Ankle and foot injuries: analysis of MDCT findings.

OBJECTIVE: The purpose of our study was to assess MDCT findings and the advantages of MDCT compared with radiography in patients referred to a level 1 trauma center for diagnostic evaluation of acute ankle and foot trauma. MATERIALS AND METHODS: During a period of 37 months, 388 patients underwent MDCT of the ankle and foot due to an acute trauma. Imaging studies (MDCT and radiography) were retrospectively reevaluated with respect to fracture location, fracture type, and mechanism of injury, and findings from the primary radiographs of the ankle and foot were compared with MDCT findings. Ligamentous, chondral, and osteochondral ankle injuries in athletes.

There is a broad spectrum of intra- and extra-articular pathologies that may result in chronic pain and loss of function after an ankle sprain in an athlete. Multiple concurrent intra- and extra-articular pathologies are common. The complex anatomy in the ankle and foot may result in overlap of the clinical features of these various conditions. Contemporary imaging modalities facilitate accurate diagnosis and appropriate and timely management. This review focuses on the particular mechanisms of injury in individual sports, the imaging manifestations of ankle ligament injuries, posttraumatic an

Mechanisms of foot and ankle injuries in soccer.

BACKGROUND: Although player-to-player contact is a risk factor in the majority of soccer injuries, the mechanisms leading to these injuries have not been analyzed. PURPOSE: To assess the relationships between foot/ankle injuries and foul play and tackle type, and to identify the position of the foot and ankle at the time of injury. STUDY DESIGN: Prospective cohort study. METHODS: Team physicians prospectively recorded each injury in four world soccer competitions, and the videotaped incident leading to the injury was retrospectively analyzed. RESULTS: Of 76 foot and ankle injuries (52 cont

Traumatic foot and ankle injuries in the athlete.

Unrecognized and inappropriately managed foot and ankle injuries can lead to significant long-term functional disability. As many of the above injuries are frequently diagnosed late or not even considered because of the benign appearance of the foot or radiograph, it behooves the sports medicine physician to carefully assess the foot and obtain appropriate radiographs, including stress views as needed. Although quick return to athletic play is among the goals of treatment for the elite athlete, this must be tempered by the fact that certain injuries require significantly greater time for compl

Ligamentous injuries about the ankle and subtalar joints.

Ligamentous injuries at the ankle and subtalar joint range from simple sprains to severe talar dislocations. While lateral ankle sprains are among the most frequently encountered injuries and do not pose a greater diagnostic problem, the surgeon must be suspicious not to overlook associated ligamentous injuries at the subtalar and midtarsal level that may result in chronic painful conditions. Syndesmotic instabilities with or without ankle fractures must be assessed carefully and treated properly, since these are prearthrotic conditions. In the treatment of chronic ankle or subtalar instabilit

Associated injuries found in chronic lateral ankle instability.

Sixty-one patients underwent a primary ankle lateral ligament reconstruction for chronic instability between 1989 and 1996. In addition to the ligament reconstruction, all patients had evaluation of the peroneal retinaculum, peroneal tendon inspection by routine opening of the tendon sheath, and ankle joint inspection by arthrotomy. A retrospective review of the clinical history, physical exam, MRI examination, and intraoperative findings was conducted on these 61 patients. The purpose was to determine the type and frequency of associated injuries found at surgery and during the preoperative e

Postural control following inversion injuries of the ankle.

Postural control was investigated following unilateral inversion injury of the ankle in 24 trained and 24 untrained subjects at least 8 weeks following injury and following resumption of high-speed activities. The two groups differed in the practice of balance exercises in one-legged stance during rehabilitation. Using a force platform the variability of the mediolateral force signal was used to quantify steadiness as each subject stood in one-legged stance with the eyes open and closed on the injured and noninjured legs. A three way analysis of variance showed that for the untrained subjects

Tendon injuries about the ankle resulting from skiing.

In essence there are three afflictions about the ankle that result from skiing: rupture of the Achilles tendon, Achilles tendinitis, and dislocation of the peroneal tendon. The cause of Achilles tendinitis and Achilles tendon rupture is, in fact, pressure within the fascial compartment of the Achilles tendon, which is caused by a swollen distal soleus muscle, occluding the circulation, and thereby producing avascular necrosis with subsequent tendinitis yielding to rupture. Four types of peroneal tendon dislocations and related problems are presented.

Injuries to the ankle and foot in athletics.

Accurate diagnosis in ligament and ligament-bone injuries of the foot and ankle demands a knowledge of the anatomy and mechanics of the foot and ankle, a good history of the mechanism of injury, clinical examination including fingertip palpation and assessment of stability, and adequate x-ray studies, perferably in the deformed position, with stress films and arthrograms in selected cases. By making an accurate diagnosis, early proper treatment is instituted, the athlete is restored to health sooner, and, almost as important, the athlete and the coach get a more realistic prognosis regarding t

Radiography in acute ankle injuries: the Ottawa Ankle Rules versus local diagnostic decision rules.

STUDY OBJECTIVE: We validate the Ottawa Ankle Rules and 2 Dutch ankle rules in distinguishing clinically significant fractures from insignificant fractures and other injuries in patients with a painful ankle presenting to the emergency department. METHODS: This prospective comparison of 3 ankle rules was conducted in the ED of a 580-bed community teaching hospital in Amsterdam from January 1998 to April 1999. Participants included 647 consecutive patients aged 18 years or older presenting with a painful ankle after trauma. All physicians received extensive and pictorial training on how to cor

Current treatment options for ankle injuries: lateral ankle sprain, Achilles tendonitis, and Achilles rupture.

The ankle is the site of many sports-related injuries. There is, however, continued debate over which treatment strategies are most effective for various conditions. This article reviews the relevant literature from the past year to shed some light on the ongoing controversy regarding the best treatment options along the treatment continuum, from conservative to operative management, for three common ankle conditions: lateral ankle sprains, Achilles tendonitis, and Achilles tendon rupture.

Common Mistakes in Diagnosis of Ankle and Foot Injuries.

Ankle and foot injuries are extremely common among athletes and nonathletes alike. Clinical findings are often nonspecific, and many injuries are difficult to diagnose with imaging, even using advanced techniques, and a delay in diagnosis may have significant adverse long-term consequences. Thus it is important to be familiar with the location and imaging appearances of these injuries. This article addresses several injuries that are commonly missed when imaging the ankle and foot.

Temporizing Care of Acute Traumatic Foot and Ankle Injuries.

Temporizing care has become a critical part of the treatment armamentarium for select foot and ankle injuries. Indications for performing temporizing care are based on the specific injury pattern, the host, associated injuries, as well as surgeon resources. Foot and ankle injuries are often associated with severe adjacent injury to the soft tissue sleeve. An acute procedure performed through a traumatized soft tissue envelope will often lead to the failure of wound healing and/or infectious complications. Thus, delayed reconstruction of acute foot and ankle injuries is often advisable in these
