Severity of infection following the introduction of new infection control measures for medical abortion.

BACKGROUND: In response to concerns about serious infections following medical abortion, in early 2006 the Planned Parenthood Federation of America changed the route of misoprostol administration from vaginal to buccal and required either routine antibiotic coverage or universal screening and treatment for chlamydia; in July 2007, the Planned Parenthood Federation of America began requiring routine antibiotic coverage for all medical abortions. We previously reported a pronounced drop in the rate of serious infections following the adoption of these new infection control measures. Our objectiv

Preparation and in-vivo evaluation of dimenhydrinate buccal mucoadhesive films with enhanced bioavailability.

Dimenhydrinate (DMH)-loaded buccal bioadhesive films for the prevention and treatment of motion sickness were prepared and optimized. This study examines the rate of drug release from the films for prolonged periods of time to reduce or limit the frequency of DMH administration. Based on preliminary studies using various polymers and concentrations, hydroxyethylcellulose (2.5, 3.0, and 3.2%), and xanthan gum (2.8%) were chosen as matrix polymers. The films were analyzed with respect to their mechanical, physicochemical, bioadhesive, swelling, and in-vitro release properties. In in-vivo pharmac

Recent development in therapeutics for breakthrough pain.

Breakthrough pain is defined as transitory flares of pain. Breakthrough pain is caused by cancer, cancer complications, treatment or comorbidities. The usual onset to maximum breakthrough pain intensity time is 3 min and duration is 30 min; therefore, the assessment for response needs to be at short intervals. The rapid onset and offset of pain results in inadequate responses when oral opioids are used to manage pain flares. Several strategies have been used to manage breakthrough pain: titration of the chronic opioid, independent titration of rescue opioids and alternative routes. Buccal fent

Efficacy, safety, route of administration of midazolam and diazepam for pediatric status epilepticus: systematic review, meta-analysis, and trial sequential analysis.

BACKGROUND: Status epilepticus (SE) is a life-threatening pediatric emergency requiring rapid anticonvulsants. While midazolam and diazepam are standard benzodiazepines, their comparative efficacy across various administration routes remains debated. We synthesized high-quality evidence to guide clinical protocols. METHODS: We searched PubMed, Scopus, Web of Science, and Cochrane for randomized controlled trials comparing buccal, intramuscular, and intravenous formulations. We evaluated treatment outcomes including therapeutic success (seizure cessation), recurrence, and drug-related side eff

Formulation development and characterization of peptide-based spray-forming in situ gel for enhanced buccal permeation and improved stability.

AIM: To develop an in situ gelling buccal spray to improve the permeation and stability of reduced glutathione through the buccal mucosa without using ascorbic acid as an antioxidant. MATERIALS AND METHODS: Glutathione, a hydrophilic thiol peptide classified as a Biopharmaceutics Classification System (BCS) class III drug, exhibits poor oral bioavailability due to limited permeability, enzymatic degradation by gamma-glutamyl transferase, and instability in aqueous environments. The formulation was optimized based on gelling time and glutathione stability and evaluated for spray pattern, in si

[Mucoadhesive tablets for oral administration of ciclopiroxolamine].

Oral mucoadhesive tablets belong to modern dosage forms, which allow controlled drug release after buccal application. They are used for the treatment of oral cavity disorders or for systemic administration of drugs with high first-pass effect or drugs instable in gastrointestinal tract. This study reports the development of oral mucoadhesive tablets containing antimycotic drug ciclopiroxolamine. Mucoadhesive properties of placebo tablets containing different ratios of carbomer and hydroxypropylmethylcellulose were evaluated in vivo in healthy human volunteers. The longest mucoadhesion for 9 h

An assessment of prochlorperazine buccal for the prevention of nausea and vomiting during intravenous patient-controlled analgesia with morphine following abdominal hysterectomy.

The effectiveness of prochlorperazine buccal as an anti-emetic for the prevention of post-operative nausea and vomiting in patients using intravenous patient-controlled analgesia with morphine following abdominal hysterectomy has been assessed in a randomized, double-blind, placebo-controlled study. Forty-nine female patients participated with 26 allocated to the prochlorperazine buccal group and the remainder to the placebo group. Each received either placebo or prochlorperazine buccal 6 mg, in each case by the buccal route, 1 h prior to anaesthesia with further doses at 6, 18, 30 and 42 h, r

Effects of topical administration of a bisphosphonate (risedronate) on orthodontic tooth movements in rats.

