Renal handling of urate in a patient with familial juvenile gouty nephropathy.

We encountered a case of familial juvenile gouty nephropathy (FJGN) with an autosomal dominant transmission pattern. Hyperuricemia in the propositus was caused by renal underexcretion of urate although his erythrocyte purine enzyme was normal. A renal biopsy specimen from the propositus showed interstitial fibrosis with tubular atrophy. On pyrazinamide and probenecid tests, the tubular secretion of urate selectively decreased without changes in either presecretory or postsecretory reabsorption of urate when his renal function was normal. Probenecid increased the urinary urate excretion and Cur

Mechanisms and Targeted Therapeutic Strategies for Inflammation and Renal Fibrosis in Hyperuricemic Nephropathy.

Hyperuricemia (HUA) is a growing global health issue driven by economic development and lifestyle changes. Approximately 75% of uric acid in humans is excreted renally. Excess uric acid deposits in renal tissues, promoting tubulointerstitial fibrosis and leading to hyperuricemic nephropathy (HN), which is characterized by urate crystal deposition, chronic interstitial nephritis, and renal fibrosis. The pathogenesis and progression of HN involves dysregulated activation of multiple signaling pathways, including MAPK, Nrf2/HO-1/NQO1, PI3K/AKT, and ASK1/JNK/c-Jun pathways, which facilitate diseas

Targeting PIEZO1-TMEM16F Coupling to Mitigate Sickle Cell Disease Complications.

A deeper understanding of sickle cell disease (SCD) pathophysiology is critical for identifying novel therapeutic targets. A hallmark of SCD is abnormal phosphatidylserine (PS) exposure on sickle red blood cells (RBCs), which contributes to anemia, thrombosis, and vaso-occlusive crises (VOC). However, the mechanisms underlying this excessive PS exposure remain unclear. Here, we identify TMEM16F, a Ca2+-activated lipid scramblase, as a key mediator of PS exposure downstream of Ca2+ influx through the mechanosensitive channel PIEZO1 in sickle RBCs. Electrophysiology, imaging, and flow cytometry

Novel Human Urate Transporter 1 Inhibitors as Hypouricemic Drug Candidates with Favorable Druggability.

Lesinurad, a human urate transporter 1 (URAT1) inhibitor approved as a medication for the treatment of hyperuricemia associated with gout in 2015, can cause liver and renal toxicity. Here, we modified all three structural components of lesinurad by applying scaffold hopping, bioisosterism, and substituent-decorating strategies. In a mouse model of acute hyperuricemia, 21 of the synthesized compounds showed increased serum uric acid (SUA)-reducing activity; SUA was about 4-fold lower in animals treated with 44, 54, and 83 compared with lesinurad or benzbromarone. In the URAT1 inhibition assay,

Restoration of the uricosuric effect of probenecid after triglycylvasopressine administration in a gouty patient.

A 35-year-old patient with severe gout and mild renal insufficiency presented very low urinary urate excretion. Volume expansion induced by fludrocortisone combined or not with a uricosuric drug (Benzbromarone) was unable to significantly increase his urate excretion. A combined Probenecid (PB) and Pyrazinamide (PZA) test was performed. These drugs being considered to affect renal tubular reabsorption or secretion. No significant modification of uric acid fractional excretion (FE.uric acid) was observed after PB and PZA. When the same test was performed after the administration of Triglycyl-ly

Elevated uric acid induces erectile dysfunction in rats by interacting with MLCK and inhibiting its ubiquitin-mediated degradation.

Erectile dysfunction, a precursor to cardiovascular diseases, is linked to metabolic disorders like gout. However, whether hyperuricemia plays a direct causative role in erectile dysfunction is unclear. Here we show that clinical data from young patients (24-49 years) reveal an over 2.5-fold increased erectile dysfunction risk with elevated serum uric acid. In spontaneous hyperuricemia rats with Urate oxidase gene knockout, hyperuricemia impairs erectile function early (20 weeks) without other metabolic comorbidities. Mechanistically, uric acid enters corpus cavernosum smooth muscle cells, int

Discovery of multi-target anti-gout agents from Eurycoma longifolia Jack through phenotypic screening and structural optimization.

Developing anti-gout medications that simultaneously reduce uric acid and exert anti-inflammatory effects represents a critical breakthrough for managing gout progression. Natural products with polypharmacological properties offer promising leads for drug discovery. In this study, β-carboline-1-propionic acid, a bioactive constituent of Eurycoma longifolia Jack, served as the starting point for drug design. Guided by a dual-target pharmacophore model, we design and synthesize 64 derivatives. Through systematic screening, 32 emerges as a drug candidate, demonstrating potent uric acid-lowering a

Deciphering and disrupting PIEZO1-TMEM16F interplay in hereditary xerocytosis.

