Bronchial-biliary fistula secondary to cholangiocarcinoma: long-term efficacy of biliary self-expandable metal stent.

A 77-year-old female with previous medical history of non-cirrhotic chronic hepatitis B and hepatocellular carcinoma treated with sequential partial hepatectomy followed by transarterial chemoembolization complained of pruritus and jaundice. Magnetic resonance cholangiopancreatography revealed a peri-hilar ill-defined stenosing lesion suggestive of malignancy. Endoscopic retrograde cholangiopancreatography with cholangioscopy confirmed a circumferential peri-hilar stenosis with fragile mucosa and tortuous dilated vessels and biopsies of this area were consistent cholangiocarcinoma. After 3 mon

Multiple Hepatic Hydatid Cysts With Cysto-Biliary Communication and Broncho-Biliary Fistula: A Pediatric Case Report.

Hydatid cyst disease is a health problem in some developing countries occurring in groups with permanent contact with dogs. It is a parasitic disease caused by Echinococcus Granulosus. The liver is the most common organ infected. If not treated appropriately; it may lead to developing life-threatening complications. Its rupture into bile ducts is the most frequent one, while hepatopulmonary fistula is a rare complication. We discuss the case of a child presenting those 2 complications as we emphasize the importance of early diagnosis and the role of the radiologist.

Utility of stent-grafts in treatment of porto-biliary fistula.

A porto-biliary fistula causing hemobilia is a known complication of percutaneous transhepatic biliary drainage (PTBD). We present two patients with hemobilia secondary to porto-biliary fistula, treated successfully by percutaneous placement of stent-grafts. In one case, the stent-graft was placed in the bile duct, and in the other case, it was placed in the intrahepatic portal vein branch. Hemobilia stopped and there were no complications except a small area of hepatic infarction, distal to the stent-graft in the portal vein.

[Methods of treatment for suppurative biliary fistulas of amebic etiology].

Experience in the treatment of 43 patients with pyobiliary fistulas which formed after abscessotomy in amebic abscess of the liver is generalized. The fistulas occurred after open drainage in 29 patients, spontaneous opening of the abscess on the external surface of the body in 8, and after operation for transdiaphragmatic opening of a hepatic abscess in 6 patients. Two groups of patients were distinguished according to the method of completion of the operation. Among the 24 patients of the first clinical group 9 were subjected to drainage of the residual cavity with excision of the fistula, b

Treatment of fistulas of the biliary tract by endoscopic insertion of endoprostheses.

Five patients with fistulas of the biliary tract are reported; three had fistulas caused by operative injury and two, by accidental blunt abdominal trauma. All patients were treated successfully by endoscopic transpapillary insertion of biliary endoprostheses. The endoprostheses were removed after four to 17 weeks in all patients, and no evidence of biliary leakage was revealed by repeat cholangiograms and other clinical follow-up study. This effective, simple and safe method allows the fistula to heal and is a good alternative to surgical treatment.

[Postoperative external biliary fistulas: personal case series].

Over the period 1974-1986, five patients with external biliary fistulas were admitted to our clinic. The fistulas set in as complications of surgical operations due in 3 cases to a Kehr drainage of the common bile duct, in 1 case to drainage of a subhepatic abscess and in 1 case to pancreatic hydatid cyst. The causes of onset of the fistulas are reported as well as the therapy adopted in each case.

Gallstone ileus with two separate biliary-enteric fistulae.

A patient with gallstone ileus associated with two separate biliary-enteric fistulae has been reported. The patient had exploratory laparotomy with removal of the gallstone from the distal ileum, repair of the two fistula tracts, and cholecystectomy. The pathology report showed adenocarcinoma of the gallbladder. The patient was discharged 14 days after surgery. It is important to point out that the diagnosis was not suspected prior to surgery, although the patient was elderly with known cholelithiasis, no prior abdominal surgery, and clinical symptoms of bowel obstruction. The plain abdominal

[Spontaneous bilio-biliary fistulae. Experience in 18 cases].