In orthodontics, undesirable movement of anchor teeth during tooth movement and relapse of moved teeth after treatment are the main causes of unsuccessful results. If these tooth movements could be prevented with pharmacological agents, a less complex orthodontic force system and less extensive retention would be required. The purpose of this study was to examine the effect of topical administration of a bisphosphonate (risedronate), a potent blocker of bone resorption, on orthodontic tooth movements in rats. In the first experiment, both the right and left upper first molars were moved buccal

Solid lipid microparticles-into-film delivering clobetasol: from buccal formulation design to pilot clinical study in patients with oral lichen planus.

Oral lichen planus is a chronic inflammatory disorder of the oral cavity, for which the therapeutic gold standard consists of the topical administration of corticosteroids, such as clobetasol-17-propionate (CLB). Nowadays, there are no commercially available pharmaceuticals intended for CLB delivery within the oral cavity. Then, in clinical practice, the drug is administered via magistral galenic preparations, formulated as adhesive semisolids. As they possess numerous drawbacks (e.g., limited contact time, poor patients' compliance, inaccurate dosing), the research is still focused on the dev

Development and biopharmaceutical evaluation of aqueous micelle based corticosteroid formulations for topical treatment of oral lichen planus.

The dearth of approved products to treat oral lichen planus (OLP) compels off-label use of dermatological corticosteroid formulations not optimized for indications in the oral cavity. The aims of this study were to develop aqueous micelle based formulations of triamcinolone acetonide (TA) and fluocinonide (FLU), using D-tocopheryl polyethylene glycol succinate, and to investigate corticosteroid delivery to the epithelium-lamina propria junction region - the anatomical target for OLP treatment - in comparison to that from marketed products. Total mucosal deposition of TA after application of Ke

Nasal fentanyl alone plus buccal midazolam: an open-label, randomised, controlled feasibility study in the dying.

INTRODUCTION: Many patients want to stay at home to die. They invariably become unable to take oral medication during their terminal phase. Symptoms are usually controlled by subcutaneous medications. There have been no studies on nasal fentanyl (NF) or buccal midazolam (BM) to control symptoms in the dying. OBJECTIVE: To establish how best to conduct a definitive, randomised controlled trial (RCT) to determine whether NF and BM administered by families, for patients dying at home, lead to faster and better symptom control and fewer community nursing visits than standard breakthrough medicati

Antiepileptic drug treatment in the end-of-life phase of glioma patients: a feasibility study.

BACKGROUND: During the end-of-life (EOL) phase of glioma patients, a rapid deterioration in neurological functioning may interfere with the oral intake of antiepileptic drugs (AEDs). We aimed to assess the feasibility of non-oral AED treatment in an out-of-hospital setting according to an expert-based guideline. METHODS: Glioma patients with a history of epilepsy, in whom further antitumor therapy was considered to be no longer meaningful, were recruited at two Dutch hospitals. As soon as swallowing difficulties developed, the patient's caregiver administered prophylactic treatment with bucca

Amlodipine-induced buccal lichenoid lesions: A case report.

Buccal lichenoid lesions (BLLs) are characterised by a unique, linear whitish striation in the buccal region and can be accompanied by ulcers, plaques, erythemas, atrophies and blisters. They are distinguished from oral lichen planus (OLP) by the association of the administration of a drug or contact with a metal. Herein, we present the case of a 42-year-old woman with underlying hypertension with amlodipine-induced BLLs. She complained of a 1-month history of right buccal whitish streaks and oral ulcers 2 months after taking amlodipine. She visited a private otorhinolaryngology clinic, and a

A randomized controlled trial of different buccal misoprostol doses in mifepristone medical abortion.

BACKGROUND: An 800-mcg dose of buccal misoprostol following mifepristone has been shown to be highly effective in terminating pregnancies through 63 days since the last menstrual period (LMP) (B. Winikoff, I.G. Dzuba, M.D. Creinin, et al., Two distinct oral routes of misoprostol in mifepristone medical abortion: a randomized controlled trial. Obstet Gynecol 2008; 112: 1303-1310). However, a two 200-mcg misoprostol pill option would simplify administration, and potentially reduce costs and increase women's satisfaction. This study compares a 400-mcg dose (Group I) to an 800-mcg dose (Group II)
