Epidural Blood Patching in Spontaneous Intracranial Hypotension-Do we Really Seal the Leak?

PURPOSE: Epidural blood patch (EBP) is a minimally invasive treatment for spontaneous intracranial hypotension (SIH). Follow-up after EBP primarily relies on clinical presentation and data demonstrating successful sealing of the underlying spinal cerebrospinal fluid (CSF) leak are lacking. Our aim was to evaluate the rate of successfully sealed spinal CSF leaks in SIH patients after non-targeted EBP. METHODS: Patients with SIH and a confirmed spinal CSF leak who had been treated with non-targeted EBP were retrospectively analyzed. Primary outcome was persistence of CSF leak on spine MRI or in

Efficacy of epidural blood patching or surgery in spontaneous intracranial hypotension: an evidence map protocol.

BACKGROUND: Spontaneous intracranial hypotension (SIH) is a debilitating disorder caused by non-iatrogenic spinal cerebrospinal fluid leaks. SIH is increasingly recognized as an important treatable cause of secondary headaches. Treatment involves either epidural blood patching or surgery, which have considerable differences in their adverse event rates, recovery times, and cost. The objective of this evidence map is to understand the breadth of studies that investigate SIH treatment efficacy and to identify knowledge gaps to inform future research. METHODS: This review will consider experimen

The effectiveness of epidural blood patch in patients with cerebral palsy treated with intrathecal baclofen implantation.

BACKGROUND: Cerebrospinal fluid leak and postdural puncture spinal headache following intrathecal baclofen therapy are known complications. Although primary treatments are conservative, epidural blood patch is an alternative in patients with persistent and severe symptoms. AIM: The purpose of this article is to review the effectiveness of epidural blood patch for the treatment of spinal headache and cerebrospinal fluid leak associated with intrathecal baclofen treatment in children with cerebral palsy. METHODS: Our database was reviewed for epidural blood patch in 341 pediatric patients with

Transforaminal Epidural Blood Patches for the Treatment of Postsurgical Dural Leaks: Two Case Reports.

UNLABELLED: Unintended dural punctures with leakage of cerebrospinal fluid (CSF) are recognized as a frequent complication of spinal surgery. Although conservative or invasive options may be used to treat postoperative CSF leaks, the existing literature does not define either an algorithmic treatment approach or a universally accepted standard of care. We believe that a transforaminal epidural blood patch (EBP) can serve as a minimally invasive, cost-effective option to treat postsurgical CSF leaks that do not resolve with conservative management. We have performed an EBP via the transforamina

Predictors of repeat epidural blood patch for postdural puncture headache after labor epidural analgesia: a single-center retrospective cohort study (2014-2024).

BACKGROUND: Epidural blood patch remains the gold standard for treating postdural puncture headache following unintentional dural puncture during labor epidural analgesia. However, epidural blood patch may fail in 17-28 % of cases, necessitating repeat procedures. Factors predicting epidural blood patch failure remain poorly understood, limiting the ability to optimize treatment strategies. We aimed to identify independent predictors of repeat epidural blood patch following postdural puncture headache. METHODS: We conducted a retrospective observational study at a tertiary maternity center fr

Unveiling the superiority: single catheter multisite epidural blood patch for the treatment of spontaneous intracranial hypotension.

Spontaneous intracranial hypotension (SIH) is a rare clinical condition that may occur due to cerebrospinal fluid leakage. The purpose of this study was to compare the safety and efficacy of single catheter multisite epidural blood patch(CA-EBP) with standard procedures, including blinded and targeted epidural blood patch (EBP) for the treatment of SIH, and to investigate the effectiveness of dexamethasone as an adjuvant. Clinical data of 68 patients with postoperative SIH who underwent EBP between March 2022 and May 2024. In this prospective cohort trial, 68 patients were allocated to four gr

Epidural blood patch for cerebrospinal fluid leak-induced subdural hematoma in a pediatric patient: illustrative case.

BACKGROUND: Intracranial hypotension (IH) is a secondary headache syndrome that can result from spontaneous or iatrogenic CSF leaks. Subdural hematomas (SDHs) are a particularly dangerous sequela of IH. Although epidural blood patch (EBP) is a recognized treatment for IH, its use in pediatric patients with SDH as a solitary intervention has not been reported. OBSERVATIONS: An 11-year-old male with newly diagnosed acute myeloid leukemia developed persistent, postural headaches following a lumbar puncture performed for diagnostic and therapeutic purposes. Initial imaging revealed a moderate lef

Initial Results of Precision Treatment of Postoperative Cerebrospinal Fluid Leak with Ultrasound-Guided Epidural Blood Patch.

BACKGROUND: Incidental durotomy, a known complication of spinal surgery, can lead to persistent cerebrospinal fluid leak and pseudomeningocele if unrecognized or incompletely repaired. We describe the use of ultrasound to visualize the site of durotomy, observe the aspiration of the pseudomeningocele, and guide the precise application of an ultrasound-guided epidural blood patch (US-EBP), under direct visualization in real time. METHODS: A retrospective review was performed to determine demographic, procedural, and outcome characteristics for patients who underwent US-EBP for symptomatic post

Long-Term Outcomes of Post Dural Puncture Headache Treated With Epidural Blood Patch: A Pilot Study.

