Treatment of a tibial bone defect with a motorized intramedullary bone transport nail: A case review with 32 months follow up.

Nowadays, massive segmental bone defects represent a surgical challenge for trauma surgeons. Most of these injuries appear in the context of high-energy trauma, not only significantly affecting the bones, but also involving severe injuries of the adjacent soft tissues. For these reasons, their treatment requires complex reconstruction surgeries. There are multiple techniques to treat bone defects, bone transport being one of the most widely used. Historically, external fixators (monolateral and circular) have been and still are the gold standard for performing this technique, although they are

Nail proximal fold stem cells participate in nail growth, orchestrating enhanced digit regeneration via bone morphogenetic protein signaling activation.

Rodent and primate digit tips exhibit a remarkable regenerative capacity following amputation, driven by highly proliferative nail stem cells with active canonical Wnt signaling. Recently, a distinct, slow-cycling population of bi-functional nail proximal fold stem cells (NPFSCs) has been identified, contributing to both peri-nail epidermis and nail plate (NP). Here, we demonstrate that NPFSCs actively participate in nail growth, orchestrating digit regeneration, with bone morphogenetic protein (BMP) signaling serving as a key regulator. Inhibition of BMP resulted in an epidermalized NP-like s

Assessment of the initial viscoelastic properties of a critical segmental long bone defect reconstructed with impaction bone grafting and intramedullary nailing.

INTRODUCTION: This study compared the initial viscoelastic properties of a segmental tibial defect stabilized with intramedullary nailing and impaction bone grafting to that of a transverse fracture stabilized with intramedullary nailing. MATERIALS AND METHODS: Seven sheep tibiae were tested in compression (1000N), bending and torsion (6Nm) in a six degree-of-freedom hexapod robot. Tests were repeated across three groups: intact tibia (Intact), transverse fracture stabilized by intramedullary nailing (Fracture), and segmental defect stabilized with a nail and impaction bone grafting (Defect).

[Congenital pseudarthrosis of the tibia and fibula in children. Results of the treatment of 18 cases with nails and bone grafts].

Eighteen cases of congenital pseudarthrosis of the leg in 17 children were treated by nailing and tibio-fibular graft with a mean follow-up of six years (from one to twelve years). Six patients had reached the end of growth. Primary union was obtained in 17 cases in a mean time of 6.5 months (from 3.5 to 18 months), but one case developed a new pseudarthrosis at the level of an associated upper tibial osteotomy. Only one case required a supplementary graft. Walking was usually achieved without any apparatus after five-and-a-half months, the tibia being protected by a nail which was left in pla

Elastic Stable Intramedullary Nailing (ESIN) in Diaphyseal Fractures of Both Forearm Bones in Children.

Background Diaphyseal fractures of both forearm bones are common in children. Although most cases can be managed conservatively, surgical treatment is indicated for unstable, irreducible, open, or complicated fractures. Elastic stable intramedullary nailing (ESIN) has emerged as a minimally invasive option with favorable functional and radiological outcomes. Methods We conducted a retrospective descriptive study including 38 children younger than 15 years with diaphyseal fractures of both forearm bones treated with ESIN in the Department of Pediatric Surgery at Moulay Ali Cherif Regional Hospi

Rigid intramedullary nailing of lower limb segments in children and adolescents with metabolic bone disease.

Children and adolescents with metabolic bone disease present to the orthopedic surgeon with pain, fractures (which may be impending), and deformity. Different modalities of orthopedic management are available. Scant literature exists on the use of rigid intramedullary nailing in this population. This study sought to evaluate the utilization of this treatment modality in the pediatric cohort, focusing on indications, techniques, and outcomes of the procedures. A retrospective review was performed over an 11-year period at a single tertiary pediatric institution in the UK. Medical records and ra

Canal Fill of the Forearm Bones When Placing Intramedullary Nails in the Pediatric and Adolescent Populations.

BACKGROUND: In the lower extremity, studies have suggested an optimal nail diameter to medullary canal diameter (ND/MCD) ratio to minimize postoperative complications. The goal of this study was to determine whether a correlation exists between the occurrence of complications, angulation, and range of motion and the ratio of the ND/MCD in the upper extremity. METHODS: A total of 85 radius and ulna fractures treated with flexible intramedullary nails had ND/MCD ratios measured. Random-effects models were developed to determine the association between complications and ND/MCD ratio, angulation

Intramedullary Nail and Plate Combination Technique for Peri-Implant Both-Bone Forearm Fractures.

