We retrospectively reviewed 207 patients suffering from traumatic bone defect who had been treated at our institution between 1973 and 1993. Three types of traumatic bone defects were identified: I, minor; II, major cortical; III, major articular. Each type was further subdivided into: A, open injury; B, closed injury. The proposed treatment modality of each type was included within each classification. Types II and III posed the greatest difficulties in management. However, massive bone allografts can be successfully used in these situations, even in the early phases of treatment, but only gi

BACKGROUND: In young patients with limited articular cartilage damage, osteochondral allografts may offer an alternative to total joint replacement. The survival of chondrocytes after transplantation and the correlation with clinical outcomes was studied.
METHODS: Between March 1987 and September 1990, nine patients received fresh osteochondral allografts. Three patients received tibial plateau transplants, three received patellar transplants, two received proximal interphalangeal joints and the remaining patient received a segmental femoral head allograft. Patient ages ranged from 16-51 year

One hundred and thirty-seven allografts used since 1986 in limb preserving operations for malignant bone tumours were reviewed. The follow up was longer than two years. There were fourteen fractures (10.2%) in twelve patients at a mean time of 22 months from the operation. Most of them were in the metaphyseal area and were related to perforations of the allograft made for stabilisation with plates, for tendon and ligament reattachment, or any other hole in the allograft. Fractures occurred always after the allograft-host junction was united. Healing was achieved in 7 cases by internal fixation

OBJECTIVE: To present long-term results after mitral valve replacement with stent mounted glutaraldehyde preserved aortic allografts in patients older than 15 years. The clinical support for this study was to combine the glutaraldehyde technique of biological tissue preservation with the advantages of allografts when compared to xenografts. This was demonstrated in previous studies using other methods of tissue processing.
METHODS: Between September 1984 and November 1994, 70 patients aged 16-77 years (mean 35.4 years) underwent mitral valve replacement with this preserved and mounted allogra

Liver transplantation and surgery : official publication of the American Association for the Study of Liver Diseases and the International Liver Transplantation Society1997eng
Little is known about the possible contribution of apoptosis to ischemia-reperfusion injury in human liver transplantation. Therefore, we studied postreperfusion surgical biopsy specimens of 16 human liver allografts using the TUNEL assay for in situ demonstration of apoptotic cells. In all patients, a variable proportion of hepatocytes and sinusoidal endothelial cells presented labeled nuclei. The mean +/- standard deviation percentages of positive hepatocytes were 18.7% +/- 12.2% in the whole section, 30.4% +/- 18.7% in the subcapsular region, 14.5% +/- 13.5% in the centrilobular zones, and

BACKGROUND: Minimizing immunosuppression after allotransplantation is desirable.
METHODS: We assessed the minimal dose of cyclosporin A for viable tracheal allografts in 50 dogs. Each tracheal transplant, consisting of a six-ring segment of cervical trachea, was harvested and heterotopically implanted into the omentum. In group I (n = 10), transplantation into each dog's own omentum was performed as a control. The remaining 40 tracheal segments were randomly assigned to four recipient groups receiving either no treatment (group II, n = 10), 10 mg.kg-1.day-1 of cyclosporin A (group III, n = 10

Renal allograft dysfunction may be due to a variety of causes including acute and chronic rejection, acute tubular necrosis, cyclosporine A toxicity, glomerulonephritis, and surgical causes. It can be difficult to distinguish between these etiologies. Ultrasound is a relatively simple, noninvasive method for imaging the transplant kidney, and is excellent for the diagnosis of surgical complications. The features of rejection include an increase in renal volume, reduced echogenicity, indistinctness of the corticomedullary junction, and splaying of the medullary pyramids. These findings, althoug

The developmental potential of nuclei can be studied by nuclear transplantation. Although amphibian blastula nuclei and other early embryonic nuclei are totipotent, to our knowledge no nucleus from an adult cell has ever been shown to be totipotent by this procedure. Transfer of Lucké renal carcinoma nuclei into enucleated eggs results in prefeeding swimming tadpoles. Inasmuch as these tadpoles die, rescue of this pluripotential tissue was attempted by grafting fragments of triploid tumor nuclear transplant tadpoles to the tails of normal diploid Rana pipiens hosts. Grafts of tumor nuclear tra

The Transplant Service at the University of Minnesota Hospitals has performed over 2,000 kidney transplants. Fourteen of these patients have developed cardiac conditions necessitating surgical intervention at intervals of 9 to 144 months (mean 67 months) following their transplantation. These individuals had a mean age of 42 years, and five (36%) were diabetic. All patients had functioning renal allografts with preoperative serum creatinine levels ranging from 1.0 to 1.8 mg/100 ml (mean 1.4 mg/100 ml). Ten patients underwent aorta-coronary saphenous vein bypass grafting. One patient underwent

