Use of a new malleable implant as a bone substitute in maxillofacial surgery.

Biocompatible osteoconductive polymer (BOP) has been used since 1979 as a substitute for bone in orthopedic surgery and neurosurgery. To date, in maxillofacial surgery, loss of bone volume as a result of either trauma or osteoporosis has been compensated for by autologous bone onlay graft or subperiosteal apposition grafts of synthetic materials. Neither of these solutions is entirely satisfactory. Autologous bone is subject to reabsorption, requiring initial overcorrection and multiple reoperations to achieve an acceptable result. Preshaped implants of Proplast, Vicryl, Gore-Tex, or silicone

[Clinical lymphographic diagnosis of posttraumatic edema in the substitution of bone defects].

The method of direct "lower" lymphography of the medial superficial collector was used for the examination of 11 patients with a moderate chronic posttraumatic edema developed against the background of atrophy of muscle tissues before and in the process of substituting bone defects after Ilizarov. In substituting bone defects against the background of a previous posttraumatic blockade of lymph vessels and their alterations there appears a complete or partial decompensation of the lymph flow, resulting in the development of edema in the atrophied earlier segments of lower extremities. Sinus floor elevation using autogenous bone or bone substitute combined with platelet-rich plasma.

OBJECTIVE: Sinus augmentation is a common approach for patients with severe alveolar ridge atrophy. However, autogenous bone sometimes results in donor site complications. Bone substitutes with platelet-rich plasma (PRP) promote early bone formation with autogenous bone. Use of PRP on autogenous bone and a bovine bone substitute were investigated in this split-mouth animal study. STUDY DESIGN: Premolars were extracted from minipigs. Each animal received sinus augmentation using a lateral approach with simultaneous insertion of 3 implants in each site. Groups were randomized using autogenous b

Usefulness of a Nanostructured Fibrin-Agarose Bone Substitute in a Model of Severely Critical Mandible Bone Defect.

Critical defects of the mandibular bone are very difficult to manage with currently available materials and technology. In the present work, we generated acellular and cellular substitutes for human bone by tissue engineering using nanostructured fibrin-agarose biomaterials, with and without adipose-tissue-derived mesenchymal stem cells differentiated to the osteogenic lineage using inductive media. Then, these substitutes were evaluated in an immunodeficient animal model of severely critical mandibular bone damage in order to assess the potential of the bioartificial tissues to enable bone re

Augmentation procedures using bone substitute materials or autogenous bone - a systematic review and meta-analysis.

AIMS: Bone substitute materials (BSM) are described as a reasonable alternative to autologous bone (AB) to simplify the grafting procedure. In a systematic review and meta-analysis, the influence of BSM compared to AB on treatment success in augmentation procedures of the edentulous jaw was analysed. MATERIAL AND METHODS: Literature analysis resulted in only two studies addressing reconstruction of the totally edentulous jaw using BSM. Therefore the literature analysis was extended to partially and totally edentulous jaws. The following augmentation procedures were analysed: maxillary sinus f

The effects of a novel-reinforced bone substitute and Colloss®E on bone defect healing in sheep.

Hydroxyappatite-β-tricalciumphosphate (HA/β-TCP) was reinforced with poly(D,L)-lactic acid (PDLLA) to overcome its weak mechanical properties. Two substitutes with porosities of 77% and 81% HA/β-TCP reinforced with 12 wt % PDLLA were tested in compression. The effects of allograft, substitute (HA/β-TCP-PDLLA), Colloss®E, and combination of substitute with Colloss®E on bone formation in vivo were evaluated. Cylindrical critical size defects were created at distal femoral condyles bilaterally in sheep. Titanium implant with concentric gap filling with one of the four materials was inserted. Afte

Preclinical Evaluation of BMP-9-Treated Human Bone-like Substitutes for Alveolar Ridge Preservation following Tooth Extraction.

