuse are scarce. METHODS: This is a retrospective review of head injuries with abnormal neuroimaging in children younger than 3 years during a 10-year period in Auckland, New Zealand. Histories were assumed to be true. Results were analyzed for incongruity then compared with data on confessed abuse and corroborated accidental injury. RESULTS: Five hundred nineteen cases were analyzed. Most cases

The medico-legal evaluation of injuries from falls in pediatric age groups.

Blunt trauma from accidental falls or intentional jumping from great heights occurs frequently in forensic medicine. The goal of this study was to investigate injuries due to falls in children under 19 years of age. Injuries from falls are the leading cause of visits to emergency departments and to deaths due to injuries. Various methods are used in the classification of falls. In this study, we have classified falls as "high-level" (≥ 5 m), "low-level" (<5 m) and "ground-level". We have retrospectively evaluated 814 boys (61.18%) and 512 girls (38.62%), making up a total of 1326 children (und

Using a co-design approach to develop aquatic reactive balance training for fall prevention.

BACKGROUND: Falls are a major health concern among older adults, leading to injury, reduced independence, and increased healthcare cost. Reactive balance training can reduce fall risk, but barriers such as fear, joint discomfort, and harness burden limit its use - barriers that training in aquatic environment  may help overcome. OBJECTIVE: To design an aquatic reactive balance training (AquaReBal) program for older adults, integrating end-user perspectives to enhance safety, accessibility, and engagement. METHODS: Using a participatory design approach grounded in Participatory Action Researc

[Fall Risk and Its Management in Parkinson's Disease].

Falls represent critical clinical events in older people, leading to functional decline, fractures, and deterioration in their ability to perform activities of daily living and decreased cognitive abilities, resulting in a bedridden state. Progression of Parkinson's disease (PD) causes postural instability, gait disturbances, gait freezing, and cognitive dysfunction, which markedly elevate the risk of falling compared with that of the general older adult population. Kempster et al. revealed that patients with PD frequently fall, reaching major milestones in PD such as dementia or institutional

Low-Cost Non-Wearable Fall Detection System Implemented on a Single Board Computer for People in Need of Care.

This work aims at proposing an affordable, non-wearable system to detect falls of people in need of care. The proposal uses artificial vision based on deep learning techniques implemented on a Raspberry Pi4 4GB RAM with a High-Definition IR-CUT camera. The CNN architecture classifies detected people into five classes: fallen, crouching, sitting, standing, and lying down. When a fall is detected, the system sends an alert notification to mobile devices through the Telegram instant messaging platform. The system was evaluated considering real daily indoor activities under different conditions: o

[Clinical and socially significant consequences of falls in elderly and senile persons (analytical review).].

Falls refers to geriatric syndromes, which is accompanied by a significant number of adverse clinically and socially consequences. For the rational organization of medical, social, psychological and other types of assistance and rehabilitation measures, separate groups of consequences developing as a result of a fall are distinguished: physical, functional, psychological, social. Every year, every fourth elderly and older person faces a fall, while about half of the victims seek medical help. Among people of older age groups who have suffered a fall episode, 20-30% have complications in the fo

Reducing harm from falls.

Serious adverse event reporting from district health boards (DHBs) brought in-hospital falls to the attention of the Health Quality & Safety Commission (the Commission) when it was incepted in 2010. In 2012, responding to the large numbers reported, the Commission began planning for a three-year programme to reduce harm from falls, initially to run 2013-2015. In this article we discuss the serious consequences of falls, and the challenges and practical considerations involved in reducing the risk of falling and the rate of falls. We explore the Commission's choice of an adaptive approach in it

Camera-based fall detection using a particle filter.

More than thirty percent of persons over 65 years fall at least once a year and are often not able to get up again. The lack of timely aid after such a fall incident can lead to severe complications. This timely aid can however be assured by a camera-based fall detection system triggering an alarm when a fall occurs. Most algorithms described in literature use the biggest object detected using background subtraction to extract the fall features. In this paper we compare the performance of our state-of-the-art fall detection algorithm when using only background subtraction, when using a particl

Postural balance and the risk of falling during pregnancy.

Pregnancy is a physiological process and many changes occur in a woman's body during pregnancy. These changes occur in all systems to varying degrees, including the cardiovascular, respiratory, genitourinary, and musculoskeletal systems. The hormonal, anatomical, and physiological changes occurring during pregnancy result in weight gain, decreased abdominal muscle strength and neuromuscular control, increased ligamentous laxity, and spinal lordosis. These alterations shift the centre of gravity of the body, altering the postural balance and increasing the risk of falls. Falls during pregnancy

Childhood Falls With Occipital Impacts.

OBJECTIVES: Falls are commonly reported in children who present with both accidental and inflicted brain injuries. Short falls rarely result in serious or life-threatening injuries. Our purpose is to describe a series of cases of short falls with occipital impact leading to subdural hemorrhage (SDH). METHODS: We present a series of 8 witnessed accounts of young children diagnosed as having SDHs after striking the back of their heads during a short fall. Child-abuse physicians were surveyed to determine if they had evaluated a child younger than 24 months diagnosed as having SDH, with or witho

[Falls in the elderly].

Falls are the main causes of accidental death and disability in elderly people, since they may especially cause hip fractures and a number of related complications. Available incidence estimates are surely in defect, because falls are often omitted both by patients, their family and caregivers. Risk factors may be classified in intrinsic and extrinsic; the former include muscular and osteoarticular diseases or other favouring conditions, whilst the latter include environmental or iatrogenic factors, such as drugs or alcohol consumption. Extrinsic factors may be rapidly modified in the elderly

Adapting Falls Prevention Evidence for People with Charles Bonnet Syndrome Across Clinical and Community Settings.

Charles Bonnet Syndrome (CBS) refers to visual hallucinations associated with a decline in vision. Historically, the functional effects of CBS have focused on its psychological impacts, but recent work has highlighted that CBS may also lead to physical outcomes by causing and increasing susceptibility to falls. As awareness of CBS among sight loss and falls prevention specialists is low, its role in the sight loss and falls mechanism may be missed. A falls prevention approach for CBS-related falls spanning primary, secondary, and community services is indicated to ensure this potential contrib

Real-Time Vision-Based Fall Detection Systems for the Elderly: A Systematic Review.

Falls represent a threat to older adults, overload healthcare systems, and reduce quality of life. Vision-based fall detection has advanced recently through deep learning, yet most proposed models lack validation on physical hardware and do not report inference-time metrics. This systematic review, following PRISMA and Kitchenham guidelines, targets this gap. We focus exclusively on vision-based systems that report inference speed on a specified device. We define real-time performance using a threshold of 10 fps, based on the reported duration of the critical fall phase in real-life falls. Fro
