Long-Term Outcomes of Surgical Treatment by In Situ Graft Reconstruction for Infected Abdominal Aortic Aneurysm.

Objectives: The optimal surgical management for primary infected abdominal aortic aneurysm (IAAA) is controversial. Here, we report the early and long-term results of surgical treatments with the resection of the IAAA and in situ graft reconstruction with pedicled omental coverage that was performed at out hospital. Methods: Between 2010 and 2017, 27 consecutive patients (26 males, 1 female, median age 69 years) with IAAA were surgically treated with the resection of the IAAA, in situ graft reconstruction and covered with a pedicled omental flap. Perioperative and long-term outcomes were revie

Ruptured thoracic aortic aneurysm infected with listeria monocytogenes: a case report and a review of literature.

A 75-year-old male with a history of alcoholic liver cirrhosis, sigmoid colon cancer, and metastatic liver cancer was admitted to our institution with a complaint of a prickly feeling in his chest. On admission, a chest radiograph revealed a normal cardio-thoracic ratio of 47%. Echocardiography revealed pericardial effusion and blood chemical analyses revealed elevated C-reactive protein levels (14.7 mg/dL). On day 3, chest radiography revealed cardiomegaly with a cardio-thoracic ratio of 58% and protrusion of the left first arch. Contrast-enhanced chest computed tomography revealed a saccular

F-18 FDG PET/CT in the management of infected abdominal aortic aneurysm due to Salmonella.

Mycotic aneurysm is a rare and life-threatening disorder. Computed tomography (CT) is considered to be the best diagnostic imaging modality that can detect an abdominal aortic aneurysm and changes in the surrounding structures. More recently, F-18 fluorodeoxyglucose (FDG) PET would seem to hold promise for the diagnosis of focal infection and during the follow-up after antibiotic treatment. We present a case of an infected abdominal aortic aneurysm due to Salmonella enteritidis. In this case, a combination of CT and FDG PET/CT provided accurate information for the diagnosis of the infected abd

Infected ruptured popliteal artery aneurysm by Listeria monocytogenes. A case report and review of the literatures.

We describe a case of a voluminous infected aneurysm of the popliteal artery, with Listeria monocytogenes (LM) associated with rupture, in a 72-year old man. After radical resection of the aneurysm a reconstruction was not necessary, because of the sufficient blood supply, due to the pre-existent good development of collateral circulation. The patient was discharged on the 12th postoperative day with primary healing of the wound and viable leg. Adequate antibiotic treatment was continued for 4 weeks. In the following 18 months the serial clinical examinations, laboratory tests and ultrasound s

Spondylodiscitis due to transmitted mycotic aortic aneurysm or infected grafts after endovascular aortic aneurysm repair (EVAR): A retrospective single-centre experience with short-term outcomes.

PURPOSE: To report the challenging therapeutic approach and the clinical outcome of patients with pyogenic spondylodiscitis transmitted due to infected retroperitoneal regions of primary infected mycotic aortic aneurysms (MAAs) or secondary infected aortic stent grafts after endovascular aneurysm repair (EVAR). METHODS: Between 2012 and 2019, all patients suffering from spondylodiscitis based on a transmitted infection after the EVAR procedure were retrospectively identified. Patient data were analysed regarding the time between primary and secondary EVAR infection and spondylodiscitis detect

Imminent Rupture of an Infected Aortic Aneurysm Presenting as Lower Back Pain in an Elderly Patient.

An infected abdominal aortic aneurysm (IAAA) is a rare and potentially life-threatening condition that can mimic common spinal disorders, leading to delayed diagnosis. We report a case of a 63-year-old male who presented with lower back pain and left leg numbness. Initially suspected to have a spinal pathology, further investigation revealed an IAAA complicated by a psoas abscess and septic shock. Despite initial antibiotic therapy, the aneurysm ruptured, necessitating emergency surgical intervention. Unfortunately, the patient died from circulatory failure on postoperative day four. This case

Suspected infected infrarenal abdominal aortic aneurysm with contained rupture in the setting of Escherichia coli prostatitis.

Contained rupture of a mycotic abdominal aortic aneurysm (AAA) is rare and potentially fatal. While most cases involve Salmonella or Staphylococcus aureus, Escherichia coli is an uncommon cause.A man in his early 60s presented with acute E. coli prostatitis and a prostatic abscess. Abdominal ultrasound incidentally revealed a 3.8 cm infrarenal saccular AAA suspicious for contained rupture. Contrast-enhanced CT confirmed focal aortic wall disruption without active extravasation. Intravenous ceftriaxone and vancomycin were initiated, and serial imaging demonstrated stability. After infection con

Mycotic Aortic Aneurysm and Infected Aortic Graft After Intravesical Bacillus Calmette-Guérin Treatment for Bladder Cancer.

