Bone Conduction: Benefits and Limitations of Surgical and Nonsurgical Devices.

Bone conduction devices (BCDs) are a well-established and effective treatment solution for children with conductive and mixed hearing loss. Evidence indicates that early intervention through BCDs can improve hearing, speech, and language, and developmental outcomes. BCDs overcome several limitations associated with traditional hearing aids, and in many cases offer children an alternative to more invasive surgical management. Despite these benefits, children with conductive hearing loss are often subject to delays in intervention. In addition, interventional challenges and technological limitat

Study of the improvement in bone conduction threshold after stapedectomy.

INTRODUCTION AND OBJECTIVES: Bone conduction threshold depression is not always a result of inner ear and cochlear nerve pathology. In fact, middle ear pathologies may be responsible for such threshold depression, as occurs in otosclerosis. The aims of this study were to evaluate the improvement of bone conduction threshold in patients with otosclerosis that underwent stapedectomy and to study the postoperative audiological results. MATERIAL AND METHODS: This was a retrospective study on 95 patients (116 ears) diagnosed with otosclerosis having conductive or mixed hearing loss that received s

Intraoperative measurement for a new transcutaneous bone conduction hearing implant.

OBJECTIVE: To investigate the possibility of using a modified reverse transfer function (RTF) measurement intraoperatively during surgery of a new transcutaneous bone conduction hearing implant to evaluate the status of the device. METHODS: Tests were performed on a cadaver skull (preclinically) and two conductive hearing loss patients implanted with a new transcutaneous bone conduction implant. During intraoperative activation, the RTF was measured using a microphone attached perpendicularly and directly to the skin in the middle section of the forehead. RESULTS: The RTF could be measured f

Is complex signal processing for bone conduction hearing aids useful?

OBJECTIVES: To establish whether complex signal processing is beneficial for users of bone anchored hearing aids. METHODS: Review and analysis of two studies from our own group, each comparing a speech processor with basic digital signal processing (either Baha Divino or Baha Intenso) and a processor with complex digital signal processing (either Baha BP100 or Baha BP110 power). The main differences between basic and complex signal processing are the number of audiologist accessible frequency channels and the availability and complexity of the directional multi-microphone noise reduction and

Inner ear contribution to bone conduction hearing in the human.

Bone conduction (BC) hearing relies on sound vibration transmission in the skull bone. Several clinical findings indicate that in the human, the skull vibration of the inner ear dominates the response for BC sound. Two phenomena transform the vibrations of the skull surrounding the inner ear to an excitation of the basilar membrane, (1) inertia of the inner ear fluid and (2) compression and expansion of the inner ear space. The relative importance of these two contributors were investigated using an impedance lumped element model. By dividing the motion of the inner ear boundary in common and

The bone conduction implant--first implantation, surgical and audiologic aspects.

OBJECTIVE: To report on preoperative assessment, surgery, and audiologic outcome of the first patient implanted with the bone conduction implant (BCI). BACKGROUND: The BCI is a bone conduction hearing device with an intact skin solution where the transducer is implanted close to the ear canal opening. By avoiding a percutaneous screw attachment to the skull, the BCI is anticipated to reduce complications associated with the Bone-Anchored Hearing Aid (BAHA) solution. METHODS: The first patient to receive a BCI was a 42-year-old woman with a unilateral mixed hearing loss due to tympanosclerosi

Bone-conduction amplification with completely-in-the-canal hearing aids.

Recent advances in miniaturization have provided clinicians with hearing aids that can be comfortably inserted as far as the bony portion of the ear canal. It is possible to take advantage of these deeply inserted hearing aids in new ways. For example, the physical vibrations of microphone and receiver components may be used to improve hearing aid gain through bone conduction. Three cases are presented that will introduce this phenomenon for two transcranial CROS fittings and for one unilateral otosclerosis fitting. In each case, functional gain measurements under headphones were obtained with

Results of bone conduction following surgery for chronic ear disease.

Preoperative and postoperative bone conduction thresholds were compared in 181 chronic ears operated on over a 5-year period between 1990 to 1994. In the majority (92%) of cases the bone conduction thresholds remained unchanged (+/-10 dB). Nine ears (5%) showed better thresholds after surgery, with improvements ranging from 11 dB to 25 dB. This improvement was especially noted in ears with severe tympanic pathology. One ear with a large labyrinthine fistula became totally deaf after surgery. In 5 ears (3%) bone-conduction thresholds deteriorated, but remained measurable at all frequencies test

Effects of contralateral masking on high-frequency bone-conduction thresholds.

The present study reports effects of contralateral masking on high-frequency threshold force levels in 28 normal-hearing subjects. High-frequency air- and bone-conduction thresholds were measured with a high-frequency auditory evaluation system using matched Koss HV/1A earphones and the Pracitronic KH 70/5 bone vibrator. Measurements were made for both unmasked and masked bone-conduction thresholds at the ipsilateral mastoid of the better ear. The contralateral masked condition was performed using 30-dB-SL 400-Hz narrow-band masking noise centered at frequency of test tone. The results demonst

Percutaneous vs. transcutaneous transducers for hearing by direct bone conduction.

There is a substantial need for improvement of the hearing situation for patients, with chronic middle ear or ear canal disorders. To improve hearing for these patients, two different bone conduction hearing systems have been developed. The Nobelpharma Auditory System HC 200-the bone-anchored hearing aid we present here-uses a percutaneous transducer; whereas the Audiant device, developed by Dr. Jack Hough, uses a transcutaneous transducer. In percutaneous transmission, the transducer is directly coupled to the bone by means of a permanent skin penetration, whereas in transcutaneous transmissi

Labyrinthine fistulae caused by cholesteatoma. Improved bone conduction by treatment.

In five cases of labyrinthine fistulae caused by extensive cholesteatoma, more than 30-dB improvement in bone conduction was observed in four postoperative cases and in one case after preoperative administration of antibiotics. In each case, a fistula of more than 2 mm in length was present at the lateral semicircular canal, and membranous labyrinthine wall was exposed when the cholesteatoma membrane was removed. These five cases were considered to be in the stage of serous labyrinthitis. The experience with these cases shows that emergent antibiotic treatment and surgery are appropriate for c

Bone conductive implants in single-sided deafness.

CONCLUSION: Bone conduction implants (BCIs) have been shown to partially restore some of the functions lost when binaural hearing is missing, such as in subjects with single-sided deafness (SSD). The use of a single BCI needs to be recommended by a clinician based on thorough counselling with the SSD subject. OBJECTIVES: To perform an overview of the present capabilities of BCIs for SSD and to evaluate the reliability of the audiological evaluation for assessing speech recognition in noise and sound localization cues, which are major problems related to the loss of binaural hearing. METHODS:

Effect of Cochlear Implantation on Air Conduction and Bone Conduction Elicited Vestibular Evoked Myogenic Potentials-A Scoping Review.

Background/Objectives: Cochlear implantation (CI) is an effective intervention for individuals with severe to profound hearing loss; however, it may impact vestibular function due to its proximity to related anatomical structures. Vestibular evoked myogenic potentials (VEMPs) assess the function of the saccule and utricle, critical components of the vestibular system. This review examines CI's impact on air conduction (AC) and bone conduction (BC) VEMP responses. Methods: A scoping review was conducted following PRISMA guidelines, using databases such as Medline, Embase, Cochrane Library, Scop
