BACKGROUND: Alexia without agraphia is a neurological syndrome that is caused by a specific lesion in the left (or the dominant) cerebral hemisphere. It is characterised by a severe reading disorder with writing ability intact.
CASE DESCRIPTION: We describe a patient who, after a fall from a staircase, could no longer read, even that which he had written shortly before. Initially, he also had problems with orientation and facial recognition. Two months after the accident, alexia without agraphia was still manifest while the other symptoms had disappeared. His ability to read showed a slow imp

Alexia without agraphia (also called pure alexia or word blindness) was the first of the disconnection syndromes to be described. It results from the loss of visual input to the language area without involvement of the language area. The most common cause is occlusion of the left posterior cerebral artery with involvement of left occipital cortex and the splenium of corpus callosum. However, it can also be caused by any lesion affecting the splenium of corpus callosum disrupting the white matter tracts from the left visual cortex to the angular gyrus. We hereby describe five cases of alexia wi

OBJECTIVE: To clarify whether agraphia or alexia occurs in lesions of the left posterior middle temporal gyrus.
METHODS: We assessed the reading and writing abilities of two patients with this lesion using kanji (Japanese morphograms) and kana (Japanese syllabograms).
RESULTS: Patient 1 first presented with pure alexia more impaired for kana after an infarction in the left middle and inferior occipital gyri and right basal occipital cortex, and after a second infarction in the left posterior middle temporal gyrus adjoining the first lesion he showed alexia with agraphia for kanji and worsene

Alexia without agraphia, or "pure" alexia, is an acquired impairment in reading that leaves writing skills intact. Repetition priming for visually presented words is diminished in pure alexia. However, it is not possible to verify whether this priming deficit is modality-specific or modality independent because reading abilities are compromised. Hence, auditory repetition priming was assessed with lexical decision and word stem completion tasks in pure alexic patients with lesions in left inferior temporal-occipital cortex and the splenium. Perceptually based, modality-specific priming models

An 84-year-old woman with a history of dry age-related macular degeneration presented with an acute inability to read, but intact writing ability (pure alexia or alexia without agraphia). She denied any difficulty speaking, paresthesias, or hemiparesis. Her visual acuity was 20/20 in each eye. Macular examination, optical coherence tomography, and fluorescein angiography demonstrated the previously diagnosed macular drusen and geographic atrophy of the retinal pigment epithelium consistent with the dry form of age-related macular degeneration both eyes. Automated perimetry revealed a right hom

Alexia without agraphia is the result of a left inferotemporal cortical lesion. This unilateral lesion provokes an almost irrecoverable word reading disability. This anatomical-clinical observation has identified as crucial an area on the left fusiform gyrus, which represents a sort of "letter cranial bump". This area, located near the left V4, has been labeled the visual word form area (VWFA). The VWFA ensures visual word processing whatever the visual field stimulated. In normal subjects, imaging methods show an activation of the VWFA during reading. In patients, the alexia that follows a VW

Disorders of reading are frequently encountered in patients with acquired cerebral lesions. Investigations in the past few decades have improved our understanding of these disorders. In this article we review the peripheral dyslexias, including neglect dyslexia, attentional dyslexia, and pure alexia (or alexia without agraphia), as well as the "central" dyslexias, including deep, surface, and phonological dyslexia. Current accounts of acquired dyslexia are also discussed. Finally, we briefly describe the reading tasks that serve to differentiate the different reading disorders.

