Metabolic and bariatric surgery in vegetarians and vegans.

BACKGROUND: Vegetarianism is constantly increasing worldwide. However, the role of metabolic and bariatric surgery (MBS) in vegetarians/vegans is unclear as there is very limited data on this topic. The aim of this study was to evaluate MBS outcomes in vegetarians or vegans. METHODS: Retrospective analysis of a prospectively maintained database of a single-bariatric surgeon was carried out. All patients with a vegetarian or vegan lifestyle undergoing MBS were included. RESULTS: Eleven patients were included; none were lost to follow-up. Ten patients were women, the mean age and Body Mass Ind

Gastric pericardial fistula in a bariatric patient.

INTRODUCTION AND IMPORTANCE: Bariatric surgery has positively affected the lives many by improving their overall health with weight loss. Unfortunately, there are complications to bariatric surgery including anastomotic leaks, bowel obstructions, hernias, and ulcers. We present a case report of a gastric-pericardial fistula in the setting of a marginal ulcer of a patient with prior Roux-en-Y gastric bypass (RNYGB). CASE PRESENTATION: The patient is a 71 year old female with a history of diabetes, hypertension, hypothyroidism and arthritis requiring many prior joint replacements and leaving th

Research progress on bariatric surgery for hyperuricemia.

Hyperuricemia is closely linked to obesity. As lifestyles and dietary patterns evolve, the prevalence of hyperuricemia has been on the rise. Bariatric surgery, an efficacious intervention for morbid obesity and its associated metabolic disorders, not only manages the weight of patients with severe obesity but also exerts beneficial therapeutic effects on hyperuricemia and gout. Moreover, it demonstrates substantial efficacy against other obesity-related metabolic conditions. However, the dramatic fluctuations in serum uric acid levels and acute gouty attacks in the immediate postoperative peri

Cognition in patients with post-bariatric hypoglycemia.

OBJECTIVE: Bariatric surgery, a highly effective treatment for obesity and associated comorbidities, may improve cognition and brain volume in parallel with cardiometabolic function. However, some post-bariatric individuals develop post-bariatric hypoglycemia (PBH), which can be frequent and severe. The impact of recurrent hypoglycemia on cognition in PBH is unknown. The objective of this study was to determine whether individuals with PBH display reduced cognitive function compared with postsurgical counterparts without hypoglycemia. METHODS: Fourteen adults with a history of Roux-en-Y gastr

Perioperative Pain Management in Bariatric Anesthesia.

Weight loss (bariatric) surgery is the most commonly performed elective surgical procedure in patients with morbid obesity. In this review, we provide an evidence-based update on perioperative pain management in bariatric anesthesia. We mention some newer preoperative aspects-medical optimization, physical preparation, patient education, and psychosocial factors-that can all improve pain management. In the intraoperative period, with bariatric surgery being almost universally performed laparoscopically, we emphasize the use of non-opioid adjuvant infusions (ketamine, lidocaine, and dexmedetomi

Rhabdomyolysis as a rare complication of bariatric surgery.

Rhabdomyolysis after bariatric surgery is a quite rare occurrence with low recognition. Due to the breakdown of striated muscle fibers, creatine kinase and myoglobin are released into systemic circulation with variable effects on renal filtering functions. Herein, it was aimed to present a patient who de- veloped rhabdomyolysis following revision bariatric surgery. This 34-year-old male patient was admitted for bariatric surgery. He had had a gastric band surgery approximately six years ago, with regain of weight starting one year after surgery gradually reaching the previous weight level. Con

Aorta, the Innocent Bystander of Bariatric Banding.

INTRODUCTION: Laparoscopic gastric banding (LAGB) is a common bariatric operation performed for obesity. Complications of LAGB commonly evolve around device malfunction and physiological changes secondary to the gastric banding. Complications of LAGB involving the aorta are rare. A case of gastric band misplacement around the aorta and subsequent successful retrieval of the misplaced device is reported. REPORT: A 45 year old obese woman presented as an emergency with food bolus obstruction secondary to gastric banding inserted 10 years previously. Investigations revealed that her gastric band

Skin manifestations after bariatric surgery.

BACKGROUND: Skin signs observed in morbid obesity may change as the weight reduces, especially post-bariatric surgery (BaS). Data concerning the skin findings exclusively in post-BaS patients remain limited. METHODS: Seventy post-BaS patients were examined for cutaneous abnormalities. The patients were divided into those with successful weight loss (% excessive body weight loss (EBWL) of at least 50%) and a non-successful group (%EBWL < 50%). RESULTS: Forty-six patients with successful weight loss demonstrated a significantly lower prevalence of acanthosis nigricans on the neck, axillae and

[Caries explosion following bariatric surgery].

