[The reconstruction of laryngeal function in subtotal laryngectomy by pedicled flaps].

OBJECTIVE: To explore the possibility of subtotal laryngectomy in the treatment of advanced laryngeal cancer and selection of reconstruction. METHODS: Forty patients were treated surgically by subtotal laryngectomy with preservation of arytenoid cartilage and perichondrium. The pedicled flaps between cricoid cartilage and arytenoid cartilage were sewed up. The new larynx was reconstructed by suturing the cricoid or trachea to the hyoid bone or the tongue base. RESULTS: The 3 and 5 year survival rates were 85.0% and 76.2% respectively. Decannulation rate was 92.5%. CONCLUSION: The reconstruc

Laser arytenoidectomy in the treatment of bilateral vocal cord paralysis.

The introduction of the CO2 surgical laser into laryngeal microsurgery has made resection of the posterior vocal cord together with the arytenoid cartilage possible. Since November 1990, 30 arytenoidectomies, 17 partial cordectomies and 18 bilateral cordectomies as described by Kashima were performed by means of a CO(2) laser in patients with bilateral paralyses of the vocal cords. In this group there were 58 women and 7 men. The patients' ages ranged from 28 to 71 years (mean, 46.7 years). In one case the operation was performed twice: the right arytenoid cartilage was excised initially and t

Successful management of an arytenoid chondrosarcoma in a dog.

An eight-year-old Doberman pinscher was presented with a four-week history of inspiratory stridor, dysphonia, inappetence and weight loss. Inspiratory stridor was apparent and became more pronounced during gentle compression of the larynx. Previous investigations, including laryngoscopy, had revealed the presence of a left-sided arytenoid mass. Histological examination of pinch biopsies was not diagnostic. The mass was removed by resection of the arytenoid cartilage through a ventral laryngotomy allowing salvage of the cuneiform process. Histological examination of the laryngeal mass was consi

[Experimental study on cricoarytenoid joint movement].

Through the study on 80 sides of the cricoarytenoid joints, it was proved that the cricoarytenoid joint was a small joint having multiple forms of movements. As the glottis opened or closed the arytenoid cartilage would slide first along the major axis of the joint facet, then round the inclined axle complemented by transverse and perpendicular movements round the joint facet. The closing movements of the glottis took place at the anterior part of the joints while the opening movements at the posterior part of the joints. The vocal process would move outwards and inwards as the arytenoid carti

[Subtotal reconstructive laryngectomy: author's experience].

The authors conclude 18 year's experience with reconstructive laryngectomy by adopting two surgical techniques for intrinsic laryngeal tumors. The first, where one or both of the arytenoids are conserved, can be applied in cases of supraglottic neoplasms extending to the glottis as well as in glottic cancers where a simple cordectomy is not feasible. In both cases this technique is indicated when at least one arytenoid is respected. The second technique involves removal of both arytenoids and is used in cases of intrinsic supraglottic or glottic tumors extended to both arytenoids. The anatomic

The assistance of coblation in arytenoidectomy for vocal cord paralysis.

BACKGROUND: There are many causes for vocal cord paralysis, which can cause difficulty in breathing in serious cases. The common surgical methods for solving vocal cord paralysis include laryngeal splitting or laser surgery, but there are limitations. Plasma radiofrequency ablation is a new treatment with good achievements in clinical applications. OBJECTIVE: To investigate the effect of coblation-assisted arytenoidectomy (CSA) in the treatment of bilateral vocal cord paralysis (BVCP). METHODS: All patients had undergone preoperative electrolaryngoscopic examination of the glottidis rima; el

Assessment of cartilage invasion in case of laryngeal cancer by means of longitudinal sectioning for histopathology - Clinical implications.

AIM: The aim of the study was to assess the accuracy of radiological diagnosis of laryngeal cartilage infiltration by histopathological examination of laryngeal specimen after total laryngectomy. BACKGROUND: Despite the development of new medical technologies and significant clinical advances allowing early diagnosis and treatment of laryngeal cancer, mortality is still on the rise. Neoplastic infiltration of the laryngeal cartilages is the most common source of error in the assessment of cancer staging. Furthermore, cartilage invasion is listed as a contraindication to partial surgical techn

[Instructions for the complete preparation of the larynx].

A method of preparing the larynx in forensic cases is described especially when death occurs by strangulation (manually or using some form of ligature). After the usual dissection of the hyoid bone and the upper horns of the thyroid cartilage, the complete larynx is clipped and fixed in formaldehyde overnight. The first part of dissection after that is to cut away the thyroid gland and the remaining muscles in front of the larynx, except for the cricothyroid muscle and the esophagus and hypopharynx from the back of the larynx. The complete thyroid cartilage is dissected out, and then a horizon

Development of endoscopic arytenoid adduction using cricoid implant.

