Morphine patient-controlled analgesia is superior to meperidine patient-controlled analgesia for postoperative pain.

The choice between morphine and meperidine for postoperative pain is usually based on the preference of the prescriber, as few objective comparative data are available. This blind, randomized study compared the efficacy and side effects of morphine and meperidine administered by patient-controlled analgesia (PCA) for postoperative pain. One hundred two consenting patients scheduled for major abdominal surgery were randomly assigned to receive PCA with morphine (0.75, 1.0, or 1.5 mg bolus dose size) or meperidine (9, 12, or 18 mg) for pain control. Postoperative assessments included pain at res

Remifentanil patient-controlled analgesia for labour analgesia: guidance from the obstetric anaesthetists' association: Recommendations for well tolerated provision of remifentanil PCA for labour analgesia.

BACKGROUND: Remifentanil patient-controlled analgesia (PCA) has emerged as an important option for labour analgesia, particularly for women who are unable to receive neuraxial analgesia. Concerns about maternal safety and staffing requirements suggest that in many maternity units, it is only infrequently used, leading to variation in practice. In 2023, the UK National Institute for Health and Care Excellence recommended that all labouring women be informed about the full range of analgesia options, including remifentanil PCA, highlighting the need for a standardised management approach. Application of dezocine patient-controlled epidural analgesia in postoperative analgesia in patients with total myomectomy.

BACKGROUND: Uterine fibroids are common benign gynecological conditions. Patients who experience excessive menstruation, anemia, and pressure symptoms should be administered medication, and severe cases require a total hysterectomy. This procedure is invasive and causes severe postoperative pain, which can affect the patient's postoperative sleep quality and, thus, the recovery process. AIM: To evaluate use of dezocine in patient-controlled epidural analgesia (PCEA) for postoperative pain management in patients undergoing total myomectomy. METHODS: We selected 100 patients undergoing total a

Single-dose intrathecal analgesia to control labour pain: is it a useful alternative to epidural analgesia?

OBJECTIVE: To examine the safety and efficacy of single-dose spinal analgesia (intrathecal narcotics [ITN]) during labour. QUALITY OF EVIDENCE: MEDLINE was searched and the references of 2 systematic reviews and a meta-analysis were reviewed to find articles on obstetric analgesia and pain measurement. The 33 articles selected included 14 studies, 1 meta-analysis, and 2 systematic reviews, all providing level I evidence. MAIN MESSAGE: The literature supports use of ITN as a safe and effective alternative to epidural anesthesia. The recent decrease in rates of episiotomies and use of forceps

Sedation, Analgesia, and Neuromuscular Blockade.

Optimal sedation and analgesia in critical illness require a structured, patient-centered approach grounded in an understanding of how critical illness alters pharmacologic needs. Analgesia-first strategies and targeted light sedation with short-acting agents enhance comfort, interaction, and ventilator synchrony while reducing oversedation and delirium. Frequent clinical reassessment of sedation level using validated clinical scales is needed, which may be complemented by objective tools such as processed electroencephalography, nociception monitoring, respiratory drive metrics, and neuromusc

Neuraxial techniques of labour analgesia.

In recent years, many neuraxial techniques have been introduced to initiate and maintain labour analgesia, with low-dose mixtures of local anaesthetics and opioids, which have improved the quality of analgesia and made it safer for both mother and neonate. An independent search of the databases of PubMed, Medline, and Cochrane controlled trial data was conducted by two researchers, and randomized controlled trials that compared different methods of neuraxial analgesia and the different techniques of maintaining labor analgesia were retrieved and analyzed. The advantages, disadvantages, and ind

Multimodal analgesia and regional anaesthesia.

Multimodal analgesia provides quality analgesia, with fewer side effects due to the use of combined analgesics or analgesic techniques. Regional anaesthesia plays a fundamental role in achieving this goal. The different techniques of regional anaesthesia that include both peripheral and central blocks in either a single dose or in continuous infusion help to modulate the nociceptive stimuli that access the central level. The emergence of the ultrasound as an effective system to perform regional anaesthesia techniques has allowed the development of new regional anaesthesia techniques that forme

Categorising methadone: Addiction and analgesia.

