[Parenteral nutrition in premature babies with a birth weight <1500g: a systematic single-center analysis and comparison with current guidelines].

BACKGROUND: Parenteral and enteral nutrition are essential for both growth and development of preterm infants. Based on the results of many studies, the rate of nutritional growth and the amount of substrate delivered parenterally are under debate. OBJECTIVE: The main aim of this study was to assess parenteral nutrition in very and extremely immature preterm infants, i.e. very low birth weight (VLBW, birth weight <1500g) and extremely low birth weight (ELBW, birth weight <1000g) neonates, and to compare the amount of parenterally delivered substrate in our neonatal intensive care unit (NICU)

Quality of EHR data extractions for studies of preterm birth in a tertiary care center: guidelines for obtaining reliable data.

BACKGROUND: The use of Electronic Health Records (EHR) has increased significantly in the past 15 years. This study compares electronic vs. manual data abstractions from an EHR for accuracy. While the dataset is limited to preterm birth data, our work is generally applicable. We enumerate challenges to reliable extraction, and state guidelines to maximize reliability. METHODS: An Epic™ EHR data extraction of structured data values from 1,772 neonatal records born between the years 2001-2011 was performed. The data were directly compared to a manually-abstracted database. Specific data values

Efficacy of phototherapy devices and outcomes among extremely low birth weight infants: multi-center observational study.

OBJECTIVE: Evaluate the efficacy of phototherapy (PT) devices and the outcomes of extremely premature infants treated with those devices. STUDY DESIGN: This substudy of the National Institute of Child Health and Human Development Neonatal Research Network PT trial included 1404 infants treated with a single type of PT device during the first 24±12 h of treatment. The absolute (primary outcome) and relative decrease in total serum bilirubin (TSB) and other measures were evaluated. For infants treated with one PT type during the 2-week intervention period (n=1223), adjusted outcomes at discharg

Effect of birth timing on early morbidity and mortality in very preterm neonates: a single-center retrospective cohort study.

BACKGROUND: Preterm birth is the leading cause of neonatal mortality and morbidity worldwide. Very preterm neonates born between 23 + 0 and 31 + 6 weeks of gestational age are at particularly high risk due to complications requiring immediate intervention. Whether the differential organization of healthcare services during working hours (full staffing) versus off-hours (on-call) affects neonatal outcomes remains debated. This study aims to evaluate the effect of in-hours versus off-hours birth timing on early morbidity and mortality in very preterm neonates. METHODS: This was a single-center,

Leveraging Health Insurance Claims Data to Complement the Centers for Disease Control and Prevention Surveillance System for Birth Defects.

BACKGROUND: Birth defect surveillance can help identify temporo-spatial clusters and teratogenic signals to inform subsequent investigations or interventions. In the United States, state surveillance systems exist but collect limited information, prompting a complementary use of health insurance claims data to describe national birth defect prevalence trends and investigate signals. METHODS: The Merative MarketScan Commercial Claims and Encounters (MarketScan) database was used to identify liveborn infants from 2016 to 2022, with linkage to maternal health care records during pregnancy. Birth

Associations between prolonged second stage of labor and maternal and neonatal outcomes in freestanding birth centers: a retrospective analysis.

BACKGROUND: Current guidelines for second stage management do not provide guidance for community birth providers about when best to transfer women to hospital care for prolonged second stage. Our goal was to increase the evidence base for these providers by: 1) describing the lengths of second stage labor in freestanding birth centers, and 2) determining whether proportions of postpartum women and newborns experiencing complications change as length of second stage labor increases. METHODS: This study is a retrospective analysis of de-identified client-level data collected in the American Ass

Maternal and neonatal outcome of births planned in alongside midwifery units: a cohort study from a tertiary center in Germany.