A 35-year-old female patient with cerebellar ataxia presented with a right periprosthetic both-bone forearm fracture after a ground-level fall. Her surgical history was significant for multiple both-bone forearm fractures treated by open reduction and internal fixation. Subsequent treatment with a combination of intramedullary nailing and plate fixation for each bone provided successful fracture union while allowing immediate return to weight-bearing and range of motion. This case report demonstrates that intramedullary nailing and plate fixation of both-bone forearm fractures provides complet

Limitations in Use of Elastic Stable Intramedullary Nailing (ESIN) in Children with Disorders of Bone Mineralization.

BACKGROUND: Elastic intramedullary nails (ESIN) have been the treatment of choice in many long bone fractures in children for more than 20 years. The introduction of ESIN has drastically reduced tissue traumatization during fracture fixation procedures and decreased the risk of growth cartilage damage, as well as allowing for preservation of the natural biology of closed fracture healing. The objective of the present report is to draw attention to a small group of patients with bone mineralization disorders, who consequently demonstrate decreased mechanical resistance of the skeletal system, i

Ilizarov trifocal lengthening followed by intramedullary nailing for massive posttraumatic tibial bone defects.

The treatment of massive tibial bone defects takes a very long time. An Ilizarov trifocal approach decreases the lengthening time. Subsequent intramedullary fixation decreases the complications of the conventional Ilizarov method. This technique was applied between June 2010 and June 2011 in 10 male patients with a mean age of 30 years. All had grade III open tibial fractures. The patients were treated at Mansoura University Hospital and a private hospital. The sequence of treatment included trifocal lengthening using the Ilizarov method, followed by removal of the fixator, temporary plaster c

Reduced systemic bone mineral density associated with a rare case of tooth and nail syndrome.

This is the first report of a rare genetic tooth and nail syndrome (TNS) diagnosed in a 14-year-old Caucasian girl with a complete absence of the permanent dentition and, additionally, reduced total and lumbar spine bone mineral density (BMD). This coincidence suggests a new clinical manifestation of the disorder in which genetic factors and/or shared mechanisms may be responsible for the deterioration of the stomatognathic system, anodontia, nail phenotype and osteopenia. Low bone mass appears to be a new component of the syndrome. There is a rationale for bone densitometry scans assuming tha

Small patella syndrome: a bone dysplasia to recognize and differentiate from the nail-patella syndrome.

We report a 5-year, 9-month-old boy with bilateral patellar hypoplasia. The radiographic skeletal survey revealed absent ossification of the ischium and inferior pubic ramus on both sides, characteristic of the newly recognized benign bone dysplasia known as small patella syndrome, or ischiopatellar dysplasia. No nail change, iliac horn, elbow anomaly, or renal disease is associated with this condition.

Ender nail fixation in long bone fractures: experience in a level I trauma center.

Early fixation of extremity fractures in polytrauma patients is a well accepted and desirable treatment. In our patient population, the indications for immediate Ender nail fixation was for Grade I and Grade II open long bone fractures and closed long bone fractures in patients with small intramedullary canals. Ender nails were also used in patients in whom reaming was undesirable. This included patients with open physes for immediate fracture fixation and in those with Grade III open wounds in a delayed fashion. These flexible nails were also implemented for use to minimize anesthesia time as

Comparative complication rates of titanium vs carbon fiber intramedullary nails in pathologic long bone fractures: A systematic review and meta-analysis.

INTRODUCTION: Carbon fiber (CF) intramedullary (IM) nails have emerged as a promising alternative to titanium (TI) implants for managing pathologic long bone fractures, offering advantages such as radiolucency for improved imaging, reduced artifact during surveillance, and minimal attenuation of therapeutic radiation. Although early studies suggest comparable complication rates between CF and TI constructs, existing evidence remains limited by small sample sizes and a lack of direct, high-quality comparisons of complication profiles. This study aimed to systematically compare complication rate
