Predictors of ear barotrauma in aircrews exposed to simulated high altitude.

INTRODUCTION: Ear barotrauma is an adverse effect related to hypobaric exposure. Ear, nose, and throat (ENT) diseases are risk factors for barotrauma in aircrews trained in a hypobaric chamber, but excluding affected subjects from exposure does not abolish the risk in asymptomatic trainees. We investigated other possible predictors, including history of ENT diseases, ENT clinical abnormalities, altitude, and subject's age. METHODS: After a complete ENT evaluation including otoscopy and tympanometry, 314 aircrews underwent hypobaric chamber training. Two altitude training profiles up to 35,000

Upper respiratory infections and barotraumas in commercial pilots: a retrospective survey.

INTRODUCTION: One of the most common causes of acute incapacitation among aircrew is barotrauma induced by pressure changes during descent in aviation. However, the incidence of ENT barotrauma during flight among commercial pilots with or without upper respiratory infections (URI) has not been described in detail. METHOD: There were 948 commercial pilots--equivalent to one-third of all commercial pilots in Denmark--visiting the Danish Aero Medical Centre during a 6-mo period who answered a questionnaire regarding signs and symptoms of URI and barotrauma incidence in relation to flying with a

[Current aspects of sinus barotrauma. The impact on treatment, prevention and fitness].

Among the barotraumatism which affects divers and airplane pilots, sinus barotraumatism is one of the less known and studied, as the scarceness of series published can testify. The interest of this study stands in doing an up to date synthesis (recap) of the bibliography, and to understand the "physio-pathologic" mechanism after the exploitation of series of 22 observations. That work will allow a better knowledge of therapeutic on the one hand, and of the "prophylactic" on the other hand.

Elevation of pulmonary artery pressure as a sign of pulmonary barotrauma (pneumothorax).

Pneumothorax occurs in approximately 25% of patients maintained on high levels of positive end-expiratory pressure. In several cases at our Institution, a sudden and rapid elevation in pulmonary artery pressure (PAP) monitored by use of the Swan-Ganz catheter accompanied the development of a pneumothorax. In the three cases described, the change in PAP was observed before a pneumothorax became otherwise clinically evident. Two of the three patients had marked restrictive lung disease associated with the adult respiratory distress syndrome and the third patient had large bilateral pleural effus

Pulmonary barotrauma in COVID-19: A systematic review and meta-analysis.

BACKGROUND: An ever-increasing number of studies have reported an increased incidence of spontaneous pulmonary barotrauma such as pneumothorax, pneumomediastinum, and subcutaneous emphysema in patients with COVID-19. We conducted this systematic review and meta-analysis to assess the value and significance of the available data. METHODS: A thorough systematic search was conducted to identify studies of barotrauma in hospitalized patients with COVID-19. Data analysis of case reports was done using a statistical package for the social sciences (SPSS) version 22, and meta-analysis was performed

Sphenoid sinus barotrauma in diving: case series and review of the literature.

About 50% of scuba divers have suffered from barotrauma of the ears and about one-third from barotrauma of paranasal sinuses. The sphenoid sinuses are rarely involved. Vital structures, as internal carotid artery and optic nerve, adjoin the sphenoid sinus. Thus, barotrauma could lead to serious neurologic disorders, including blindness. After searching the literature (Medline) and other sources (Internet), we present some cases of sphenoid sinus barotrauma, because these injuries may be underreported and misdiagnosed due to the lack of awareness and knowledge. Therefore, information is provide

The application of endoscopic sinus surgery to the treatment of recurrent sinus barotrauma.

Recurrent sinus barotrauma is an uncommon condition but it may terminate the career of an aviator. Sinus barotrauma occurs almost exclusively on descent and probably results from occlusion of the sinus ostia through a combination of mucosal disease and anatomical abnormalities. Traditional methods of treating sinus barotrauma have achieved mixed results so we have employed functional endoscopic sinus surgery (FESS) since 1990. The presentation and outcome of 39 patients with recurrent sinus barotrauma managed by FESS have been reviewed. Ninety-five per cent were able to resume their full flyin

Barotrauma in patients with severe coronavirus disease 2019-retrospective observational study.

BACKGROUND: Coronavirus disease 2019 (COVID-19) is an infectious disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus. Although it is known that the COVID-19 acute respiratory distress syndrome (ARDS) is associated with higher incidence of pulmonary barotrauma, unique mechanisms causing the aforementioned complication are still to be investigated. The goal of this research was to investigate the incidence of barotrauma among COVID-19 patients treated in the intensive care unit (ICU) and to examine different clinical outcomes among those subjects. METHODS: This

[Abdominal compartment syndrome by tension pneumoperitoneum secondary to barotrauma. Presentation case].