BACKGROUND: For healthy women entering birth after uneventful pregnancy, midwife-led models of care have the potential to reduce interventions and increase the vaginal birth rate. In Germany, 98.4% of women are giving birth in consultant-led obstetric units. Alongside midwifery units (AMU) have been established in 2003. We compared the outcome of women registered for planned birth in the AMU at our hospital with a matched group of low-risk women who gave birth in standard obstetric care during the same period of time. METHODS: We used a retrospective cohort study design. The study group consi

Birth Outcomes, Intervention Frequency, and the Disappearing Midwife-Potential Hazards of Central Fetal Monitoring: A Single Center Review.

INTRODUCTION: Many birth units use central fetal monitoring (CFM) under the assumption that greater surveillance improves perinatal outcomes. The unexpected loss of the CFM system at a tertiary unit provided a unique opportunity to evaluate outcomes and staff attitudes toward CFM. METHODS: This retrospective cohort study compared patient data from 2,855 electronically monitored women delivering over a 12-month period, where CFM was available for the first 6 months but unavailable for the following 6 months. Primary outcomes relating to neonatal morbidity and secondary outcomes relating to int

Primary repair of esophageal atresia in extremely low birth weight infants: a single-center experience and review of the literature.

BACKGROUND: Advances in neonatal intensive care have led to an increased survival of very low birth weight (VLBW, <1,500 g) and extremely low birth weight infants (ELBW, <1,000 g). Several abnormalities may occur in these children, e.g. esophageal atresia (EA), imperforate anus or abdominal wall defects. Correction of EA is often performed as a staged procedure in this group of patients. OBJECTIVES: To evaluate the feasibility of a primary correction of EA in 4 ELBW and VLBW infants. METHODS: Between 2002 and 2004, 4 infants below 1,200 g were operated on in our institution with a diagnosis

The effects of the World Trade Center event on birth outcomes among term deliveries at three lower Manhattan hospitals.

The effects of prenatal exposure to pollutants from the World Trade Center (WTC) disaster on fetal growth and subsequent health and development of exposed children remain a source of concern. We assessed the impact of gestational timing of the disaster and distance from the WTC in the 4 weeks after 11 September on the birth outcomes of 300 nonsmoking women who were pregnant at the time of the event. They were recruited at delivery between December 2001 and June 2002 from three hospitals close to the WTC site. Residential and work addresses of all participants for each of the 4 weeks after 11 S

Racial Disparities in Birth and Lactation Outcomes Among Neonates with Congenital Heart Defects at a Regional Mid-Atlantic Heart Center.

BACKGROUND: Evidence regarding ways in which systemic racism impacts birth and lactation among neonates with congenital heart defects (CHD) is limited. Thus, we sought to examine racial disparities among a cohort of neonates with CHD by exploring associations between birth and lactation outcomes and individual and community-level indicators of systemic racism. METHODS: This was a post hoc analysis of retrospective electronic health record (EHR) data from hospitalized neonates who had cardiac surgery and resided in the same county as the pediatric hospital (n = 20; 2016-2020). Association of discordant neonatal birth weight and maternal height with risk of preeclampsia: a multi-center retrospective cohort study in China.

BACKGROUND: Uteroplacental mismatch contributes to development of preeclampsia, involving of reduced uteroplacental blood supply and/or increased fetoplacental demands. Maternal height and neonatal weight, which positively related to uteroplacental supply and fetoplacental demands were respectively shown to associated with altered risk of preeclampsia. However, data to comprehensively evaluate the integrated associations of neonatal weight and maternal height with the risk of preeclampsia is lacked. METHODS: A respective cohort study was conducted among pregnant women delivered at 38 secondar

Prevalence of and Risk Factors for Extremely Low Birth Weight Infants in Saudi Arabia: A Four-Year Single-Center Experience.

Introduction Globally, extremely low birth weight (ELBW) infants present a distinct challenge for neonatologists, representing a significant portion of the most vulnerable and high-risk cases admitted to the neonatal intensive care units (NICUs). Saudi Arabia has a high prevalence of ELBW, which leads to a significant risk of morbidity and mortality, as many of these extreme preterm infants who are admitted to NICUs may not survive till discharge. This study aimed to evaluate the prevalence of ELBW infants and associated risk factors over a four-year period. Methods Two study designs were used