The success of dental implant treatment after tooth extraction is generally maximized by preserving the alveolar ridge using cell-free biomaterials. However, these treatments can be associated with inflammatory reactions, leading to additional bone volume loss hampering dental implant positioning. Our group developed a self-assembled bone-like substitute constituted of osteogenically induced human adipose-derived stromal/stem cells (hASCs). We hypothesized that a bone morphogenetic protein (BMP) supplementation could improve the in vitro osteogenic potential of the bone-like substitute, which

The Granule Size Mediates the In Vivo Foreign Body Response and the Integration Behavior of Bone Substitutes.

The physicochemical properties of synthetically produced bone substitute materials (BSM) have a major impact on biocompatibility. This affects bony tissue integration, osteoconduction, as well as the degradation pattern and the correlated inflammatory tissue responses including macrophages and multinucleated giant cells (MNGCs). Thus, influencing factors such as size, special surface morphologies, porosity, and interconnectivity have been the subject of extensive research. In the present publication, the influence of the granule size of three identically manufactured bone substitute granules b

Aspergillus fumigatus biofilm formation on different bone substitutes used in maxillary sinus augmentation: an in vitro analysis.

BACKGROUND: Fungus ball (FB) typically affects healthy adults, and Aspergillus fumigatus is the most frequent etiologic agent: iatrogenic factors represent an important issue in FB pathogenesis. Moreover, a recent study suggested a significant association between the use of anorganic bovine bone as sinus grafting material and subsequent development of FB. The aim of the present investigation is to evaluate in vitro eventual differences in the ability of Aspergillus fumigatus to colonize different bone grafting materials and grow on them as biofilm. FINDINGS: Five different bone substitutes (d

Antibiotic-loaded bone substitutes in the surgical management of diabetic foot osteomyelitis: Current evidence and clinical perspectives.

Diabetic foot osteomyelitis (DFO) represents a severe manifestation of diabetic foot syndrome and a leading cause of lower-limb amputation globally. Surgical intervention is frequently required, often leaving residual bone defects that can promote persistent infection and impair healing. In this context, bioabsorbable antibiotic-loaded bone substitutes have emerged as a potential adjunct in surgical management. These biomaterials enable high local antibiotic delivery within the surgical cavity, exceeding minimum inhibitory concentrations for common pathogens, while avoiding systemic toxicity.

Bone substitutes in adolescent idiopathic scoliosis surgery using sublaminar bands: is it useful? A case-control study.

INTRODUCTION: In order to avoid pseudarthrosis in adolescent idiopathic (AIS) patients, it is recommended to bring additional bone graft or substitute. Modern rigid instrumentations have been shown to provide less pseudarthroses even without bone substitutes. The aim of our study was to determine the impact of using bones substitutes on fusion rates in adolescent idiopathic scoliosis patients undergoing PSF with sublaminar bands. METHOD: AIS patients scheduled to undergo PSF with sublaminar bands were prospectively enrolled into this study and not given any bone substitutes (no-substitute gro

Bone healing of distal radius nonunion treated with bridge plating with bone graft substitutes in combination with systemic romosozumab administration: A case report.

Romosozumab is a humanized, anti-sclerostin monoclonal antibody used to treat osteoporosis, which increases bone formation and decreases bone resorption. It enhances fracture healing and systemic romosozumab administration may have therapeutic potentials for accelerating bone healing of even nonunion. Herein, a 61-year-old heavy smoker male with distal radius nonunion who achieved successful bone union by combination therapy of romosozumab and spanning distraction plate fixation with bone graft substitutes was presented. Novel use of cranial epidural space in rabbits as an animal model to investigate bone volume augmentation potential of different bone graft substitutes.

BACKGROUND: The success of bone augmentation to a major degree depends on the biomechanics and biological conditions of the surrounding tissues. Therefore, an animal model is needed providing anatomical sites with similar mechanical pressures for comparing its influence on different biomaterials for bone regeneration. The present report describes the new bone formation associated to biomaterial in a bursa created in the epidural space, between dura mater and cranial calvaria, under the constant pressure of cerebrospinal fluid. METHODS: Five adult California rabbits were used for the trial. In
