Management of congenital dysfibrinogenemia in pregnancy: A challenging patient case.

Afibrinogenemia and congenital dysfibrinogenemia (CD) are rare conditions with limited information available for appropriate management. Previous case reports have demonstrated the safe and efficacious use of fibrinogen replacement therapy (FRT) as a therapeutic approach to prevent hemorrhage and fetal loss in pregnant women with CD. In this case report, we present a 28-year-old pregnant woman who sought testing for CD given her family history. She denied any current or previous bleeding symptoms. Laboratory testing confirmed the diagnosis of CD. She was treated with FRT and prophylactic antic

Total knee arthroplasty in a patient with hypofibrinogenemia.

Patients with afibrinogenemia or hypofibrinogenemia present a unique challenge to the arthroplasty surgeon as fibrinogen is a key contributor to hemostasis. Patients with these disorders are known to have a higher risk for postsurgical bleeding complications. We present the case of a patient with hypofibrinogenemia who underwent an elective total knee arthroplasty. Our colleagues in hematology-oncology guided us initially to achieve and maintain appropriate fibrinogen levels in the early perioperative period. However, the patient developed an acute joint effusion and subsequent infection 4 wee

Individualized Periprocedural Management of Congenital Dysfibrinogenemia during Transvaginal Oocyte Retrieval: A Case Report.

Congenital dysfibrinogenemia is a qualitative fibrinogen disorder with phenotypes ranging from isolated laboratory abnormalities to bleeding or thrombosis. Transvaginal ultrasound-guided oocyte retrieval is minimally invasive but may cause pelvic bleeding, and peri-procedural management in patients with congenital dysfibrinogenemia is not standardized. We report a 34-year-old woman with tubal-factor infertility and persistently low functional fibrinogen who pursued in vitro fertilization. Targeted sequencing of coagulation-related genes identified a heterozygous fibrinogen gamma chain gene (FG

Congenital Fibrinogen Deficiencies: Not So Rare.

Congenital fibrinogen deficiencies (CFDs), traditionally considered rare monogenic disorders, are now recognized as more prevalent and genetically complex than previously thought. Indeed, the symptoms manifested in CFD patients, such as bleeding and thrombosis, are likely to result from variation in several genes rather than solely driven by variants in one of the three fibrinogen genes, FGB, FGA, and FGG. This review highlights recent advances in understanding the genetic causes of CFD and their variability, facilitated by the growing use and availability of next-generation sequencing data. U

The β-Chain Mutation p.Arg17Stop Impairs Fibrinogen Synthesis and Secretion: A Nonsense Mutation Associated With Hypofibrinogenemia.

BACKGROUND: Congenital hypofibrinogenemia, a quantitative fibrinogen disorder, is characterized by abnormally low levels of both functional and antigen fibrinogen. We identified a heterozygous nonsense mutation, p.Arg17Stop, in the fibrinogen Bβ chain of a three-month-old female infant. METHODS: Coagulation testing, gene analysis, in vitro plasmid construction, and functional analyses were conducted to investigate the underlying pathogenic mechanisms. RESULTS: Plasma fibrinogen levels showed decrease in the proband. DNA sequencing of the proband revealed a heterozygous point mutation (c.139C

Absence of Missense Variant Detection in Inherited Dysfibrinogenemia May Result from a Poor Raw Data Analysis Algorithm or Mosaicism.

Variant identification underlying inherited dysfibrinogenemia quite exceptionally fails. We report on two dysfibrinogenemia cases whose underlying DNA variant could not be identified by Sanger analysis. These failures result from two distinct mechanisms. The first case involved raw signal overcorrection by a built-in software, and the second constituted the first description of mosaicism for one of the fibrinogen genes. This mosaicism was subsequently identified by next-generation sequencing reanalysis of the sample.

Mutations Accounting for Congenital Fibrinogen Disorders: An Update.

