Electrophysiologic neuromonitoring during repair of the thoracoabdominal aorta by anesthesiologists.

BACKGROUND: Surgical repair of the thoracoabdominal aorta is associated with risk of spinal ischemia. Electrophysiologic neuromonitoring reduces this risk, but is usually performed by neurophysiologists not always available. In this study repair of the thoracoabdominal aorta monitored by anesthesiologists has been investigated. METHODS: Somatosensory and transcranial electrical motor evoked potentials were monitored in 20 patients. A lumbar intraspinal fluid drainage was inserted. Dacron graft replacement of the aorta was performed by sequentially clamping during partial cardiopulmonary bypas

Chronic pain management: American Society of Anesthesiologists Closed Claims Project.

BACKGROUND: The practice of chronic pain management has grown steadily in recent years. The purpose of this study was to identify and describe issues and trends in liability related to chronic pain management by anesthesiologists. METHODS: Data from 5,475 claims in the American Society of Anesthesiologists Closed Claims Project database between 1970 and 1999 were reviewed to compare liability related to chronic pain management with that related to surgical and obstetric (surgical/obstetric) anesthesia. Acute pain management claims were excluded from analysis. Outcomes and liability characteri

The evolving concepts in visceral obesity: What anesthesiologists need to know.

Obesity, an ever-evolving global epidemic, continues to offer some of the most controversial and at times, polarizing debates in the modern-day medicine; whether it is for the formers' ethnic intricacies, or for that matter, its potential outcome associations. Given the far-reaching implications of obesity however, the recent developments in characterization of the same, are of special interest to an anesthesiologist caring for this predilected cohort of patients. The expert propositions, in this regard, outline the much-required need to think beyond the body mass index (BMI), traditionally us

The occupational risk to dental anesthesiologists of acquiring 3 bloodborne pathogens.

OBJECTIVE: To estimate the occupational risk to dental anesthesiologists of contracting 3 bloodborne pathogens: hepatitis B (HBV), hepatitis C (HCV), and human immunodeficiency virus (HIV). METHODS: Through an anonymously returned, mailed questionnaire, dental anesthesiologists in Canada and the United States provided information regarding percutaneous and mucocutaneous contacts with contaminated fluid during the treatment of patients under deep sedation and general anesthesia as well as other general practice information. A mathematical model was applied to determine the occupational risk.

Production pressure in the work environment. California anesthesiologists' attitudes and experiences.

BACKGROUND: Pressure to put efficiency, output, or continued production ahead of safety has caused catastrophic accidents in various industries. The authors assessed the attitudes and experiences of anesthesiologists concerning production pressure. METHODS: A random, repeated-mailing survey was conducted among 647 members of the American Society of Anesthesiologists residing in California. Questions were asked about attitudes toward production pressure and other patient safety issues, frequency of occurrence of various operating room events, encounters with situations involving unsafe actions

Psychoactive substance use among American anesthesiologists: a 30-year retrospective study.

The purpose of this study was to assess the cumulative incidence of substance use among anesthesiologists during training and practice, the effect of stress on drug use, and deterrent efficacy of institutional prevention programmes. The 260 anesthesiologists who had trained at the Medical College of Wisconsin between 1958-1988 were surveyed by mail regarding psychoactive substance use. Analysis of 183 responses focused on demographic and psychosocial factors. Substances used most frequently included: alcohol (91.6%), marijuana (30.8%) and cocaine (9.4%). Perioperative POCUS: Technical Considerations and Applications for the Pediatric Anesthesiologist.

Background: Point-of-care ultrasound (POCUS) use continues to expand across specialties caring for pediatric patients in the perioperative period, offering noninvasive, diagnostic information that complements clinical assessment in real time. Although many POCUS applications were developed and validated in adults, pediatric POCUS requires unique considerations. This review examines the current evidence of diagnostic POCUS within pediatric perioperative care. Methods: A structured search of PubMed was performed for literature published through December 2025, using terms related to point-of-care

An Anesthesiologist's Moral Distress With Organ Procurement: A Narrative Review.

