Cicatricial pemphigoid is a chronic vesiculobullous disease of the mucosal epithelium that primarily involves the oral cavity and the eyes. The clinical and histologic features are identical to those of bullous pemphigoid, and these features often can be nonspecific for other disease processes. It is not unusual for a period of 1 year or more to elapse before a diagnosis is made. The diagnosis of cicatricial pemphigoid requires characteristic lesions and histopathologic evidence of immunoglobulin deposition along the basement membrane, as well as a high index of suspicion. The authors detail a

TOPIC IMPORTANCE: Small airways diseases pose several challenges to the practicing clinician. The small airways are involved in common disorders such as asthma and COPD but are also the primary site of pathology in a host of bronchiolitis syndromes. Diagnosis is challenging and often delayed because of a lack of uniform diagnostic criteria and the limited ability of standard pulmonary testing to capture small airways dysfunction until considerable involvement has occurred. Data to guide management beyond general strategies targeting airflow obstruction are limited and largely derived from smal

Different introducers are available to assist with tracheal intubation. Subtle differences in the design of introducers can have a marked effect on safety and performance. The Difficult Airway Society's Airway Device Evaluation Project Team proposal states that devices should only be purchased for which there is at least a case-control study on patients assessing airway devices. However, resources are not currently available to carry out a case-control study on all introducers available on the market. This study comprised a laboratory and manikin-based investigation to identify introducers tha

Huge hydrocephalus is defined as a head circumference larger than the length of the child. We discuss the perioperative anesthetic management of a case of huge hydrocephalus during ventriculoperitoneal shunt placement, focusing primarily on the airway management. The patient was a 4-month-old with a midline supratentorial lesion causing obstructive hydrocephalus due to compression of the sylvian aqueduct. For optimum positioning for direct laryngoscopy, a pillow was placed below the baby's torso, to achieve a slight extension at the atlantoaxial joint. This maneuver decreased the angle between

PURPOSE OF REVIEW: The number of anaesthetic procedures in the ambulatory setting is steadily increasing. During the last years, more and more patients with complex medical conditions undergo ambulatory interventions. This review will focus on airway management practices, but especially on techniques used in the ambulatory setting. It will highlight recent literature published in the focus of ambulatory anaesthesia and will discuss relevant findings of the last year.
METHODS: Literature search and analysis performed independently by two reviewers.
RECENT FINDINGS: Practical points to improve

OBJECTIVE: To report our experience with propranolol in managing airway infantile hemangiomas.
DESIGN: Case series of 3 consecutive patients who had extensive, symptomatic airway infantile hemangiomas treated with propranolol.
SETTING: Tertiary academic medical center.
PATIENTS: Three infants with facial cutaneous hemangiomas who developed stridor that progressed to respiratory distress, which according to laryngoscopic examination results was confirmed to be caused by extensive subglottic hemangiomas. These patients underwent follow-up during their course of therapy, ranging from 3 weeks t

Forestier's disease, also called diffuse idiopathic skeletal hyperostosis (DISH), is a noninflammatory enthesopathy, ossifying the anterolateral spine and sparing the disc and joint space in elderly men, mostly at thoracic levels. Intubation difficulty and spinal cord injury are potential problems when managing the airway in DISH patients. We report a patient with Forestier's disease who was admitted for osteophyte resection. After taking a detailed history, we evaluated the airway carefully. Also, preparation for difficult intubation was done. After a rapid sequence induction, we performed ma

The upper airway can obstruct due to soft tissues, blood, secretions and infections. In patients who are sedated, anaesthetised or unconscious, the commonest cause is due to loss of airway muscle tone. The usual site of obstruction is the soft palate followed by the epiglottis and occasionally the tongue. To relieve the obstruction, a head tilt, chin lift jaw thrust and open mouth manoeuvre should be performed. This will tense the muscles in the floor of the mouth causing the hyoid and larynx to be lifted away from the posterior pharyngeal wall. Oral and nasal airways are useful adjuncts to im

We have developed a modern strategy for the anaesthetic management of the pediatric airway using anaesthetic drugs such as sevoflurane, propofol, mivacurium and remifentanil, together with new techniques including the laryngeal mask. This strategy provides optimal conditions for the examiner and maximum safety for the pediatric patient. The endoscopic investigation of pediatric airways has become important for the diagnosis and treatment of many pediatric diseases, and is often performed with the support of a pediatric anaesthetist. Important indications include acute and chronic stridor, chro

The operative repair of craniofacial defects presents the surgeon with critical problems regarding airway management. Thirty-seven per cent of our series of 100 children operated upon for a variety of cranofacial deformities had some difficulty in airway management. Those patients with mandibular dysostoses and those requiring midface advancement (Le Fort III) had the highest incidence of airway problems. Our suggested management of the airway is described with emphasis upon the importance of close cooperation between the plastic surgeon, otolaryngologist and anesthesiologist.

Airway management in a patient with Forestier's disease can be challenging clinically because this disease may cause not only dysphagia but also airway obstruction due to the compression of the pharynx and esophagus caused by the ossification of anterior longitudinal ligament. We report our anesthetic management in a patient with Forestier's disease. Meanwhile, we studied the causes of difficult airway and the most suitable airway device for a patient with this disease from a standpoint of anatomy of upper airway. Our study indicated the possibility that the most suitable airway device differe

BACKGROUND: Ingestion and/or aspiration of foreign bodies (FB) are avoidable incidents. Children between 1 and 3 years are common victims for many reasons: exploration of the environment through the mouth, lack of molars which decreases their ability to properly chew food, lack of cognitive capacity to distinguish between edible and inedible objects, and tendency to distraction and to perform other activities, like playing, whilst eating. Most FBs are expelled spontaneously, but a significant percentage impacts the upper aerodigestive tract. Approximately 80% of children's choking episodes are

OBJECTIVE: To investigate etiology and airway management in children with tracheobronchomalacia.
METHOD: Bronchoscopic examinations were performed in 671 children. The cases with tracheomalacia and bronchomalacia were analyzed in etiopathogenesis and summarized their therapy simultaneously.
RESULT: Bronchoscopic examination indicated tracheomalacia and bronchomalacia in 148 cases, tracheomalacia in 77 cases and bronchomalacia in 71 cases. Among the cases with tracheomalacia, compression by vascular rings was found in 46 cases, incorporated congenital esophageal atresia with tracheoesophageal

A tracheal bronchus is an aberrant, accessory or ectopic bronchus arising almost invariably from the right lateral wall of the trachea, causing hypoxaemia, atelectasis, or both, during anaesthesia. We describe two patients with a tracheal bronchus found before anaesthesia. One tracheal bronchus was found by tracheobronchoscopy and the other by chest x-ray. Because of recognition of the anomaly before operation, anaesthesia was uneventful in each patient.