BACKGROUND: Pneumoperitoneum is defined as the existence of extraluminal air in the abdominal cavity. In 80-90% of cases is due to perforation of a hollow organ. However, in 10-15% of cases, it is nonsurgical pneumoperitoneum. OBJECTIVE: The case of a patient undergoing mechanical ventilation, developing abdominal compartment syndrome tension pneumoperitoneum is reported. CLINICAL CASE: Female, 75 years old asking for advise due to flu of long term duration. Given her respiratory instability, admission to the Intensive Care Unit is decided. It is then intubated and mechanically ventilated. C

Massive subcutaneous emphysema and pneumomediastineum after finger subtotal amputation with barotrauma.

BACKGROUND: Advancement of pressurized air through subcutaneous tissue after barotraumas involving skin laceration has been documented in the literature. The type and anatomic location of injury, amount of pressure, and time elapsed all play a role in determining the destination of the air advancing through tissues. OBJECTIVES: To report a case demonstrating the vascular system as the anatomic pathway for subcutaneous pressurized air resulting from an industrial accident. CASE REPORT: We present the case of a 28-year-old laborer wounded by an air valve blast. An enormous accumulation of air

Sinus barotrauma--late diagnosis and treatment with computer-aided endoscopic surgery.

Sinus barotrauma is usually easy to diagnose, and treatment achieves good results. We present two severe cases where delayed diagnosis caused significant morbidity. The signs and symptoms were atypical and neither the patients themselves, nor the initial examiners recognized that the onset of symptoms coincided with descent in a commercial airliner. CT and MRI scans of the brain were normal, but in both cases showed opafication of the sphenoid sinuses, which lead to the correct diagnosis. Subsequent surgical intervention consisting of endoscopic computer-aided surgery showed blood and petechia

Interleukin-2 receptor-specific fusion toxin inhibits barotrauma-induced arterial atherosclerosis.

Immunocytochemical analyses of human plaques and experimental arterial lesions have implicated activated lymphocytes and monocytes in the pathogenesis of atherosclerosis, as demonstrated by the expression of interleukin-2 (IL-2) membrane receptors and major histocompatibility complex class II epitopes. The objective is to determine if targeting these cells with an IL-2 receptor-specific chimeric toxin, DAB486-IL-2, can inhibit experimental post-angioplasty vascular neointimal thickening. Twenty-two atherogenically modeled rabbits were treated in vivo with DAB486-IL-2 (0.1 mg/kg per day i.v.; n

Endotracheal Oxygen Insufflation Associated with Life-Threatening Barotrauma during Apnea Testing.

Apnea testing is a standard method when diagnosing brain or more specifically brainstem death; however, it is imperative to acknowledge the potential for lung injury during this procedure and follow clinical practice guideline recommendations that may reduce the risk of complications. Two cases of barotrauma leading to pneumothorax occurred during apnea tests that used higher-than-recommended oxygen flow rates. Additional data are necessary to clarify the mechanism and incidence of this life-threatening complication.

Maxillary sinus barotrauma with infraorbital nerve paraesthesia after breath-hold diving.

Barosinusitis, or sinus barotrauma, is a sinonasal injury and/or inflammation that results when the aerated spaces of the nose and sinuses are exposed to an uncompensated change in ambient pressure. We describe a 19-year-old male diver who presented to our clinic on the fourth day following a breath-hold diving session. During descent on a constant weight monofin dive at the South Cyprus World Championship he began to experience symptoms due to the inability to equalise the pressure, particularly in the Eustachian tubes and middle ear cavities. He felt pain and pressure in the upper left half

Triplegic Stroke After Air Embolism From Bronchogenic Cyst Secondary to Barotrauma.

Emergency medical assistance is rare regarding air travel. Pulmonary barotrauma during air travel can occur in asymptomatic patients who have underlying intraparenchymal pulmonary pathology such as bullae or bronchogenic cysts. During aircraft travel the resultant decrease in pressure during the ascent can lead to expansion of cyst volume, culminating in tears and leakage of air into the surrounding vasculature and thus the potential for air embolism. We describe a case of massive cerebral air embolism secondary to pulmonary barotrauma in a previously asymptomatic patient.

Barotrauma after liquid nitrogen ingestion: a case report and literature review.

A 25-year-old man developed a gastric perforation after ingesting a homemade drink containing liquid nitrogen. Surgical repair had to be postponed to seek consultations with experts because the available practitioners in this case, including emergency physicians, surgeons, and anesthesiologists, had little experience and knowledge about the source of the patient's pneumothorax and subcutaneous emphysema. The patient ultimately underwent exploratory laparotomy with general anesthesia, considering that delaying the operation would lead to a longer duration of bacterial peritonitis and delay the
