Pharmacotherapy for alcohol misuse: Views of patients, providers, and leaders in an Alaska Native healthcare system.

INTRODUCTION: Alcohol misuse is responsible for a significant public health burden in the United States and remains undertreated despite approved treatments for alcohol misuse such as naltrexone. The current study characterizes the views of stakeholders in an Alaska Native healthcare system regarding pharmacotherapy for alcohol misuse. METHODS: A secondary, mixed-methods analysis was conducted using qualitative data from 70 interview participants (patients, providers, and health system leaders) and survey data completed by 944 stakeholders (patients, providers, leadership, and administrative

Multi-level determinants of vaccination of the American Indian and Alaska Native population: a comprehensive overview.

CONTEXT: American Indians and Alaska Natives (AIANs) are historically disadvantaged, losing 20 million (95%) of their population largely through epidemics since 1,520 and continuing lower overall vaccination coverage than other races. Determinants of this lower coverage are underexamined. METHODS: Among peer-reviewed relevant articles since 1968, 39 studied AIANs solely; 47 drew general population samples, including AIANs. We employed rigorous economic definitions and framework of Individual Decision-Making Under Uncertainty. The Social-Ecological model identified determinants and mechanisms

Grief and Historical Trauma Among American Indian and Alaska Native Populations: A Cluster Analysis.

American Indian and Alaska Native (AI/AN) communities face significant social, economic, and health disparities rooted in historical traumas stemming from colonization. These disparities manifest in disproportionate rates of poverty, unemployment, and early mortality among AI/AN populations. High rates of premature deaths contribute to a persistent cycle of grief and loss, perpetuating mental and physical health problems. The current study explores the relationships between grief, historical trauma, and psychological and physical health outcomes among AI/AN individuals who have experienced the

Conducting an implementation intervention study with American Indian and Alaska Native communities: Methodological considerations.

BACKGROUND: Incorporating American Indian and Alaska Native (AI/AN) traditional practices and knowledge into healthcare can support AI/AN health. Drug overdose deaths disproportionately impact AI/AN communities due to colonization, genocide, historical trauma, discriminatory policies, and under-resourced healthcare. Medications for opioid use disorder (MOUD; e.g., buprenorphine, methadone, naltrexone) are considered the most effective treatment for reducing mortality. Integrating AI/AN cultural practices with MOUD may increase acceptability and uptake of MOUD within AI/AN communities. METHODS

Sleep Problems and Health Outcomes Among Urban American Indian and Alaska Native Adolescents.

IMPORTANCE: Adolescent sleep problems are prevalent, particularly among racial and ethnic minority groups, and can increase morbidity. Despite the numerous strengths of their racial and ethnic group, urban American Indian and Alaska Native adolescents face significant health disparities but are rarely included in health research. Understanding how sleep problems are associated with health outcomes among American Indian and Alaska Native adolescents may elucidate novel targets for interventions to promote health equity. OBJECTIVE: To assess whether baseline sleep problems are associated with c

Urgent versus elective surgical disparities among American Indian and Alaska Native patients.

BACKGROUND: American Indian and Alaska Native (AIAN) health issues are understudied despite documentation of lower-than-average life expectancy. Urgent surgery is associated with higher rates of postsurgical complications and postoperative death. We assess whether American Indian and Alaska Native (AIAN) patients in Washington State are at greater risk of requiring urgent rather than elective surgery compared with non-Hispanic Whites (NHW). METHODS: We accessed data for the period 2009-2014 from the Washington State Comprehensive Hospital Abstract Reporting System (CHARS) database, which capt

A comparison of alcohol and commercial tobacco use in populous American Indian/Alaska Native states.

BACKGROUND: Modern research suggests that the racial influence on smoking and drinking behaviors may not be as prevalent as once thought. This study utilizes national survey data to compare binge drinking and commercial cigarette smoking behaviors between American Indians/Alaskan Natives (AI/AN) and non-Hispanic Whites in the five most populous AI/AN U.S. states. METHODS: Data were analyzed from the Behavioral Risk Factor Surveillance System (BRFSS) from 2018 to 2022; White (n=79,405), AI/AN (n=11,007). Data analyses include frequency statistics, followed by two model evaluations comparing ma

Depression management interests among Alaska Native and American Indian adults in primary care.

BACKGROUND: Depression remains the second leading cause of disability worldwide. Symptoms of depression are expressed and experienced differently across cultural groups, impacting treatment decisions. Patient preferences predict service utilization, treatment selection and persistence, as well as health outcomes for medical and behavioral health conditions, including depression. We identified depression management preferences of Alaska Native and American Indian (AN/AI) people who receive care within a comprehensive, integrated, tribally owned and operated healthcare facility in Anchorage, Ala

American Indian and Alaska Native Men's Use of Sexual Health Services, 2006-2010.

CONTEXT: American Indian and Alaska Native men experience poorer sexual health than white men. Barriers related to their sex and racial identity may prevent them from seeking care; however, little is known about this population's use of sexual health services. METHODS: Sexual health service usage was examined among 923 American Indian and Alaska Native men and 5,322 white men aged 15-44 who participated in the 2006-2010 National Survey of Family Growth. Logistic regression models explored differences in service use by race and examined correlates of use among American Indians and Alaska Nativ

Impact of heptavalent pneumococcal conjugate vaccine on invasive disease, antimicrobial resistance and colonization in Alaska Natives: progress towards elimination of a health disparity.

We evaluated invasive pneumococcal disease (IPD), antimicrobial resistance and nasopharyngeal colonization before and after introduction of pneumococcal conjugate vaccine (PCV7) in Alaska Natives (AN), a population with high IPD rates. We obtained IPD rates from population-based surveillance. Colonization was determined from annual surveys among rural AN of all ages and from urban children. After vaccine introduction, vaccine-type IPD rates declined by 91% among AN children <2 years, by 80% among non-Natives <2 years, and by 40% for adults of all races (P<0.001 each). IPD decreased for isolate