Cell-surface exposure of phosphatidylserine (PS) is essential for phagocytic clearance and blood clotting. Although a calcium-activated phospholipid scramblase (CaPLSase) has long been proposed to mediate PS exposure in red blood cells (RBCs), its identity, activation mechanism, and role in RBC biology and disease remain elusive. Here, we demonstrate that TMEM16F, the long-sought-after RBC CaPLSase, is activated by calcium influx through the mechanosensitive channel PIEZO1 in RBCs. PIEZO1-TMEM16F functional coupling is enhanced in RBCs from individuals with hereditary xerocytosis (HX), an RBC

Gout and organ transplantation.

Acute and chronic gout are common complications following organ transplantation. Risk factors include those shared with the general population (eg, diuretic use) and transplant-specific risk factors (eg, cyclosporine). Clinical features of gout are similar to those seen in the general population, although tophi may be more common. A definitive diagnosis requires demonstration of monosodium urate crystals within synovial fluid or tophi. Treatment is often empiric, although a poor response should prompt joint aspiration to exclude septic arthritis. Corticosteroids are commonly used to treat acut

[Pharmacological agents affecting uric acid metabolism].

A large number of pharmacological agents affect the serum concentration of uric acid. Some drugs raise serum uric acid level by an increase in uric acid production or a decrease in uric acid excretion, while others lower serum uric acid level by a decrease in uric acid production or an increase in uric acid excretion. In addition, some drugs show so-called biphasic effect; ie, hyperuricemic in lower doses but hypouricemic in higher doses. Hyperuricemic agents contribute to gout and/or urate nephropathy. Among them, pyrazinamide is often used to determine the defective site(s) of uric acid tran

[Summary of the Dutch College of General Practitioners' "Gout" Standard].

The typical form of acute gout can be clinically diagnosed. The term 'complicated gout' is used if there are more than three acute attacks of gout per year, tophi or urate stones in the urinary tracts. In the case of recurrent probable acute gout, a diagnostic fine needle aspirate from the joint during an attack is indicated. First choice treatment of acute gout consists of NSAIDs. Colchicine is the second choice treatment and the third choice treatment consists of corticosteroids. Excessive alcohol use should be limited. Treatment of chronic gout depends on the uric acid excretion in the 24-h

Evaluation of Treatment Success Rate Among Antihyperuricemic Using Real-World Data.

In this study, treatment and the serum uric acid (UA) level were compared using medication history generated by prescription order records of antihyperuricemic to examine the treatment success rate. We examined the treatment success rate among these patients based on the serum UA level during 120-180 days after the initiation of treatment, which was set as the endpoint. The number of patients whose UA level before the start of treatment was > 8.0 mg/dL but decreased to < 6.0 mg/dL after the treatment, which is the target treatment success, was 92 (success rate of 14.2%), 50 (53.2%), 76 (41.5%)

Virtual and in vitro bioassay screening of phytochemical inhibitors from flavonoids and isoflavones against xanthine oxidase and cyclooxygenase-2 for gout treatment.

Synthetic drugs such as allopurinol and benzbromarone are commonly used to treat the complex pathogenesis of gout, a metabolic disease that results from an inflammation of the joints caused by precipitation of uric acid. We seek to discover novel phytochemicals that could treat gout, by targeting the xanthine oxidase and cyclooxygenase-2 enzymes. In this study, we report the screening of nine compounds of flavonoids from the ZINC and PubChem databases (containing 2092 flavonoids) using the IGEMDOCK software tool against the xanthine oxidase and cyclooxygenase-2 3D protein structures. Pseudotumor of gout in the patella of a kidney transplant recipient.

BACKGROUND: A 33-year-old renal transplant recipient presented with painless swelling of the right knee. Physical examination revealed an impressive knee joint effusion with no signs of inflammation. The patient did not remember a recent trauma, but he mentioned a strain 3 years earlier; radiographic findings had been normal at that time. The patient had suffered from end-stage renal disease due to chronic glomerulonephritis and had previously undergone two transplantations. At presentation, his kidney function was stable under treatment with ciclosporin, azathioprine and steroids. INVESTIGAT

Subjective sensation of heaviness in gout patients.

The main objective of the treatment of gout is to protect the major organs. Almost all gout patients require medication throughout their lives. Gout patients are asymptomatic during intercritical periods, even if they are not receiving antihyperuricemic drugs. Some patients, therefore, misinterpret that their gout is cured and may cease taking medication of their own accord. Thirty-two gout patients who had received continuous treatment with antihyperuricemic drugs for more than 3 months were selected. The sensation of heaviness was relieved by antihyperuricemic drugs in 14 (44%) of 32 patient