BACKGROUND: Post dural puncture headache (PDPH) is a known complication which may occur in the setting of patients undergoing lumbar punctures (LP) for diagnostic or therapeutic purposes. The gold standard for treating a PDPH is an epidural blood patch (EBP). There have been few publications evaluating the long-term outcomes of PDPH treated with EBP. The aim of this pilot study was to examine the incidence of chronic headaches in dural puncture patients who received EBP versus those who did not. METHODS: A retrospective case control study was performed at a single large center institution. Fo

Accidental dural puncture, postdural puncture headache, intrathecal catheters, and epidural blood patch: revisiting the old nemesis.

One of the controversial management options for accidental dural puncture in pregnant patients is the conversion of labor epidural analgesia to continuous spinal analgesia by threading the epidural catheter intrathecally. No clear consensus exists on how to best prevent severe headache from occurring after accidental dural puncture. To investigate whether the intrathecal placement of an epidural catheter following accidental dural puncture impacts the incidence of postdural puncture headache (PDPH) and the subsequent need for an epidural blood patch in parturients. A retrospective chart review

Successful epidural blood patch in a patient with headache for 6 months after lumbar root decompression.

Prolonged headache following dural puncture is an uncommon problem that may occur after a spinal tap, often as a complication of epidural anaesthesia. This problem has also been described after long-term epidural or spinal anaesthesia, myelography or spinal surgery. A case of prolonged postdural puncture headache following lumbar nerve root decompression is described in a healthy young man. No other cause could be found either clinically or with the aid of scanning by computerized tomography or magnetic resonance imaging techniques at the spinal level involved. The symptoms were successfully t

Low-Angle Paramedian Approach versus Midline Approach for Remote Access Epidural Blood Patch: A Case Report.

While mid-thoracic CSF leaks (T7-T8) require targeted epidural blood patches, direct thoracic puncture carries a significant risk of spinal cord injury due to the narrow epidural space. To mitigate this risk, a safety-oriented remote-access strategy from the T12-L1 level, with cranial advancement of the catheter, was employed to prevent direct thoracic spinal cord injury. We present a 46-year-old male with a T7-T8 leak in whom a standard midline approach failed due to poor catheter steerability. By adopting a low-angle (<30°) paramedian approach, we achieved precise ipsilateral catheter positi

Targeted Versus NonTargeted Epidural Blood Patch for Spontaneous Intracranial Hypotension: A Systematic Review and Meta-Analysis.

BACKGROUND: Spontaneous intracranial hypotension (SIH) is characterized by CSF leakage leading to orthostatic headaches and neurological deficits. In some cases, it can be extremely severe to cause nausea, vomiting, and vertigo. Despite conservative treatment, epidural blood patch remains the optimal approach for empiric resolution of symptoms. However, the medical community lacks consensus regarding the best approach to use: targeted epidural blood patch (TEBP) versus nontargeted epidural blood patch (NTEBP). METHODS: A systematic review and metanalysis was conducted on multiple databases wi

Targeted Epidural Blood Patch Injection Through a Mini-Open Approach for Treatment of Spontaneous Intracranial Hypotension.

BACKGROUND AND OBJECTIVES: Spontaneous intracranial hypotension is recognized as a cause for refractory headache. Treatment can range from blind blood patch injection to microsurgical repair of the cerebrospinal fluid (CSF) leak. The objective of the study was to investigate the safety and efficacy of the targeted blood patch injection (TBPI) technique through a mini-open approach in treatment of refractory intracranial hypotension. METHODS: We retrospectively reviewed cases of 20 patients who were treated for spontaneous intracranial hypotension at our institute between 2011 and 2022. Head a

Reversal of Progressive Conscious Disturbance with Epidural Blood Patch for Cerebrospinal Fluid Leakage at C2 Level.

Intracranial hypotension syndrome (IHS) is generally caused by cerebrospinal fluid (CSF) leakage. Complications include bilateral subdural hygroma or haematoma and herniation of the cerebellar tonsils. Epidural blood patch (EBP) therapy is indicated if conservative treatment is ineffective. We reported the case of a 46-year-old man with a history of postural headache and dizziness. The patient was treated with bed rest and daily hydration with 2000 mL of fluid for 2 weeks. However, dizziness and headache did not resolve, and he became drowsy and disoriented with incomprehensible speech. Magnet

High thoracic/cervical epidural blood patch for spontaneous cerebrospinal fluid leak: a new challenge for anesthesiologists.

Spontaneous cerebral spinal fluid leakage is increasingly recognized as a cause of headache due to low intracranial pressure. The site of leakage can be identified with radionuclide cisternography, and anesthesiologists are increasingly requested to provide epidural blood patch for their management. This series of case reports demonstrates some of the issues relating to the management of this condition.

[A case of cerebrospinal fluid hypovolemia caused by Ommaya reservoir successfully treated by epidural blood patch].

Here, we report a case of cerebrospinal fluid hypovolemia caused by lumbar puncture for implantation of an Ommaya reservoir. A 55-year-old man with central nervous system lymphoma developed meningeal dissemination. He was treated by spinal radiotherapy, and had an Ommaya reservoir implanted for intrathecal injection of anticancer drugs. After one month, he complained of orthostatic neck pain. MRI revealed bilateral subdural effusion and diffuse meningeal enhancement. He was diagnosed as having cerebrospinal fluid hypovolemia. Because no improvement in symptoms was seen after 2 weeks of conserv