Six cholecyst-hepatic, nine cholecyst-hepatic-choledochic, and three cholecyst-choledochic spontaneous fistulae were observed in subjects (89% females) aged 28-76 (average 52) yr, forming 0.9% of a series of bile duct operands. Lithiasis is seen as the origin of these forms. A large calculus wedged in the gallbladder basis leads, due to decubitus, to symphysis between the basis and the main bile duct. Later, infection results in necrosis of the walls and gives rise to the fistula. The symptoms are not pathognomonic. Clinical diagnosis is virtually impossible and the condition is usually an une

Surgery for colorectal liver metastases: Predictive factors of biliary fistula.

BACKGROUND: Colorectal liver metastases (CRLM) are the most common secondary site of colorectal cancer. Hepatic resection remains the potentially curative standard treatment, but postoperative morbidity remains substantial, with biliary fistula representing the most frequent and clinically significant specific complication. This study aimed to identify predictive factors of biliary fistula following CRLM surgery. METHODS: A retrospective single-center study was conducted including 129 patients who underwent surgery for CRLM at Mongi Slim Hospital, La Marsa, between January 2020 and December 2

Atypical Biliary Fistula After Revisional Bariatric Surgery: A Case Report.

Obesity affects about 39% of the global population, and bariatric surgery is a leading treatment option. As the number of primary bariatric procedures increases, so does the demand for revisional bariatric surgeries (RBSs), which are often more complex and associated with increased risks of complications. This case report details an unusual complication of RBS that surgeons should be aware of. A 41-year-old male patient with morbid obesity, after an initial sleeve gastrectomy, experienced weight regain due to gastric pouch dilatation and underwent conversion to mini-gastric bypass. Postoperati

Endoscopic Treatment of Duodenocolic Fistula Caused by Migrating Biliary Stents.

BACKGROUND Endoscopic biliary stent implantation is a recognized and effective method for the treatment of benign and malignant diseases of the bile duct and pancreas, ensuring smooth bile drainage. Currently, stent migration is considered a long-term and complex process, and in most cases, stents are removed through endoscopy or expelled from the body through the intestinal cavity. In rare cases, stents lead to formation of duodenocolic fistulas. CASE REPORT We report a case of duodenal colon fistula caused by a biliary stent penetrating the duodenum and entering the ascending colon. We remov

[Biliary-duodenal fistula following radiofrequency ablation therapy for hepatocellular carcinoma].

Hepatocellular carcinoma (HCC) is one of the most common malignant neoplasms occurring worldwide. Surgical resection currently provides the best chance of long-term tumor free survival, but the most HCCs are not candidates for surgical excision due to poor liver function or poor medical background. Numerous noninvasive alternatives to surgical resection have been introduced to treat liver cancers. Radiofrequency thermal ablation has begun to receive much attention as an effective and minimally invasive technique for the local control of HCC. The biliary system related complications after radio

Internal biliary fistula due to cholelithiasis: a single-centre experience.

AIM: To discuss about the perioperative problems encountered in patients with internal biliary fistula (IBF) caused by cholelithiasis. METHODS: In our hospital, 4,130 cholecystectomies were carried out for symptomatic cholelithiasis from January 2000 to March 2004 and only 12 patients were diagnosed with IBF. The perioperative data of these 12 IBF patients were analyzed retrospectively. RESULTS: The incidence of IBF due to cholelithiasis was nearly 0.3%. The mean age was 57 years. Most of the patients presented with non-specific complaints. Only two patients were considered to have IBF when

[Persistent external biliary fistula after surgery for hepatic hydatid cyst].

Post-operative persistent biliary fistulae due to hydatid cysts are difficult to manage. We report 3 cases of external biliary fistulae complicating surgery for hepatic hydatid cyst (2 cases) and intraperitoneal hydatid cyst (1 case). Endoscopic sphincterotomy was successful with closure of the fistulae in all patients. This procedure should be the first-line treatment for post-operative external biliary fistulae related to liver hydatid disease. Usually difficult and haemorrhagic surgery can thus be avoided.
