Osseointegrated dental implants in the vicinity of oral squamous cell carcinoma (OSCC) will become more common given the increasing popularity of dental implants. Reports and studies of OSCC around dental implants are rare, as is the topic of how to handle OSCC surgically when implants are in contact with the tumour. In view of this uncertainty, a histological study was performed to assess tumour behaviour around implants. The aim was to determine whether an implant facilitates inward growth of the tumour and how this should be taken into account during staging and treatment planning. A total

The survivorship of cementless orthopaedic implants may be related to their initial stability; insufficient press-fit can lead to excessive micromotion between the implant and bone, joint pain, and surgical revision. However, too much interference between implant and bone can produce excessive strains and damage the bone, which also compromises stability. An understanding of the nature and mechanisms of strain generation during implantation would therefore be valuable. Previous measurements of implantation strain have been limited to local discrete or surface measurements. In this work, we dev

It has been a matter of debate as to whether dental implant therapies are suitable for patients subjected to long-term use of bisphosphonates (BPs). This report presents a case of a 76-year-old woman who developed BPs-related osteonecrosis of the jaw (BRONJ) in the left hemimandible after dental implant exposure. The implants and the necrotic crestal bone were removed, and postoperatively, a delay in tissue healing with bone exposure was noticed. The histologic analysis of the block biopsies revealed a lamellar bone tissue exhibiting necrotic areas and bacterial colonies associated with the bo

Bone condensation is thought to densify interfacial bone and thus improve implant primary stability, but scant data substantiate either claim. We developed a murine oral implant model to test these hypotheses. Osteotomies were created in healed maxillary extraction sites 1) by drilling or 2) by drilling followed by stepwise condensation with tapered osteotomes. Condensation increased interfacial bone density, as measured by a significant change in bone volume/total volume and trabecular spacing, but it simultaneously damaged the bone. On postimplant day 1, the condensed bone interface exhibite

PURPOSE: The aim of this study was to evaluate and compare the effect of changing five macrostructural design parameters of dental implants on the peak strains experienced by the interfacial bone.
MATERIALS AND METHODS: Five geometric variables, including three body-related (implant length, diameter, and taper) and two thread-related (thread depth and thread angle) parameters, were defined. The alveolar bone was modeled as a block with anisotropic and linearly elastic properties with 20-mm height and 12-mm buccolingual and mesiodistal dimensions. PURPOSE: This study was designed to precipitate hydroxyapatite coating on the surface of commercially pure titanium (cpTi) implants using an electroplating technique. After characterization of the hydroxyapatite coating, the bone-implant interface and bone integration of both cpTi and hydroxyapatite-coated implants were assessed.
MATERIALS: Twenty implants were divided equally into 2 main groups (n = 10). Ten cpTi implants were utilized as received, while the others were hydroxyapatite coated and then sterilized. The implants of both groups were inserted in the tibiae of New Zealand rabbits.

A case is reported where a female patient with bilateral otosclerosis received a bone-anchored and skin-penetrating titanium implant on which a hearing aid was mounted to improve conductive hearing loss. The patient developed an infection that did not cease despite intensive local treatment and skin-grafting. Eventually the implant was removed. The histological examination of the interface between implant and surrounding bone and soft tissues showed an inflammatory reaction in the superficial parts of the soft tissues whereas the deeper portions of the soft tissues and all of the bone tissue w

It remains uncertain whether replacing posterior zygomatic implants with pterygoid implants reduces the stress exerted on anterior zygomatic implants in the canine region. This three-dimensional finite element analysis (FEA) study aims to compare stress and strain patterns between two implant configurations under the influence of masticatory muscle forces. A 3D (FEM) model of an edentulous, atrophic maxilla was developed using data from a cone-beam computed tomography (CBCT) scan. Two configurations were examined: Model A, which included bilateral zygomatic implants, and Model B, featuring one

Recent developments have seen poly[aryl-ether-ether-ketone] (PEEK) being increasingly used in vertebral body fusion. More novel approaches to improve PEEK have included the introduction of titanium-PEEK (Ti-PEEK) composites and coatings. This paper aims to describe a potential complication of PEEK based implants relating to poorer integration with the surrounding bone, producing a "PEEK-Halo" effect which is not seen in Ti-PEEK composite implants. We present images from two patients undergoing anterior lumbar interbody fusion (ALIF). The first patient underwent an L5/S1 ALIF using a PEEK impla

Interconnected porous lattices are commonly added to the surfaces of 3D-printed orthopedic implants to facilitate mechanical interlocking with interfacing bone. Despite the biological advantages, the porous surface layers introduce stress concentrations at the solid-lattice interface that can lead to implant fracture, particularly if complete osseointegration is not reached. This study investigated how tissue regeneration within the porous network influences the mechanical properties of implants through simulating bone ingrowth using PMMA based bone cement. Bone cement was selected as a surrog

Osseointegrated implants have been developed to allow direct skeletal fixation of a prosthesis as an alternative to traditional socket-fitted prostheses for patients who have suffered from a major limb amputation. The implants contribute to improvements in functional outcome and quality of life and radiological evaluation plays a crucial role in pre- and post-operative assessment. This article acts as a guide for radiologists who may be tasked with providing the radiological information required by surgeons and prosthetists. We also look at the radiological appearances of complications that ma

Osteointegration was defined as a "direct structural and functional connection between ordered living bone and the surface of a load-carrying implant." Although osteointegration was meant originally to describe a biologic fixation of the titanium dental implants, it is now used to describe the attachment of other materials used for dental and orthopedic applications as well. Analyses of material-bone interface showed that osteointegrated implants can have an intervening fibrous layer or direct bone apposition characterized by bone-bonding depending on the composition and surface properties of

Osseointegration at the site of orthopedic implants is dependent on the recruitment, attachment, and differentiation of osteogenic cells. Data concerning the effect of a patient's underlying joint disease on the modulation of the cellular activity and the long-term survival of joint prostheses is limited. In this study, immunocytochemistry was used to investigate the osteogenic cell phenotype within the bone-implant interface fibrous membrane in 60 patients with different underlying joint disease. Tissue specimens were removed during revision operations performed at variable times following im

BACKGROUND: Obesity can cause higher failure rates and inferior clinical outcomes after Total Knee Arthroplasty (TKA). This study aimed to analyze the biomechanical consequences of a short or long-extension stem on primary TKAs in a BMI-dependent manner.
METHODS: Several finite element models (FEM) were developed to assess the biomechanical effects of adding stem extension in primary TKA on stress distribution, resistance to sinking, motion, and shear forces at the bone/implant interface as a function of different BMIs.
RESULTS: Adding an extension stem improves stress distribution and resis

A two-stage endosseous implant, placed in the retromolar area of the mandible was utilized as rigid anchorage to translate two molars 10-12 millimeters mesially into an atrophic endentulous ridge. Despite substantial anchorage demand over a three year period, the endosseous implant remained rigid ("osseointegrated"). At the end of treatment the implant and adjacent, intravitally labeled bone were recovered. Microradiographic and polarized light analyses revealed that about 80 percent of the endosseous portion of the implant was in direct contact with mature lamellar bone. Bone labels demonstra

The determining factors for the fixation of uncemented screws in bone are the bone-implant interface and the peri-implant bone. The goal of this work was to explore the role of the peri-implant bone architecture on the mechanics of the bone-implant system. In particular, the specific aims of the study were to investigate: (i) the impact of the different architectural parameters, (ii) the effects of disorder, and (iii) the deformations in the peri-implant region. A three-dimensional beam lattice model to describe trabecular bone was developed. Various microstructural features of the lattice wer
