[Angioplasty not always the answer in stable angina].

The number of percutaneous coronary interventions (PCIs) has more than doubled in the Netherlands over the past 15 years. In part, this is due to an increase in PCI for stable angina, and it is questionable whether this is justified since recently published trials, including the Courage study, show that there is no survival benefit of PCI for stable angina. This paper discusses the treatment of stable angina, the importance of optimal medical treatment, and the indication for PCI in selected patients.

Angioplasty for renovascular hypertension in 78 children.

OBJECTIVES: To evaluate the outcome of percutaneous transluminal angioplasty (PTA) in children with renovascular hypertension (RVH) treated at a single centre over 29 years. METHODS: A retrospective study of the medical charts of all children with RVH who underwent PTA between 1984 and 2012. The primary outcome measurement was blood pressure (BP) achieved after the procedure. The BP before the procedure was compared with that at last available follow-up, 6 (range 0.6-16) years after the initial procedure. RESULTS: Seventy-eight children with median (range) age of 6.5 (0.5-17) years were stud

Carotid angioplasty and stenting in octogenarians: is it safe?

PURPOSE: Elderly patients have a higher risk of complications in carotid endarterectomy. The aim of the study was to evaluate whether carotid artery stenting (CAS) performed in octogenarians also increases the procedure related risk. METHODS: 870 patients (male 626) mean age 70.9 +/- 9.3 years underwent 930 CAS for de novo lesions (n = 851) restenoses (n = 54) post radiation (n = 14) inflammatory arteritis (n = 9) post trauma aneurysms (n = 2). Indications for treatment: symptomatic carotid stenosis > or = 70% (n = 577) or asymptomatic stenosis > or = 80%. Patients were separated into two age

Current indications for carotid angioplasty and stenting.

Carotid angioplasty and stenting has been established as a minimally invasive alternative to endarterectomy for patients with significant extracranial carotid occlusive disease. Its indications, however, continue to evolve, as more controlled data from large clinical trials are being accumulated. The purpose of this article is to review the current evidence supporting the application of carotid angioplasty and stenting in certain subsets of patients and the relative contraindications for its use.

Intracranial angioplasty and stenting in the awake patient.

BACKGROUND AND PURPOSE: Endovascular treatment for intracranial atherosclerosis is evolving, but complications remain an issue. Most interventions are performed under general anesthesia, preventing intraprocedural clinical evaluations. We describe our approach to intracranial angioplasty and stenting, using local rather than general anesthesia, and intraprocedural neurological assessment. METHODS: We prospectively collected procedural and outcome information on all patients undergoing intracranial angioplasty and stenting. Patients underwent interventions under local anesthesia with mild intr

[Balloon angioplasty of native coarctation of the aorta].

The use of balloon dilatation to treat native coarctation of the aorta is gaining acceptance among interventional pediatric cardiologists, but is still controversial. We describe our experience with this procedure in 21 children, mean age 5.6 years and mean weight 21.1 kg. Most had an additional congenital heart defect, most commonly a bicuspid aortic valve. 17 were asymptomatic, 3 had tachypnea and 1 infant had severe congestive heart failure and was ventilated. The mean systolic blood pressure was 129.7 mm Hg. Balloon dilatation was successful in 90% (19), decreasing the mean maximal systoli

The role of angioplasty in the treatment of cerebrovascular disease.

Percutaneous transluminal angioplasty (PTA), an established treatment of arterial stenosis in coronary, renal, and other peripheral sites, is being applied to the cerebrovascular territory with greater frequency. Early results suggest that PTA may be safe and efficacious in the treatment of extracranial arterial stenosis secondary to atherosclerosis and fibromuscular dysplasia. PTA is also being used with promising results in treating symptomatic intracranial arterial stenosis from atherosclerosis. This review examines PTA in the treatment of cerebrovascular disease, current indications, and r

Biomechanical and structural assessment of transluminal angioplasty.

The increasing incidence of stroke is frequently maintained by the processes narrowing precerebral and cerebral arteries. That is why the surgical treatment of cerebrovascular disorders has increasingly included percutaneous transluminal angioplasty. However, the influence of transluminal angioplasty on the biomechanical properties of arterial wall have not been assessed so far. The cylindrical segments of left and right internal carotid artery, middle cerebral artery, anterior cerebral artery, vertebral artery, basilar artery and posterior cerebral artery were taken from 40 autopsy subjects a

Restenosis, the Achilles' heel of coronary angioplasty.

Coronary angioplasty is widely performed for the management of symptomatic coronary artery disease. With improvements in technique, operator experience, and tools, more complex lesions are being treated. Unfortunately, luminal renarrowing continues to limit the long-term success of the procedure, resulting in the need for repeat revascularization in approximately 30% of patients within 6 months. As the pathophysiologic process of restenosis is better defined, various pharmacologic and mechanical interventions have been tried to attenuate the process. Some agents are antithrombotics, antiplatel

[Intracoronary stents in the treatment of angioplasty complications].

Acute or subacute occlusion of the dilated artery is the main cause of complication after percutaneous transluminal coronary angioplasty. The occlusion incidence ranges between 2 and 10% of the procedures and the mechanisms include dissection, subintimal hemorrhage, thrombosis, vasospasm and elastic recoil. Intracoronary stent is a metallic support which may seal dissection due to angioplasty, resists elastic recoil and geometric remodeling and prevents other occlusion mechanisms. Data from recent studies demonstrate the benefit of stenting acute occlusive coronary dissection in a large artery
