Non-anesthetic Effects of Anesthetics and Organ Protection.

Ischaemia reperfusion (I/R) injury is a physiological phenomenon whereby hypoxic tissue damage can be perpetuated by tissue reperfusion; this can occur in the setting of pathology or as a surgical complication. Naturally, tissues sensitive to hypoxic episodes such as the brain, heart, kidney, liver and lung tissue are most often affected. Current treatments for I/R injury focus of limiting the pathological response to reperfusion through ischemic preconditioning (IPC) and medications that mimic the IPC response. Anesthetic preconditioning (APC) and anesthetic postconditioning (APoC) can produc

Nephrotoxicity and Modern Volatile Anesthetics: A Narrative Review.

Volatile anesthetics, while increasingly utilized in intensive care medicine, are associated with significant renal adverse effects. A critical safety concern-particularly with sevoflurane-involves its potential impact on renal function. Pathophysiologically, inorganic fluoride levels exceeding 50 µmol/L are recognized as a threshold for nephrogenic diabetes insipidus, a condition generally considered reversible. Additionally, the sevoflurane degradation product "compound A" has been implicated in direct renal tubular and glomerular toxicity. Specifically, exposure has been correlated with glo

How obesity affects the disposition of intravenous anesthetics.

PURPOSE OF REVIEW: Understanding the changes in drug disposition of intravenous anesthetics in patients with obesity and administering appropriate doses are critical to avoid intraoperative awareness with recall because of underdosing and over-sedation and delayed emergence due to overdosing. Pharmacokinetic simulation or target-controlled infusion (TCI) using models that have been adapted to patients with obesity are necessary to select appropriate dosing regimens. This review aimed to describe the pharmacokinetic concepts underpinning the use of intravenous anesthetics, including propofol, r

The Effects of General Anesthetics on Synaptic Transmission.

General anesthetics are a class of drugs that target the central nervous system and are widely used for various medical procedures. General anesthetics produce many behavioral changes required for clinical intervention, including amnesia, hypnosis, analgesia, and immobility; while they may also induce side effects like respiration and cardiovascular depressions. Understanding the mechanism of general anesthesia is essential for the development of selective general anesthetics which can preserve wanted pharmacological actions and exclude the side effects and underlying neural toxicities. Local anesthetics for brain tumor resection: current perspectives.

This review summarizes the added value of local anesthetics in patients undergoing craniotomy for brain tumor resection, which is a procedure that is carried out frequently in neurosurgical practice. The procedure can be carried out under general anesthesia, sedation with local anesthesia or under local anesthesia only. Literature shows a large variation in the postoperative pain intensity ranging from no postoperative analgesia requirement in two-thirds of the patients up to a rate of 96% of the patients suffering from severe postoperative pain. The only identified causative factor predicting

Can scleroderma be induced by anesthetics? Case report.

Occupational exposure to solvents is consistently associated with the risk of systemic sclerosis. We pose the question of whether the occupational exposure to anesthetics may favor the occurrence of this disease. This paper describes an incidence of systemic sclerosis in the case of an anesthesiologist who had been occupationally exposed to high doses of anesthetic in the operating room. Anecdotal cases in the literature give limited support to the causal association of the occupational exposure to anesthetics and systemic sclerosis. The extremely limited number of workers exposed makes it nec

Pain management via local anesthetics and responsive hydrogels.

Acute and chronic pain control is a significant clinical challenge that has been largely unmet. Local anesthetics are widely used for the control of post-operative pain and in the therapy of acute and chronic pain. While a variety of approaches are currently used to prolong the duration of action of local anesthetics, an optimal strategy to achieve neural blockage for several hours to days with minimal toxicity has yet to be identified. Several drug delivery systems such as liposomes, microparticles and nanoparticles have been investigated as local anesthetic delivery vehicles to achieve prolo

[Effects of anesthetics on the higher brain function].

Number of surgeries for the elderly is increasing year by year. Postoperative cognitive dysfunction, POCD, and delirium are typical failure's of higher brain function after surgery. The mechanism of POCD and delirium has been suggested to be associated with inflammation, but its details are unknown. Alzheimer's disease leads to derangement in cognitive function. The number of patients with Alzheimer's disease is expected to increase rapidly. There is a possibility that inhalation anesthesia exacerbates the pathology of patients with impaired higher brain function such as Alzheimer's disease. O

Additives to local anesthetics for peripheral nerve blockade.

