Obesity, Diabetes, and Birth Outcomes Among American Indians and Alaska Natives.

Objectives To examine the relationships between prepregnancy diabetes mellitus (DM), gestational diabetes mellitus (GDM), and prepregnancy body mass index, with several adverse birth outcomes: preterm delivery (PTB), low birthweight (LBW), and macrosomia, comparing American Indians and Alaska Natives (AI/AN) with other race/ethnic groups. Methods The sample includes 5,193,386 singleton US first births from 2009-2013. Logistic regression is used to calculate adjusted odds ratios controlling for calendar year, maternal age, education, marital status, Kotelchuck prenatal care index, and child's s

Assessing the Everyday Discrimination Scale among American Indians and Alaska Natives.

The Everyday Discrimination Scale (EDS) has been used widely as a measure of subjective experiences of discrimination. The usefulness of this measure for assessments of perceived experiences of discrimination by American Indian and Alaska Native (AI/AN) peoples has not been explored. Data derived from the Special Diabetes Program for Indians--Healthy Heart Demonstration Project (SDPI-HH), a large-scale initiative to reduce cardiovascular risk among AI/ANs with Type 2 diabetes. Participants (N = 3,039) completed a self-report survey that included the EDS and measures of convergent and divergent

Survival of immersions during recreational boating events in Alaska, 1999-2004.

OBJECTIVE: This retrospective study analyzed records of boating or other watercraft-related events in Alaska from 1999-2004, where at least one drowning occurred, to identify factors associated with survivors of these same events. DATA: Records of recreational boating events involving at least one fatality were obtained through the U.S. Coast Guard. Group rescue responses and rescue assistance responses by others outside the parties were categorized and analyzed. We conducted t tests and odds ratios to analyze inter-group differences. RESULTS: There were 32 immersion events involving 36 fata

Infectious disease hospitalizations among older American Indian and Alaska Native adults.

OBJECTIVES: American Indians and Alaska Natives (AI/AN) adults > or = 65 years of age (older adults) have the second highest age group-specific infectious disease (ID) hospitalization rate. To assess morbidity and disparities of IDs for older AI/AN adults, this study examined the epidemiology of overall and specific infectious disease hospitalizations among older AI/AN adults. METHODS: ID hospitalization data for older AI/AN adults were analyzed by using Indian Health Service hospital discharge data for 1990 through 2002 and comparing it with published findings for the general U.S. population

Probable destructive meningioma in an archaeological adult male skull from Alaska.

Several intracranial pathological conditions can affect the bones of the skull. The most common cause of these conditions is tumor, but infection and other diseases are also known to affect the bones of the skull. Distinguishing between the various causes of intracranial skeletal pathology in archaeological human remains is usually a challenging exercise, and a specific diagnosis will often be impossible. Meningiomas are tumors that arise in arachnoid tissues embedded in the outer layer of the dura. Because of this association, they occur almost exclusively in the skull and vertebral column. U

Two atypical cases of cystic echinococcosis (Echinococcus granulosus) in Alaska, 1999.

Before 1999, clinical experience demonstrated that the sylvatic (or Northern) biotype of Echinococcus granulosus seen in Alaska produced fewer complications and serious sequelae than infection with the pastoral (or European) biotype found in other parts of the world. Two cases of E. granulosus with severe sequelae occurred in Alaska in 1999. The adverse outcomes could have been rare complications that are part of the clinical spectrum of disease caused by sylvatic cystic echinococcus, an indication that the sylvatic biotype, especially when affecting the liver, has potential for severe clinica

Epidemiology of Invasive Haemophilus influenzae Type a Disease in Alaska, 2018-2022.

We used statewide surveillance data to describe the epidemiology of invasive Haemophilus influenzae type a disease in Alaska during 2018 to 2022. Of 52 cases identified, 39 (75%) occurred among Alaska Native children aged <5 years who lived in rural areas of southwest or northern Alaska. The average annual incidence was 17.8 per 100 000 among children aged <5 years vs 0.3 among persons aged ≥5 years. Among 43 cases in children aged <5 years, 16 (37%) presented with meningitis and 6 (14%) died. Characterizing H influenzae type a disease epidemiology can help direct prevention strategies, includ

Urgent versus elective surgical disparities among American Indian and Alaska Native patients.

BACKGROUND: American Indian and Alaska Native (AIAN) health issues are understudied despite documentation of lower-than-average life expectancy. Urgent surgery is associated with higher rates of postsurgical complications and postoperative death. We assess whether American Indian and Alaska Native (AIAN) patients in Washington State are at greater risk of requiring urgent rather than elective surgery compared with non-Hispanic Whites (NHW). METHODS: We accessed data for the period 2009-2014 from the Washington State Comprehensive Hospital Abstract Reporting System (CHARS) database, which capt

Prevalence and Correlates of Homelessness Among American Indian and Alaska Native Youth.

