A Depolarizing Leak in Sodium Bicarbonate Cotransporter NBCe1 Causes Brain Edema.

OBJECTIVES: SLC4A4 encodes electrogenic sodium bicarbonate cotransporter NBCe1, prominently expressed in kidney and brain. Recessive loss-of-function variants in SLC4A4 cause proximal renal tubular acidosis, no brain edema. In the brain, NBCe1 is expressed by astrocytes, where it regulates pH and mediates astrocyte volume changes. Here we describe a novel dominant variant in SLC4A4 in patients with brain edema and investigate how it affects NBCe1 function. METHODS: Genetic studies identified a novel gene variant in three unrelated pediatric patients with the same MRI pattern of cerebral subco

C5aR1+ microglia exacerbate neuroinflammation and cerebral edema in acute brain injury.

Microglia rapidly respond to acute brain injury and contribute to neuroinflammation that drives cerebral edema, a major cause of mortality and disability in stroke and traumatic brain injury (TBI). However, microglial heterogeneity complicates precise therapeutic targeting because specific disease-associated subtypes remain poorly characterized. Here, we define a previously unrecognized C5a receptor 1 (C5aR1)-expressing microglial subtype enriched in human cerebral edema tissue from decompressive surgery for TBI and intracerebral hemorrhage (ICH). In preclinical models, C5aR1+ microglia engage

MRI radiomic signature predicts peritumoral brain edema resolution following meningioma surgery.

BACKGROUND: Intracranial meningiomas(IM) are often associated with peritumoral brain edema(PTBE), visible as hyperintensities on T2/FLAIR MRI. Postoperative persisting PTBE-like changes likely represent gliosis that, in turn, contributes to surgical morbidity. Since the human eye is unable to distinguish between PTBE and gliosis on MR images, we hypothesized that radiomic analysis of preoperative peritumoral T2/FLAIR hyperintensities could distinguish preoperatively established gliosis from reversible edema. METHODS: MRI of patients with gross totally resected IM were retrospectively analyzed

Noninvasive measures of brain edema predict outcome in pediatric cerebral malaria.

BACKGROUND: Increased brain volume (BV) and subsequent herniation are strongly associated with death in pediatric cerebral malaria (PCM), a leading killer of children in developing countries. Accurate noninvasive measures of BV are needed for optimal clinical trial design. Our objectives were to examine the performance of six different magnetic resonance imaging (MRI) BV quantification measures for predicting mortality in PCM and to review the advantages and disadvantages of each method. METHODS: Receiver operator characteristics were generated from BV measures of MRIs of children admitted to

Extensive vasogenic edema of anti-aquaporin-4 antibody-related brain lesions.

OBJECTIVE: Using neuroimaging, we analyzed the nature of extensive brain lesions in five anti-aquaporin-4 (AQP4) antibody-positive patients with neuromyelitis optica spectrum disorders. RESULTS: Extensive brain lesions involved white matter in three, and basal ganglia and corpus callosum in one each. Four patients showed high diffusivity on apparent diffusion coefficient maps and three demonstrated increased choline/creatine ratios and decreased N-acetyl-aspartate/creatine ratios on (1)H-magnetic resonance spectroscopy. These findings suggested that the lesions were vasogenic edema associated

Brain edema after intracerebral hemorrhage: the effects of systemic complement depletion.

The complement cascade is activated after experimental intracerebral hemorrhage (ICH) and may play an important, role in edema formation. This study investigated the effects of systemic complement depletion on brain edema formation following ICH. Thirty-six pentobarbital-anesthetized Sprague-Dawley rats were used. Treatment animals were complement-depleted with cobra venom factor (CVF) while controls received an equal volume of saline injection (i.p.). In both treatment and control rats, autologous blood (100-microL) was infused stereotactically into the right basal ganglia. Rats were sacrific

Inhibition of brain glutamine accumulation prevents cerebral edema in hyperammonemic rats.

The mechanism of brain swelling during hyperammonemia is not understood, but glutamine accumulation is consistently observed. We tested the hypothesis that brain swelling associated with hyperammonemia is a consequence of the osmotic effect of intracellular glutamine accumulation in brain. Increases in plasma ammonium levels from 31 +/- 3 to 601 +/- 38 mumol/l (+/- SE) were produced by 6 h of infusion of ammonium acetate in anesthetized rats. Hyperammonemia resulted in increased brain water content accompanied by more than a tripling of brain glutamine concentration compared with control rats

[Fatty liver, metabolic acidosis and brain edema--Reye's syndrome in adults?].

