Network generalized estimating equations for complexly correlated data with applications to cluster randomized trials.

Estimating parameters corresponding to mean outcomes and their intricate association structures in cluster randomized trials (CRTs) can pose significant methodological challenges. This paper introduces a novel framework that leverages network concepts to represent complex dependency structures and estimate these parameters using generalized estimating equations (GEE). We focus on modeling complex correlation structures by partitioning observations into potentially overlapping groups of interrelated data, where observations are assumed locally exchangeable within each group. This network GEE fr

Use of instrumental variables in electronic health record-driven models.

Precision medicine presents various methodological challenges whose assessment requires the consideration of multiple factors. In particular, the data multitude in the Electronic Health Records poses interoperability issues and requires novel inference strategies. A problem, though apparently a paradox, is that highly specific treatments and a variety of outcomes may hardly match with consistent observations (i.e., large samples). Why is it the case? Owing to the heterogeneity of Electronic Health Records, models for the evaluation of treatment effects need to be selected, and in some cases, t

Influence diagnostics for count data under AB-BA crossover trials.

This paper aims to develop diagnostic measures to assess the influence of data perturbations on estimates in AB-BA crossover studies with a Poisson distributed response. Generalised mixed linear models with normally distributed random effects are utilised. We show that in this special case, the model can be decomposed into two independent sub-models which allow to derive closed-form expressions to evaluate the changes in the maximum likelihood estimates under several perturbation schemes. The performance of the new influence measures is illustrated by simulation studies and the analysis of a r

Regression analysis of clustered interval-censored data with informative cluster size.

Interval-censored data are commonly found in studies of diseases that progress without symptoms, which require clinical evaluation for detection. Several techniques have been suggested with independent assumption. However, the assumption will not be valid if observations come from clusters. Furthermore, when the cluster size relates to response variables, commonly used methods can bring biased results. For example, in a study on lymphatic filariasis, a parasitic disease where worms make several nests in the infected person's lymphatic vessels and reside until adulthood, the response variable o

Edward B. Lewis: His radiation leukemia article in Science journal: A turning point in cancer risk assessment.

Edward B. Lewis published what is widely considered the most influential article in the history of cancer risk assessment. He was successful in changing the radiation risk assessment focus from transgenerational birth defect concerns to cancer due to his success in getting his article published in Science journal, receiving a highly laudatory editorial endorsement, and quickly becoming a prominent speaker to the U.S. Congress and the focus of a major story in Life magazine, amongst other notably rapid recognitions. However, what is generally not appreciated is that Lewis had no education and t

Latent Class Analysis: Insights about design and analysis of schistosomiasis diagnostic studies.

Various global health initiatives are currently advocating the elimination of schistosomiasis within the next decade. Schistosomiasis is a highly debilitating tropical infectious disease with severe burden of morbidity and thus operational research accurately evaluating diagnostics that quantify the epidemic status for guiding effective strategies is essential. Latent class models (LCMs) have been generally considered in epidemiology and in particular in recent schistosomiasis diagnostic studies as a flexible tool for evaluating diagnostics because assessing the true infection status (via a go

Augmented therapeutic efficacy of 5-fluorouracil in conjunction with lantibiotic nisin against skin cancer.

Chemotherapy, a gold standard for treating most of the cancers, involves drastic side-effects and multidrug resistance. An attractive alternative is development of combination therapy employing antimicrobial peptides with chemotherapeutic drugs. In vivo studies: Anti-cancer therapeutic efficacy of 5-fluororuacil (5-FU) in conjunction with nisin (50 mg/kg body weight) was evaluated against murine skin cancer, in terms of tumor biostatistics, histopathology, electron microscopy, infrared spectroscopy and transcriptional studies. Definition of normal blood pressure at 11 to 14 weeks' gestation according to risk.

OBJECTIVE: To define normal blood pressure at first trimester of pregnancy by performance for pre-eclampsia prediction. METHOD: A cohort study in singleton pregnancies, according to the competing risks model for pre-eclampsia. Blood pressure was measured according to a standardized method at 11 to 14 weeks' gestation. RESULTS: Among 797 pregnancies included, there were 40 (5.0%, IC95 3.6; 6.4) deliveries with pre-eclampsia. The systolic, diastolic, and mean arterial blood pressures were respectively of 101(9), 68(8) and 79(8) mmHg in pregnant women who did not develop pre-eclampsia, against

Risk factors for the development of hospital-acquired pediatric venous thromboembolism-Dealing with potentially causal and confounding risk factors using a directed acyclic graph (DAG) analysis.

INTRODUCTION: Hospital-acquired venous thromboembolism (HA-VTE) in children comprises multiple risk factors that should not be evaluated separately due to collinearity and multiple cause and effect relationships. This is one of the first case-control study of pediatric HA-VTE risk factors using a Directed Acyclic Graph (DAG) analysis. MATERIAL AND METHODS: Retrospective, case-control study with 22 cases of objectively confirmed HA-VTE and 76 controls matched by age, sex, unit of admission, and period of hospitalization. Descriptive statistics were used to define distributions of continuous va

How to Deal With Temporal Relationships Between Biopsychosocial Variables: A Practical Guide to Time Series Analysis.

OBJECTIVE: Longitudinal data allow for conclusions about the temporal order of events and interactional dynamics between several processes. The aim of this article is to provide a concise and pragmatic description of time series analyses (TSAs) of patient samples with numerous (or daily) repeated biological, behavioral, or psychological measurements. In addition, the article demonstrates how to implement the described analyses with the software R. METHODS: To illustrate the concrete application of the time series method, we use two case series of patients with anorexia nervosa. Upon awakening

Z-scores-based methods and their application to biological monitoring: an example in professional soccer players.

