Frequency-dependent effects of diltiazem on the atrioventricular node during experimental atrial fibrillation.

Calcium channel blockers depress atrioventricular (AV) nodal properties in vivo in a frequency-dependent manner, suggesting that selective drug action during supraventricular arrhythmias may result from use-dependent properties. The present study was designed to examine whether or not the rate-dependent actions of diltiazem account for its therapeutic effects during atrial fibrillation. The determinants of the ventricular response to atrial fibrillation (concealed AV nodal conduction and AV node functional refractory period, AVFRP) were evaluated at multiple cycle lengths (with extrastimulus t

Atrioventricular node reentry: intravenous verapamil as a method of defining multiple electrophysiologic types.

Fourteen patients with recurrent supraventricular tachycardia (SVT) underwent electrophysiological evaluation. Each patient was shown to have reentry confined to the region of the atrioventricular (AV) node. Verapamil, 0.075 to 0.15 mg/kg was administered intravenously to each patient during a stable episode of SVT, resulting in termination in each instance. There was more than one mechanism for termination of SVT. Nine patients showed termination by anterograde AV node block preceded by an increase in conduction time in the anterograde limb of the tachycardia circuit (Ae-H intervals) with no

A modified electrocardiographic algorithm for differentiating typical atrioventricular node re-entrant tachycardia from atrioventricular reciprocating tachycardia mediated by concealed accessory pathway.

Non-invasive prediction of tachycardia mechanism is becoming clinically important in the era of catheter ablation for curing supraventricular tachycardia. Twelve-lead electrocardiograms (ECGs) during sinus rhythm and atrioventricular node re-entrant tachycardia (AVNRT) or atrioventricular reciprocating tachycardia (AVRT) with a narrow QRS complex were obtained from 154 consecutive adult patients who had received successful radiofrequency catheter ablation. The ECGs of initial 104 patients were analysed by three observers without knowledge of the electrophysiological diagnosis. The two arrhythm

Long-Term Outcomes of Radiofrequency Atrioventricular Node Ablation in a Real-World Population.

BACKGROUND: While interventional strategies have expanded the options for long term rhythm restoration, rate control continues to play a pivotal role in the treatment of atrial arrhythmias; however, pharmacologic strategies alone often fail in achieving effective rate control. AIMS: To examine long-term outcomes in patients who have undergone atrioventricular node ablation (AVNA) for the management of symptomatic atrial arrhythmias. METHODS: This observational Study Assessed Patients Who Underwent AVNA For Rate Control of Atrial Fibrillation (AF) Or Atrial Tachycardia (AT) Between April 2014

Cardioneuroablation for pediatric patients with functional sinus node dysfunction and paroxysmal atrioventricular block.

INTRODUCTION: Severe transitory episodes of bradycardia with subsequent syncope in children are common, and generally portend a benign prognosis. Rarely, patients may experience prolonged asystolic episodes secondary to significant sinus pauses (SP) or paroxysmal atrioventricular block (AVB). Cardioneuroablation (CNA) is a catheter-based intervention, used to identify and ablate the epicardial ganglionated plexi (GP), which results in disruption of the vagal-mediated parasympathetic input to the sinus and atrioventricular node. OBJECTIVE: Describe the methodology and role of CNA for treatment

Long-term experience of atrioventricular node ablation in patients with refractory atrial arrhythmias.

Atrial fibrillation and other atrial tachyarrhythmias are increasing with age and concomitant morbidity. First options in symptomatic patients are drug treatment and catheter ablation. Nevertheless, a considerable number of patients suffer from refractory atrial tachyarrhythmias despite treatment. Atrioventricular node ablation (AVNA) may be helpful in many of these patients. Therefore, we investigated AVNA patients with a long-term follow-up. We enrolled 82 patients with a follow-up longer than 1 year receiving AVNA for drug- and ablation-resistant atrial tachyarrhythmias (AA) in a retrospect

Atrial fibrillation in cardiac resynchronization therapy patients: should we ablate the atrioventricular node?

Prevalence of persistent atrial fibrillation in patients undergoing cardiac resynchronization therapy remains high. Several factors, including suboptimal biventricular capture rates, compound the significant mortality and response rates in patients with atrial fibrillation and severe heart failure. Atrioventricular nodal ablation provides the best mean of rate control in this population. In this article, the authors evaluate a recently published trial addressing this issue in a prospective fashion and discuss the results and clinical applications.

Electrophysiologic and clinical effects of oral encainide in paroxysmal atrioventricular node reentrant tachycardia.

The electrophysiologic effects of oral encainide (75 to 150 mg daily) were evaluated in 14 patients (6 male and 8 female, aged 49 +/- 9 years) with atrioventricular (AV) node reentrant tachycardia of the slow-fast type. The patients were studied in control conditions and after 2 to 12 days of treatment. Encainide increased the AH interval from 67 +/- 10 to 82 +/- 23 ms (p less than 0.02). Anterograde Wenckebach cycle length was increased in three patients, reduced in four, unchanged in one; it was not measurable in the remaining patients because tachycardia was induced. Retrograde Wenckebach p

De subitaneis mortibus. XXVII. Histological abnormalities in the sinus node, atrioventricular node and His bundle associated with coarctation of the aorta.

