[Multiple thromboembolism associated with anti-thrombin III deficiency].

Congenital deficiency of antithrombin III is considered as one serious primary hypercoagulable state. The authors present a case of deficiency of antithrombin III in a 28 years old young male, with aortic valve disease and several thromboembolic events. When discussing their diagnostical and therapeutical aspects they call the attention to the need for investigation of the situations in which thrombophilia presents with recurrent venous and arterial thrombosis.

Molecular basis of SERPINC1 mutations in Japanese patients with antithrombin deficiency.

BACKGROUND: Congenital antithrombin (AT) deficiency, which arises from various SERPINC1 defects, is an autosomal-dominant thrombophilic disorder associated with a high risk of recurrent venous thromboembolism. PATIENTS/METHODS: We investigated SERPINC1 defects in Japanese patients with congenital AT deficiency who developed venous thromboembolism or had a family history of deep vein thrombosis. We analyzed the full DNA sequences of SERPINC1 exons and exon-intron junctions by PCR-mediated direct sequencing. If no mutation was found, multiplex ligation-dependent probe amplification (MLPA) was c

Antithrombin III concentrates--are they clinically useful?

Treatment with AT III concentrates is a good example for the discrepancy between the optimistic expectations based on theoretical considerations or animal experiments and the result of clinical studies. 15 years after the introduction into clinical practice, a benefit for patients treated with AT III concentrates has not been proven. In hereditary antithrombin III deficiency, randomized clinical trials are completely lacking and only few and small sized randomized studies were performed in patients with acquired AT III deficiency. In none of these trials, a significant clinical benefit with re

[Hereditary antithrombin deficiency and hyperhomocysteinemia in venous thromboembolic disease].

Hypercoagulability state is the result of the interplay of genetic predisposition and risk factors. Many key enzymes and reactions in coagulation and anti-coagulation system are involved. Hereditary antithrombin deficiency is one of the major risk factors of venous thromboembolic disease (VTE), whereas hyperhomocysteinemia may also play a role. This article reviews the recent researches on the contributions of these two factors to VTE.

A case that illustrates the challenges of managing pregnant patients with antithrombin deficiency: More questions than answers.

Using an illustrative case of a patient with antithrombin (AT) deficiency who developed a recurrent venous thromboembolism (VTE) in pregnancy despite therapeutic low-molecular-weight heparin (LMWH), we highlight what is known in the literature and address areas of controversy through a series of questions around the case. The questions we address include the role of anti-Xa monitoring for patients with past VTE on antepartum LMWH, what treatment regimen is recommended for pregnant patients who develop a recurrent VTE while on therapeutic anticoagulation, the role of antepartum AT concentrate p

Comparison of antithrombin activity assays in detection of patients with heparin binding site antithrombin deficiency: systematic review and meta-analysis.

Antithrombin (AT) deficiency increases the risk for venous thromboembolism, therefore, a highly sensitive assay to identify this condition is crucial. The aim of this paper was to perform a meta-analysis comparing AT activities measured by different AT activity assays in patients with heparin binding site AT deficiency. In addition, the diagnostic sensitivity of selected assays was compared depending on the available data. An extensive literature search was performed considering results with publication date up to July 10, 2021. Seven relevant English-language observational studies, comparing

Bridging laboratory assays, genetics, and clinical phenotypes in antithrombin deficiency: Rethinking the diagnostic paradigm.

BACKGROUND: Diagnosis of antithrombin deficiency (ATD), the most severe inherited thrombophilia, commonly relies on functional assays despite their uncertain sensitivity. Moreover, routine characterization of ATD remains uncommon due to limited supporting clinical data. OBJECTIVES: This study aimed to evaluate the diagnostic sensitivity of commercial antithrombin (AT) activity assays and assess clinical differences among ATD types to refine the current diagnostic approach. METHODS: Eighty-eight patients with decreased AT activity and 124 consecutive patients with unprovoked venous thromboemb

A novel SERPINC1 c.119G>A (p.Cys40Tyr) mutation with variable clinical expression in an Indian family.

Hereditary antithrombin (AT) deficiency due to mutations in SERPINC1 is known to be the most severe form of thrombophilia. We report three members in a family with hereditary AT deficiency with a novel mutation in exon 2 of SERPINC1, that is c.119 G>A (p.Cys40Tyr). Two brothers presented with acute pulmonary thromboembolism (PTE) at 18 and 21 years of age, whereas their 58-year-old father did not have any thrombotic episode till date. The in-silico prediction of the variant was found to be highly damaging by PolyPhen-2, SIFT and MutationTaster. Clinical exome sequencing did not show any strong

Two case reports of inherited antithrombin deficiency: a novel frameshift mutation and a large deletion including all seven exons detected using two methods.

