Post-concussion symptoms after mild traumatic brain injury in adults: Management of neuropsychological symptoms.

BACKGROUND: Mild traumatic brain injury (mTBI), or concussion, is common in the adult civilian population and is associated with an array of physical, cognitive and affective symptoms. Many individuals recover from these symptoms rapidly and experience a full return to pre-injury work/study and broader life demands. The proportion of individuals who continue to report elevations in symptoms more than 3 months after injury is substantial, however.  OBJECTIVE: This paper aims to provide clinically relevant, evidence-based advice for general practitioners who are treating patients with post-concu

Functional network disruptions in youth with concussion using the Adolescent Brain Cognitive Development study.

OBJECTIVE: This study aimed to compare psychosocial outcomes and functional neuroimaging among youth with concussion, youth with anxiety, and age- and sex-matched controls. METHODS: Using archival data from the Adolescent Brain Cognitive DevelopmentSM Study, we analyzed between-group differences in psychosocial outcomes measured by the Child Behavior Checklist's internalizing and externalizing problem scales, and assessed brain function using resting-state fMRI network-region connectivity (specifically frontoparietal network (FPN) and default mode network (DMN) connectivity with the amygdala)

Increased brain age and relationships with blood-based biomarkers following concussion in younger populations.

OBJECTIVE: Brain age is increasingly being applied to the spectrum of brain injury to define neuropathological changes in conjunction with blood-based biomarkers. However, data from the acute/sub-acute stages of concussion are lacking, especially among younger cohorts. METHODS: Predicted brain age differences were independently calculated in large, prospectively recruited cohorts of pediatric concussion and matched healthy controls (total N = 446), as well as collegiate athletes with sport-related concussion and matched non-contact sport controls (total N = 184). Effects of repetitive head in

Multiple Object Tracking Scores Predict Post-Concussion Status Years after Mild Traumatic Brain Injury.

The diagnosis of concussion remains challenging, particularly in cases where several months have passed between a traumatic brain injury (TBI) and clinical assessment. Tracking multiple moving objects in three-dimensional (3D) space engages many of the same cognitive processes that are affected by concussion, a form of mild TBI (mTBI), suggesting that tests of 3D multiple object tracking (3D-MOT) may be sensitive to post-concussion syndrome (PCS) after a brain injury has occurred. To test this, we evaluated 3D-MOT performance (using NeuroTrackerTM) against Sports Concussion Assessment Tool res

When Is It Safe to Drive after Mild Traumatic Brain Injury/Sports-related Concussion?

Driving an automobile has inherent danger, and driving after mild traumatic brain injury (mTBI)/sports-related concussion (SRC) can make this task even more hazardous. There is evidence to suggest that restriction from driving for the first 24 to 48 h after mTBI/SRC is probably reasonable. However, after the first 48 h have passed, there is insufficient evidence to make a recommendation regarding return to driving. Clinicians need to weigh the evidence and make decisions on how to advise their patients on a case by case basis. Further research is required to provide a standardized recommendati

Mild concussion impairs extinction of avoidance and alters respective brain circuits in male rats.

Concussive brain injury is a risk factor for anxiety disorders. Pre-clinical models demonstrate that concussion increases passive fear responses, such as conditioned freezing, yet provide limited insight to active responses like avoidance of perceived threats. This is important because persistent avoidance is characteristic of anxiety disorders. Moreover, brain injury can induce an imbalance of the gut microbiome, which can alter emotions. Adult male rats were trained on a platform-mediated avoidance task where they learned to step onto a platform to avoid a foot shock following a conditioned

Structural brain alterations and predictors of clinical improvement in functional cognitive disorder after concussion.

Functional cognitive disorder (FCD) is an under-studied yet likely prevalent subtype of functional neurological disorder (FND), with neural mechanisms that remain poorly understood. While studies of other FND subtypes have revealed structural and functional brain alterations across several large-scale brain networks, no study to date has investigated neural alterations in FCD. Here, we examined structural grey matter brain differences as measured from magnetic resonance imaging in 37 adults with FCD after concussion compared to 25 post-concussion controls. We compared FreeSurfer-based cortical

Acute Changes in the Resting Brain Networks in Concussion Patients: Small-World Topology Perspective.

BACKGROUND: The acute changes that occur in the small-world topology of the brain in concussion patients remain unclear. Here, we investigated acute changes in the small-world organization of brain networks in concussion patients and their influence on persistent post-concussion symptoms. METHODS: Eighteen concussion patients and eighteen age-matched controls were enrolled in this study. All participants underwent computed tomography, magnetic resonance imaging (MRI), susceptibility weighted imaging, and blood oxygen level-dependent functional MRI. Network analysis applied to post-concussion symptoms in two mild traumatic brain injury samples.