A twenty-year-old carpenter died suddenly and unexpectedly nine years after surgical treatment of coarctation of the aorta. Both before and after surgery he had paroxysmal atrial fibrillation, T wave inversions in the ECG and persisting cardiac hypertrophy. At postmortem examination there was focal fibromuscular dysplasia narrowing the sinus node artery, but other small coronary arteries were normal and there was no focal fibrosis of the ventricular myocardium. Within the sinus node there were several small glomera surrounding branches of the sinus node artery. Pericardial fibrosis was present

Plasma cell leukemia accompanied by polyclonal gammopathy and congenital tumor in the atrioventricular node.

A 32-year-old man with plasma cell leukemia and congenital tumor of the atrioventricular node is described. Although M protein could not be detected by immunoelectrophoresis, the examination of bone marrow atypical plasma cells with immunofluorescent and immunoperoxidase showed that most of these plasma cells produced IgG or IgA. Also some plasma cells possessed concomitantly both IgG and IgA. An electrocardiogram revealed complete atrioventricular block. The patient died of aspergillosis and bleeding in the lung. Postmortem examination revealed the infiltration of atypical plasma cells in the

Beyond the lens: Unveiling the invisible atrioventricular node in the era of high-density mapping.

Numerous studies have clarified the histological characteristics of the area surrounding the atrioventricular (AV) node, commonly referred to as the triangle of Koch (ToK). Although it is suggested that the conduction of electric impulses from the atria to the ventricles via the AV node involves myocytes possessing distinct conduction properties and gap junction proteins, a comprehensive understanding of this complex conduction has not been fully established. Moreover, although various pathways have been proposed for both anterograde and retrograde conduction during atrioventricular nodal reen

Twin atrioventricular nodes, arrhythmias, and survival in pediatric and adult patients with heterotaxy syndrome.

BACKGROUND: Heterotaxy syndrome is likely to involve arrhythmias from associated conduction system abnormalities, which are distinct in different subtypes of isomerism and may change further after interventions and remodeling. OBJECTIVE: The purpose of this study was to understand the risk of arrhythmias and its relation to isomerism subtypes. METHODS: Patients diagnosed between 1980 and 2019 as having heterotaxy syndrome were enrolled and grouped as right atrial isomerism (RAI), left atrial isomerism (LAI), or indeterminate isomerism. RESULTS: Of the 366 patients enrolled, 326 (89.1%) had

Utility of the aVL lead in the electrocardiographic diagnosis of atrioventricular node re-entrant tachycardia.

AIMS: Reciprocating atrioventricular tachycardia can be categorized into common slow-fast atrioventricular node re-entrant (AVNRT) and orthodromic atrioventricular reciprocating tachycardia (AVRT). The electrocardiogram (ECG) during tachycardia is useful in distinguishing these two mechanisms. The presence of a pseudo-R'-wave in lead V1 or pseudo-S-wave in the inferior leads has been widely used, although the value of an isolated aVL lead has not been evaluated yet. To determine whether an isolated aVL lead of the surface 12-lead ECG is useful for the differential diagnosis between AVNRT and A

Reentrant tachycardia using two discrete atrioventricular nodes and a concealed atriofascicular pathway.

We describe a case of reentrant supraventricular tachycardia using two discrete atrioventricular (AV) nodes in the antegrade direction and a concealed atriofascicular pathway in the retrograde direction in a 21-year-old woman with superior-inferior ventricles and ventricular inversion. Using the Carto endocardial mapping system, two discrete AV nodes were identified as well as a separate decremental AV pathway located midway between the two nodes. Ablation of the separate pathway resulted in elimination of inducible tachycardia without loss of either AV node.

An interindividual variability in the sensitivity of atrioventricular node to diltiazem in patients with paroxysmal supraventricular tachycardia.

To study the sensitivity of atrioventricular (AV) node to diltiazem in seven patients with paroxysmal supraventricular tachycardia (PSVT), we analyzed the plasma concentration-response relationship of this Ca-antagonist using AH interval as an index for assessing its Ca channel blocking effect on the AV node after an IV infusion (0.4 mg/kg). The postdose AH intervals were prolonged compared with the baseline, and their percentage changes correlated significantly (P less than 0.01) with log-diltiazem concentrations in all patients. However, drug concentrations associated with a 20% prolongation

Electrophysiological actions of dantrolene sodium in isolated sinoatrial and atrioventricular nodes and in a model of ischemia.

Recent experiments have suggested that the electrophysiological actions of dantrolene sodium in cardiac tissues may be mediated in part by slow inward current blockade. The authors report here the effects of this agent in tissues the electrical activity of which depends mainly on the slow inward current. In isolated rabbit sinoatrial node tissue, dantrolene (35.1 microM) significantly diminished maximal diastolic potential, action potential amplitude and upstroke velocity, and it prolonged action potential duration and spontaneous cycle length. Most effects were reversible upon washout. In iso

A new electrocardiographic algorithm using retrograde P waves for differentiating atrioventricular node reentrant tachycardia from atrioventricular reciprocating tachycardia mediated by concealed accessory pathway.

OBJECTIVES: The purpose of this study was to use an electrocardiographic (ECG) algorithm, derived from the results of radiofrequency ablation, to discriminate atrioventricular node reentrant tachycardia (AVNRT) from atrioventricular reciprocating tachycardia (AVRT) and to localize a concealed accessory pathway, prospectively. BACKGROUND: Information about ECG criteria for differentiating AVNRT from AVRT is limited and has not been confirmed by surgical or catheter ablation.