In a general dental practice, attention was drawn to a patient by numerous deep carious cavities following bariatric surgery. In the research literature, indications can be found for a negative influence of bariatric surgery on oral health. Risk factors for caries, such as the number of cariogenic bacteria in saliva, inadequate quantity and quality of saliva, gastrooesophageal reflux and vomiting may be become more pronounced. Additionally, dietary habits will change due to the altered anatomy of the gastro-intestinal tract. Negative effects of bariatric surgery on periodontal tissue have also

Bariatric surgery: a potential cure for asthma?

Asthma incidence and severity are increased in obese populations. Systematic reviews have shown benefit from weight-loss interventions on asthma outcomes, but the role of bariatric surgery is still unclear. In this review, cohorts of obese asthmatic patients undergoing bariatric surgery were examined regarding different asthma outcomes. The available data on patients who were followed up showed improvements in asthma control, exacerbation risk, asthma-related hospitalisation, medication use and airway hyperresponsiveness, with some patients not requiring further treatment for asthma. Follow-up

Considerations for bariatric surgery in patients with cirrhosis.

With the ever increasing global obesity pandemic, clinical burden from obesity related complications are anticipated in parallel. Bariatric surgery, a treatment approved for weight loss in morbidly obese patients, has reported to be associated with good outcomes, such as reversal of type two diabetes mellitus and reducing all-cause mortality on a long term basis. However, complications from bariatric surgery have similarly been reported. In particular, with the onslaught of non-alcoholic fatty liver disease (NAFLD) epidemic, in associated with obesity and metabolic syndrome, there is increasin

Outcome of bariatric surgery in older patients.

BACKGROUND: People are living longer than they were expected to 2 decades ago. Increased life expectancy and reduced mortality encompasses a simultaneous increase in the number of older adults with obesity that entails an increase of co-morbidities, such as diabetes, hypertension, cancer, and many other diseases. The aim of our study was to compare the outcomes of bariatric surgery in patients age ≥65 in comparison with younger patients. METHODS: This retrospective study compares bariatric surgeries performed in a private institution between the years 2013 and 2015. The study included 9044 pa

Nutritional Status of Bariatric Surgery Candidates.

Obesity is a global epidemic affecting populations globally. Bariatric surgery is an effective treatment for morbid obesity, and has increased dramatically. Bariatric surgery candidates frequently have pre-existing nutritional deficiencies that might exacerbate post-surgery. To provide better health care management pre- and post-bariatric surgery, it is imperative to establish the nutritional status of prospective patients before surgery. The aim of this study was to assess and provide baseline data on the nutritional status of bariatric candidates. A retrospective study was conducted on obese

Bariatric surgery and gene expression in the gut.

PURPOSE OF REVIEW: The current review provides an overview of recent literature on new findings related to bariatric surgery and gut gene expression. RECENT FINDINGS: Bariatric surgery modulates the expression of intestinal genes. Experimental and clinical investigations have demonstrated the association of gut rearrangement with changes in intestinal expression of genes related to glucose metabolism. Recent data suggest that bariatric surgery also affects expression of genes belonging to other pathways, including nutrient transporters and metabolism of vitamin B12, decreasing pathway-encodin

Obesity, Bariatric Surgery, and Vitamin D.

The high prevalence of obesity is a worldwide problem associated with multiple comorbidities, including cardiovascular diseases. Vitamin D deficiency with secondary hyperparathyroidism is common in obese individuals and can be aggravated after bariatric surgery. Moreover, there is no consensus on the optimal supplementation dose of vitamin D in postbariatric surgical patients. We present new data on the variability of 25(OH)D response to supplementation in postmenopausal obese women. It is important to recognize and treat vitamin D deficiency before bariatric surgery to avoid postoperative com

PURPOSE OF REVIEW: The purpose of this review is to provide an update of recent additions to our understanding of the prevalence of nutrient deficiencies and the potential role of preoperative weight loss in contributing to these deficiencies in obese individuals planning to undergo bariatric surgery. RECENT FINDINGS: Recent reports that have included bariatric surgery candidates from sites around the world have shown consistent deficiencies in a variety of nutrients. Although protein-energy malnutrition is uncommon preoperatively, micronutrient deficiencies occur commonly with multiple defic

Nutrition in Pregnancy Following Bariatric Surgery.

The widespread use of bariatric surgery for the treatment of morbid obesity has led to a dramatic increase in the numbers of women who become pregnant post-surgery. This can present new challenges, including a higher risk of protein and calorie malnutrition and micronutrient deficiencies in pregnancy due to increased maternal and fetal demand. We undertook a focused, narrative review of the literature and present pragmatic recommendations. It is advisable to delay pregnancy for at least 12 months following bariatric surgery. Comprehensive pre-conception and antenatal care is essential to achie