OBJECTIVE: A latent disease explanation cannot exclusively explain post-concussion symptoms after mild traumatic brain injury (mTBI). Network analysis offers an alternative form of explanation for relationships between symptoms. The study aimed to apply network analysis to post-concussion symptoms in two different mTBI cohorts; an acute treatment-seeking sample and a sample 10 years post-mTBI. METHOD: The treatment-seeking sample (n = 258) were on average 6 weeks post-injury; the 10 year post mTBI sample (n = 193) was derived from a population-based incidence and outcomes study (BIONIC). Emergency department assessment and management of pediatric acute mild traumatic brain injury and concussion.

Mild traumatic brain injury (mTBI) and concussion, a subtype of mTBI, commonly present to the emergency department (ED) and may present with symptoms identical to those associated with more severe TBI. The development and use of clinical decision rules, increased awareness of the risk of radiation associated with head computed tomography, and the potential for patient observation has allowed emergency clinicians to make well-informed decisions regarding the need for imaging for patients who present with mTBI. For patients who present to the ED with concussion, appropriate diagnosis, management

Microstructural changes in the brain after long-term post-concussion symptoms: A randomized trial.

A recent randomized controlled trial in young patients with long-term post-concussion symptoms showed that a novel behavioral intervention "Get going After concussIoN" is superior to enhanced usual care in terms of symptom reduction. It is unknown whether these interventional effects are associated with microstructural brain changes. The aim of this study was to examine whether diffusion-weighted MRI indices, which are sensitive to the interactions between cellular structures and water molecules' Brownian motion, respond differently to the interventions of the above-mentioned trial and whether

Examining the relationship between neuroticism and post-concussion syndrome in mild traumatic brain injury.

Objective: We sought to examine the relationship between personality traits and post-concussion symptom reporting in individuals with and without a self-reported history of concussion. Methods: Data were collected via a cross-sectional electronic survey from 619 individuals via Amazon Mechanical Turk (mTurk). Participants completed a background demographic questionnaire, as well as both the Post-Concussion Symptom Scale (PCSS) and IPIP-NEO-120 personality inventory. Results: Significant relationships were seen between concussion symptom reporting and personality traits of neuroticism, extraver

Mapping brain recovery after concussion: From acute injury to 1 year after medical clearance.

OBJECTIVE: To test the hypothesis that concussion-related brain alterations seen at symptomatic injury and medical clearance to return to play (RTP) will have dissipated by 1 year after RTP. METHODS: For this observational study, 24 athletes with concussion were scanned longitudinally within 1 week after injury, at RTP, and 1 year after RTP. A large control cohort of 122 athletes were also scanned before the season. Each imaging session assessed global functional connectivity (Gconn) and cerebral blood flow (CBF), along with white matter fractional anisotropy (FA) and mean diffusivity (MD). T

Eye tracking detects disconjugate eye movements associated with structural traumatic brain injury and concussion.

Disconjugate eye movements have been associated with traumatic brain injury since ancient times. Ocular motility dysfunction may be present in up to 90% of patients with concussion or blast injury. We developed an algorithm for eye tracking in which the Cartesian coordinates of the right and left pupils are tracked over 200 sec and compared to each other as a subject watches a short film clip moving inside an aperture on a computer screen. We prospectively eye tracked 64 normal healthy noninjured control subjects and compared findings to 75 trauma subjects with either a positive head computed

Quantitative assessment of post-concussion syndrome following mild traumatic brain injury using robotic technology.

Post-Concussion Syndrome (PCS) is a common sequelae of mild Traumatic Brain Injury (mTBI). Currently, there is no reliable test to determine which patients will develop PCS following an mTBI. As a result, clinicians are challenged to identify patients at high risk for subsequent PCS. Hence, there is a need to develop an objective test that can guide clinical risk stratification and predict the likelihood of PCS at the initial point of care in an Emergency Department (ED). This paper presents the results of robotic-assisted neurologic testing completed on mTBI patients in the ED and its ability

From motion to deformation: a large-scale study of concussive versus non-concussive head acceleration events and brain strains in Canadian university football.

Sport-related concussion arises from complex brain loading during head acceleration events (HAEs). However, tissue-level responses in Canadian university football remain poorly characterized. This study quantified head kinematics and finite element-derived brain strain in concussive and non-concussive HAEs. Helmet-mounted sensors recorded linear acceleration and rotational velocity during games and practices from 250 players across seven seasons (2013-2019). Medically diagnosed concussions (n = 45) were identified, and causative HAEs determined through expert consensus of available data. A set

Associations between brain drawings following mild traumatic brain injury and negative illness perceptions and post-concussion symptoms at 4 years.

Characteristics of patient's drawings have been linked to short-term health-related outcomes across a range of health conditions. This study examined associations between brain drawings at 1 month and illness perceptions and post-concussion symptoms at 4 years in 92 adults following mild traumatic brain injury. Greater damage depicted at 1 month was correlated with perceived greater impact on life, duration of injury, symptoms of brain injury, emotional consequences and late-onset post-concussion symptoms. Results indicate that brain drawings shortly after traumatic brain injury offer a simple
