Femoral neck anteversion of the less involved side in unilateral cerebral palsy: kinematics and radiological considerations.

This research aims to investigate femoral neck anteversion (FNA) on the less involved side in unilateral cerebral palsy (CP) and examine its impact on hip rotation during gait. Sixty-nine patients with unilateral CP, with a mean of 21 years, were included study. Static and dynamic hip rotation ranges were quantified via physical examination and three-dimensional motion analysis. Patients were stratified into five levels of involvement according to modified Winters' classification. FNA differences between modified Winters' classification types and correlation with static and dynamic hip rotatio

Pseudodislocation of the Shoulder After a Pathologic Scapula Fracture: A Rare Case Report.

Shoulder pseudodislocation is a rare clinical entity characterized by radiographic malposition of the humeral head relative to the glenoid despite preserved joint articulation. We present the case of a 79-year-old female with a complex oncologic history involving the scapula who presented to the emergency department with acute shoulder pain. Initial radiographs suggested a glenohumeral dislocation; however, given the patient's complex oncologic history, a closed reduction was deferred in favor of advanced imaging. Subsequent computed tomography (CT) definitively identified the presentation as

Total Hip Arthroplasty and the Spinopelvic
Relationship: What's the Latest!

Although total hip arthroplasty (THA) has proved to be a successful surgical procedure, both prosthetic and bone impingement resulting in dislocation continue to occur. Studies have shown that spine pathology resulting in lumbar stiffness and hip arthritis often coexist. Spinopelvic mobility patterns during postural changes affect three-dimensional acetabular component position, which affects the incidence of prosthetic impingement and THA instability. Several spinopelvic risk factors that may affect THA stability have been identified. Numerous reports recommend performing a preoperative spino

Neglected congenital dislocation of the hip. Role of computed tomography and computer-aided design for total hip arthroplasty.

Computed tomography (CT) provides important three-dimensional anatomic details in congenital dislocation of the hip that are useful for total hip arthroplasty (THR) and are not obtainable with conventional radiographic evaluation. In this study, 84 patients (119 hips) with congenital dislocation of the hip were evaluated with CT before surgery. Specifically, both the acetabulum and the femur were analyzed to make the best selection of the prosthesis. The average anteversion of the acetabulum was 23 degrees, with an opening of 30.9 mm and a depth of 14.7 mm. The bone stock of the true acetabulu

Femoral shaft fractures treated by intramedullary nailing. A follow-up study focusing on problems related to the method.

One hundred and sixteen patients with 120 femoral fractures treated by reamed intramedullary nailing were reviewed. All fractures but one healed without additional procedures. One comminuted fracture had a bone transplant after 8 months to enhance bone remodelling in the lateral part of the fracture area. Three patients developed adult respiratory distress syndrome; all patients survived. Deep infection complicated one osteosynthesis. Thromboembolism was recorded in five cases. Twenty-three patients had a true torsional deformity (anteversion difference of 15 degrees or more), but only nine ha

Sagittal alignment of the spine-pelvis-lower extremity axis in patients with severe knee osteoarthritis: A radiographic study.

OBJECTIVES: Normal sagittal spine-pelvis-lower extremity alignment is crucial in humans for maintaining an ergonomic upright standing posture, and pathogenesis in any segment leads to poor balance. The present study aimed to investigate how this sagittal alignment can be affected by severe knee osteoarthritis (KOA), and whether associated changes corresponded with symptoms of lower back pain (LBP) in this patient population. METHODS: Lateral radiograph films in an upright standing position were obtained from 59 patients with severe KOA and 58 asymptomatic controls free from KOA. Sagittal alig

Precision of using contralateral lesser trochanter as a landmark to prevent rotational malalignment in fracture shaft of femur: a human cadaveric study.

BACKGROUND: Rotational Malalignment is abnormal rotational alignment between proximal and distal parts of femoral bone. If there is more than 15 degrees of rotational malalignment in femoral bone, the patient will have abnormality of gait and they will have difficulties to perform their daily life activities. The chances to have rotational malalignment in femoral fracture are increased after intramedullary nailing which is more than plate and screws fixation. In this study, we used contralateral Lesser Trochanter to be the reference for rotational determiner. MATERIAL AND METHOD: 17 pairs of

Femoral neck radiography: effect of flexion on visualization.

OBJECTIVES: To determine whether flexion improves radiographic visualization of the femoral neck when the femur is externally rotated. METHODS: Five human femora, with varying neck-shaft and anteversion angles, were measured and immobilized. Degree of flexion required to bring the femoral neck horizontal was measured, varying the rotation. Next, one bone was radiographed in 16 positions, varying rotation in 15 degrees and flexion in 10 degrees increments. Radiographs were presented in randomized blinded fashion to 15 staff radiologists for scoring of femoral neck visualization. Following this

Comparison of radiological and functional results in osteoporotic distal femur fractures operated with single plating, lateral incision, and double plating, anterior paramedial incision: A retrospective study.

