IMPORTANCE: Approximately 70% of individuals critically buried in avalanche debris die within 35 minutes as a result of asphyxial cardiac arrest. An artificial air-pocket device (AAPD) that separates inhaled air from exhaled air may delay the onset of severe hypoxemia and eventual asphyxia during snow burial.
OBJECTIVE: To investigate the efficacy of a new AAPD during snow burial in a supine position.
DESIGN, SETTING, AND PARTICIPANTS: This comparative effectiveness trial was performed in winter 2016 with data analysis in November 2016 and November 2022. Each trial used a simulated critical

AIM: The primary purpose of an avalanche airbag is to prevent burial during an avalanche. Approximately twenty percent of avalanche victims deploying airbags become critically buried, however. One avalanche airbag actively deflates three minutes after deployment, potentially creating an air pocket. Our objective was to evaluate this air pocket and its potential to prevent asphyxiation.
METHODS: Twelve participants were fitted with an airbag and placed prone on the snow. Participants deployed the airbag and were buried in 1.5 m of snow for 60 min with vital signs including oxygen saturation (S

BACKGROUND: Continuous positive airway pressure (CPAP) is the primary treatment for obstructive sleep apnea (OSA), but randomized evidence for arrhythmia reduction is lacking. Nocturnal arrhythmia avalanches (NAAs)-frequent, nonsustained arrhythmic episodes associated with cardiovascular mortality-may provide enhanced statistical power for detecting treatment effects.
OBJECTIVE: This study aimed to determine whether CPAP therapy reduces NAA burden in patients with OSA and whether this effect is dependent on treatment adherence.
METHODS: We conducted a secondary analysis of electrocardiograph

BACKGROUND: Accidental hypothermia with severe frostbite is a rare combination of injuries with a high risk for long-term sequelae. There are widely accepted recommendations for the management of avalanche victims and for frostbite treatment, but no recommendation exists for the treatment of frostbite in severe hypothermic patients, specifically for the management of hypothermic avalanche victims presenting with frostbite.
CASE PRESENTATION: We present a case of a previously healthy, 53-year-old male skier who was critically buried by an avalanche at 2300 m of altitude at an ambient temperatu

To provide guidance to the general public, clinicians, and avalanche professionals about best practices, the Wilderness Medical Society convened an expert panel to revise the evidence-based guidelines for the prevention, rescue, and resuscitation of avalanche and nonavalanche snow burial victims. The original panel authored the Wilderness Medical Society Practice Guidelines for Prevention and Management of Avalanche and Nonavalanche Snow Burial Accidents in 2017. A second panel was convened to update these guidelines and make recommendations based on quality of supporting evidence.

OBJECTIVE: There are few studies on oxidative stress in patients with post-traumatic stress disorder (PTSD). The thiol/disulfide homeostasis is a new marker of oxidative stress. This study aimed to examine the oxidative DNA damage and thiol/disulfide homeostasis after 6 months in patients who developed PTSD after an avalanche disaster and to compare them with healthy controls.
METHODS: A total of 31 patients who developed PTSD after 2 consecutive avalanche disasters that occurred in Van on February 4 and 5, 2020, resulting in 42 deaths, and 33 healthy volunteers were included in the study. Th

Wound-healing disorders are common complications in bilateral reduction mammaplasty. Traditional electrosurgical devices generate large amounts of thermal energy, often causing extensive thermal-related collateral tissue damage. This study aimed to retrospectively analyse the operative performance of a novel low-thermal plasma dissection device (pulsed electron avalanche knife-PEAK PlasmaBlade™) compared with traditional electrosurgery. Twenty patients with breast hypertrophy were randomly treated with PEAK PlasmaBlade™ on one breast and conventional electrosurgery on the other. Primary outcom

