Atrial fibrillation in the elderly.

Atrial fibrillation is the most common arrhythmia in the United States, whose incidence is greatest in the elderly population. This rhythm disorder can be paroxysmal or chronic and is associated with a range of clinical conditions from palpitations and dyspnea to stroke and death. In the elderly the mainstay of treatment of atrial fibrillation should utilize drug therapy. The main goals of drug therapy should be effective rate control to avoid tachycardia-induced cardiomyopathy, anticoagulation to reduce the risk of stroke and thromboembolism, and maintenance of sinus rhythm to prevent adverse

Atrial fibrillation in the elderly.

Atrial fibrillation is the most common sustained arrhythmia seen in clinical practice. Although it occurs in any age patients, its frequency increases with age and is very common in the elderly. Atrial fibrillation causes substantial symptoms and morbidity and is an important cause of thromboembolism and stroke. The two approaches for therapy in those patients with intermittent or persistent atrial fibrillation are (1) maintenance of sinus rhythm with an antiarrhythmic drug or nonpharmacologic therapy, and (2) maintenance of atrial fibrillation with rate control. At the present time there are

Effect of atrial fibrillation ablation on gastric motility: the atrial fibrillation gut study.

BACKGROUND: Collateral damage to the vagal nerve and the upper gastrointestinal (UGI) system during atrial fibrillation ablation has not been systematically evaluated. METHODS AND RESULTS: We performed a prospective, observational study assessing the effect of atrial fibrillation ablation on the function of the vagus nerve/UGI system. All patients underwent esophageal manometry, gastric emptying study, and sham-feeding test (corresponding to esophageal, gastric, and small intestinal function evaluation, respectively) before ablation (baseline) and subsequently at 24 hours, 90 days, and 180 da

Atrial fibrillation and paroxysmal atrial fibrillation detection in patients with acute ischemic stroke.

BACKGROUND: Studies about continuous electrocardiographic (ECG) monitoring in detection of paroxysmal atrial fibrillation (PAF) in Asian patients with acute ischemic stroke are very limited. We looked for the prevalence and associated factors of atrial fibrillation (AF) and PAF in Thai patients with acute ischemic stroke. METHODS: In all, 204 patients with acute ischemic stroke were prospectively included. Snapshot 12-lead ECG and continuous ECG monitoring for at least the first 24 hours were performed. Multivariate analyses were performed to find out the associated factors of AF and PAF. RE

[Atrial fibrillation: practical questions. Who are the best candidates for atrial fibrillation ablation?].

Ablation of paroxysmal AF in symptomatic patients is a clear indication after failure of anti-arrhythmic therapy (class IIa according to the recommendations). Ablation of persistent AF is feasible, as second line after failure of medical treatment, although there is no real consensus on ablative techniques : this procedure is long and complex, exposure to ionizing radiation is important. Regarding ablation of persistent AF, a careful selection of patients and multiple interventions are often necessary to increase the success rate. Standardization of the procedure and better reproducibility of

Stroke in paroxysmal atrial fibrillation: report from the Stockholm Cohort of Atrial Fibrillation.

AIMS: Knowledge about stroke risk in paroxysmal atrial fibrillation (PxAF) is limited. Although current guideline recommendations advocate the same treatment as in permanent atrial fibrillation (PermAF), most patients with PxAF do not receive prophylactic anticoagulation. The aim of this study is to investigate whether there are differences in stroke risk between PxAF and PermAF. METHODS AND RESULTS: All patients with PxAF (n = 855) and PermAF (n = 1126) treated for atrial fibrillation (AF) during 2002 at one of Scandinavia's largest hospitals were followed-up for 3.6 years regarding incidenc

Propafenone for conversion and prophylaxis of atrial fibrillation. Propafenone Atrial Fibrillation Trial Investigators.

Pharmacologic therapy is widely used for restoration of sinus rhythm and prevention of recurrences of atrial fibrillation. Because concerns have been raised about their potential proarrhythmic effects, therapeutic regimens should be evaluated by placebo-controlled studies to determine their efficacy and safety. One hundred thirty-six patients with persistent atrial fibrillation were randomized to receive propafenone 2 mg/kg over 30 minutes, followed by oral propafenone 150 mg 3 times daily or matching placebo. Nonresponders to intravenous therapy underwent direct-current cardioversion. Both re

Estimation of atrial fibrillation burden as a function of monitoring time without atrial fibrillation.

BACKGROUND: Atrial fibrillation (AF) burden is associated with adverse outcomes; in some patients, very low AF burden may not be actionable. OBJECTIVE: This study aimed to model AF burden as a function of monitoring time without AF. METHODS: We included 11,639 US patients (median age 62 years [interquartile range 46‒73], 40.6% male) without permanent/persistent AF, monitored for ≥14 days in 2021 with the PocketECG device (MEDICALgorithmics). The overall 14-day AF burden was modeled as a function of monitoring duration without AF episodes lasting ≥30 seconds to <6 minutes, ≥6 minutes to 1 hou

Discrimination of Paroxysmal and Persistent Atrial Fibrillation in Patients With New-Onset Atrial Fibrillation.

Discrimination between paroxysmal and persistent atrial fibrillation (PAF and persistent AF) is important for determining the therapeutic strategy in patients with new-onset AF. We evaluated various clinical factors and P wave morphology to discriminate PAF and persistent AF patients in patients with new-onset AF.The study population consisted of 79 patients with new-onset AF (70.3 ± 10.8 years, female:male 33:46) who were retrospectively selected from 8,632 AF patients in the Kitasato University Hospital ECG storing system. PAF (n = 38) and persistent AF (n = 41) patients were diagnosed by wh

Evolution of Paroxysmal Atrial Fibrillation to Persistent or Permanent Atrial Fibrillation: Predictors of Progression.

