The epidemiology of fractures in infants--Which accidents are preventable?

INTRODUCTION: In children, fractures have a huge impact on the health care system. In order to develop effective prevention strategies exact knowledge about the epidemiology of fractures is mandatory. This study aims to describe clinical and epidemiological data of fractures diagnosed in infants. METHODS: A retrospective analysis of all infants (children<1 year of age) presenting with fractures in an 11 years period (2001-2011) was performed. Information was obtained regarding the location of the fractures, sites of the accident, circumstances and mechanisms of injury and post-injury care. R

Retinal and visual function in infants with non-accidental trauma and retinal hemorrhages.

PURPOSE: To investigate retinal function and visual outcomes in infants with retinal hemorrhages due to non-accidental trauma (NAT). METHODS: This is a retrospective review of full-field or multifocal electroretinogram (ERG) recordings, visual acuity in log minimum angle of resolution (logMAR), clinical status, and neuroimaging. Multifocal ERGs from the central 40° were compared to corresponding fundus imaging. Visual acuity was measured by Teller cards at follow-up. ERGs were compared to controls recorded under anesthesia. RESULTS: Sixteen children met inclusion criteria (14 recorded during

Optic nerve damage in shaken baby syndrome: detection by beta-amyloid precursor protein immunohistochemistry.

BACKGROUND: Rapid acceleration-deceleration of an infant's head during intentional shaking should in theory exert stretch or shear forces upon the optic nerves sufficient to cause axonal injury. beta-Amyloid precursor protein (beta-APP) immunohistochemistry recently has been shown to be a highly effective method for identifying diffuse axonal injury in the brains of infants with shaken baby syndrome. In this study, we investigated the utility of beta-APP in identifying optic nerve damage in infants who have sustained fatal whiplash shaking. MATERIALS AND METHODS: beta-Amyloid precursor protei

Imaging of epiphyseal injuries.

There have been many advances in the diagnosis and treatment of epiphyseal injuries in the 30 years since the publication of the landmark article by Drs Robert Salter and William Harris. They are the subject of this review. The anatomic features of the physis, epiphysis, and metaphysis are presented, and histologic studies of human and experimental physeal injuries are described. The recently recognized histologic, anatomic, and imaging characteristics of bone bridging of the physis resulting in growth disturbances are reviewed. Modification in and additions to the original Salter-Harris class

Childhood maltreatment, childhood social support, and child abuse potential in a Basque sample.

The present study obtained data on childhood physical and sexual abuse in a Basque sample in order to compare abuse rates with a similar study conducted in the United States. The study also examined the relationships between childhood physical and sexual abuse, childhood social support, and abuse potential in the Basque sample. Although the overall rates of physically abusive behaviors were similar in the Basque and US samples, the rates of physical abuse sequelae (bruises/welts, cuts/scratches, dislocations, burns, and/or bone fractures) were lower in the Basque sample. In addition, while the

Homicidal parents.

This paper describes a series of 13 cases of parents who have killed their children. A review of the literature suggests that child murder is infrequent and committed in most instances by the parents. Most attention has been directed to the universal phenomenon of child abuse. The killing of a child in our culture is viewed much more seriously than the killing of a newborn infant, legally defined as infanticide. Only a few authors have reported on the former, and their studies tend to demonstrate that a higher proportion of these crimes are perpetrated by mothers. Homicidal behaviour in parent

[A postmortem view on "pure" subdural hemorrhages in infants and toddlers].

In the last years, the discussion concerning the causes of infantile subdural hemorrhages became controversial. Many authors still suppose that child abuse is the predominant cause of such cases. On the other hand, reports presenting series of accidental cases were published, and the fear of an overdiagnosis of the shaken baby syndrome has been expressed. Our autopsy material concerning all lethal head injuries of infants and toddlers from 2 decades was reviewed. 17 of these 64 cases were characterized by the following: history of no trauma or only an insignificant event; children found dead o

Serial radiography in the infant shaken impact syndrome.

Certain CT and/or MRI abnormalities have been used medicolegally to time intracranial injuries from the infant shaken impact syndrome (ISIS). For example, parenchymal hypodensities on CT scans are said to arise only after 6-48 h have elapsed postinjury, and the presence of chronic or mixed subdural hematomas suggests injury that occured 1-4 weeks prior. However, these statements are based largely upon inference from data obtained in other conditions such as ischemic anoxic injury and chronic subdural hemorrhage in adults. Direct evidence about the evolution of intracranial injuries in infants

Understanding fatal child abuse.