The clinical and biological follow-up of individuals, such as the biological passport for athletes, is typically based on the individual and longitudinal monitoring of hematological or urine markers. These follow-ups aim to identify abnormal behavior by comparing the individual's biological samples to an established baseline. These comparisons may be done via different ways, but each of them requires an appropriate extra population to compute the significance levels, which is a non-trivial issue. Moreover, it is not necessarily relevant to compare the measures of a biomarker of a professional

Second Order Segmented Polynomials for Syphilis and Gonorrhea Prevalence and Incidence Trends Estimation: Application to Spectrum's Guinea-Bissau and South Africa Data.

This paper presents two approaches to smoothing time trends in prevalence and estimating the underlying incidence of remissible infections. In the first approach, we use second order segmented polynomials to smooth a curve in a bounded domain. In the second, incidence is modeled instead and the prevalence is reconstructed using the recovery rate which is assumed to be known. In both approaches, the number of knots and their positions are estimated, resulting in non-linear regressions. Akaike Information Criterion is used for model selection. The method is illustrated with Syphilis and Gonorrhe

Targeting pre-exposure prophylaxis among men who have sex with men in the United States and Peru: partnership types, contact rates, and sexual role.

BACKGROUND: We aim to identify optimal strategies for deploying pre-exposure prophylaxis among men who have sex with men (MSM) in the United States and Peru to maximize population-level effectiveness in an efficient manner. We use epidemic models to simulate the impact of targeting strategies. Most studies have focused on targeting either the general population or high-risk MSM. Alternative strategies, including serodiscordant couples, may better balance effectiveness and efficiency. METHODS: We use dynamic stochastic sexual network models based on exponential-family random graph modeling, pa

A joint frailty model to estimate the recurrence process and the disease-specific mortality process without needing the cause of death.

In chronic diseases, such as cancer, recurrent events (such as relapses) are commonly observed; these could be interrupted by death. With such data, a joint analysis of recurrence and mortality processes is usually conducted with a frailty parameter shared by both processes. We examined a joint modeling of these processes considering death under two aspects: 'death due to the disease under study' and 'death due to other causes', which enables estimating the disease-specific mortality hazard. The excess hazard model was used to overcome the difficulties in determining the causes of deaths (unav

Cumulative sum control charts for monitoring geometrically inflated Poisson processes: An application to infectious disease counts data.

In this work, we study upper-sided cumulative sum control charts that are suitable for monitoring geometrically inflated Poisson processes. We assume that a process is properly described by a two-parameter extension of the zero-inflated Poisson distribution, which can be used for modeling count data with an excessive number of zero and non-zero values. Two different upper-sided cumulative sum-type schemes are considered, both suitable for the detection of increasing shifts in the average of the process. Aspects of their statistical design are discussed and their performance is compared under v

Nonidentical twins: Comparison of frequentist and Bayesian lasso for Cox models.

One important task in translational cancer research is the search for new prognostic biomarkers to improve survival prognosis for patients. The use of high-throughput technologies allows simultaneous measurement of genome-wide gene expression or other genomic data for all patients in a clinical trial. Penalized likelihood methods such as lasso regression can be applied to such high-dimensional data, where the number of (genomic) covariables is usually much larger than the sample size. There is a connection between the lasso and the Bayesian regression model with independent Laplace priors on t

A comparison of 12 algorithms for matching on the propensity score.

Propensity-score matching is increasingly being used to reduce the confounding that can occur in observational studies examining the effects of treatments or interventions on outcomes. We used Monte Carlo simulations to examine the following algorithms for forming matched pairs of treated and untreated subjects: optimal matching, greedy nearest neighbor matching without replacement, and greedy nearest neighbor matching without replacement within specified caliper widths. For each of the latter two algorithms, we examined four different sub-algorithms defined by the order in which treated subje

Bounds on the effect of vaccine induced immune response on outcome.

A major goal of vaccine development is the identification of immune responses that are responsible for vaccine efficacy. In theory, modest vaccines could be successfully improved by increasing such immune responses. And for a vaccine with a great benefit in one population, inducing such immune response in a different population could help one conclude the vaccine would have great benefit there. Such identification is tricky because the immune response to vaccination can only be measured in the vaccine group and thus immune responses might only be identifying individuals with a constitutional a

The standard strong ion difference, standard total titratable base, and their relationship to the Boston compensation rules and the Van Slyke equation for extracellular fluid.

A general formalism for calculating physiological acid-base balance in multiple compartments is extended to the combined interstitial, plasma, and erythrocyte multicompartment system in humans using the Siggaard-Andersen approximation for interstitial fluid. The resulting equations for total titratable base and strong ion difference reproduce the experimental in vivo carbon dioxide titration curve as well as the experimental strong ion difference value of the interstitial, plasma, and erythrocyte system in normal man. Forensically relevant anatomical brain regions cannot be sub-differentiated by RNA expression analysis.

The contextualization of biological traces generated by severe head injuries can be beneficial for criminal investigations. Here we aimed to identify and validate mRNA candidates for a robust sub-differentiation of forensically and traumatologically relevant brain regions. To this purpose, massively parallel sequencing of whole transcriptomes in sample material taken from four different areas of the cerebral cortex (frontal, temporal, parietal, occipital lobe) was performed, followed by bioinformatical data analysis, classification, and biostatistical candidate selection. Candidates were evalu