Many additives to local anesthetics to prolong the duration of analgesia for peripheral nerve blocks have been studied. In this review, the authors focus on the more commonly described additives, including epinephrine, clonidine, dexmedetomidine, buprenorphine, dexamethasone, tramadol, sodium bicarbonate, and midazolam. While the primary focus of this review is the effect of the additive on the duration of analgesia, neurotoxicity and other safety concerns are also discussed.

Inhibition of learning and memory by general anesthetics.

PURPOSE: Today's general anesthetics were developed empirically according to their ability to produce memory blockade, analgesia, immobility, and unconsciousness. Thus, a major outstanding question remains: How do anesthetics produce their desirable behavioural end points at the molecular level? Understanding the mechanisms underlying memory blockade is of particular importance, because some patients experience the unexpected recall of events during anesthesia while others experience persistent memory deficits in the postoperative period. This review provides a brief summary of the acute memor

Effects of anesthetics on systemic hemodynamics in mice.

The aim of this study was to compare the systemic hemodynamic effects of four commonly used anesthetic regimens in mice that were chronically instrumented for direct and continuous measurements of cardiac output (CO). Mice (CD-1, Swiss, and C57BL6 strains) were instrumented with a transit-time flow probe placed around the ascending aorta for CO measurement. An arterial catheter was inserted into the aorta 4 or 5 days later for blood pressure measurements. After full recovery, hemodynamic parameters including stroke volume, heart rate, CO, mean arterial pressure (MAP), and total peripheral resi

Effects of local anesthetics on pregnant uterine muscles.

Obstetric analgesia and anesthesia should provide optimal pain relief for the parturient with minimal risks. The local anesthetic agents are most commonly used for obstetric analgesia and anesthesia. We investigated the effect of local anesthetic agents: prilocaine, bupivacaine, ultracaine on myometrium in pregnant rats. In our study, we evaluated the effects of three local anesthetics at cumulative concentrations on contractions of myometrium isolated from pregnant rats. The following characteristics of the contractions were analyzed after the addition of drugs: frequency, mean duration, ampl

Intravenous anesthetics differentially modulate ligand-gated ion channels.

BACKGROUND: Heteromeric neuronal nicotinic acetylcholine receptors (nAChRs) are potently inhibited by volatile anesthetics, but it is not known whether they are affected by intravenous anesthetics. Ketamine potentiates gamma-aminobutyric acid type A (GABAA) receptors at high concentrations, but it is unknown whether there is potentiation at clinically relevant concentrations. Information about the effects of intravenous anesthetics with different behavioral profiles on specific ligand-gated ion channels may lead to hypotheses as to which ion channel effect produces a specific anesthetic behavi

Behavioral effects of volatile anesthetics in Caenorhabditis elegans.

BACKGROUND: The nematode Caenorhabditis elegans offers many advantages as a model organism for studying volatile anesthetic actions. It has a simple, well-understood nervous system; it allows the researcher to do forward genetics; and its genome will soon be completely sequenced. C. elegans is immobilized by volatile anesthetics only at high concentrations and with an unusually slow time course. Here other behavioral dysfunctions are considered as anesthetic endpoints in C. elegans. METHODS: The potency of halothane for disrupting eight different behaviors was determined by logistic regressio

Synthesis of the fluorine-18 labeled inhalation anesthetics.

Fourteen compounds (fluoroalkanes and fluoroethers), including the two most utilized inhalation anesthetics Isoflurane (CF3CHClOCF2H) and Halothane (CF3CHBrCl), have been labeled with fluorine-18 via a facile 18F-for-19F exchange reaction. The compounds include ten inhalation anesthetics which span a ten-fold range in potency and four structurally related non-anesthetics. All the compounds possess a trifluoromethyl group (CF3) adjoining a carbon atom with an acidic alpha-hydrogen and at least one halogen or a strong electron withdrawing group (X), [CF3CHXR]. We postulate the isotopic fluoride

Mechanisms of chronotropic effects of volatile inhalation anesthetics.

The chronotropic effects of all currently available volatile anesthetics were investigated in isolated rat atrial preparations. Anesthetic ethers, diethyl ether, methoxyflurane, and enflurane elicited a dose-dependent positive chronotropic effect. Fluroxene produced a slight depression at low concentrations. The halogenated hydrocarbon anesthetics, halothane, chloroform, and trichloroethylene, did not show a uniform pattern. Halothane's effect was small and biphasic. Chloroform caused a dose-dependent decrease in heart rate, and trichloroethylene caused a marked positive chronotropic effect. T