Fibrinogen is a complex protein that plays a key role in the blood clotting process. It is a hexamer composed of two copies of three distinct chains: Aα, Bβ, and γ encoded by three genes, FGA, FGB, and FGG, clustered on the long arm of chromosome 4. Congenital fibrinogen disorders (CFDs) are divided into qualitative deficiencies (dysfibrinogenemia, hypodysfibrinogenemia) in which the mutant fibrinogen molecule is present in the circulation and quantitative deficiencies (afibrinogenemia, hypofibrinogenemia) with no mutant molecule present in the bloodstream. Phenotypic manifestations are variab

Congenital Hypofibrinogenemia; An Unexpected Culprit Of Blindness In An Infant.

Congenital hypofibrinogenemia is a rare haematological disorder in which the production of functional fibrinogen is impaired because of the inherited mutation. Hypofibrinogenemia affects the coagulation cascade leading to bleeding diathesis and one of its manifestations can be recurrent Vitreous haemorrhages, sometimes leading to irreversible loss of vision. Therefore, Hypofibrinogenemia must be included in the differential diagnosis of Vitreous haemorrhage, particularly in young children. We report a case of a four months old female infant who was brought by her mother to the unit since she w

Congenital hypofibrinogenemia with bleeding risk: mutations in the FGA, FGB, and FGG genes.

INTRODUCTION: Congenital hypofibrinogenemia is a genetic disorder caused by defects in the fibrinogen gene. We identified a case of congenital hypofibrinogenemia with mutations in the FGA, FGB, and FGG genes associated with bleeding risk and conducted experimental studies to explore the condition's pathogenesis. METHODS: To investigate the bleeding risk in the proband, we performed coagulation screening and genetic analysis, supplemented by sodium dodecyl sulfate polyacrylamide gel electrophoresis, electron microscopy, sequence conservation analysis, and thromboelastography to elucidate the p

Hereditary hepatic fibrinogen storage disease with a novel fibrinogen variant FGG c.1113T>A ( fibrinogen Seoul III ).

Hereditary hepatic fibrinogen storage disease (HHFS) is an extremely rare genetic disorder caused by mutations in the fibrinogen gamma ( FGG ) gene. Clinical presentations range from asymptomatic cases to severe symptoms. We present the case of a 9-year-old boy with persistently elevated liver enzymes who exhibited no clinical symptoms. A liver biopsy revealed hepatic fibrinogen storage disease, and genetic testing identified a heterozygous mutation, c.1113T>A, p.(Asn371Lys) in FGG . This is the first reported case of HHFS in Korea, as well as the first report of this novel mutation. Multigene

Congenital fibrinogen disorders are rare pathologies of the haemostasis, comprising afibrinogenaemia, hypofibrinogenaemia, dysfibrinogenaemia and hypodysfibrinogenaemia. Phenotypic manifestations are variable, patients may be asymptomatic or suffer from bleeding or thrombosis. Most of congenital fibrinogen disorders are coincidentally discovered. Fibrinogen concentrate is used to treat bleeding, whereas low-molecular weight heparin is most often administered for the treatment of thrombotic complications. The aim of this review is to provide an update of the knowledge of congenital fibrinogen d

Laboratory Testing for Fibrinogen Disorders: From Routine Investigations to Research Studies.

Congenital and acquired fibrinogen disorders often have heterogeneous clinical phenotypes and are challenging from a laboratory perspective. Fibrinogen determination using the Clauss method remains the gold standard, while the reproducibility and significance of the thrombin time and the reptilase time are limited. Molecular testing for causative mutations in fibrinogen genes is now recommended to confirm the diagnosis of congenital fibrinogen disorders. Research assays are used to evaluate alterations to fibrin formation and properties of plasma and purified fibrinogen-derived clots, characte

Etiology and management of hypofibrinogenemia in trauma.

PURPOSE OF REVIEW: Fibrin polymerization is essential for stable clot formation in trauma, and hypofibrinogenemia reduces hemostasis in trauma. This review considers fibrinogen biology, the changes that fibrinogen undergoes after major trauma, and current evidence for lab testing and treatment. RECENT FINDINGS: Fibrinogen is a polypeptide that is converted to fibrin by the action of thrombin. During trauma, fibrinogen levels are consumed and reduce within the first few hours because of consumption, dilution, and fibrinolysis. Fibrinogen levels usually rebound within 48 hours of injury and can
