Immediate Angioscopic Investigation After Cerebral Artery Thrombectomy Revealed Aortogenic Cerebral Infarction.

BACKGROUND: Aortogenic embolism is one of the causes of embolic stroke of undetermined source, which can be difficult to diagnose. METHODS: We present the case of a 74-year-old male patient with a history of Bentall surgery and thoracic endovascular aortic repair who was transported to our hospital with sudden-onset right-sided hemiplegia and total aphasia. Magnetic resonance imaging revealed an acute left middle cerebral artery occlusion. Subsequently, an emergency mechanical thrombectomy was performed, through which the left middle cerebral artery was partially reperfused. We immediately in

Five-year histological evaluation of paclitaxel-eluting stent for an infrapopliteal lesion: a case report.

BACKGROUND: The long-term vascular response to paclitaxel-eluting stents (PESs) in infrapopliteal (IP) lesions remains histologically uncharacterized. CASE SUMMARY: A 78-year-old woman with chronic limb-threatening ischaemia underwent implantation of two overlapping paclitaxel-eluting stents from the tibioperoneal trunk to the peroneal artery for an ischaemic ulcer. Multimodality follow-up imaging was obtained at both 3 and 5 years, including optical frequency domain imaging (OFDI) and angioscopy performed immediately before amputation. At 3 years, OFDI and angioscopy demonstrated uncovered s

[Coronary thrombosis imaging in humans].

The identification of coronary thrombosis in humans has important prognostic and therapeutic implications. Its recognition calls for invasive techniques: angioscopy, coronarography, and endocoronary ultrasound. Angioscopy allows visualisation of the arterial surface and the detection of thrombus with great sensitivity. Its presence is exclusive to the acute coronary syndromes: 67% in unstable angina, and 75% in myocardial infarction, although it is only present in 27% of cases of stable angina. The relative complexity of its use has led to the abandonment of this technique. Coronarography allo

Does anticoagulation therapy prevent thrombosis in coronary stent grafts?

A polytetrafluoroethylene (PTFE)-covered stent was deployed for enlarging coronary aneurysms in a 73-year-old man. Optical coherence tomography (OCT) and coronary angioscopy performed 3 months later revealed naked stent struts and thin patches of red thrombi covering the PTFE membrane even under dual antiplatelet therapy (DAPT). After 5-month warfarin administration under DAPT, OCT and angioscopy showed a clear extinction of the thin patches of red thrombi on the PTFE membrane and stent struts, although most of the struts were still naked. The addition of anticoagulation therapy to DAPT may re

Impact of Angioscopic Evaluation for Femoropopliteal In-Stent Restenosis Before and After Excimer Laser Atherectomy.

In-stent restenosis (ISR) is a prevalent problem following stenting of femoropopliteal lesions. A potential novel treatment modality for ISR including excimer laser atherectomy (ELA) has become available. We performed ELA for in-stent chronic total occlusion (CTO) of femoropopliteal lesions and evaluated lesion morphology before and after ELA by angioscopy in 2 patients. The angioscopic findings clearly showed removal of in-stent thrombi after ELA. Thus, ELA may be effective for in-stent CTO of femoropopliteal lesions. This is the first report describing the direct visualization of ELA effect

Optimal Dual Antiplatelet Therapy Duration For Carotid Artery Stenting On the Basis of Angioscopic Findings.

BACKGROUND: Dual antiplatelet therapy plays a crucial role in preventing thrombosis in the perioperative period of carotid artery stenting (CAS); however, it has been associated with a risk of hemorrhagic complications. A standard regimen for the duration and combination of antiplatelet therapy is yet to be established. Angioscopy enables direct visualization of neointimal coverage and might help determine the optimal duration of dual antiplatelet therapy through the assessment of post-CAS neointimal coverage. METHODS: This prospective study enrolled 20 patients who were scheduled to undergo

Accuracy of multimodal intravascular imaging in evaluating neointimal tissue in a swine femoropopliteal artery stent model.