While methadone was first developed as an analgesic, and used for this purpose before it was adopted as a therapy for drug dependence, it is this latter use which has saturated its identity. Most of the literature and commentary on methadone discusses it in the context of methadone maintenance therapy (MMT). But one of the effects of the liberalization of opiate prescription for chronic pain which took place in the 1990s was the re-emergence of methadone as a painkiller. This article examines the relationship between methadone the painkiller and methadone the addiction treatment as it is const

Postoperative analgesia in elderly patients.

Elderly people represent the fastest-growing segment of our society and undergo surgery more frequently than other age groups. Effective postoperative analgesia is essential in these patients because inadequate pain control after surgery is associated with adverse outcomes in elderly patients. However, management of postoperative pain in older patients may be complicated by a number of factors, including a higher risk of age- and disease-related changes in physiology and disease-drug and drug-drug interactions. Physiological changes related to aging need to be carefully considered because agin

[Intravenous remifentanyl for labor analgesia].

BACKGROUND: Intravenous remifentanil may be the preferred analgesic when regional techniques are contraindicated. OBJECTIVE: To perform a systematic review on the use of remifentanil for analgesia in labor. METHODS: We searched MEDLINE (January 1995-August 2007) for studies on obstetric analgesia with remifentanil. RESULTS: We found 32 references representing the use of remifentanil in 257 women in labor. In most cases, patients reported relief of pain and a high level of satisfaction, with no severe side effects in mothers or neonates. When compared with meperidine and nitrous oxide in cli

Thoracic paravertebral block (PVB) analgesia.

Paravertebral blockade is a safe and effective technique for intraoperative and postoperative management of acute surgical pain. The block may be performed as a single injection or a catheter may be inserted to allow for the continuous or bolus administration of a long acting local anaesthetic. This mode of analgesia is not considered to be a stand alone technique (in contrast to epidural analgesia) and most patients will require additional analgesia (usually parenteral patient controlled analgesia). Compared to epidural analgesia, the paravertebral administration of analgesia is considered to

[Labor pain and obstetric analgesia].

GOAL AND METHODS: Labor pain is of major concern since most parturients experience significant pain of extremely severe intensity for many. The purpose of this review was to provide an overview of the mechanisms and pathways of labor pain (including new insights on integration of the nociceptive signal) and to emphasize the need of effective labor pain relief. RESULTS: Labor pain can have deleterious effects on the mother, on the fetus and on labor outcome itself. Among the current methods of obstetric analgesia, regional analgesia (the most widespread technique being epidural analgesia) offe

Epidural analgesia in gastrointestinal surgery.

BACKGROUND: The ideal perioperative analgesia should provide effective pain relief, avoid the detrimental effects of the stress response, be simple to administer without the need for intensive monitoring, and have a low risk of complications. METHODS: This review defines the physiological effects of epidural analgesia and assesses whether the available evidence supports its preferential use in gastrointestinal surgery. All papers studied were identified from a Medline search or selected by cross-referencing. RESULTS: Epidural analgesia is associated with a shorter duration of postoperative i

[Narcolepsy and epidural obstetrical analgesia ].

We report the case of a parturient with a history of narcolepsy that delivered under epidural anaesthesia without any complication. Patients with narcolepsy might be at risk of delayed recovery or of apneic episodes after general anaesthesia while narcolepsy-cataplexy fits might be more frequent during regional anaesthesia. Avoiding sedative premedication and continuing medical therapy on the day of surgery are recommended to avoid these complications.

Paediatric caudal and epidural analgesia.

PURPOSE OF REVIEW: Epidural blocks (caudal, lumbar or thoracic) are of common use in paediatric patients for special procedures such as open fundoplication as well as for postoperative analgesia. However, because neonates and infants have lower metabolic capacities and specific anatomy as compared to adults, the dose of local anaesthetics and the way of their administration need careful attention. RECENT FINDINGS: Recent pharmacokinetic studies have more precisely identified the differences in pharmacokinetics between age groups, and between local anaesthetic agents. In that respect, ropivaca

Epidural analgesia-anaesthesia in obstetrics.

Epidural analgesia is the most effective and innocuous technique for obstetrics. Pain relief is its main indication but maternal diseases that might be decompensated by labour and delivery are also accepted indications. Low doses of long-acting local anaesthetics alone or in combination with low doses of fentanyl or sufentanil provide good quality analgesia and are safe for mother and fetus. Test doses in parturients lack sufficient specificity and sensitivity for detecting inadvertent intravascular injection, and subarachnoid migration of the catheter is possible at any time during the proced
