Intermittent exogenous ketosis does not reduce acute mountain sickness during early terrestrial high-altitude acclimatization.

PURPOSE: Intermittent exogenous ketosis via ketone monoester (KE) ingestion has been shown to enhance physiological responses and reduce acute mountain sickness (AMS) symptoms during acute normobaric hypoxia. Its effects during early acclimatization to terrestrial high-altitude, however, remain unclear. METHODS: Thirty-four participants were randomised to an intermittent exogenous ketosis (IEK, n=17) or placebo (PLA, n=17) group and underwent a near sea level trial (295 m) without supplementation, followed by a four-day sojourn at 3375 m during which they received regular KE (IEK) or placebo

[Overview of the most important (patho)physiological mechanisms of high-altitude acclimatization in healthy individuals].

More and more people are spending time at high altitudes, either recreationally or permanently. As altitude increases, the partial pressure of inspired oxygen decreases, leading to hypobaric hypoxia and triggering a wide range of physiological adaptations. This article discusses the most relevant (patho)physiological acclimatization effects at high altitudes in healthy subjects. Hypobaric hypoxia primarily affects respiration, circulation, blood, as well as sleep and brain function. Respiration responds immediately with increased ventilation (hypoxic ventilatory response), reducing CO₂ levels

Seasonal Metabolic Acclimatization Varies in Direction and Magnitude among Years in Two Arid-Zone Passerines.

Adaptive plasticity in avian thermal physiology is increasingly apparent, with a well-studied example being metabolic upregulation during cold winters in small birds inhabiting temperate and boreal latitudes. Recent studies have revealed greater variation in the direction and magnitude of seasonal metabolic adjustments among subtropical/tropical birds experiencing milder winters compared with higher-latitude counterparts, suggesting that patterns could vary among years within populations. We quantified seasonal metabolic variation (summer vs. winter) in Kalahari Desert populations of two Afrot

Nocturnal periodic breathing during acclimatization at very high altitude at Mount Muztagh Ata (7,546 m).

RATIONALE: Quantitative data on ventilation during acclimatization at very high altitude are scant. Therefore, we monitored nocturnal ventilation and oxygen saturation in mountaineers ascending Mt. Muztagh Ata (7,546 m). OBJECTIVES: To investigate whether periodic breathing persists during prolonged stay at very high altitude. METHODS: A total of 34 mountaineers (median age, 46 yr; 7 women) climbed from 3,750 m within 19-20 days to the summit at 7,546 m. During ascent, repeated nocturnal recordings of calibrated respiratory inductive plethysmography, pulse oximetry, and scores of acute mount

Alterations in cerebral dynamics at high altitude following partial acclimatization in humans: wakefulness and sleep.

We tested the hypothesis that, following exposure to high altitude, cerebrovascular reactivity to CO2 and cerebral autoregulation would be attenuated. Such alterations may predispose to central sleep apnea at high altitude by promoting changes in brain PCO2 and thus breathing stability. We measured middle cerebral artery blood flow velocity (MCAv; transcranial Doppler ultrasound) and arterial blood pressure during wakefulness in conditions of eucapnia (room air), hypocapnia (voluntary hyperventilation), and hypercapnia (isooxic rebeathing), and also during non-rapid eye movement (stage 2) slee

α-Adrenergic regulation of blood pressure in acclimatizing lowlanders and Andean highlanders at high altitude.

Elevated muscle sympathetic nerve activity (MSNA) at high altitude is associated with blunted transduction of sympathetic signals to blood pressure in lowlanders and indigenous Andean highlanders. However, it is unclear whether this is due to reduced adrenergic communication or other factors (e.g., augmented dilatory signaling). Therefore, we quantified the contribution of α-adrenoreceptor activity to 1) resting systemic sympathetic transduction and 2) pressor responses to sympathoexcitation in acclimatizing lowlanders (9 M and 4 F) and Andean highlanders (15 M) at 4,300 m. MSNA (microneurogra

The Impact of High-Altitude Acclimatization on Frontal Lobe Executive Function: The Mediating Effect of Creatinine.

