Comprehensive meta-analysis of antibiotic-impregnated bone cement versus plain bone cement in primary total knee arthroplasty for preventing periprosthetic joint infection.

PURPOSE: Antibiotic-loaded bone cement (ALBC) was usually used to prevent periprosthetic joint infection (PJI) in primary total knee arthroplasty (PTKA), but whether to use ALBC or plain bone cement in PTKA remains unclear. We aimed to compare the occurrence rate of PJI using two different cements, and to investigate the efficacy of different antibiotic types and doses administered in preventing surgical site infection (SSI) with ALBC. METHODS: The availability of ALBC for preventing PJI was evaluated by using a systematic review and meta-analysis referring to the Preferred Reporting Items fo

Injectable photosensitive bone cement enhancing angiogenesis and osteogenic differentiation for the treatment of bone nonunion.

Nonunion fractures present a significant clinical challenge because of their complex microenvironment, which includes poor vascularization, insufficient osteogenesis, infection, and separation of fracture ends. The current clinical treatments have certain limitations. Inspired by this phenomenon, sandcastle worms secrete adhesive proteins that bind sand grains, shell fragments, and mineral particles, thereby constructing their "castles." In this study, we developed an injectable bone cement using methacryloyl chitosan (CSMA) combined with a specific concentration of oyster shell nanoparticles

A Model of the Action of the Shockwave Generated by a Multichannel Discharge on the Union of Bone Tissue with Bone Cement.

BACKGROUND: In reoperation of femoral prostheses, there is a higher percentage of complications associated with the extraction of bone cement from the femoral channel, which is technically and time consumingly tedious. Shockwaves have been used in medicine for years, particularly in urology for the destruction of concretions and orthopaedics, as a method of treating pain in patients with calcifying enthesopathy. MATERIALS AND METHODS: We studied the use of shockwaves generated by a new source based on the multichannel discharge principle in facilitating extraction of bone cement. Femurs of ex

Is Bone Cement Implantation Syndrome Actually Caused by Cement? A Systematic Review of the Literature Using the Bradford-Hill Criteria.

BACKGROUND: Debate surrounding the use of cemented femoral components in hip arthroplasty persists. A proposed risk of cement fixation is bone cement implantation syndrome (BCIS), a phenomenon characterized by intraoperative hypotension, hypoxia, and/or cardiovascular collapse. The purpose of this study was to analyze the literature to determine if enough evidence exists to support a causal relationship between cement and BCIS. METHODS: A systematic review of articles on BCIS published from 2010 to 2023 was performed. Using the Bradford-Hill criteria, a set of nine epidemiological principles

Intracardiac bone cement embolism as a complication of vertebroplasty: management strategy.

BACKGROUND: Vertebroplasty carries multiple complications due to the leakage of polymethylmethacrylate (PMMA) into the venous system through the iliolumbar or epidural veins. The rate of venous cement complications may vary from 1 to 10 %, with cement extravasation into the venous system in 24 % of patients. Emboli may further migrate into the right heart chambers and pulmonary arteries. Patients may vary in presentation from asymptomatic or symptoms such as syncope to life-threatening complications. CASE REPORT: We present a case of a 57-year-old lady diagnosed with osteoporosis who underwen

Does bone cement in percutaneous vertebroplasty act as a stress riser?

STUDY DESIGN: An in vitro cadaveric study. OBJECTIVE: To determine whether percutaneous vertebroplasty (PVP) with a clinically relevant amount of bone cement is capable of causing stress peaks in adjacent-level vertebrae. SUMMARY OF BACKGROUND DATA: It is often suggested that PVP of a primary spinal fracture causes stress peaks in adjacent vertebrae, thereby leading to additional fractures. The in vitro studies that demonstrated this relationship, however, use bigger volumes of bone cement used clinically. METHODS: Ten fresh-frozen vertebrae were loaded until failure, while registering forc

Complications of hydroxyapatite bone cement in secondary pediatric craniofacial reconstruction.

Hydroxyapatite cement has become a popular alternative to bone grafts in reconstructing the calvarium. Although animal studies have shown promising results with use of hydroxyapatite, human clinical studies have shown mixed results including significant rates of infection. This is a retrospective chart review during a 7-year period (1997-2003) of 20 patients who underwent secondary forehead cranioplasty with hydroxyapatite cement (Norian Craniofacial Reconstruction System). Basic demographics including age, sex, and diagnosis were identified. Characteristics of the defects were recorded includ

Antibiotic-impregnated cement spacer for bone defects of the forearm and hand.

Management of bone loss, particularly in the face of open or contaminated wounds, presents a reconstructive challenge. Polymethylmethacrylate impregnated with antibiotics has been used successfully in the treatment of infected total joint arthroplasty and open fractures. The cement delivers high-dose local concentrations of antibiotics while filling a space. This article presents our use of an antibiotic-impregnated cement spacer for bone loss in the forearm or hand in the face of open or infected wounds. The cement spacer fills a potential space, prevents the involution of the surrounding sof