Introduction: Paroxysmal atrial fibrillation (PAF) eventually progresses to persistent and permanent AF. The predictors of progression from PAF to persistent and permanent AF are poorly understood. Methods: Electronic medical records of 437 patients with PAF were reviewed in a retrospective cohort study. Patients were followed in time and progression to persistent/permanent AF was recorded. Demographic, clinical and echocardiographic information was collected. A logistic regression analysis was performed to identify predictors of progression to persistent/permanent AF. Results: Over a mean dur

Follow-up of atrial fibrillation: The initial experience of the Canadian Registry of Atrial Fibrillation.

Previous reports of the follow-up of patients with atrial fibrillation have been confusing because of the variety of clinical presentations, heterogeneity of underlying pathology, and the initiation of follow-up at various stages of the patient's disease. The Canadian Registry of Atrial Fibrillation (CARAF) is a non-interventional, follow-up study of patients enrolled at the time of their initial diagnosis with atrial fibrillation at seven Canadian centres. At baseline, a comprehensive database recorded clinical, laboratory, and echocardiographic variables. No specific intervention was initiat

Effect of family history of atrial fibrillation on recurrence after atrial fibrillation ablation: A report from the Chinese Atrial Fibrillation Registry Study.

BACKGROUND: To evaluate the impact of family history of atrial fibrillation (FAF) on postablation atrial tachyarrhythmia (AT) recurrence. METHODS: All the 8198 patients undergoing initial AF ablation registered in the Chinese Atrial Fibrillation Registry study were analyzed. FAF was defined as having first-degree relatives diagnosed as AF at age 65 years or younger, and before the time the case in this study was diagnosed. Cox proportional hazards models were used to evaluate the impact of FAF on postablation AT recurrence. Age, sex, body mass index, AF type, history of congestive heart failu

Atrial high-rate episodes and thromboembolism in patients without atrial fibrillation: The West Birmingham Atrial Fibrillation Project.

BACKGROUND: Patients with cardiac implantable electronic device (CIED) developing atrial high-rate episodes (AHRE) have a significant risk of thromboembolic events (TEs), but risk factors have been scarcely investigated. OBJECTIVES: To analyze risk factors for TEs in a contemporary cohort of patients with CIED. METHODS: Consecutive non-AF patients without anticoagulation at baseline were followed up after the CIED implantation. The role of newly-developed AHRE and other risk factors for TEs were analyzed using a time-dependent Cox regression model and Kaplan-Meier analysis with log-rank test

Correlation between atrial fibrillation driver locations and complex fractionated atrial electrograms in patients with persistent atrial fibrillation.

INTRODUCTION: The aim of this study was to evaluate a spatial correlation between active atrial fibrillation (AF) drivers measured by electrocardiographic imaging and complex fractionated atrial electrograms (CFAEs) in patients with persistent AF. METHODS: Sixteen patients with persistent AF were included. A biatrial geometry relative to an array of 252-body-surface-electrodes was obtained from a noncontrast computed tomography scan. The reconstructed unipolar AF electrograms were signal-processed (ECVUE™, CardioInsight Technologies Inc., Cleveland, OH, USA) to identify AF drivers. Before dri

Longer longitudinal atrial dimension in patients with idiopathic paroxysmal atrial fibrillation: A possible cause of atrial fibrillation.

BACKGROUND: The ostium of the superior pulmonary veins or superior vena cava has been reported to be an important source of the ectopic beats that initiate paroxysmal atrial fibrillation (PAF). The structural details of the atria in patients with idiopathic PAF, however, remain unknown. METHODS: We studied 113 patients (92 men and 21 women) with idiopathic PAF and 128 normal control subjects (100 men and 28 women). None of the subjects in either group were found to have any evidence of structural cardiac disease. The echocardiographic measurements were performed in the apical 4-chamber view d

Possible role of supernormal atrial conduction in the genesis of atrial fibrillation in patients with idiopathic paroxysmal atrial fibrillation.

OBJECTIVE: The occurrence of supernormal excitability and conduction in the atrium has been attributed to the internodal pathways in several animal experiments. However, little is known about the role of supernormal atrial conduction (SNC) in the genesis of arrhythmias. The specific aim of this study was to evaluate prospectively the relationship between SNC, atrial conduction defects and atrial fibrillation in patients with idiopathic paroxysmal atrial fibrillation. METHODS: Programmed atrial stimulation was performed in 38 control patients (group 1), and 21 patients with idiopathic paroxysm

Saudi Atrial Fibrillation Survey: national, observational, cross-sectional survey evaluating atrial fibrillation management and the cardiovascular risk profile of patients with atrial fibrillation.

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia. The Saudi Atrial Fibrillation Survey registry was designed to provide epidemiological and clinical data on patients with AF. The registry included 400 consecutive patients who met the eligibility criteria. Control of AF at the time of the initial visit was achieved by 211 (52.75%) patients. Cardiovascular risk profile of the patients with AF was smoking 92 (23.5%), hypertension 253 (63.25%), diabetes 192 (48%), and dyslipidemia 173 (44%). Rate control was the most frequent management strategy (in 265 patients, 66.2%) whe