Organ procurement procedures have evolved significantly over the past decade, resulting in new moral uncertainties. The dead donor rule (DDR), a foundational principle, mandates that organ procurement occurs only after death is confirmed. However, developments such as donation after circulatory death (DCD) and normothermic regional perfusion (NRP) have raised ethical issues, creating moral distress and moral injury among anesthesiologists. This review highlights the recent changes in the organ procurement processes and the potential impact on anesthesiologists and also discusses the strategies

What an anesthesiologist should know about pediatric arrhythmias.

Identifying and treating pediatric arrhythmias is essential for pediatric anesthesiologists. Pediatric patients can present with narrow or wide complex tachycardias, though the former is more common. Patients with inherited channelopathies or cardiomyopathies are at increased risk. Since most pediatric patients present for anesthesia without a baseline electrocardiogram, the first identification of an arrhythmia may occur under general anesthesia. Supraventricular tachycardia, the most common pediatric tachyarrhythmia, represents a broad category of predominately narrow complex tachycardias. S

Perioperative implications of amyloidosis and amyloid cardiomyopathy: A review for anesthesiologists.

It is well recognized that amyloid protein can infiltrate many regions of the body. This can include the peripheral nerves, the liver, kidney, spleen, the gastrointestinal tract, and most importantly the myocardium. The amyloid proteins that cause cardiomyopathy may come from genetically altered liver genes (transthyretin amyloid, ATTR) or from the bone marrow with malignant plasma cells (light chain amyloid, AL) generating the aberrant protein. These two types of amyloidosis cause significant damaging effects on both the myocardial cells as well as the conduction system of the heart. The resu

Common street drug names for the anesthesiologist and pain physician.

People who use or sell drugs develop their own in-group terms and language, much like any other group of people with a common experience. Slang terms are derived from a wide variety of sources. These might include the physical appearance and/or type of drug, the place where it originates, the effect it has on users, or how it is packaged for sale. To assist and educate the clinical practitioner who may deal with this nomenclature, we have compiled a list of some of the most common street names and some explanations (when known) of their origins.

What does the anesthesiologist need to know about monkeypox?

PURPOSE: Monkeypox (or "mpox" as preferred by the World Health Organization) is an emerging infectious disease with sustained global transmission occurring outside of West Africa and the Democratic Republic of Congo. The recent 2022 mpox outbreak has involved widespread atypical presentations. Infected patients requiring surgery can increase the exposure of health care professionals and other patients to the virus. As it is a relatively new infectious disease internationally, there is less familiarity in managing this risk, especially in the surgical and anesthesia setting. This paper aims to

Pediatric lower respiratory tract infection: Considerations for the anesthesiologist.

Neonatal and childhood infectious diseases continue to be a global health problem. Acute respiratory tract infections are typically classified as upper respiratory tract infection and lower respiratory tract infections. The most common lower respiratory infections in childhood are pneumonia and bronchiolitis. Vaccination against measles, diphtheria, pertussis, Haemophilus influenzae, pneumococcus, and influenza resulted in a significant reduction in the incidence of acute respiratory tract infection globally. Though the global burden of the disease has decreased, the mortality rates still are

Maternal safety: recent advances and implications for the obstetric anesthesiologist.

PURPOSE OF REVIEW: Recognition of the increasing maternal mortality rate in the United States has been accompanied by intense efforts to improve maternal safety. This article reviews recent advances in maternal safety, highlighting those of particular relevance to anesthesiologists. RECENT FINDINGS: Cardiovascular and other chronic medical conditions contribute to an increasing number of maternal deaths. Anesthetic complications associated with general anesthesia are decreasing, but complications associated with neuraxial techniques persist. Obstetric early warning systems are evolving and ho

The Role of the Anesthesiologist in Preventing Severe Maternal Morbidity and Mortality.

Anesthesiologists are responsible for the safe and effective provision of analgesia for labor and anesthesia for cesarean delivery and other obstetric procedures. In addition, obstetric anesthesiologists often have a unique role as the intensivists of the obstetric suite. The anesthesiologist is frequently the clinician with the greatest experience in the acute bedside management of a hemodynamically unstable patient and expertise in life-saving interventions. This review will discuss (1) risks associated with neuraxial and general anesthesia for labor and delivery, and (2) clinical scenarios
