Comparison of Interscalene Brachial Plexus, Anterior Suprascapular Nerve, and Costoclavicular Brachial Plexus Blocks in Arthroscopic Shoulder Surgery: A Prospective Observational Study.

Background: Interscalene brachial plexus block (ISB) remains the gold standard anesthesia method in shoulder surgery. However, risk of diaphragm paralysis is a major concern among anesthesiologists. Recent studies on anterior suprascapular nerve block (ASB) and costoclavicular brachial plexus block (CCB) have given promising results for preventing diaphragm paralysis and providing sufficient analgesia. Methods: Forty-six patients who underwent arthroscopic shoulder surgery under one of three regional anesthesia techniques, including ISB (n = 15), ASB (n = 15), and CCB (n = 16), were included i

Sonoanatomy of the vasculature at the supraclavicular and interscalene regions relevant for brachial plexus block.

BACKGROUND: Our aim in this observational study was to utilize ultrasound, as well as anatomic dissection, to document the frequency with which branches of the subclavian vessels are found in close association with the brachial plexus at the locations of supraclavicular and interscalene brachial plexus block. METHODS: Ultrasound was utilized to document the presence of branches of the subclavian vein and artery, adjacent to the brachial plexus in the supraclavicular and in the interscalene region in 50 patients undergoing shoulder surgery. The position, depth, and dimensions of the vessels we

A magnetic resonance imaging study of modifications to the infraclavicular brachial plexus block.

A previously described infraclavicular brachial plexus block may be modified by using a more lateral needle insertion point, while the patient abducts the arm 45 degrees or 90 degrees. In performing the modified block on patients abducting 45 degrees, we often had problems finding the cords of the brachial plexus. Therefore, we designed an anatomic study to describe the ability of the recommended needle direction to consistently reach the cords. Additionally, we assessed the risk of penetrating the pleura by the needle. Magnetic resonance images were obtained in 10 volunteers. From these image

[Anatomic basis of continuous axillary block of the brachial plexus with thoracic reservoir].

Although continuous axillary block is effective for periods of up to four to six weeks, pain clinic patients with severe chronic pain in the upper extremities, mainly resulting from neuropathic disease, can require continuous drug delivery by catheter-reservoir for up to three months or longer. We studied possible locations for implanting reservoirs or catheters from the perivascular axillary space in fresh cadavers, checking for possible vascular or nerve damage after applying the usual technique for reservoir implantation. The ideal location for the reservoir is the subcutaneous tissue of th

[The interaction of sedativa with etidocaine in patients with brachial plexus block (author's transl)].

The influence of diazepam and phenobarbital on plasma etidocaine concentrations is studied. It is shown that there is no statistical relevant interaction. The mean peak levels occurred within 40 min after application of local anaesthetics and amounted to 1,439 micrograms/ml. The mean half times were calculated and are 135 min.

Comparative Evaluation of Dexmedetomidine and Dexamethasone as Adjuvants in Supraclavicular Brachial Plexus Block.

Introduction Currently, peripheral nerve block has shown immense potential with effective patient satisfaction. In the event of upper limb surgeries, the supraclavicular brachial plexus approach under ultrasound guidance renders quick and dense anesthesia. In addition, the clinical utility of adjuvants with local anesthetics elicits a good quality of nerve block with improved duration and inset. So the aim of the present study was to compare the block characteristics of dexmedetomidine and dexamethasone during supraclavicular brachial plexus block in patients undergoing upper limb surgeries. Ultrasound-guided supraclavicular vs infraclavicular brachial plexus blocks in children.

BACKGROUND: Supraclavicular brachial plexus blocks are not common in children because of risk of pneumothorax. However, infraclavicular brachial plexus blocks have been described in paediatric patients both with nerve stimulation and ultrasound (US)-guidance. US-guidance reduces the risk of complications in supraclavicular brachial plexus blocks in adults. OBJECTIVE: To compare the success rate, complications and time of performance of US-guided supraclavicular vs infraclavicular brachial plexus blocks in children. MATERIAL AND METHODS: Eighty children, 5-15 years old, scheduled for upper li

1,001 subclavian perivascular brachial plexus blocks: success with a nerve stimulator.

