Acromion is a major associated structure of subacromial impingement syndrome, one of the most common diagnoses in chronic shoulder pain world-wide. The aims of this study are to study morphometry of acromion and to find risk group using acromial morphometry. Total samples were 392 scapulae. The samples were of both sexes, ranging from the age of 31 to 90. Acromion type and osteophytes were observed. Acromial parameters were measured. The relationships were analyzed among acromion type, acromial osteophyte, acromial parameters, age group, sex and side. Curved acromion had the highest prevalence

PURPOSE: Quantitative and qualitative kinematic analyses of subacromial impingement by 1.2T open MRI were performed to determine the location of impingement and the involvement of the acromioclavicular joint.
METHODS: In 20 healthy shoulders, 10 sequential images in the scapular plane were taken in a 10-s pause at equal intervals from 30° to maximum abduction in neutral and internal rotation. The distances between the rotator cuff (RC) and the acromion and the acromioclavicular joint were measured. To comprehend the positional relationships, cadaveric specimens were also observed.
RESULTS: A

INTRODUCTION: Acromion and scapular spine stress fractures can be catastrophic complications following reverse shoulder arthroplasty (RSA). A variety of host, implant, and technical factors have been identified that increase the risk of this complication. The glenoid component in particular has been closely evaluated for its impact on rates of stress fractures following RSA. The goal of this biomechanical study is to evaluate if humeral stem version has an impact on acromion and scapular spine strain after RSA.
METHODS: Eight cadaveric specimens were tested on a custom dynamic shoulder frame.

The purpose of this study was to analyze age-related changes in the coracoacromial arch and correlate these degenerative changes with rotator cuff tears. We obtained 80 shoulders from 40 cadavers. The mean age at death was 58.4 years. We performed a gross examination of the rotator cuff and the acromion and histological examination of the coracoacromial ligament. The statistical significance of any difference for each group considered was determined by Student's t-test. The rotator cuff was normal in 66 specimens; there was an articular-side partial tear in 4 cases, a bursal-side partial tear

BACKGROUND: The purpose was to analyze the influence of deltoid lengthening due to different implant designs and anatomic variations of the acromion and scapular spine (SS) in the parasagittal plane on strain patterns after reverse shoulder arthroplasty (RSA).
METHODS: Ten cadaveric shoulders with strain rosettes placed on the surface of the acromial body (Levy II) and SS (Levy III) were tested using a shoulder simulator. RSA using humeral onlay (+3, +5, +8, +10, +13 mm) and glenosphere lateralization (0, +6 mm) was performed. Arm lengthening and magnitude of strain on acromion/SS were measur

BACKGROUND: The axillary nerve is at risk for iatrogenic injuries in surgical procedures involving the lateral aspect of the shoulder joint. To date and to our knowledge, there have been no studies that have compared the relevant distances in the common arm abduction positions used in these types of surgery as well as the relative risks of each position.
PURPOSE: To evaluate the effect of arm abduction position on the distance from the acromion process to the axillary nerve in the common abducted arm positions.
STUDY DESIGN: Descriptive laboratory study.
METHODS: The shoulders of 10 fresh-f

PURPOSE: To evaluate acromial shape in relation to age, sex, symmetry, and presence of subacromial enthesophytes.
MATERIALS AND METHODS: Three hundred ninety-four cadaveric scapulas were reviewed. Specimens were categorized by sex and age (age range, 20-89 years). Acromial morphology was typed according to the Bigliani classification: type I, flat; type II, curved; and type III, hooked. The presence and degree of subacromial enthesopathy was recorded. Selective radiographic correlation was obtained.
RESULTS: The relative percentages of acromial types I, II, and III were 22.8% (90 acromions),

A twenty-eight-year-old multiple trauma patient had a nondisplaced acromion fracture that was not detected until after it had displaced. Open reduction internal fixation was performed without complication and the patient achieved excellent shoulder abduction strength. Nondisplaced acromion fractures may displace if not protected. Open reduction internal fixation of displaced acromion fractures should be considered if deltoid muscle strength is important to the patient.

INTRODUCTION: There are many variations of scapula morphology. Evaluation of scapula morphology is necessary to elucidate shoulder joint movement. We aimed to analyze the bone morphology of various parts constituting the scapula to identify certain conserved features among them.
MATERIALS AND METHODS: Thirty-one healthy individuals were the subjects. We created a scapular three-dimensional (3D) model using computed tomography (CT). X and Y axes were set on a glenoid surface. We measured the approximate plane of the upper and lower scapular bodies and scapular spine and the central axis of the

HYPOTHESIS: Reverse shoulder prostheses depend on deltoid function. An injury to the acromion, either fracture or meso-acromion, or deltoid injury, may affect the outcomes of patients after receiving a reverse shoulder prosthesis.
METHODS: Four-hundred and fifty-seven consecutive reverse shoulder prostheses were implanted between January 1992 and June 2003 by one of seven surgeons in five separate centers. Forty-one patients were noted to have a pre-operative lesion of the acromion or scapular spine. Twenty-three presented with an os acromiale (meso-acromion). Seventeen had fracture or fragme

