Systematic Review of Methemoglobinemia in Acetaminophen Poisoning.

BACKGROUND: Acetaminophen (N-acetyl-para-aminophenol, paracetamol, (APAP) toxicity is one of the commonly encountered poisonings by emergency physicians. Methemoglobinemia is an uncommon association and rarely seen in APAP poisoning. METHODS: Retrospective analysis of all the published reports on APAP induced methemoglobinemia from 1968 to 2019. RESULTS: In total there were 14 cases with 9 females and 6 males. The median age of the study cohort was 59 years. The most common presenting feature was altered mentation (9 patients) followed by cyanosis (3 patients). The intent of consumption was

Acetaminophen (paracetamol) for the common cold in adults.

BACKGROUND: Acetaminophen is frequently prescribed for treating patients with the common cold, but there is little evidence as to whether it is effective. OBJECTIVES: To determine the efficacy and safety of acetaminophen in the treatment of the common cold in adults. SEARCH METHODS: We searched CENTRAL 2013, Issue 1, Ovid MEDLINE (1950 to January week 5, 2013), EMBASE (1980 to February 2013), CINAHL (1982 to February 2013) and LILACS (1985 to February 2013). SELECTION CRITERIA: We included randomised controlled trials (RCTs) comparing acetaminophen to placebo or no treatment in adults with

Intravenous acetylcysteine for the treatment of acetaminophen overdose.

IMPORTANCE OF THE FIELD: Acetaminophen is a leading cause of overdose-related hepatotoxicity. Although acetylcysteine prevents or minimizes acetaminophen-induced hepatotoxicity and reduces mortality, some patients presenting with complicated overdose scenarios (massive ingestions or combination or modified-release formulations) may develop toxicity despite administration of recommended dosage regimen. AREAS COVERED IN THIS REVIEW: The article evaluates evidence regarding intravenous acetylcysteine's effectiveness in patients with acute overdoses who receive treatment within 10 h or > 10 h, pa

Role of acetaminophen in acute myocardial infarction.

Acetaminophen, the active ingredient in Tylenol, is a widely used drug that is well known for its analgesic and antipyretic properties. Acetaminophen is a commonly used alternative to nonsteroidal anti-inflammatory drugs, which have recently been demonstrated to increase mortality after acute myocardial infarction (AMI). The safety and potential cardioprotective properties of acetaminophen in the setting of AMI have recently been investigated; however, the results from these studies have been inconclusive. Using both large (ovine) and small (rabbit) collateral-deficient animal models, we studi

Use of acetaminophen in young subelite athletes.

AIM: The purpose of the present investigation was to look for other drugs besides doping substances in the urine of subelite athletes submitted to heavy training. METHODS: One hundred and forty-one young subelite athletes (in sprint, cycling, middle distance running and handball) were included in the study, with a control group of 89 high school pupils. Drugs were researched by high performance liquid chromatography using a diode array detector. RESULTS: Among the 212 subjects who agreed to give a urine sample, acetaminophen was detected: 9.5% for the subelite athletes versus 1.3% for the co

Acetaminophen in the post-ischemia reperfused myocardium.

Acetaminophen was administered acutely at the onset of reperfusion after 20 min of low-flow, global myocardial ischemia in isolated, perfused guinea pig hearts (Langendorff) to evaluate its influence in the postischemia, reperfused myocardium. Similarly prepared hearts were treated with vehicle or with uric acid (another phenol for comparison). Functionally, acetaminophen-treated hearts (0.35 mM) achieved significantly greater recovery during reperfusion. For example, left ventricular developed pressures at 40 min reperfusion were 38 +/- 3, 27 +/- 3, and 20 +/- 2 in the presence of acetaminoph

Perioperative pharmacodynamics of acetaminophen analgesia in children.

BACKGROUND: There are no adequate pharmacodynamic data relating concentrations of acetaminophen in serum to analgesia. METHODS: Children undergoing outpatient tonsillectomy were administered acetaminophen either orally, 0.5-1.0 h preoperatively (n = 20), or per rectum at induction of anesthesia (n = 100). No other analgesic agents were administered. Individual concentrations of acetaminophen in serum and pain scores (0-10) measured over a 4-h postoperative period were analyzed using a nonlinear mixed-effects model (NONMEM). RESULTS: Mean (% CV) estimates of population pharmacokinetic paramet

Extrahepatic lesions induced by acetaminophen in the mouse.

Acetaminophen in acute overdose is primarily recognized as potentially hepatotoxic with few descriptions of extrahepatic lesions other than nephrotoxicity. Fasted adult, male mice, both standard and germ-free, were given acetaminophen orally and killed at selected times, from 30 minutes to 48 hours after treatment. In addition to the expected hepatic effects after 600 mg acetaminophen/kg, degenerative and necrotic changes were found in four non-hepatic tissues. Nephrosis developed 2 to 4 hours after treatment and paralleled the course of hepatic damage. Necrosis of bronchiolar epithelium in th

Acetaminophen hepatotoxicity in alcoholics. A therapeutic misadventure.

