Ventilator autocycling and delayed recognition of brain death.

BACKGROUND: Improvements in technology play an important role in caring for critically ill patients. One example is the advance in ventilator design to facilitate triggering of mechanical breaths. Minimal changes in circuit flow unrelated to respiratory effort can trigger a ventilator breath and may mislead caregivers in recognizing brain death. METHODS: We observed patients with devastating brain injuries in a mixed medical/surgical intensive care unit (ICU) with a high clinical suspicion for brain death including the absence of cranial nerve function with apparent spontaneous breathing duri

What can be learned from brain-death models?

Brain death of the donor is an important risk factor influencing graft outcome. In addition to its nonspecific effects, it potentiates graft immunogenicity and increases host alloresponsiveness. Thus brain death in addition to other unspecific injuries such as organ procurement, preservation and consequences of ischemia/reperfusion injury, contributes towards the change of an inert organ to an immunological altered graft. Prior to engraftment, brain death initiates a cascade of molecular and cellular events including the release of proinflammatory mediators leading to cellular infiltrates. Tho

Computed tomographic angiography for diagnosis of brain death.

The authors report two patients with suspected brain death who required confirmatory tests other than clinical examination because of prolonged barbiturate administration for intracranial hypertension. Absence of intracranial blood flow was documented on CT angiography and confirmed by CT perfusion images. Cerebral angiography confirmed the findings consistent with brain death. CT angiography with CT perfusion may represent a rapid noninvasive method for diagnosis of brain death.

Simulation of brain death from fulminant de-efferentation.

BACKGROUND: Guillain-Barré syndrome (GBS) classically presents with a subacutely evolving areflexic paralysis, with typical laboratory findings of elevated cerebrospinal fluid protein and abnormal nerve conduction studies. There is now an increasing recognition of GBS variants that differ in clinical presentation, prognosis, electrophysiology and presumed pathogenesis. Fulminant cases of GBS have been reported in which a rapid deterioration evolves to a clinical state resembling "brain death". METHODS: A retrospective analysis of two such cases of fulminant neuropathy are described, that incl

Cell death and birth in multiple sclerosis brain.

The hallmark of the brain pathology in multiple sclerosis is the white matter plaque, characterized by myelin destruction and oligodendrocyte loss. To examine the role that cell death plays in the development of MS lesions, we used the in situ TUNEL technique, a method that sensitively detects DNA fragmentation associated with death at the single cell level. We found that patchy areas within acute MS lesions have massive numbers of inflammatory and glial cells undergoing cell death. The punched out areas of some long-standing chronic lesions also had labeled glial cells showing that the attack

The undulating toe flexion sign in brain death.

Brain-dead patients may exhibit gross spontaneous and reflex movements (e.g., Babinski sign, stereotypic flexion of one or more limbs, and Lazarus sign). We report three brain-dead patients who had unusual complex sequential movements of the toes. Undulating toe flexion was elicited by noxious stimuli to the lower extremities, and consisted of initial plantar flexion of the great toe, followed by sequential brief plantar flexion of the second, third, fourth, and fifth toes. The undulating toe flexion sign differs from previously described responses characterized by plantar flexion of the toes

[Brain death from the viewpoint of transplantation surgery].

If in the case of brain death breathing and circulation are continued artificially we deceive ourselves emotionally not to except death of the so treated organism. But on scientific medical grounds brain death defines the end of the organism as a whole beyond doubt. No rational reasons can be seen, why organs should not be taken from the brain dead provided that diagnosis of brain death and the organ retrieval are done by independent teams of doctors and an opposing will of the dead person is not known. Honour should be payed to the remains of the dead organ donor. Donation of living donors sh

Clinical brain death with preserved cerebral arterial circulation.

We present a case of a 44-year old male suffering from severe subarachnoid haemorrhage secondary to a ruptured basilar artery aneurysm. Clinical examination on 3 consecutive days fulfilled the criteria for the diagnosis of clinical brain death. Nevertheless, cerebral angiograms showed intracranial filling until the third day after the bleeding. Autopsy showed acute infarction of the brain stem and the cerebellum.

Age-specific characteristics of brain death in children.

Clinical and neuropathologic characteristics of 45 children who met criteria for brain death were analyzed. Children between 2 months and 1 year of age were compared with children older than 1 year and children older than 5 years. The observation period to fulfill brain death criteria was not different between the age groups. Deep tendon and spinal reflexes were preserved significantly less frequently in children younger than 1 year old. Diabetes insipidus and the necessity of inotropic support were significantly more frequent in children older than 5 years. Fifty-eight percent (26/45) of pati

Duration of apnea needed to confirm brain death.

To determine the duration of respiratory arrest needed to attain a PaCO2 level high enough to provide maximal stimulation of respiration, we evaluated changes in PaCO2, PaO2 and apH during periods of apnea lasting as long as 10 minutes in 10 apparently brain-dead subjects. Before apnea, mean PaCO2 was 33 mm Hg. In seven subjects who did not breathe for 10 minutes, the mean rate of rise of PaCO2 was 3.2 mm Hg per minute. PaCO2 at 4 minutes was 50 mm Hg and at 10 minutes was 67 mm Hg. Three subjects breathed, two after less than 2 minutes of apnea, when PaCO2 was 47 and 54 mm Hg, and one after 4

Differential Effects of Brain Death and Circulatory Death on Myocardial Integrity and Transplant Outcomes.

Heart transplantation is the definitive treatment for end-stage heart failure, yet the persistent scarcity of donor organs has necessitated expanded criteria for donor selection, particularly the inclusion of donors after brain death (DBD) and circulatory death (DCD). These two mechanisms of donor death result in distinct pathophysiological alterations that impact myocardial viability, inflammatory activation, and immune recognition. DBD is characterized by a catecholamine surge, hormonal collapse, and systemic inflammation, contributing to endothelial dysfunction and immunologic priming. In c

Cell death in the brain during parasitic infections.

Programmed cell death is an integral arm of immunity, especially in response to intracellular pathogen infection, for the release of inflammatory mediators, and the clearance of pathogen-infected cells. In the central nervous system, cell death as a form of immunity may seem paradoxical; neuronal health is critical to survival and neurogenesis is nominal post gestation. However, the brain is composed of various glial cell types that have a greater regenerative capacity than neurons, thus allowing for cell death to potentially be utilized as a protective immune response. While the brain is an i

Brain Death Donors on Extracorporeal Membrane Oxygenation Support.

OBJECTIVES: We investigated donors with brain death on extracorporeal membrane oxygenation support, a clinically challenging situation due to hemodynamic instability frequently encountered in these donors, which may threaten organ function. MATERIALS AND METHODS: We described our experience with 15 utilized brain death donors on extracorporeal membrane oxygenation support, consecutively admit-ted in our intensive care unit (which is a referral center for extracorporeal membrane oxygenation) from 2018 to 2023. We investigated whether utilization rate for brain death donors on extracor-poreal m

Brain Death Diagnosis in Primary Posterior Fossa Lesions.

BACKGROUND: New controversies have raised on brain death (BD) diagnosis when lesions are localized in the posterior fossa. OBJECTIVE: The aim of this study was to discuss the particularities of BD diagnosis in patients with posterior fossa lesions. MATERIALS AND METHODS: The author made a systematic review of literature on this topic. RESULTS AND CONCLUSIONS: A supratentorial brain lesion usually produces a rostrocaudal transtentorial brain herniation, resulting in forebrain and brainstem loss of function. In secondary brain lesions (i.e., cerebral hypoxia), the brainstem is also affected l
