[The effects of innominate ostèotomy on the opposite acetabulum (author's transl)].

The study on dissected pelvis shows that after innominate osteotomy, the acetabulum still faces too much anteriorly. Its anterior wall remains deficient. The innominate osteotomy is able to diminish the excessive antetorsion of the acetabulum by rotating the distal fragment outwards. But this movement seems to increase the antetorsion of the opposite acetabulum. Axial X. Rays of the pelvis authors think that in case of great anterior defect, and in case of bilateral hip dysplasia, a triple pelvic osteotomy will be preferred to Salter's osteotomy.

Central fractures of the acetabulum: a critical analysis and review of literature.

Central fracture of the acetabulum is an uncommon traumatic lesion of the pelvis that is usually best managed by conservative measures. Adequate roentgenographic evaluation and closed manipulation followed by skeletal traction is essential to attain and maintain congruity of the weight-bearing dome with the femoral head. Reduction should be maintained with traction continued uninterrupted for three months. Open reduction and internal fixation may be warranted if the actabular dome elements are of sufficient size for reconstruction of the pelvic wall. An early exercise program will improve and,

Does Acetabulum Remodel Following Varus Derotation Osteotomy for Perthes' Disease?

Objective  We investigated the effect of disease stage, patient's age and final contour of femoral head on acetabulum contour following varus derotation osteotomy of proximal femur (VDRO) in unilateral Perthes's disease. Methods  The study is a retrospective analysis of case records of 23 children aged ≥6 years with unilateral Perthes' disease who underwent primary VDRO procedure for containment. Acetabular index (AI) and center edge angle (CEA) were calculated bilaterally in preoperative and follow-up radiographs and compared statistically. Results  There were 15 boys and 8 girls. Six hips we

Bilateral asymmetrical hip dislocations with acetabulum fractures; a case report.

Bilateral hip dislocation is a rare injury and asymmetrical hip dislocations are even very rare entities. We are reporting two cases of bilateral asymmetrical hip dislocations with associated acetabulum fractures. Mechanism of injury in first case was fall from height and in other case road traffic accident. Associated injuries were midshaft femur fracture and sciatic nerve (Peroneal part). Anterior hip dislocation in both cases is reduced closely but posterior hip dislocation in both cases relocated openly due to ipsilateral femur fracture in first case and unstable hip in second case. Standard Approaches to the Acetabulum Part 1: Kocher-Langenbeck Approach.

Historically, standard approaches for surgical treatment of displaced acetabular fractures were the KocherLangenbeck approach, the ilioinguinal approach and the extended iliofemoral approach (12). Presently, several modifications of these approaches are accepted alternatives, especially anterior modifications based on the intrapelvic approach described by Hirvensalo (8). Single access approaches allowing visualization of one acetabular column are the posterior Kocher-Langenbeck approach and the anterior ilioinguinal approach (12) and the use of a single approach is favoured (9, 24). Retroversion of the contralateral adult acetabulum after previous Perthes' disease.

Retroversion of the adult acetabulum following a previous Legg-Calvé-Perthes disease (LCPD) is found in 30-40% of the patients. It is known that in LCPD there may also be morphologic changes in the nonaffected controlateral acetabulum. We investigated the prevalence of retroversion of the contralateral acetabulum in a group of adult patients with a history of LCPD. We found this morphology in 11 out of 36 patients (31%). The aetiology and its relation to Perthes' disease are discussed.

Intraoperative fractures of the acetabulum during primary total hip arthroplasty.

BACKGROUND: The intraoperative occurrence of an acetabular fracture is a rare complication of primary total hip arthroplasty. Previous reports have lacked a sufficiently large number of subjects to allow for an analysis of the causes and appropriate treatment of this problem. METHODS: Between 1990 and 2000, 7121 primary total hip arthroplasties were performed at our institution. We retrospectively reviewed the records in our Total Joint Registry and found that twenty-one patients (twenty-one hips) had sustained an intraoperative acetabular fracture. Nineteen of these patients (nineteen hips)

Total hip arthroplasty in acute acetabulum fractures: a systematic review.

INTRODUCTION: The incidence of acetabular fractures has increased significantly in recent years due to demographic change. It can be divided into high-energy acetabular fractures primarily seen in adult patients versus geriatric patients mostly suffering from low-energy trauma. Historically, acetabular fractures have been treated either non-operatively or surgically following open reduction and internal fixation (ORIF), whereas nowadays total hip arthroplasty (THA) provides a true alternative. Aim of this study was to perform a systematic review on the outcome following THA in acute acetabulum

Managing bone loss in revision total hip arthroplasty: the acetabulum.

Osteolysis is the greatest threat to the viability of modern acetabular implants. The etiology and natural history of osteolysis remain unknown. Patients with cementless implants who have osteolysis are challenging to treat. There are no standards for follow-up methods or frequency, and there is little evidence to support timing of the decision to begin treatment in these patients. The natural history of proposed treatment options is also unknown. Reconstruction of the acetabulum in the presence of bone loss is difficult. Classifications have been developed to guide management. Development of acetabulum after closed reduction in developmental hip dysplasia.

The radiographic study of acetabulum development after closed reduction of 87 congenitally dislocated hips in 64 patients who were treated between 1980 and 1990 was conducted retrospectively. An average patient's age at the moment of closed hip reduction was 16 months (range between 6 to 24 months). On the average, patients were 16 months old (range between 6 to 24 months) at the moment of closed hip reduction. Development of bony acetabulum was followed for 8 years by measuring the AC angle. According to the age at the moment of closed reduction patients were sorted into three groups, the fir

Bilateral fractures of acetabulum in a young girl with osteogenesis imperfecta and epilepsy.

We report a case of bilateral displaced acetabular fractures including both columns fractures with protrusio acetabuli in a female patient, aged 15 years, with osteogenesis imperfecta (OI) (Sillence Type I), epilepsy, blue sclera and bilateral ipoacusia. Since OI is a rare genetic disorder characterized by an increased propensity to osteopenia, intraoperative fracture risks and hemorrhagic diathesis, we opted for the open reduction and internal fixation of the acetabula in two surgical steps and using two different approaches. Although the clinical outcomes are not excellent in this report (HH

A Modified Extensile Anterior Approach to the Acetabulum for Severe Acetabular Defects.

A modified extensile Smith-Petersen approach was used to access the acetabulum and pelvis in cases with complex acetabular defects requiring extensive reconstructions. Between 2010 and 2014, a total of 49 hips (48 patients) with a variety of acetabular defects were reconstructed using highly porous hemispherical acetabular cups and different reconstruction methods. Preoperative diagnoses included isolated aseptic loosening of the acetabular component, aseptic loosening of both acetabular and femoral components, infection, and other. Five patients (5 hips) died prior to the 2-year follow-up, wi