An inherited antithrombin deficiency is an autosomal dominant thrombotic disorder. We identified two pedigrees of inherited type I antithrombin deficiency and two responsible mutations in each. A novel 21-22delAA appeared to have caused a frameshift with a premature termination at amino acid +63 in one patient and a large deletion including all seven exons was identified by multiplex ligation-dependent probe amplification in the other. Some asymptomatic relatives of the second patient had the same mutation. The present findings support the value of using more than one method of gene analysis a

Ischemic stroke due to deficiency of coagulation inhibitors. Report of 10 young adults.

BACKGROUND AND PURPOSE: Deficiencies in coagulant inhibitors protein C, protein S, and antithrombin III increase the risk of venous thrombosis. We describe 10 young adults with cerebral arterial thrombosis due to deficiencies in these factors. METHODS: Sixty patients younger than 45 years were hospitalized because of acute ischemic stroke diagnosed through computed tomography or magnetic resonance imaging. Cerebral angiography was performed in 54 cases. Hematologic and coagulation profiles, autoantibody screen, syphilis serology, and lupus anticoagulant were analyzed in all patients. Among th

Coagulation abnormalities in idiopathic portal venous thrombosis.

BACKGROUND: Portal vein thrombosis, usually idiopathic, is the cause of portal hypertension in 46% of Indian patients, who present with a variceal bleed. The presence of lupus anticoagulant (LA) and antithrombin III deficiency have been reported to be associated with an increased tendency to venous thrombosis. METHODS AND RESULTS: We studied 30 patients with portal venous thrombosis diagnosed by ultrasound. Two patients were positive for a lupus anticoagulant and both had very prolonged partial thromboplastin time with kaolin. None of the patients had antithrombin III deficiency.

Clinical and functional characterization of rare compound heterozygous mutations in the SERPINC1 gene causing severe thrombophilia.

INTRODUCTION: Hereditary antithrombin (AT) deficiency is a rare autosomal dominant disorder with significant clinical heterogeneity. In the study, we identified a patient with AT deficiency caused by compound heterozygous mutations in the SERPINC1 gene. METHODS: A total of 9 individuals from three generations were investigated. The mutations were identified by direct sequencing of SERPINC1. Multiple in silico tools were programmed to predict the conservation of mutations and the effect on the AT structure. The coagulation state was evaluated by the thrombin generation assay. Recombinant AT wa

[Use of antithrombin III concentrates].

Concentrates of antithrombin III (AT III) have been applied in congenital as well as in acquired AT III deficiency. Congenital defects only require substitution during surgery and in the case of pregnancy. Otherwise prophylaxis of thrombosis can be successfully carried out with oral anticoagulants. In acquired AT III deficiency substitution was found to be useful in cases of advanced DIC. When AT III activity was constantly kept around 100% the duration of DIC could be considerably shortened. No beneficial effect of additional administration of heparin was found. In cases of DIC due to septic

Diagnostic Performance of Commercial Antithrombin Activity Assays: Do We Get What We Expect?

Hereditary antithrombin (AT) deficiency is a rare autosomal dominant disorder that predisposes to the development of recurrent venous thromboembolism (VTE). Diagnosis is based on the measurement of reduced AT activity in plasma. However, not all commercial AT activity assays are equally suited for diagnosis since they differ in sensitivity toward different AT mutations. The aim of this study was to compare the ability of commonly used AT activity assays to diagnose inherited antithrombin deficiency, with a focus on type II AT deficiencies. We used samples from genetically confirmed AT deficient

Myocardial infarction and severe thromboembolic complications. As seen in an estrogen-dependent transsexual.

An estrogen-dependent genetic-male transsexual had an extensive anterior wall myocardial infarction, despite insignificant coronary artery disease, a subsequent mural thrombosis, and resultant multiple cardioarterial thromboembolic events, despite heparin therapy. With an otherwise lack of cardiac risk factors, the patient was demonstrated to have an antithrombin III deficiency that resolved when conjugated estrogen therapy was withdrawn. Although congenital, plasminogen-activator dysfunction, or heparin-induced etiologies could not be ruled out, we believe this case demonstrated an estrogen-i

[The application of molecular biology to thrombosis and hemostasis].

Current biological science has been revolutionized by a series of new investigative molecular techniques developed within the last 15 years. These techniques allow the detection of gene mutations responsible for congenital diseases including thrombophilia and hemorrhagic disorders. Here, we provide a short introduction to polymerase chain reaction (PCR), single-strand conformational polymorphism (SSCP), restriction fragment length polymorphism (RFLP) and northern blot techniques and present the results of DNA sequence analyses of 4 different types of antithrombin III abnormalities and one IIB-
