Nail-gun injuries have become an increasingly prevalent source of penetrating intracranial trauma. Few cases of intracranial nail-gun injuries disturbing major cerebrovascular structures have been reported, and none entailing basilar artery involvement. We report here the case of a 51-year-old male with an intracranial nail-gun injury involving penetration of the distal basilar artery. Operative removal was accomplished under direct vision using a double concentric cranioorbital zygomatic osteotomy for a trans-Sylvian approach. We highlight the principles involved in removing foreign bodies pe

In view of the poor prognosis for untreated patients with basilar artery occlusion, rapid delivery of effective treatment has the highest priority. Early recanalization by intra-arterial thrombolysis can improve the outcome in some patients. Apart from this method, specialized centers are increasingly using mechanical devices and glycoprotein IIb/IIIa inhibitors to achieve recanalization. This review focuses on current developments in the diagnosis and treatment of acute basilar artery occlusion.

A case of ruptured middle saccular aneurysm arising from basilar artery fenestration is reported. Defects of the medial wall at the junctures of the fenestrated segments explain the association between vertebrobasilar aneurysms and basilar artery fenestration. A 47 year-old woman had a perimesencephalic subarachnoid hemorrhage. Immediate angiography revealed a ventral vertebrobasilar aneurysm and an endovascular coil occlusion was performed. Embryological development, pathogenesis and therapeutic difficulties are discussed.

The cause of transient global amnesia (TGA) remains controversial. Focal cerebral ischemia, seizure, venous congestion, and migraine have all been proposed as underlying mechanisms. We describe a patient presenting with typical TGA who two days later developed a posterior circulation stroke due to basilar artery occlusion. He was treated successfully with intra-arterial thrombolytic therapy. Shortly thereafter, he had recurrent basilar artery thrombosis and jugular vein thrombosis, and was found to have a mucinous adenocarcinoma believed to be causing a hypercoagulable state. We believe this c

We present a case of a patient with vertigo, disequilibrium and facial palsy. Nistagmus features pointed to a diagnosis of central vertigo. MRI showed a basilar aneurysm that compressed the brain stem and caused a stroke. We review the diagnosis.

Acute basilar artery occlusion has a mortality rate approaching 90%. The authors describe a case of acute basilar artery occlusion managed successfully with endovascular embolectomy. A 31-year-old man sought treatment for confusion, dysarthria, and right-sided weakness. He soon became unresponsive and was found to have a vertebral artery dissection and an associated basilar artery embolism. The dissection was managed with endovascular stenting, and the basilar artery embolus was removed with a clot retriever at 7 hours. The patient recovered without neurologic deficit.

Basilar artery (BA) fenestrations are the most frequently observed fenestrations of the cerebral arteries. Our goal was to examine the magnetic resonance (MR) angiographic incidence, location, and characteristic configuration of BA fenestration. Between April 1996 and March 2000, 600 cranial MR angiographies were performed at our institution. The majority of the patients examined had or were suspected to have cerebrovascular disease. We retrospectively reviewed these 600 MR angiograms. A 1.5 Tesla scanner was used in all studies, and maximum intensity projection (MIP) images obtained using the

PURPOSE: To report a case of optic tract compression caused by a dolichoectatic basilar artery.
DESIGN: Observational case report.
METHODS: A 74-year-old man with progressive loss of vision over 13 years and no other neurologic signs or symptoms was found to have bilateral optic nerve head pallor and a left homonymous hemianopia.
RESULTS: Magnetic resonance imaging and angiography revealed a severe dolichoectatic basilar artery compressing the right optic tract.
CONCLUSION: Basilar artery dolichoectasia may rarely cause compression of the optic tract and progressive visual loss.

BACKGROUND AND PURPOSE: Percutaneous angioplasty of the intracranial arteries still carries the risk of dissection, with acute closure and embolization. Stenting has been shown to improve the safety and durability of angioplasty in every circulatory bed in which it has been applied. However, stenting of the intracranial arteries has been limited by the availability of stents that can be reliably deployed intracranially.
METHODS: Twelve patients underwent elective stenting of the basilar artery after episodes of vertebrobasilar ischemia. In all patients, either medical therapy had failed or th

The reason cerebral edema in postpartum cerebral angiopathy (PPCA) occurs preferentially in the posterior brain is poorly understood. The authors present two patients with PPCA who showed vasospasm occurring earlier and more severely in the basilar artery than in the middle cerebral artery. Our patients demonstrate the difference in vascular change between the anterior and posterior cerebral vessels, explaining the susceptibility of the posterior brain to PPCA.

We report a case of a basilar artery migraine in a 17-year-old boy with transient CT and MR abnormalities after each of two migraine episodes. A repeat MR study 6 months after the last event showed complete resolution of the lesion. Transient abnormalities on brain images similar to those shown in our case have been reported in patients with migraine and other neurologic conditions and are most likely related to cerebral vasogenic edema.

OBJECTIVE AND IMPORTANCE: Distal basilar artery aneurysms represent 5 to 8% of intracranial aneurysms. It is crucial to preserve all of the basilar apex perforating vessels when dissecting in this region. This report is the first to describe a rostral basilar perforating artery that was the anatomic origin of a cerebral aneurysm.
CLINICAL PRESENTATION: A 56-year-old woman presenting with subarachnoid hemorrhage underwent initial four-vessel cerebral angiography that did not demonstrate the source of her hemorrhage. A follow-up cerebral angio-gram 9 days later suggested a small aneurysm in the

Twenty-two patients with ischemic stroke, as a single event, in the territory of basilar artery (BA) are reported. On the basis of the findings from computerized tomography (CT) and clinico-radiologic features, the authors propose that this heterogeneous entity--the basilar artery (BA) syndrome--can be divided into five subtypes. Type 1 (complete type), characterized by infarctions in the whole territory of BA, is incompatible with life; type 2, with extensive brain stem infarct, may result in a locked-in state; and type 3, with infarctions in part of the BA territory (incomplete form or "part

Two cases of aneurysm occurring on a fenestrated basilar artery are reported. One patient had a saccular aneurysm in the typical location at the vertebrobasilar junction at the proximal end of the fenestrated basilar segment. The aneurysm sac projected dorsally into the brain stem. The other patient had a blister-like aneurysm distal to a fenestration of the terminal basilar artery. The development of a distal aneurysm on a fenestrated basilar artery is predictable based on the reported morphology of the vessel wall in this vascular anomaly.

On the basis of clinical analysis and pathological findings in 25 patients dying after occlusion of the basilar artery the authors describe the main signs and course of the disease. Their observations made possible isolation of two types of the disease: 1) acute, with sudden consciousness disturbances leading to gradually increasing coma with flaccid extremities or decerebrate rigidity, stiff pupils not reacting to light and, frequently, with divergent squint, 2) subacute--with headache and hemiparaesthesiae and later quadriparesis and paresis of cranial nerves III to VII or IX, X, XII. The lower portion of the basilar artery is a site of significant disease, both aneurysmal and stenotic, but has been considered inaccessible via traditional surgical approaches. A baboon model was used to evaluate the feasibility of a direct surgical reconstruction via a transoral-transclival approach, utilizing temporary occlusion of the basilar artery. An arteriotomy was performed and either repaired primarily or a venous patch graft was sutured in place. All animals survived the surgery; one suffered fatal postoperative upper airway obstruction, but the other four were neurologically intact