BACKGROUND AND OBJECTIVES: Among the supraclavicular approaches to the brachial plexus, the subclavian perivascular technique is a well-established method of anesthesia of the upper extremity. Ever since Kulenkampf described his technique, eliciting a paresthesia has been almost mandatory ("no paresthesia, no anesthesia"). Lately, nerve stimulators have become more popular. However, up to the present time, clinical studies involving the nerve stimulator have failed to show success rates comparable to paresthesia techniques. METHODS: Data from 1,001 consecutive, subclavian perivascular blocks

Brachial plexus block. A comparison of the supraclavicular lateral paravascular and axillary approaches.

BACKGROUND AND OBJECTIVES: Anesthesia of the brachial plexus has been associated with injuries to adjacent structures (e.g., pneumothorax, vascular penetration). It is not uncommon to have only partial block of the upper extremity, hindering completion of the surgical procedure. The supraclavicular lateral paravascular approach to brachial plexus anesthesia has been proposed as an effective, safe alternative to the traditional approaches to brachial plexus anesthesia. METHODS: This prospective, randomized study sought to determine if the supraclavicular lateral paravascular (SCLP) approach is

Traction injury of the brachial plexus confused with nerve injury due to interscalene brachial block: A case report.

INTRODUCTION: Shoulder surgery is often performed with the patient in the so called "beach-chair position" with elevation of the upper part of the body. The anesthetic procedure can be general anesthesia and/or regional block, usually interscalenic brachial plexus block. We present a case of brachial plexus palsy with a possible mechanism of traction based on the electromyographic and clinical findings, although a possible contribution of nerve block cannot be excluded. PRESENTATION OF THE CASE: We present a case of a 62 year-old female, that suffered from shoulder fracture-dislocation. Open

Supraclavicular block versus interscalene brachial plexus block for shoulder surgery: A meta-analysis of clinical control trials.

BACKGROUND: The ultrasound-guided interscalene block (ISB) has been considered a standard technique in managing pain after shoulder surgery. However, this method was associated with the incidence of hemi-diaphragmatic paresis. In contrast to ISB, supraclavicular block (SCB) was suggested to provide effective anaesthesia for shoulder surgery with a low rate of side-effects. Thus, we performed a meta-analysis of randomised controlled trials (RCTs) to compare SCB with ISB for evaluating the efficacy and safety. METHOD: The literature was searched from PubMed, Wiley Online Library, EMBASE, and th

[Ultrasound-guided axillary block: anatomical variations of terminal branches of the brachial plexus in relation to the brachial artery].

OBJECTIVES: To describe the distribution of the terminal branches of the brachial plexus at the axillary level and define distribution patterns after ultrasound evaluation. MATERIAL AND METHOD: Fifty volunteers underwent ultrasound bilateral axillary brachial plexus scanning exploration. Nerve distribution around the humeral artery was described and the distance between each nerve and the center of the artery was measured. The distance and relationship between the ulnar nerve and the humeral vein were also recorded. RESULTS: The median nerve was located in the anterolateral quadrant (-29±40°

Ultrasound-guided retroclavicular approach infraclavicular brachial plexus block for upper extremity emergency procedures.

The America Society of Anesthesiology guidelines recommend multimodal analgesia that combines regional anesthetic techniques with pharmacotherapy to improve peri-procedural pain management and reduce opioid related complications. Commonly performed emergency procedures of the upper extremity such as fracture and dislocation reduction, wound debridement, and abscess incision and drainage are ideal candidates for ultrasound-guided (USG) regional anesthesia of the brachial plexus. However, adoption of regional anesthesia by emergency practitioners has been limited by concerns for potential compli

Anatomic variation of subclavian artery visualized on ultrasound-guided supraclavicular brachial plexus block.

Use of ultrasonography for performance of nerve and plexus blocks has made the process simpler and safer. However, at times, variant anatomy of the visualized structures can lead to failure of blocks or complications such as intravascular injections. This is especially true in case of novice operators. We report a case of a variant branch of subclavian artery, possibly the dorsal scapular artery passing through the brachial plexus nerve bundles in the supraclavicular area. Since this variation in anatomy was visualized in the scout scan prior to the performance of the block, it was possible to

The vertical infraclavicular brachial plexus block: a simulation study using magnetic resonance imaging.

The recommended needle trajectory for the vertical infraclavicular brachial plexus block is anteroposterior, caudad to the middle of the clavicle. We studied the risk of pneumothorax and subclavian vessel puncture and the precision of this method by using magnetic resonance imaging in 20 adult volunteers. The trajectory aimed at the lung in six subjects, five of whom were women. However, pleural contact could be avoided in all subjects by halting needle advancement after contact with the subclavian vessels, plexus, or first rib. The subclavian vein was reached by the trajectory in three and th