Treatment of osteoporotic distal femur fractures is often complicated by a high rate of nonunion and varus collapse. For such fractures, lateral plating with lateral incision and double plating with anterior paramedial incision have shown promising results in the recent literature. The hypothesis of this study was that bilateral plating of comminuted distal femur fractures in osteoporotic patients would result in higher union rates and lower revision rates compared to an isolated lateral locking plate. The study included 56 patients (23 males, 33 females) with supracondylar femur fracture. Acc

Simple guidelines for evaluating intraoperative alignment after the reduction of intertrochanteric fractures.

The incidence of intertrochanteric femoral fractures has rapidly increased with the extended lifespan of the elderly population. Surgery enables early ambulation by achieving anatomic reduction and stable internal fixation. However, reduction usually involves postoperative evaluation. Here, we present reliable parameters obtained from analyses of three-dimensional computed tomography images from cadavers to serve as guidelines during the reduction of intertrochanteric fractures. We included 184 three-dimensional modeling samples from cadavers placed in two standardized positions, similar to C-

The influence of osteophyte depiction in CT for patient-specific guided hip resurfacing procedures.

PURPOSE: An accurate fit of a patient-specific instrument guide during an intervention is one of the critical factors affecting accuracy of the surgical procedure. In this study, we investigated how well osteophytes, which are abnormal bone growths that form along joints, are depicted in clinical preoperative CT scans and estimated the influence of such depiction errors on the intraoperative accuracy of the guide. METHODS: In 34 hip resurfacing patients, 227 osteophyte surface points on the anterior aspect of the femoral neck were collected intraoperatively, using an optoelectronic navigation

Mechanobiological predictions of growth front morphology in developmental hip dysplasia.

Developmental dysplasia of the hip (DDH) is the most common orthopedic problem of newborn children. Most clinicians and researchers agree that the primary cause of DDH is abnormal mechanical forces on the head of the femur due to limb position, pressure from the womb, or ligament laxity. The abnormal mechanical forces result in altered growth and bony deformities, in particular large neck-shaft and anteversion angles in the proximal femur and a shallow acetabulum. Previous studies have suggested that intermittent octahedral shear stress promotes growth and ossification, while intermittent hydr

Deep learning-based workflow for hip joint morphometric parameter measurement from CT images.

Objective.Precise hip joint morphometry measurement from CT images is crucial for successful preoperative arthroplasty planning and biomechanical simulations. Although deep learning approaches have been applied to clinical bone surgery planning, there is still a lack of relevant research on quantifying hip joint morphometric parameters from CT images.Approach.This paper proposes a deep learning workflow for CT-based hip morphometry measurement. For the first step, a coarse-to-fine deep learning model is designed for accurate reconstruction of the hip geometry (3D bone models and key landmark p

Fluoroscopic demonstration of femoroacetabular impingement during hip arthroscopy.

Femoroacetabular impingement is a cause of hip pain that in selected cases can be treated by removal of impinging bone (osteoplasty). No absolute parameters for osteoplasty exist. We present a technique for dynamic evaluation of bony impingement and control of arthroscopic osteoplasty using intraoperative fluoroscopy. With the patient supine on a fracture table, the C-arm is positioned between the legs. Femoral anteversion is measured with the C-arm vertical and the hip flexed 90° (Dunn view). The C-arm is then tilted back 25° from vertical to create a profile view of the anterior rim, and the

Acetabuloplasty in the treatment of congenital dislocation of the hip.

The results in 102 acetabuloplasties have been assessed in eighty-six patients, with a minimum follow-up of five years. Clinical and radiological assessment was carried out according to a score system very similar to that used in previous studies, and the results were expressed as excellent, good, fair and poor according to the score obtained. Any hip requiring further stabilising procedures was automatically grouped as a failure. Subtrochanteric femoral ostestomy was used when femoral anteversion was excessive. The results revealed that 73 per cent of the hips were satifactory (excellent or g

Positive Correlation Between Acetabular Anteversion and Abduction in Developmental Dysplasia of the Hip: A CT-Based Morphological Study.

OBJECTIVE: Developmental dysplasia of the hip (DDH) exhibits abnormalities in hip anatomy, so changes in the acetabular and femoral angles hold clinical relevance. This study aimed to investigate the correlations between acetabular anteversion (AAV) and acetabular abduction (AAB), as well as between combined anteversion and combined abduction in patients with DDH, attempting to give evidence for synchronous torsion of the proximal femur and acetabulum. This study also aimed to propose a new method for predicting acetabular anteversion and combined anteversion angles, respectively, based on ace