OBJECTIVES: We sought to develop a less-invasive alternative to conventional arytenoid adduction using a cricoid implant. METHODS: We performed a preliminary study with excised human and canine larynges. A nail-shaped stainless steel rod and an insertion device were designed for an in vivo animal trial. After unilateral recurrent laryngeal denervation surgery in 5 adult mongrel dogs, the implants were inserted endoscopically through a small mucosal incision over the cricoarytenoid joint. Acoustic and aerodynamic data were obtained from each animal before serial euthanasia followed by examinat

A new paramedian approach to arytenoid adduction and strap muscle transposition for vocal fold medialization.

OBJECTIVE: To develop a prosthesis-free medialization laryngoplasty for the treatment of glottal incompetence. STUDY DESIGN: Twenty-two consecutive patients with glottal incompetence underwent vocal fold medialization using a new paramedian approach to arytenoid adduction and/or strap muscle transposition. METHODS: Under local anesthesia, the thyroid lamina on the involved side was parasagittally separated 5 mm off the midline. The inner perichondrium was carefully freed from the overlying thyroid cartilage. After dividing the thyrohyoid and cricothyroid membranes, the lamina was retracted l

Surgical Treatment of Laryngeal Paralysis.

Unilateral arytenoid lateralization is the most commonly used technique to treat laryngeal paralysis. It is important not to overabduct the arytenoid cartilage during the unilateral lateralization to minimize exposure of the rima glottides. Dogs with laryngeal paralysis treated with unilateral lateralization have a good long-term prognosis. Idiopathic polyneuropathy is the most common cause of laryngeal paralysis in dogs.

Physiologic assessment of arytenoid adduction.

Arytenoid adduction as described by Isshiki is a surgical technique used to improve vocal quality by adducting the arytenoid cartilage of a paralyzed vocal fold, medializing the fold, and closing the posterior glottic aperture. Surgical results of this operation were evaluated by preoperative and postoperative voice recordings, laryngoscopy, and stroboscopy. Objective measurements of vocal jitter, shimmer, and signal to noise ratio were done to assess changes in the vibratory patterns, and analysis of data from 12 patients revealed improved glottic function postoperatively. Often an anterior m

[Electrostimulation of laryngeal muscles with fluctuating currents in the treatment of patients with unilateral laryngeal paralysis].

Intralaryngeal electrostimulation of the laryngeal muscles with fluctuating currents with simultaneous mobilization of the arytenoid cartilage and paralysed vocal cord were used in 42 patients with unilateral laryngeal paralysis. The treatment was combined with phonopedic lessons. The control group consisted of 32 patients receiving standard electrostimulation with diadynamic currents. The effect was evaluated by changes in vocal acoustic parameters and stroboscopic parameters. In patients with paramedian fixation of the vocal cords voice improvement was obtained irrespective of electrostimula

The historical Latin and etymology of selected anatomical terms of the larynx.

The etymological evolution of the anatomical terms larynx, cricoid, glottis, epiglottis, and thyroid (cartilage) dates to antiquity. Human dissection replaced animal in the 16th and 17th centuries and terms evolved. This evolution was recorded in the literature largely in Latin. We translated key studies of laryngeal anatomy from the 16th century to better understand this evolution. We present the Latin with our translations, and historical commentary as essential to this understanding. Vesalius favored the Latin scutiform (shield) for the thyroid cartilage, but recognized peltalis (shield). T

[Aspiration pneumonia following various types of partial horizontal laryngectomy].

The authors present a retrospective study of 463 partial horizontal laryngectomy of various types (horizontal supraglottic laryngectomy and its modifications, supracricoid laryngectomy with CHP and CHEP, glottic laryngectomy) performed in the years 1970-1991 in the ENT Clinic of Pomeranian Medical Academy in Szczecin. The aspiration pneumonia in 46 (about 10%) patients was observed as a result of insufficient lower airway protection. In cases of extended supraglottic laryngectomy (including the removal of one arytenoid cartilage and/or one vocal cord) aspiration pneumonia appeared more often (

[Mediafixation of the vocal cord through the external approach in the patients presenting with unilateral laryngeal paralysis].

The objective of the present work was to develop a method for the modification of the vocal cord during the treatment of the patients presenting with unilateral laryngeal paralysis taking into consideration individual peculiarities of stereotopometric characteristics of the larynx in the subjects of different gender, neck structure, and body constitution. The earlier proposed method of laryngostereotopometry was employed in this study. The following body features were measured prior to surgery: the total length, acromial diameter, chest circumference, neck length and circumference; the degree