OBJECTIVE: High-altitude hypoxia impairs frontal executive functions. This study examines whether serum creatinine (CR) mediates the relationship between altitude acclimatization (AAI) peripheral oxygen saturation (SpO2)/hematocrit (HCT) ratio and frontal neural oscillations. METHODS: In 190 adults at 3,650 m (Lhasa), we measured SpO2, hematocrit, CR, and resting-state frontal electroencephalography. Power spectral density was calculated across multiple frequency bands (1-80 Hz) and Z-standardized. Mediation analysis with bootstrap tested the indirect effect of AAI on each band via CR. RESUL

Acetazolamide fails to decrease pulmonary artery pressure at high altitude in partially acclimatized humans.

In this randomized, double-blind placebo controlled trial our objectives were to determine if acetazolamide is capable of preventing high altitude pulmonary edema (HAPE) in trekkers traveling between 4250 m (Pheriche)\4350 m (Dingboche) and 5000 m (Lobuje) in Nepal; to determine if acetazolamide decreases pulmonary artery systolic pressures (PASP) at high altitude; and to determine if there is an association with PASP and signs and symptoms of HAPE. Participants received either acetazolamide 250 mg PO BID or placebo at Pheriche\Dingboche and were reassessed in Lobuje. The Lake Louise Consensus

Adaptive acclimatization yields a Bacillus velezensis strain with enhanced nitrate metabolism for remediating salinized soil.

Microbial inoculants show potential for remediating nitrate-rich, salinized soils. However, native strains often exhibit suboptimal performance under high-salinity and high-nitrate conditions, limiting their practical application. To develop a nitrate-reducing strain suitable for saline soil remediation, we adaptively acclimated a nitrate-reducing Bacillus velezensis strain BV-1 under high-nitrate conditions to enhance its salt tolerance and nitrogen metabolic capacity. The acclimatized strain exhibited significantly upregulated nar genes (4.71- to 7.56-fold) and nirD expression (1.36-fold), i

Closing the loop: autonomous intelligent control for hypoxia pre-acclimatization and high-altitude health management.

Hypobaric hypoxia at high altitudes threatens the health of high-altitude residents. The development of effective methods to guarantee the safety of frequent human activities in high-altitude locations is therefore needed. Pre-acclimatization at sea level is an effective approach to mitigate subsequent altitude sickness for rapid ascent, which offers a viable substitute to on-site acclimatization, minimizes the associated risks that are linked to prolonged exposure in high-altitude environments and can be personalized to individual hypoxic responses. Another critical aspect to prevent long-ter

Why is VO2 max after altitude acclimatization still reduced despite normalization of arterial O2 content?

Acute hypoxia (AH) reduces maximal O2 consumption (VO2 max), but after acclimatization, and despite increases in both hemoglobin concentration and arterial O2 saturation that can normalize arterial O2 concentration ([O2]), VO2 max remains low. To determine why, seven lowlanders were studied at VO2 max (cycle ergometry) at sea level (SL), after 9-10 wk at 5,260 m [chronic hypoxia (CH)], and 6 mo later at SL in AH (FiO2 = 0.105) equivalent to 5,260 m. Pulmonary and leg indexes of O2 transport were measured in each condition. Both cardiac output and leg blood flow were reduced by approximately 15

Increased calcium current in carotid body glomus cells following in vivo acclimatization to chronic hypoxia.

1. Rat pups were gestated and born in normoxia (inspired O2 pressure 149 mmHg) or chronic hypoxia (insured O2 pressure 80 mmHg) to test whether chronic hypoxia alters carotid body glomus cell calcium currents. Carotid bodies were removed from 5- to 8-day-old-pups under halothane anesthesia, at which time blood hematocrits averaged 52 +/- 1% (mean +/- SE) in the chronically hypoxic pups and 36 +/- 1% in the normoxic pups (P < 0.05). Glomus cells were then enzymatically isolated from the carotid bodies, and calcium currents were recorded with whole cell patch clamp. 2. Compared with normoxic glo

[The characteristics and changes in high altitude ventilatory response during acclimatization at 5000 meters altitude].