Stent failure, including in-stent restenosis (ISR) and occlusion, remains a major challenge in femoropopliteal arteries. Since the underlying cause of stent failure, such as fibrin-rich thrombus versus organized fibrous plaque, likely dictates the optimal treatment strategy, accurate characterization of neointimal tissue is crucial. However, pathological validation for key intravascular imaging modalities in this setting is lacking. A total of 22 femoral arteries with an in-stent restenosis model in healthy swine after stent implantation were evaluated. One month later, intravascular imaging u

Angioscopic Ulcerated Plaques in the Femoropopliteal Artery Associated with Impaired Infrapopliteal Runoff.

PURPOSE: To determine the correlation between upstream atherosclerosis in the femoropopliteal arteries, assessed using angioscopy, and impaired infrapopliteal runoff. MATERIALS AND METHODS: Thirty-one patients with peripheral arterial disease who underwent endovascular therapy and angioscopy were prospectively included. Yellow plaque color scores were semiquantitatively determined as 0, 1, 2, or 3. Irregular plaques with rough surfaces, similar to gastric ulcers, were defined as ulcerated plaques (UPs). Angioscopic data were correlated with angiographic runoff scores (ARS). RESULTS: UPs were

Novel Endovascular Therapy, AGET, for In-Stent Occlusions in Iliac and Femoropopliteal Arteries.

Catheter-directed intra-arterial thrombolysis (CDT) is useful for not only patients with acute limb ischemia but also those with chronic total occlusions (CTOs) of the lower extremity arteries. However, it is difficult to determine whether CTO lesions have significant thrombi, which can be treated by CDT, or not in a clinical setting. Angioscopy can accurately detect thrombi. We investigated the clinical impact of angioscopy guided endovascular therapy following thrombolysis (AGET) for in-stent occlusions (ISOs) in iliac or femoropopliteal arteries. We performed AGET in 7 patients with ISOs who

Angioscopic observations after percutaneous thrombectomy of thrombosed hemodialysis grafts.

PURPOSE: To use angioscopy to evaluate and compare the amount of residual thrombus and endoluminal wall damage in hemodialysis grafts after percutaneous thrombectomy procedures. MATERIALS AND METHODS: Thirty-nine thrombectomy and angioscopy procedures were performed in 35 patients. Percutaneous thrombectomy methods included eight different mechanical thrombectomy devices and the "lyse and wait" technique. Videotaped images of 33 angioscopic examinations were independently reviewed by three radiologists. Two parameters-the amount of residual thrombus and degree of endoluminal wall damage-were

Plaque characteristics and prognosis in patients with peripheral arterial disease by angioscopic analysis.

BACKGROUND: The plaques with higher grade of yellow color by angioscopy are reported to be associated with vulnerability leading to adverse outcomes in coronary artery diseases. However, no studies have been performed for peripheral artery disease (PAD). We aimed to evaluate the relationship of angioscopic findings of peripheral arteries with the long-term prognosis. METHODS: Angioscopy of iliac or femoropopliteal artery was performed before endovascular therapy in patients with PAD. The local plaque color and presence of thrombus were evaluated. Multivariable Cox regression models were used

Identifying Abnormal Ostial Morphology in Anomalous Aortic Origin of a Coronary Artery.

BACKGROUND: Anomalous origin of a coronary artery from the contralateral sinus of Valsalva is associated with exercise-induced ischemia and sudden death. That is thought to be due to aortic enlargement in patients with an elliptical ostium. We hypothesize that virtual angioscopy can identify abnormal coronary ostial morphology in these patients. METHODS: We retrospectively analyzed 55 consecutive pediatric coronary artery magnetic resonance imaging studies from January 2006 to January 2010 with the diagnosis of anomalous right (n = 20), or left (n = 7) coronary artery, or normal coronary orig

Cardiac magnetic resonance imaging characterizes stenosis, perfusion, and fibrosis preoperatively and postoperatively in children with anomalous coronary arteries.