Two Japanese patients with pure alexia, SH and YH, who showed right homonymous hemianopia following a left occipital lobe lesion, demonstrated letter-by-letter (LBL) reading in pronouncing Japanese kana words and nonwords. In contrast to alphabetic letters, each Japanese kana character has an invariant and identical pronunciation whether it appears in isolation or as a component of any word and nonword string. It is important to investigate the eye movements as well as reading latency and duration in Japanese-speaking LBL readers. Relative to normal controls, these patients demonstrated a more

The finding that visual processing of a word correlates with the number of its letters has an extensive history. In healthy subjects, a variety of methods, including perceptual thresholds, naming and lexical decision times, and ocular motor parameters, show modest effects that interact with high-order effects like frequency. Whether this indicates serial processing of letters under some conditions or indexes low-level visual factors related to word length is unclear. Word-length effects are larger in pure alexia, where they probably reflect a serial letter-by-letter strategy, due to failure of

In 1892, the French neurologist Jules Déjerine suggested that pure alexia resulted from an occipital lesion that selectively disconnected visual input from a region of the brain that housed "optical images of words." In this issue of Neuron, Gaillard and colleagues offer evidence consistent with Déjerine's proposal and provide new insights to the functional role of the "visual word form area."

To be a neurologist, training in cognitive/behavioral neuroscience is essential. In this review article, the author tries to emphasize the localization relationship between focal brain lesions and language dysfunction, by discussing from split-brain syndromes, hemispheric asymmetry, to pure word dumbness, pure word deafness (verbal auditory agnosia), pure agraphia, and pure word blindness (pure alexia; visual word agnosia), and finally to aphasia and aprosodia in general. Hopefully, after getting familiar with all the terms, the readers will feel free exploring language disturbances and behavi

Post-stroke language disorders are frequent and include aphasia, alexia, agraphia and acalculia. There are different definitions of aphasias, but the most widely accepted neurologic and/or neuropsychological definition is that aphasia is a loss or impairment of verbal communication, which occurs as a consequence of brain dysfunction. It manifests as impairment of almost all verbal abilities, e.g., abnormal verbal expression, difficulties in understanding spoken or written language, repetition, naming, reading and writing. During the history, many classifications of aphasia syndromes were estab

A 42-year-old lady presented with acute aneurysmal subarachnoid haemorrhage and developed difficulty recognising faces (prosopagnosia), inability to process visual information in busy environments (simultagnosia) and difficulty to read (alexia). She was subsequently found to have superficial siderosis on MRI.

Two cases of alexia without agraphia, one due to a cerebrovascular accident which improved spontaneously and the other in a cerebral abscess partially recovered after surgical treatment are presented. Both were anatomically verified by computed tomography. The second was also verified at surgery. A small well localized lesion of the fusiform and lingual gyri in the dominant hemisphere can lead to alexia without agraphia. The lack of involvement of the optic radiation and calcarine fissure explain the abscence of visual field defects and hence the purest type of this syndrome as shown in our fi

A word-length effect is often described in pure alexia, with reading time proportional to the number of letters in a word. Given the frequent association of right hemianopia with pure alexia, it is uncertain whether and how much of the word-length effect may be attributable to the hemifield loss. To isolate the contribution of the visual field defect, we simulated hemianopia in healthy subjects with a gaze-contingent paradigm during an eye-tracking experiment. We found a minimal word-length effect of 14 ms/letter for full-field viewing, which increased to 38 ms/letter in right hemianopia and t

Alexia without agraphia is readily recognized in its pure (without other neurologic findings) but rare form. However, this deficit is more common when associated with other behavioral disturbances that result from trauma or cerebral infarction to the posterior cerebral hemispheres. Two cases presented with alexia without agraphia following infarction within the posterior cerebral artery territory. Because of initial confusion and disorientation the patients were diagnosed as demented and unsuitable for rehabilitation. Both subjects had a visual field deficit and color agnosia. Prosopagnosia, s

PURPOSE OF REVIEW: Reading impairments after left or right hemisphere stroke are common yet receive little attention from clinicians and therapists. In this review, we focus on the classification of acquired alexia and the current theory and practice underlying the rehabilitation of this diverse set of disorders.
RECENT FINDINGS: The underlying behavioural impairments that dictate reading ability in the acquired alexias are becoming better understood; this, in turn, has led to targeted therapies being undertaken, mainly on a single subject basis. In hemianopic alexia, the most 'peripheral' of