BACKGROUND: Small changes in deltoid tension and moment arm due to glenosphere lateralization may be associated with an increase in acromion or scapular spine strain in reverse shoulder arthroplasty (RSA), which can lead to stress fracture. The coracoacromial ligament (CAL) may be protective and lower the strain seen on the acromion or scapular spine. This biomechanical study investigated the impact of glenosphere lateralization and CAL integrity on acromion and scapular spine strain after RSA.
METHODS: Ten cadaveric specimens were tested on a custom dynamic shoulder frame. Acromial and scapu

The coracoacromial ligament (CAL), normally a superior restraint against humeral translation, is frequently involved in rotator cuff impingement pathology. However, surgical excision of the CAL is not always clinically successful. Little anatomic information exists about the morphology and function of this ligament. The CAL and glenohumeral joint in 56 cadaveric shoulders were examined in 31 cadavers. Nineteen dimensional parameters were obtained by direct measurement. In 16 shoulders, specific attention was directed at the anterior band of the CAL. Variation exists in the morphology of the CA

BACKGROUND: Subacromial erosion remains a major concern after surgical fixation of acromioclavicular (AC) joint using a clavicular hook plate. To minimize postoperative subacromial erosion, we investigated the structural relationship between distal clavicle and acromion around the AC joint by considering the surgical fixation of the joint using the hook plate technique.
METHODS: Computed tomography scans of 101 AC joints without any inherent pathology were analyzed. The angle between the distal clavicle and acromion around the AC joint (AC angle), depth of the acromion, differences in height

BACKGROUND: The etiology of rotator cuff disease is age related, as documented by prevalence data. Despite conflicting results, growing evidence suggests that distinct scapular morphologies may accelerate the underlying degenerative process. The purpose of the present study was to evaluate the predictive power of 5 commonly used radiologic parameters of scapular morphology to discriminate between patients with intact rotator cuff tendons and those with torn rotator cuff tendons.
METHODS: A pre hoc power analysis was performed to determine the sample size. Two independent readers measured the

BACKGROUND: The axillary nerve may be injured during percutaneous fixation of proximal humerus fractures. This study investigated the kinematic behavior of the superior and inferior borders of the axillary nerve under varying shoulder positions. This information may reduce iatrogenic neurologic injury during fracture reduction and hardware placement.
METHODS: The lateral deltoid approach was performed on 7 fresh frozen shoulders. The inferior and superior borders of the axillary nerve were tagged. Screws were placed in the anterior, middle, and posterior acromion as landmarks. Three-dimension

PURPOSE: Primary glenohumeral osteoarthritis is commonly associated with static posterior subluxation of the humeral head. Scapulae with static/dynamic posterior instability feature a superiorly and horizontally oriented acromion. We investigated whether the acromion acts as a restraint to posterior humeral translation.
METHODS: Five three-dimensional (3D) printed scapula models were biomechanically tested. A statistical shape mean model (SSMM) of the normal scapula of 40 asymptomatic shoulders was fabricated. Next, a SSMM of scapular anatomy associated with posterior subluxation was generate

OBJECTIVE: An exact assessment of shoulder movement is of special importance both in the diagnosis of and in the therapy for different shoulder diseases. Therefore, we developed a feasible method for the analysis of shoulder movement.
METHODS: On the basis of an ELITE system and 6 skin markers (marker positions. acromion, humerus, olecranon, proc. styloideus ulnae, cervical and thoracic spine), movement analysis was performed during continuous abduction of the arm over 15 s. With the help of a purpose made software we determined the exact angle of abduction and, in addition, the acceleration

An 80-year-old woman with repeated hemarthrosis of the right shoulder was examined and treated arthroscopically. Plain radiographs indicated narrowing of the acromiohumeral interval (3 mm) with large acromial spur. Under fluoroscopic visualization, the impingement between the humeral head and the medial edge of the acromion was seen at 50 degrees abduction. The area of impingement of the humeral head seemed to be just medial to the greater tuberosity. Arthroscopically, a crater formation was seen at this site. Active bleeding was seen at the center of the crater, which was interpreted as the p

Twenty-five patients with suspected impingement syndrome on the basis of clinical examination were evaluated by magnetic resonance imaging (MRI). In paracoronal and parasagittal planes, different measurements of bony structures of the subacromial space and of the glenoid joint were performed, as in the anatomical study reported in the companion paper. A healthy, age-and sex-matched group of 25 persons underwent the same MRI-procedure and the results were compared with those in the patients. Ruptures of the rotator cuff were excluded by means of standard X-rays, physical examination, and ultras