To assess the characteristics and changes of high altitude ventilation response during acclimatization at 5000m altitude. The poikilocapnic hypoxic ventilatory responses (PHVR) were tested on nineteen healthy men who sojourned 5000m altitude for 12 weeks. The results were that there was a linear correlation between predicated ventilations which were calculated for 5000m altitude depended on PHVR values before ascent and actual ventilations tested when arrived at altitude. The PHVR was increased at second week after ascent, but at twelth week it was dropped near the level before ascent. The aut

The effect of partial acclimatization to high altitude on loop gain and central sleep apnoea severity.

BACKGROUND AND OBJECTIVE: Loop gain is an engineering term that predicts the stability of a feedback control system, such as the control of breathing. Based on earlier studies at lower altitudes, it was hypothesized that acclimatization to high altitude would lead to a reduction in loop gain and thus central sleep apnoea (CSA) severity. METHODS: This study used exposure to very high altitude to induce CSA in healthy subjects to investigate the effect of partial acclimatization on loop gain and CSA severity. Measurements were made on 12 subjects (age 30 ± 10 years, body mass index 22.8 ± 1.9,

Body composition in air and road inductees at high altitude during the initial days of acclimatization.

This study assesses body composition changes and their time course during the initial days of acclimatization to high altitude (HA). Comparisons were made between gradual and acute induction to HA using 60 male lowlander volunteers (24-28 years of age) divided into two equal groups for inducting them to HA. Thirty subjects were air-lifted from sea level (SL) to 3500 m HA in 1 h. These subjects were air inductees (AI). The other 30 subjects were transported in 4 days by road to the same location at 3500 m. These were road inductees (RI). After remaining for 15 days at 3500 m both groups were in

Acclimatization and real-time performance of botanical biofilters eliminating indoor volatile organic compounds using SIFT-MS.

Irrespective of the level of development, people are exposed to a mix of indoor volatile organic compounds (VOCs), including carcinogens such as benzene and formaldehyde. A botanical biofilter (BB) integrates plants, substrates, and their associated microbiomes to eliminate VOCs. Nonetheless, the BB startup time, the impact of indoor conditions, and the contribution of aerial biomass remain insufficiently examined. This work examines the role of acclimatization and varying boundary conditions through real-time VOC quantification. Two BBs (Epipremnum pinnatum) were acclimatized at a constant ai

Steady-state chemoreflex drive captures ventilatory acclimatization during incremental ascent to high altitude: Effect of acetazolamide.

Ventilatory acclimatization (VA) is important to maintain adequate oxygenation with ascent to high altitude (HA). Transient hypoxic ventilatory response tests lack feasibility and fail to capture the integrated steady-state responses to chronic hypoxic exposure in HA fieldwork. We recently characterized a novel index of steady-state respiratory chemoreflex drive (SSCD), accounting for integrated contributions from central and peripheral respiratory chemoreceptors during steady-state breathing at prevailing chemostimuli. Acetazolamide is often utilized during ascent for prevention or treatment

The influence of short-term high-altitude acclimatization on cerebral and leg tissue oxygenation post-orthostasis.

PURPOSE: Orthostasis at sea level decreases brain tissue oxygenation and increases risk of syncope. High altitude reduces brain and peripheral muscle tissue oxygenation. This study determined the effect of short-term altitude acclimatization on cerebral and peripheral leg tissue oxygenation index (TOI) post-orthostasis. METHOD: Seven lowlanders completed a supine-to-stand maneuver at sea level (450 m) and for 3 consecutive days at high altitude (3776 m). Cardiorespiratory measurements and near-infrared spectroscopy-derived oxygenation of the frontal lobe (cerebral TOI) and vastus lateralis (l
