Loeys-Dietz syndrome (LDS) is characterised by a mutation in the transforming growth factor beta receptor, and is strongly associated with aortic aneurysms and rupture. Most cases of LDS present in the second decade of life, with the average life expectancy being 37 years. We report a case of suspected LDS (owing to significant family history of the disorder) that presented in the seventh decade of life. Our patient had aortic root dilation and was initially believed to have Marfan syndrome. However, subsequent tests were negative. On further investigation of her family history, it was reveale

The prognosis of Marfan syndrome in both adult and pediatric patients is primarily related to the cardiovascular complications. In infantile Marfan syndrome, although involvement of the mitral valve is the most frequently encountered cardiovascular lesion, the aortic root can be more worrisome because of its excessive dilatation, leading to aortic insufficiency or dissection. If the role of elective surgery is relatively well defined for adult patients, it is still debated during childhood. We report two patients, aged 22 months and 5 years, each presenting an aortic root aneurysm related to M

BACKGROUND: The aortic root has unique embryological development and is a highly sophisticated and complex structure. In studies that report on the biomechanical characteristics of the thoracic aorta, distinction between the aortic root and ascending aorta regions is nonexistent. Our objective is to determine the maximal pressures at which dissection occurs or tissue failure occurs in the aortic root compared to that of the ascending aorta in the presence of aortic aneurysms. This may help guide preoperative monitoring, diagnosis and the decision for operative intervention for aortic root aneu

BACKGROUND: The aim of this study was to analyze the mid-term results of aortic root reconstruction.
METHODS: Between September 1995 and March 2001, 32 patients (25 males, 7 females, median age 58 +/- 21 years) underwent aortic root reconstruction as described by T. David. Indications for operation were aortic insufficiency with ascending aortic aneurysm in 27 patients, and acute aortic dissection (Stanford type A) in 5 patients. In all patients the native valve was preserved and suspended inside a tubular prosthesis with reimplantation of the coronary arteries. In 10 patients the classic rec

OBJECTIVE: Atheromatous ascending aortic aneurysms (AAA) frequently present with aortic regurgitation (AR) from dilatation of the sino-tubular junction (STJ) and extension of the pathological process into the root. Experience suggests that root dilatation begins in the non-coronary, then right coronary sinus. Rather than employ aortic root replacement or the David procedure, we have elected to replace the ascending aorta and remodel the STJ and involved sinuses. We studied the outcome after selective sinus replacement in 29 consecutive AAA patients between 1995 and 2001.
METHODS: There were n

The hypothesis that replacement of the aortic root with a valved composite graft is a safe and effective therapy for aneurysms involving the sino-tubular segment of the aorta was tested by a retrospective review. From September 1978 to January 1995, 335 consecutive patients underwent prosthetic aortic root replacement as the primary operation for aneurysm (222) or dissection (acute 59, chronic 54). There were 248 men (74%) and 87 women (26%) with a mean age of 50 +/- 14 years. 22 patients (6.6%) experienced hospital death primarily due to cardiac causes. Mortality was significantly higher in p

Prosthetic aortic valve dehiscence is a rare but potentially life-threatening complication that can occur after aortic valve replacement surgery. This condition occurs when the prosthetic valve becomes detached or dislodged from its original position leading to aortic valve regurgitation and congestive heart failure. The most common risk factors for prosthetic valve dehiscence include infective endocarditis, ascending aortic aneurysm, and severe calcification of the aortic valve. Ankylosing spondylitis, non-infectious aortitis, and accompanying vasculitis can also cause aortic valve dehiscence

OBJECTIVE: Surgery for ascending aneurysms in bicuspid aortic valve syndrome primarily includes Bentall root replacement, aortic valve replacement with supracoronary ascending aorta replacement (AVRSCAAR), and valve-sparing root reimplantation (VSRR). Comparative analysis of long-term clinical and functional outcomes of these procedures is detailed.
METHODS: From 1997 to 2017, 635 patients with bicuspid aortic valve undergoing root complex-focused procedures electively were stratified by valvulopathy (ie, aortic stenosis vs aortic insufficiency) and substratified into ascending or root aneury

BACKGROUND: This study reports our 10-year experience with the David technique and technical modifications to create neosinuses.
METHODS: From January 1996 to February 2009, the David procedure was performed in 151 consecutive patients in our department. Mean age was 59 ± 13 years (range, 22 to 78 years). All patients had ascending aortic aneurysm (mean diameter, 6.0 ± 1.1 cm); 59 patients had additional arch aneurysm. Fifty-four patients underwent the standard David procedure, with a pseudosinus created in 42 patients (28%) and neosinuses in 55 patients (36%) by plicating the base and sinotu

BACKGROUND: Aortic root dimension (ARD), an echocardiographic marker of aortopathy, has recently been associated with intracranial aneurysm eccentricity, suggesting a common underlying pathophysiology between intracranial aneurysm and aortopathy.
OBJECTIVE: To identify specific intracranial aneurysm-related parameters that independently correlate with ARD in patients with subarachnoid hemorrhage.
METHODS: Clinical and aneurysm-related parameters obtained from 151 patients with ruptured intracranial aneurysms, and who also underwent echocardiography with ARD measured, were examined. Univariat

Aortic valve-sparing reimplantation remains an effective technique for repair of aortic root aneurysms. Studies indicate that the Florida Sleeve procedure is dimensionally stable and durable in the early postoperative period; however, our technique has evolved. We describe a 10-year institutional experience and the technical update of the Florida Sleeve repair for root aneurysms.

Systemic lupus erythematosus (SLE) is an autoimmune disease in which organs, tissues, and cells undergo damage mediated by tissue-binding autoantibodies and immune complexes. We describe the case of a 23-year-old African American woman with a history of recurrent pneumonias. Computed tomography, magnetic resonance imaging (MRI), and echocardiographic evaluations, as well as clinical and laboratory findings, indicated a diagnosis of SLE with inflammatory aortitis secondary to SLE vasculitis. A repeat MRI revealed a rapidly expanding aortic root and ascending aorta that required prompt operative

PURPOSE: The advantages of aortic valve-preserving surgery are still hampered by a higher rate of reoperations compared to root replacement with valved conduits. This study evaluates whether valve deterioration rate is related to the method or depends on stringent adherence to technical concepts, which might be lost once trainees perform this complex surgery on their own.
METHODS: From 1991 to 2011, the David procedure was performed in 209 consecutive patients. Mean age was 57 ± 14 years. The patients were operated either by the senior author or trainees under his supervision (group 1, n = 13