OBJECTIVES: Using cardiac magnetic resonance imaging (MRI) with virtual angioscopy, we sought to evaluate coronary anatomy, myocardial injury, and left ventricular function in children with interarterial anomalous aortic origin of coronary artery before and after surgery. METHODS: We prospectively enrolled children 5 to 19 years old with interarterial anomalous coronary artery. Cardiac MRI was performed with respiratory-navigated steady-state free-precession 3-dimensional data set acquisition. Virtual angioscopy was used to evaluate the coronary ostia directly. Surgery consisted of the modifi

What you see is what you hear: microangioscopy-guided management of carotid-cochlear dehiscence in pulsatile tinnitus.

Angioscopy is an emerging technology in neurointervention.1-4 The Vena MicroAngioscope (MA) is a sterile, single-use, full-color, forward-viewing angioscope delivered through a guiding or distal access catheter, with or without flow arrest, and saline irrigation. The device is 160 cm in length, with a 3 Fr distal radiopaque tip, and connects to the Vena Camera Control Unit for illumination and real-time image capture (see video 1). We present a video demonstrating the second-generation MA used with the SmartGUIDE deflectable tip guidewire in treating a carotid-cochlear dehiscence causing pulsa

Role of vascular endoscopy in aortoiliac and femoropopliteal thromboendarterectomy.

Over the last few years the technique of thromboendarterectomy in the aortoiliac and femoropopliteal position has experienced a renaissance. Nearly 15 years ago thromboendarterectomy was found to have an increased early occlusion rate, especially in the femoropopliteal position (8-10%), and a high late complication of myointimal hyperplasia with subsequent stenosis. Bypass procedures were preferred to thromboendarterectomy because of these reasons. A prospective study was started in Lippstadt after good results of a retrospective study from the University of Ulm (1970-1990), in which angioscop

Intravascular imaging of serial changes of disease in saphenous vein grafts after coronary artery bypass grafting.

To clarify the structural changes of saphenous vein grafts after coronary artery bypass grafting, intravascular ultrasound and angioscopic images were obtained from 23 grafts in vivo and 5 grafts and 3 new veins in vitro; the images were compared with histologic findings. Intravascular ultrasound demonstrated a single-layered appearance at new veins and all of the angiographically normal grafts within 6 months after surgery. A triple-layered appearance that might be related to the remarkably proliferative and degenerated intima was revealed histologically at 73.3% of the normal sites of grafts

Angioscopic Evaluation of Spontaneously Ruptured Aortic Plaques.

BACKGROUND: Spontaneous ruptured aortic plaques (SRAP), which might cause atheromatous embolization, are thought to be mainly iatrogenic and have not been observed directly. OBJECTIVES: The purpose of this study was to clarify the incidence, types, and dimensions of SRAP using angioscopy. METHODS: In a cross-sectional study, 324 consecutive patients diagnosed with or suspected of having coronary artery disease were subjected to intra-aortic scans with nonobstructive angioscopy. Samples of SRAP were taken from the aorta. The dimensions of cholesterol crystals of atheromatous materials were me

In-Stent Yellow Plaque at 1 Year After Implantation Is Associated With Future Event of Very Late Stent Failure: The DESNOTE Study (Detect the Event of Very late Stent Failure From the Drug-Eluting Stent Not Well Covered by Neointima Determined by Angioscopy).

OBJECTIVES: This study examined whether coronary angioscopy-verified in-stent yellow plaque at 1 year after drug-eluting stent (DES) implantation is associated with future event of very late stent failure (VLSF). BACKGROUND: Atherosclerosis detected as yellow plaque by angioscopy has been associated with future events of acute coronary syndrome. Development of in-stent neoatherosclerosis is a probable mechanism of VLSF. METHODS: This study included 360 consecutive patients who received successful angioscopic examination at 1 year after implantation of a DES. They were clinically followed up

Non-contact ultra-widefield retinal imaging and fundus fluorescein angiography of an infant with incontinentia pigmenti without sedation in an ophthalmic office setting.

When fluorescein angioscopy or angiography is required in an infant, it is usually performed in the operating theater or neonatal unit. We report a case of an infant with incontinentia pigmenti in whom we were able to acquire angiographic information in an office setting by using an ultra-widefield non-contact system with oral fluorescein.