Although intravesical therapy with bacillus Calmette-Guérin (BCG) has proven its efficiency in the treatment of early-stage bladder cancer, infectious complications can occur and mycotic aneurysms represent a rare but life-threatening complication. Here, we report the case of an aortic graft infection in a patient with abdominal aortic aneurysm who received BCG instillations for the treatment of bladder cancer. Based on the current knowledge on this rare vascular complication, we discuss factors that may have contributed to its occurrence and review issues to optimize its management and early

Infected "Mycotic" Aneurysm of the Common Carotid Artery-A Differential Diagnosis to Tumor of the Neck.

Introduction: Infected "mycotic" Aneurysm (IA) of the extracranial carotid artery is a rare condition that can be fatal if mistaken for other pathology. An 83-year-old man presented with a mass on the neck initially suspected malignant. Weeks later it grew rapidly and was found to be an IA, thus requiring acute surgery. Via this case report, we discuss diagnostics and approach when diagnosing masses in relation to vessels of the neck not readily explained. Case Report: After diagnostic imaging and clinical assessment an unknown primary tumor of the neck was suspected. Fine needle aspiration wa

Explantation of infected aortic aneurysm and endograft with ascending aorta to mesenteric bypass for mesenteric ischemia.

A 65-year-old man presented with an infected perivisceral aortic aneurysm after previous treatment of an abdominal aortic aneurysm with an endograft. On presentation, he was septic and had occlusion of the celiac, superior mesenteric, inferior mesenteric, and bilateral renal arteries. He underwent a three-stage procedure: first, axillobifemoral bypass; then resection of the thoracoabdominal aorta; and finally bypass from the ascending aorta to the celiac and superior mesenteric arteries with a rifampin-soaked Gelsoft graft (Vascutek, Renfrewshire, Scotland). The abdominal pain resolved, and th

A surgical case of excision of infected aneurysm arising from anterior interosseal artery following infectious endocarditis.

Infected aneurysm (IA) of the anterior interosseal artery (AIA), the first branch of the ulnar artery, is an infrequent but serious complication of infectious endocarditis (IE). We report a successful case of excision of IA arising from AIA. In this case, the IA expanded and adhered to the ulnar artery, resulting in occlusion of the ulnar artery. Reconstruction of the ulnar artery was not needed by the preoperative evaluation and the intraoperative occlusion testing. We discuss surgical treatment of IA following IE in upper extremities.

Contemporary management of infective native aortic aneurysms.

PURPOSE OF REVIEW: Infective native aortic aneurysms (INAAs) are challenging and complex with both bacteraemia and an aneurysm prone to rupture. They are fatal unless recognized and treated promptly. The rarity of the disease makes it unfamiliar to most physicians, which might delay both diagnosis and treatment. The present review aims to outline the recently published diagnostic algorithm of INAAs, the main characteristics of the disease and present an overview of contemporary treatment strategies. RECENT FINDINGS: New terminology, definition, diagnostic algorithm and reporting standards have

Salmonella infection of a ventricular aneurysm with mural thrombus.

We describe a case of salmonella infection of a left ventricular aneurysm with a mural thrombus and review 12 cases described in the literature. This entity should be looked for in any patient with persistent or relapsing salmonella bacteremia in whom an intracardiac thrombus is demonstrated. Nuclear imaging may help in the diagnosis. A combined medical and surgical approach should be aggressively pursued because patients who do not undergo an aneurysmectomy are unlikely to survive.

Cryptogenic Salmonella-infected ruptured aortic aneurysms.

Salmonella-infected ruptured aortic aneurysms are a difficult surgical problem. Five patients with Salmonella-infected ruptured aortic aneurysms are presented. All patients were initially treated by in situ graft replacement, wide debridement of the infected aortic tissue and long-term systemic antibiotic therapy. The surgical technique of choice in acute situations, such as a rupture of the aneurysm, remains controversial. In the recent literature 26 reported cases were reviewed. Based on our clinical experience and these case reports, in situ bypass reconstruction can be justified as a surgi

Brain abscess and infections aneurysm of extravascular origin.

Acute and chronic ear infection can lead to intracranial involvement. We report on a case in which two typical cerebral complications followed a chronic otitis media. It seemed that there was no causal relationship between brain abscess and meningitis. The meningitis itself was complicated by rupture of an infectious intracranial aneurysm with fatal outcome. We have not been able to find a similar case in the literature. Clinical picture and therapeutic management are discussed.