Youth homelessness is a serious national challenge affecting millions of young people every year. However, due to their relatively small population size, together with limitations related to data and research efforts on homelessness to date, prevalence estimates and evidence of homelessness experiences among American Indian and Alaska Native (AIAN) youth have been scarce. This is particularly the case at the national level. We report findings on the prevalence, characteristics, and correlates of AIAN youth experiencing homelessness that are based on a nationally representative survey on homele

Problem-gambling severity and psychiatric disorders among American-Indian/Alaska native adults.

INTRODUCTION: Little is known about the association between problem-gambling severity and psychiatric disorders among American-Indian/Alaska-Native (AI/AN) individuals. Thus, we examined these factors among a nationally representative sample of AI/AN and other American adults in the USA. METHOD: Using the National Epidemiologic Survey on Alcohol and Related Conditions (NESARC) data, we conducted separate Wald tests and multinomial logistic regression analyses comparing AI/AN to black/African American, white/Caucasian, and all other racial/ethnic groups, respectively. RESULTS: Relative to oth

A Process Evaluation of the Alaska Native Colorectal Cancer Family Outreach Program.

This article presents the results of a process evaluation of the Alaska Native (AN) Colorectal Cancer (CRC) Family Outreach Program, which encourages CRC screening among AN first-degree relatives (i.e., parents, siblings, adult children; hereafter referred to as relatives) of CRC patients. Among AN people incidence and death rates from CRC are the highest of any ethnic/racial group in the United States. Relatives of CRC patients are at increased risk; however, CRC can be prevented and detected early through screening. The evaluation included key informant interviews (August to November 2012) w

The public health foundation of health services for American Indians & Alaska Natives.

The integration of public health practices with federal health care for American Indians and Alaska Natives (AI/ANs) largely derives from three major factors: the sovereign nature of AI/AN tribes, the sociocultural characteristics exhibited by the tribes, and that AI/ANs are distinct populations residing in defined geographic areas. The earliest services consisted of smallpox vaccination to a few AI/AN groups, a purely public health endeavor. Later, emphasis on public health was codified in the Snyder Act of 1921, which provided for, among other things, conservation of the health of AI/AN pers

Autoimmune hepatitis in the Alaska Native population: autoantibody profile and HLA associations.

BACKGROUND & AIMS: The Alaska Native population is one of few populations in the world with a high prevalence of autoimmune hepatitis. The objective of this study was to determine the frequency and HLA and clinical associations of autoantibodies in Alaska Native people with autoimmune hepatitis. METHODS: Alaska Native individuals with autoimmune hepatitis were recruited in clinics conducted statewide. Sera were tested for the presence of autoantibodies described in either autoimmune hepatitis or rheumatic disease. Associations between autoantibodies and HLA alleles and clinical features were

Assessment of mammography experiences and satisfaction among American Indian/Alaska Native women.

BACKGROUND: American Indian/Alaska Native (AI/AN) women have lower breast cancer (BCA) screening and 5-year survival rates than non-Hispanic Whites. Understanding reasons for low screening rates is important to combatting later stage diagnoses. The purpose of this study was to assess mammography experiences and satisfaction among AI/AN women. METHODS: Nine focus groups were held with rural (N = 15) and urban (N = 38) AI/AN women 40 years and older in Kansas and Kansas City, Missouri, living both near and far from Indian Health Service (IHS) and tribal facilities, to examine experiences and sa

Coxiella burnetii in northern fur seals and Steller sea lions of Alaska.

Coxiella burnetii, a zoonotic bacterium, has recently been identified in several marine mammal species on the Pacific Coast of North America, but little is known about the epidemiology, transmission, and pathogenesis in these species. We tested sera archived from northern fur seals (NFS, Callorhinus ursinus; n=236) and Steller sea lions (SSL, Eumetopias jubatus; n=72) sampled in Alaska for C. burnetii antibodies, and vaginal swabs from NFS (n=40) for C. burnetii by qPCR. The antibody prevalence in NFS samples from 2009 and 2011 (69%) was significantly higher than in 1994 (49%). The antibody pr

Identifying risk factors for child maltreatment in Alaska: a population-based approach.

BACKGROUND: Child maltreatment has been linked to multiple negative health outcomes and many leading causes of death. Statewide population-based evaluations are needed to identify high-risk populations early in life for targeted interventions. PURPOSE: To assess the utility of combining Pregnancy Risk Assessment Monitoring System (PRAMS) data with child protective services (CPS) records to identify risk factors associated with Protective Services Reports (PSR) suggestive of child maltreatment. METHODS: This was a retrospective population-based cohort study conducted in the spring of 2010 usi