We have treated 6 chronic alcoholics and identified an additional 19 reported in the literature who developed severe hepatotoxicity from acetaminophen taken in apparently moderate doses. The clinical disease in these 25 patients had a characteristic pattern: mild to moderate jaundice; mild to severe coagulopathy; and strikingly abnormal aminotransferase levels, values inconsistent with either acute alcoholic hepatitis or viral hepatitis. The possible causes for the injury from ostensibly nontoxic drug levels appear to be either the induction by chronic alcohol intake of the cytochrome P-450 sy

Interaction between acetaminophen and organophosphates in mice.

This study was undertaken to investigate whether depletion of hepatic nonprotein sulfhydryls (NPSH) by acetaminophen would potentiate the toxicity of organophosphates which are detoxified by glutathione transferases. Acetaminophen caused a dose dependent decrease of NPSH in mouse liver. At the dose of 600 mg/kg, which decreased hepatic NPSH by 90%, acetaminophen did not potentiate the effects of the organophosphorus insecticides methylchlorpyrifos, methylparathion or dichlorovos on esterases. On the other hand, depletion of hepatic NPSH by diethylmaleate increased the toxicities of the insecti

Interaction of ethanol with acetaminophen metabolism in the baboon.

To evaluate the effects of ethanol on acetaminophen metabolism and toxicity, twelve female baboons were studied using three experimental designs. In the first one, animals fed ethanol chronically and their pair-fed controls received acetaminophen intravenously (40 mg/kg), and drug metabolism was studied for 6 hr in blood and urine. Elimination of acetaminophen from plasma was accelerated significantly in baboons fed alcohol chronically, and urinary excretion of mercapturic acid conjugate was increased. In the second, the experiments were repeated with the addition of ethanol infusion (120-160

Development of an enzyme-based assay for acetaminophen.

A new and novel method for determination of serum acetaminophen is described. The assay, which can be completed in less than 5 min, is based on the enzymatic hydrolysis of acetaminophen, with subsequent colorimetric detection of the aminophenol so produced. Various possible means of aminophenol estimation are described; the final reaction conditions have been optimized for maximum sensitivity and assay speed. This assay compares favorably with other available procedures; it requires only small sample volumes; it is rapid, simple, and highly specific for the parent drug; and it requires neither

Nalbuphine, acetaminophen, and their combination in postoperative pain.

In a double-blind study with the use of subjective reports of patients as indices of analgesia, we compared the analgesic effect of oral nalbuphine and acetaminophen and determined the contribution of each to the efficacy of their combination. In this parallel 2 X 2 factorial study, 129 inpatients after surgery were randomly assigned to treatment with a single oral dose of nalbuphine hydrochloride (30 mg), acetaminophen (650 mg), the combination of nalbuphine (30 mg) and acetaminophen (650 mg), or placebo. In the factorial analysis, both the nalbuphine and acetaminophen effects were significan

Acetaminophen and aspirin challenges in subgroups of asthmatics.

To determine whether the different modes of action of aspirin and acetaminophen are associated with different incidences of bronchoconstriction, and whether there is cross-reactivity in terms of intolerance, selected asthmatics were challenged with both analgesics. Clinical profiles were observed to characterize more precisely the aspirin-intolerant asthmatic. Inhalational challenges with PGF2 alpha were then performed on nine selected subjects. It was determined that a positive history of aspirin intolerance is a reliable indicator of potential for anaphylactoid reactions but is not associate

Propoxyphene and acetaminophen. Possible effects on the fetus.

Propoxyphene (Darvon) and acetaminophen (Tylenol) are widely prescribed analgesic agents. Both can cross the placenta, and propoxyphene can produce serious withdrawal symptoms in newborns. Neither propoxyphene nor acetaminophen is considered a teratogen, yet, three malformed infants who were antenatally exposed to propoxyphene have previously been reported. We report a fourth case of an infant, with withdrawal symptoms and cranial-facial and digital malformations, born to a women who used propoxyphene and acetaminophen throughout her pregnancy. We suggest that possibility that the antepartum u

Effects of aspirin and acetaminophen in pregnancy and in the newborn.

Inhibitors of prostaglandin synthesis are increasingly recommended for delaying premature delivery. However, such inhibitors--eg, aspirin, indomethacin, or naproxen--may interfere with uterine contractility, with maternal, fetal, and neonatal platelet function, and especially with fetal circulation and postnatal adaptation. The instillation of aspirin (50 to 90 mg/kg of fetal body weight) into the rumina of fetal lambs resulted in significant increase of pulmonary arterial pressure, which was directly related to constriction of the ductus arteriosus. Such pressure elevation may act as a stimul

Two family members developed severe hepatitic dysfunction in association with infectious mononucleosis and acetaminophen administration. Since severe hepatitis is an extremely rare complication of infectious mononucleosis, we postulate that the hepatic dysfunction was induced by acetaminophen.