There has been a huge impact of the COVID-19 pandemic on global healthcare systems. Advisories across the world have appealed to people to stay at home and observe social distancing to slow down the pandemic. However it is important to realize as to how this is affecting acute cardiovascular care. Recent studies from Europe and USA have reported > 50% reduction in hospital admissions for ACS and declining rates of coronary interventions. The possible reasons for this noticeable reduction in patients with ACS/STEMI during the COVID-19 pandemic are multi-factorial. On one hand, it is due to chan

Snow sports in the backcountry have seen a steep increase in popularity, and therefore preparedness for efficient companion and organized rescue is important. While technical rescue skills are widely taught, there is a lack of knowledge regarding first aid for avalanche patients. The stressful and time-critical situation for first responders requires a rule-based decision support tool. AvaLife has been designed from scratch, applying mathematical and statistical approaches including Monte Carlo simulations. New analysis of retrospective data and large prospective field test datasets were used

The data and estimation methods presented in this article are associated with the research article, "Cut-off values of serum potassium and core temperature at hospital admission for extracorporeal rewarming of avalanche victims in cardiac arrest: a retrospective multi-centre study" [1]. In this article we estimate recommended cut-off values for in-hospital triage with respect to extracorporeal rewarming. With only 6 survivors of 103 patients collected over a period of 20 years the ability to estimate reliable threshold values is limited. In addition, because the number of avalanche victims is

Silicon avalanche photodiode (APD) single-photon detectors in space are continuously affected by radiation, which gradually degrades their dark count performance. From August 2016 to June 2023, we conducted approximately seven years (2507 days) of in-orbit monitoring of the dark count performance of APD single-photon detectors on the Micius Quantum Science Experimental Satellite. The results showed that due to radiation effects, the dark count growth rate was approximately 6.79 cps/day @ -24 °C and 0.37 cps/day @ -55 °C, with a significant suppression effect on radiation-induced dark counts at

BACKGROUND: Perioperative cardiac arrest after pancreaticoduodenectomy is uncommon but often catastrophic. Hypokalemia is a modifiable risk factor for malignant arrhythmia, particularly in insulin-treated diabetic patients exposed to bowel preparation and fasting. We report one of the few well-documented cases demonstrating a rapid decrease in serum potassium from a normal value to life-threatening hypokalemia within 72 h. CASE PRESENTATION: A 67-year-old woman with type 1 diabetes and hypertension underwent elective laparoscopic pancreaticoduodenectomy. The serum potassium concentration measu

INTRODUCTION: Mortality following complete avalanche burial remains high, primarily due to asphyxiation. Rebreathing-related CO₂ accumulation and oxygen depletion in the air pocket and surrounding snow are considered key mechanisms contributing to asphyxia. Artificial air pocket devices (AAPD) may mitigate this effect by separating exhaled from inhaled air. The present trial aimed to assess this effect in a simulated avalanche burial setting using continuous transcutaneous CO2 monitoring.
METHODS: In this randomized, double-blind, crossover trial, 11 healthy participants (3 female) underwent

BACKGROUND: In patients with acute coronary syndrome (ACS), current international guidelines recommend newer potent and predictable P2Y12 inhibitors as first-line treatment despite a greater bleeding risk compared with clopidogrel.
AIM: To determine if platelet function testing can predict bleeding in real-life patients with ACS treated with newer P2Y12 inhibitors.
METHODS: In this retrospective study, all consecutive adults admitted to the Lariboisière University Hospital for ACS, whatever the P2Y12 inhibitor prescribed, who had platelet function testing (vasodilator-stimulated phosphoprote

AIM: Evidence of existing guidelines for the on-site triage of avalanche victims is limited and adherence suboptimal. This study attempted to find reliable cut-off values for the identification of hypothermic avalanche victims with reversible out-of-hospital cardiac arrest (OHCA) at hospital admission. This may enable hospitals to allocate extracorporeal life support (ECLS) resources more appropriately while increasing the proportion of survivors among rewarmed victims.
METHODS: All avalanche victims with OHCA admitted to seven centres in Europe capable of ECLS from 1995 to 2016 were included