Medical, social service and coroner reports were reviewed for 14 cases of fatal child abuse and neglect identified at a children's hospital from 1988-1992. Twelve cases involved physical abuse and two neglect. The median age was 6.5 months (range 24 days to 3 years). Six families (43%) had prior protective service involvement; however, four of the referrals involved a sibling. Only two of 12 physical abuse victims had a history of a prior suspicious injury. Clinical and postmortem findings are presented. The cause of death in all physically abused patients was blunt impact head injury; one als

Interpersonal relationships and child-rearing practices in 214 parents of battered children.

The self-reported child-rearing practices of 214 parents of battered babies were characterized in a few but not all respects by demanding behaviour which exceeded that to be expected in relation to their social class and age. Inconsistency in child management was noted in the comparison between lack of demonstrativeness and emotional over-involvement, and between physical punishment and a tendency to be lax in the supervision of the child, and was reminiscent of parents of delinquents. Unhappiness and hostility in relationships with members of their families of origin, with unsupporting partne

Intimate violence in the lives of homeless and poor housed women: prevalence and patterns in an ethnically diverse sample.

In a study of 436 homeless and poor housed mothers, nearly two-thirds reported severe physical violence by a childhood caretaker, 42% reported childhood sexual molestation, and 61% reported severe violence by a male partner. Comparisons of homeless and housed women are presented, and implications for prevention and intervention are discussed in light of recent welfare reform legislation.

Unilateral retinal hemorrhages in documented cases of child abuse.

PURPOSE: To describe the occurrence of unilateral retinal hemorrhages in four cases of documented child abuse, including a case in which retinal hemorrhages were an incidental finding on routine examination. METHODS: Case reports. RESULTS: Three children, 5 to 17 months of age, with suspected child abuse had fundus examinations with a dilated pupil as part of their evaluation. An additional child, 6 months of age, received fundus examination with a dilated pupil as part of follow-up for regressed retinopathy of prematurity. Each of the four children had extensive retinal or preretinal hemorr

The floppy infant: a new manifestation of nonaccidental injury.

A 9-week-old infant who sustained a nonaccidental cervical spine fracture with spinal cord injury is reported. Such an injury has not been previously reported in such a young child. The clinical features raised initial difficulties in diagnosis, and management was hampered by the lack of recorded experience of such an injury in such a young infant. Conservative management was employed and the infant has made a partial recovery. The distinctive features of spinal injury in young children are discussed.

Unexpected non-HIV causes of death in children born to HIV-infected mothers. Pediatric Pulmonary and Cardiac Complications of Vertically Transmitted HIV Infection Study Group.

INTRODUCTION: A high incidence of sudden, unexplained deaths in infants born to HIV-infected mothers has been noted in several epidemiologic studies. During the course of a prospective study of heart and lung disease in children born to HIV-infected mothers, we noted that of 5 unexpected non-HIV-related deaths, 4 were attributed to traumatic events. METHODS: The Pediatric Pulmonary and Cardiac Complications of Vertically Transmitted HIV Infection (P2C2) study is a multicenter, prospective investigation of the incidence of heart and lung disease in HIV-infected children. A total of 805 childre

Shaken baby syndrome: identification, intervention, and prevention.

More than 1 million children are severely abused annually. Shaken baby syndrome (SBS), one example of physical abuse, is a leading cause of morbidity and mortality in infants. There is a national annual incidence in the United States of 750 to 3,750 cases of SBS. Shaken baby syndrome is defined as vigorous manual shaking of an infant who is being held by the extremities or shoulders, leading to whiplash-induced intracranial and intraocular bleeding with no external signs of head trauma. Shaken baby syndrome should be suspected in infants with a wide spectrum of clinical signs and symptoms. The

Initial predictive factors of outcome in severe non-accidental head trauma in children.

OBJECT: The aim of this study is to evaluate the outcome of young children hospitalized for non-accidental head trauma in our PICU, to evaluate PRISM II score in this sub-population of pediatric trauma and to identify factors that might influence the short-term outcome. MATERIALS AND METHODS: Files of all children less than 2 years old with the diagnosis of non-accidental head trauma over a 10-years period were systematically reviewed. We collected data on demographic information, medical history, clinical status, and management in the PICU. Three severity scores were then calculated: PRISM I

Maternal and paternal filicides: a retrospective review of filicides in Finland.

The purpose of this retrospective study was to illustrate the differences in maternal and paternal filicides in Finland during a 25-year period. In the sample of 200 filicides [neonaticides (n = 56), filicide-suicides (n = 75), other filicides (n = 69)], the incidence was 5.09 deaths per 100,000 live births: 59 percent of filicides were committed by mothers, 39 percent by fathers, and 2 percent by stepfathers. The mean age of the maternal victims (1.6 y) was significantly lower than that of the paternal victims (5.6 y), but no correlation between the sex of the victim and the sex of the perpet
