Revisiting the risk for aneuploidy in fetuses with isolated pyelectasis.

OBJECTIVE: Previous studies are contradictory with regard to the association of isolated pyelectasis and aneuploidy. Our objective was to test the hypothesis that isolated pyelectasis is associated with aneuploidy and calculate likelihood ratios using a large ultrasound database. METHODS: A retrospective cohort study of pregnancies presenting to our prenatal ultrasound unit at 16 to 22 weeks was conducted. Pyelectasis was defined as fetal renal pelvis> 4 mm in the anterior-posterior dimension. Fetuses with co-existing major structural anomalies or markers of aneuploidy were excluded. Associat

Screening for fetal aneuploidies at 11 to 13 weeks.

Effective screening for major aneuploidies can be provided in the first trimester of pregnancy. Screening by a combination of fetal nuchal translucency and maternal serum free-β-human chorionic gonadotrophin and pregnancy-associated plasma protein-A can identify about 90% of fetuses with trisomy 21 and other major aneuploidies for a false-positive rate of 5%. Improvement in the performance of first-trimester screening can be achieved by firstly, inclusion in the ultrasound examination assessment of the nasal bone and flow in the ductus venosus, hepatic artery and across the tricuspid valve, an

The influence of sex chromosome aneuploidy on brain asymmetry.

The cognitive deficits present in individuals with sex chromosome aneuploidies suggest that hemispheric differentiation of function is determined by an X-Y homologous gene [Crow (1993); Lancet 342:594-598]. In particular, females with Turner's syndrome (TS) who have only one X-chromosome exhibit deficits of spatial ability whereas males with Klinefelter's syndrome (KS) who possess a supernumerary X-chromosome are delayed in acquiring words. Since spatial and verbal abilities are generally associated with right and left hemispheric function, such deficits may relate to anomalies of cerebral asy

Klinefelter syndrome and other sex chromosomal aneuploidies.

The term Klinefelter syndrome (KS) describes a group of chromosomal disorder in which there is at least one extra X chromosome to a normal male karyotype, 46,XY. XXY aneuploidy is the most common disorder of sex chromosomes in humans, with prevalence of one in 500 males. Other sex chromosomal aneuploidies have also been described, although they are much less frequent, with 48,XXYY and 48,XXXY being present in 1 per 17,000 to 1 per 50,000 male births. The incidence of 49,XXXXY is 1 per 85,000 to 100,000 male births. In addition, 46,XX males also exist and it is caused by translocation of Y mate

The impact of gestational age on the sonographic detection of aneuploidy.

OBJECTIVE: The purpose of this study was to identify the influence of gestational age on the detection of sonographic markers of aneuploidy. STUDY DESIGN: Using a retrospective chart review, cases of trisomy 13, 18, and 21 with fetal ultrasounds between 14 1/7 and 36 4/7 weeks' gestation were identified. Sonographic markers were subdivided into major abnormalities and "soft" sonographic markers: nuchal thickening > or = 5 mm, humerus or femur length <10th percentile, intracardiac echogenic focus, echogenic bowel, pyelectasis > or = 4 mm, and choroid plexus cysts. Markers were stratified by ka

The value of minor ultrasound markers for fetal aneuploidy.

As a result of improvements in ultrasound image quality and scanning technique, an increasing number of subtle morphological changes in fetal anatomy have been identified in the second trimester. Most of these ultrasound features were originally described as normal variants of development with no clinical significance. However, subsequent studies in high-risk populations showed that some of these variants were more prevalent in fetuses with chromosomal defects and therefore proposed as prenatal markers for the detection of aneuploidy. The implications for pregnancy management when one of these

DNA aneuploidy in invasive carcinoma of the uterine cervix.

Forty cases of squamous cell carcinoma and twenty cases of adenocarcinoma of the cervix were subjected to DNA ploidy analysis to find out the frequency of aneuploid DNA pattern in different types of invasive carcinoma of the uterine cervix and their possible correlation with the clinical stage and age of the patients. From the DNA analysis an increasing trend of DNA aneuploidy was observed from well differentiated (63.6%) to moderately differentiated (70.8%) to poorly differentiated squamous cell carcinoma (83.3%). However, 85% adenocarcinomas of endocervix showed aneuploid DNA pattern. It ind

First-trimester nuchal edema as a marker of aneuploidy.