Thirty-one renal allografts placed in 25 recipients with renal failure from biopsy-documented focal segmental glomerulosclerosis (FSGS) were reviewed. These represent all of the cases with this renal histology transplanted over 13 years. Recurrence of the lesion was demonstrated histologically in five recipients. A nephrotic syndrome occurred in all five patients and failure of the graft in two. Of 20 recipients who did not show a nephrotic syndrome, allograft histology in 12 did not show FSGS in any. From these data and a review of the literature, the risks of transplantation in patients with

In 18 patients who received renal allografts, tubulo-interstitial lesions were studied, according to the types of rejection or the possible recurrence of primary disease. Immunofluorescent (IF) investigation revealed tubular basement membrane (TBM) deposits of IgG and/or C3 and sometimes other components of Complement. The pattern of these deposits was: "linear" in 3 cases, "granular" in 9 cases and "atypical" with linear and granular segments in 6 cases. The IF study of own kidneys of 3 patients, before the transplantation, revealed no TBM deposits. TBM deposits were accompanied by arterial w

The tympanic membrane allograft myringoplasty is used for the radical treatment of chronic middle ear disease and the repair of tympanic perforations which are sequellae of the chronic middle ear conditions. In the first part, basic principles, tissue bank and surgical procedures, are explained in detail. The second part shows that this technique ensures not only total eradication of the pathology, but is favoured by the specific morpho-, physio- and bio-compatibilities of the allograft.

The indications, complications and results of 54 allograft nephrectomies undertaken in 44 patients have been reviewed. There were 7 deaths following 39 nephrectomies done within 6 months of transplantation and complications occurred in a further 14 instances. Many of these resulted from pre-existing infection. When nephrectomy was carried out 6 months after transplantation there were no deaths and morbidity was less. If graft failure is diagnosed within 6 months of transplantation immediate nephrectomy is recommended to prevent future complications. When graft failure occurs later it is sugges

Bone allografts facilitate bone reconstruction in orthopedic surgery. Appropriate donor selection and processing of grafts minimize the risk of disease transfer. After implantation, bone allografts become necrotic but are incorporated and remodeled in the recipient bone with good regenerative capacity. We use bone allografts in about 3% of all elective orthopedic operations in our hospital. We use bone allografts regularly in acetabuloplasty in cases of severe residual hip dysplasia. These transplants simplify and shorten the operation and improve the results in our hands. Bone allografts are

Tympano-ossicular allografts are human transplants of the tympanic membrane alone or with the ossicles included. The authors review its use in their department since the introduction of the technique 40 years ago by J. Marquet. The advantages of the technique (near normal anatomical and physiological reconstruction) are discussed, as well as its disadvantages (time consuming harvesting, risk of disease transmission). Some technical modifications for specific indications have been adapted in our department and are described in detail. The authors believe that the tympano-ossicular allograft is

We performed limb allografts in three inbred rat strains immunosuppressed with cyclosporine. As controls, 10 autografts and 10 isografts exhibited an excellent result. In minor-mismatched allografts (Lewis to Fischer, n = 45), with the use of cyclosporine, the grafted limbs survived and the articular cartilage retained normal architecture and cell viability 52 weeks after grafting, but without cyclosporine treatment severe degeneration and destruction of articular cartilage were shown by 16 weeks after operation. In major-mismatched allografts (Brown Norway to Fischer, n = 35), the articular c

BACKGROUND: Allograft tendons are perceived to have a high ACL graft failure rate in primary anterior cruciate ligament (ACL) reconstruction (ACLR). Historical series may be biased by graft processing methods that degrade the biomechanical properties of donor tendons such as irradiation. Supercritical carbon dioxide (SCCO2) is a validated method of terminally sterilizing biomaterials at physiological temperatures without irradiation, but in vivo use of SCCO2-processed tendon allografts for primary ACLR has not been reported to date.
HYPOTHESIS: ACLR with SCCO2 allografts would result in accep

Background Surgical options for patients with recurrent wrist pain after salvage procedures, such as proximal row carpectomy (PRC), are limited and primarily result in total wrist arthrodesis (TWA). Osteochondral allografting (OCA) offers an additional option to address refractory wrist pain due to arthritis while preserving some range of motion. Case Description A 65-year-old, right hand dominant male developed chronic wrist pain, secondary to untreated scapholunate and triangular fibrocartilage complex ligament tears. The patient underwent a PRC, radial styloidectomy, and partial wrist den

BACKGROUND: Autologous saphenous vein is the preferred conduit for below-the-knee bypasses in patients with critical limb-threatening ischemia. Alternative graft must be considered for patients without (autologous saphenous vein). The aim of this article is to evaluate the mid-term performance of arterial allograft (AA) and venous allograft (VA) used as alternative conduits.
METHODS: This retrospective study included patients with critical limb-threatening ischemia, with or without a history of homolateral femoropopliteal bypass, and no autologous veins were available who underwent infraingui
