Valgus torque combined with deceleration produces high compression and shear forces acting on the posteromedial olecranon and the posteromedial trochlea. This valgus extension overload process may cause posteromedial trochlea chondromalacia, chondral flap formation, osteochondrosis, subchondral erosion, a subchondral insufficiency fracture, and marginal exostosis formation. Olecranon pathologies include proximal stress reaction, a posteromedial tip stress fracture, a transverse proximal process stress fracture, exostosis formation, exostosis fragmentation, and intra-articular loose bodies. Posttraumatic stiffness is a common complication after injury to the elbow. The loss of motion may significantly limit an individual's ability to perform normal activities of daily living. If conservative measures including medication, physical therapy, static splinting, and selective injection fail to restore functional range of motion, surgical intervention may be warranted. Open procedures have been described with reasonable success in restoring motion. However, arthroscopic techniques may provide several advantages in these patients who have often undergone previous extensive open surgery.

Arthroscopic complications are infrequent but when they occur can cause significant morbidity in the equine patient. This article reviews intraoperative and postoperative complications along with ways to avoid them. Additionally, therapeutic methods of managing these complications also are discussed.

Wrist arthroscopy is a commonly used procedure that has undergone many modifications and improvements since it was first described. The advent of new portals (both dorsal and volar) means that the wrist joint can be viewed from virtually any perspective ("box concept"). Indications for wrist arthroscopy have continued to expand and include diagnostic and reparative procedures and, more recently, reconstructive, soft-tissue, and bony procedures. Arthroscopic grading of Kienböck's disease better describes articular damage compared with plain radiographs and can help guide surgical treatment opti

Elbow arthroscopy has become to be the most useful tool for the treatment of many intra-articular affections of the elbow. Radiological statement is necessary including plain radiographs and CT or MR arthrography before performing arthroscopy. Loose bodies are the more frequent indication, they are often related with an other intra-articular pathology. The others indications for an elbow arthroscopy can be, osteochondritis dissecans, synovial fringe, synovitis especially rheumatoid arthritis and arthritic elbow. The treatment of this different pathologies is discribe keeping in mind the potent

We investigated the clinical outcomes of patients with specific symptoms and physical findings of subtalar pathology with nonspecific radiographic findings after subtalar arthroscopy. Preoperative workup included plain radiographs in all of the patients, magnetic resonance imaging in seven patients, and bone scan in six patients. Based on these studies, no patient had significant subtalar arthrosis. Twelve patients underwent subtalar arthroscopy. Clinical outcome was rated with the American Orthopaedic Foot and Ankle Society Hindfoot Score. Preoperatively, the score ranged from 54 to 75, with

Ankle arthroscopy has grown in popularity over the past decade as a diagnostic and therapeutic procedure. We assessed 24 patients who underwent ankle arthroscopy during the two year period 1991-1993. In 18 cases a mini-arthrotomy was performed based on the arthroscopic findings. All patients were reviewed at an average of 26 months and a detailed evaluation was carried out according to an objective scoring system which grades five parameters; pain, swelling, stiffness, limp, and activity level. Twenty-two patients (92%) had good or excellent results. There were two complications transient tars

A retrospective review of the first 612 patients undergoing consecutive ankle arthroscopy in the practices of two experienced arthroscopists was under-taken. All inpatient records, outpatient charts, and operative reports were reviewed. Indications for surgery included pain, swelling, locking, and instability that failed to respond to nonoperative management. The results of our investigation revealed an overall complication rate of 9.0% (55 complications). There were 27 neurological complications (4.4% of all arthroscopies) accounting for 49.1% of the complications noted. Specifically, the sup

Arthroscopy is highly useful for the diagnosis and treatment of loose bodies of the elbow. Not all loose bodies are detected radiographically, and often there are multiple loose bodies, sometimes both anteriorly and posteriorly. When not complicated by an underlying disorder such as arthritis, loose bodies can be removed from any part of the elbow to provide excellent results in a predictable manner. This article describes techniques developed for the safe and effective application of this procedure in the elbow.

An 18-year-old male presented with left foot ischemia secondary to emboli from a pseudoaneurysm of the left popliteal artery. His past history was significant only for two arthroscopies of his left knee. After evaluation, the patient was successfully treated with obliteration of the aneurysm and a reverse saphenous vein bypass graft.

The development of shoulder arthroscopy is reviewed since its beginnings in 1931. The technique of posterior introduction of the arthroscope and positioning of the patient is presented. An additional portal for arthroscopic surgery is also described. The main indications for the procedure are reviewed, based on a survey of the literature and the author's experience, and five cases are presented illustrating some of the situations for which the technique proved beneficial.

Using ankle arthroscopy in conjunction to open reduction internal fixation of traumatic ankle injuries can play an important role in the management of these injuries by way of treating intra-articular pathologies, leading to improved patient outcomes. While a majority of these injuries are not treated with concurrent arthroscopy, its addition may lead to more prognostic information to dictate the patient's course. This article has illustrated its use in managing malleolar fractures, syndesmotic injuries, pilon fractures and pediatric ankle fractures. While additional studies may be needed to f

With advancements in surgical instrumentation and techniques, the role of arthroscopic and arthroscopic-assisted surgical procedures is ever-growing. Arthroscopy offers direct, magnified visualization of pathology and reductions and is more accurate than relying on intraoperative fluoroscopy alone. It also minimizes soft tissue stripping, which is of particular importance to smaller fracture fragments whose vascularity is precarious and can be injured through open approaches.

Pain after hip arthroscopy can be severe, yet we lack a consensus method for non-narcotic analgesia. Here we describe anatomic elements of hip arthroscopy and our current understanding of the relevant sensory innervation as a prelude to the evaluation of locoregional analgesic techniques. Many regional nerve blocks and local anesthetic infiltration techniques are reviewed, including 2 newer ultrasound fascial plane blocks. Further study of targeted, motor-sparing approaches, either ultrasound-guided or under direct surgical visualization is needed.

BACKGROUND: The anterocentral portal is not a standard portal in anterior ankle arthroscopy due to its proximity to the anterior neurovascular bundle. However, it provides certain advantages, including a wide field of vision, and portal changes become redundant. The purpose of this study was to evaluate the neurovascular complications after anterior ankle arthroscopy using the anterocentral portal.
METHODS: We retrospectively identified patients who had undergone anterior ankle arthroscopy with an anterocentral portal at our institution from 2013 to 2018. Medical record data were reviewed and

Patellar osteonecrosis is a rare condition, although knee osteonecrosis has been described in the arthroplasty literature. This is the first case describing knee arthroscopy as the cause of osteonecrosis. A 50-year-old woman who was experiencing knee pain during her marathon training and singles tennis underwent knee arthroscopy for a torn medial meniscus. The result of her partial medial meniscectomy led to patellar osteonecrosis 3 months following the index procedure. Osteonecrosis of the knee has been reported in the arthroplasty and sports medicine literature following surgical interventio