Targeted ultrasonographic screening for nuchal fluid accumulation during the first trimester (9-13 weeks) seems to be a recommendable method for the detection of Down's syndrome and other chromosomal anomalies in pregnant women. It compares favorably with current methods of maternal serum screening performed during the second trimester. Using a 4-mm cut-off value, the detection rate of aneuploidy among our population was found to be 57.1%, with a false-positive rate of 0.7% and a positive predictive value of 72.7%. Chromosomal analysis should be considered for fetuses with nuchal translucency

Criteria to evaluate patterns of segmental and complete aneuploidies in preimplantation genetic testing for aneuploidy results suggestive of an inherited balanced translocation or inversion.

OBJECTIVE: To define criteria for determining when preimplantation genetic testing for aneuploidy (PGT-A) results are suggestive of a potential balanced chromosomal rearrangement in the egg or sperm source and warrant karyotyping. DESIGN: Performance evaluation of criteria developed to assess PGT-A results for patterns of imbalances suggestive of a balanced chromosomal rearrangement in the egg or sperm source. SETTING: A single PGT-A laboratory and multiple in vitro fertilization centers. PATIENTS: Reproductive couples who underwent routine PGT-A testing. INTERVENTIONS: Karyotyping of repr

Preimplantation genetic testing for aneuploidy: challenges in clinical practice.

Preimplantation genetic testing for aneuploidy (PGT-A) has been used widely during in vitro fertilization procedures in assisted reproductive centers throughout the world. Despite its wide use, concerns arise from the use of PGT-A technology in clinical decision-making. We address knowledge gaps in PGT-A, summarizing major challenges and current professional guidelines. First, PGT-A is a screening test and not a diagnostic test. Second, mosaicism is much higher in the blastocyst stage from PGT-A than had been recognized previously and a mosaic embryo may not accurately represent the genetic di

iPSC reprogramming-mediated aneuploidy correction in autosomal trisomy syndromes.

Trisomy 21, 18, and 13 are the major autosomal aneuploidy disorders in humans. They are mostly derived from chromosome non-disjunction in maternal meiosis, and the extra trisomic chromosome can cause several congenital malformations. Various genes on the trisomic chromosomes are intricately involved in the development of disease, and fundamental treatments have not yet been established. However, chromosome therapy has been developed to correct the extra chromosome in cultured patient cells, and it was recently reported that during reprogramming into iPSCs, fibroblasts from a Down syndrome pati

Preimplantation genetic testing for aneuploidy in uterus transplant patients.

Uterus transplantation is an emerging treatment for uterine factor infertility. In vitro fertilization with cryopreservation of embryos prior is required before a patient can be listed for transplant. Whether or not to perform universal preimplantation genetic testing for aneuploidy should be addressed by centers considering a uterus transplant program. The advantages and disadvantages of preimplantation genetic testing for aneuploidy in this unique population are presented. The available literature is reviewed to determine the utility of preimplantation genetic testing for aneuploidy in uteru

Prenatally Diagnosed Fetal Aneuploidy: Natural History and Subsequent Management.

Aims Our aim was to determine the number of cases of aneuploidy which were prenatally diagnosed in our tertiary referral unit from 2005-2015 and to analyse the subsequent outcomes of Trisomies 13, 18 and 21(T13, T18 and T21). Methods This was a retrospective observational study. We firstly determined the total number of prenatally diagnosed aneuploidies. We then analysed their subsequent outcomes including average gestation at miscarriage or IUD, mode of delivery and neonatal outcome. Results 402 cases of T13, T18 or T21 were identified of which 40% opted for expectant management. T18 was the

Mechanisms of oocyte aneuploidy associated with advanced maternal age.

It is well established that maternal age is associated with a rapid decline in the production of healthy and high-quality oocytes resulting in reduced fertility in women older than 35 years of age. In particular, chromosome segregation errors during meiotic divisions are increasingly common and lead to the production of oocytes with an incorrect number of chromosomes, a condition known as aneuploidy. When an aneuploid oocyte is fertilized by a sperm it gives rise to an aneuploid embryo that, except in rare situations, will result in a spontaneous abortion. As females advance in age, they are a

Comparative cephalometric analysis in patients with X-chromosome aneuploidy.

The influence of abnormal sex-chromosome constitution on cranial growth, shape and position of the craniofacial structures was analysed. Reduced cranial growth was found in women with Turner's syndrome (45 XO chromosome constitution) and in men with Klinefelter's syndrome (47 XXY chromosome constitution). Lack of or an extra X chromosome produced opposite effects on cranial-base flexion, jaw displacement and maxillary and mandibular inclination to the anterior cranial base. An extra X chromosome affected deviation in sagittal jaw relation while lack of an X chromosome influenced mandibular sha