Three, previously healthy, patients developed profuse vomiting. In the further course of the disease there was a marked deterioration in their general condition with massive metabolic acidosis, cerebral symptoms and, finally, deep coma which ended in death, without any evidence of poisoning. Post-mortem examination revealed diffuse, droplet fatty infiltration of the liver of the degenerative type as well as cerebral oedema without inflammatory signs, findings of Reye's syndrome, rarely seen beyond childhood. The aetiology of this prognostically unfavourable disease remains uncertain, clinical

[Computer tomographic measuring procedure in generalized brain edema after craniocerebral trauma].

Computer tomography enables for the first time the direct demonstration of the consequences of a craniocerebral trauma. In the computer tomogram, the cerebral oedema is dominating as an area of reduced density (hypodense region). Three different measuring techniques are compared and their application in 43 craniocerebral traumata discussed. Among the measuring methods employed, the area index shows the greatest diagnostic reliability in the computer-tomographic findings of a posttraumatic cerebral oedema. The partial volume analysis without application of a contrast medium is of little use for

Astrocyte TrkB promotes brain injury and edema formation in ischemic stroke.

Following ischemic stroke astrocytes undergo rapid molecular and functional changes that may accentuate tissue damage. In this study we identified the neurotrophin receptor TrkB in astrocytes as a key promoter of acute CNS injury in ischemic stroke. In fact, TrkB protein was strongly upregulated in astrocytes after human and experimental stroke, and transgenic mice lacking astrocyte TrkB displayed significantly smaller lesion volume, lower brain atrophy and better motor performance than control animals after transient middle cerebral artery occlusion. Neuropathological studies evidenced that e

EXTENSIVE PERITUMORAL BRAIN EDEMA IN A SMALL CLINOIDAL MENINGIOMA: CLINICAL CASE.

Peritumoral brain edema (PTBE) is seen in 40-78% of all cases of intracranial meningiomas. It may vary in shape and size, occasionally being two to three times larger than the tumor. We present a case of a 62-year-old female patient, suffering from seizure and progressive headache. She was diagnosed with left medial sphenoid wing meningioma and referred for treatment to Uzhhorod Regional Center of Neurosurgery and Neurology. The patient had no major focal neurological deficit and Karnofsky Performance Scale (KPS) of 70 on admission. The preoperative magnetic resonance imaging (MRI) with and wi

Asymmetric brain edema after cardiac transplantation: cerebroautoregulatory failure and relative hyperperfusion.

Asymmetric brain edema is a rare neurologic complication after cardiovascular surgery. We describe the clinical and imaging features of an asymmetric brain edema syndrome in a 52-year-old man following cardiac transplantation who presented with facial myoclonus and left hemiparesis in the postoperative period. To our knowledge, this is the first case report of asymmetric brain edema syndrome after cardiac transplant and the second following cardiac surgery. Arterial bypass cannula malposition in the ascending aorta or brachiocephalic artery with subsequent cerebral hypoperfusion and subsequent

Intracranial pial single-channel arteriovenous fistula presenting with significant brain edema.

The authors report a rare case of pial single-channel arteriovenous fistula presenting with significant brain edema. A 51-year-old woman was admitted with a 5-day history of headache and nausea, followed by consciousness disturbance. Computed tomography showed cerebellar swelling with obstructive hydrocephalus. Magnetic resonance imaging revealed extensive vasogenic edema in the cerebellum bilaterally. Angiography demonstrated 2 different arteriovenous shunts (AVSs) at peripheral branches of the right anterior inferior cerebellar artery. One was located on the suboccipital surface. It drained

Hypertensive brain stem encephalopathy: clinically silent massive edema of the pons.

Hypertensive encephalopathy is a medical emergency whose clinical manifestations are associated with bilateral parieto-occipital lesions. We describe a case of hypertensive brainstem encephalopathy in which high blood pressure was accompanied only by nuchal headaches of violent onset. T2-weighted magnetic resonance images showed hyperintensity and edema of the pons without any parietooccipital lesions, but with hyperintense lesions at the level of the basal nuclei, insula and temporal lobes. The lesions rapidly regressed once the hypertension had been controlled.

Cerebral blood flow and edema in perinatal hypoxic-ischemic brain damage.

The relationship between cerebral blood flow (CBF) and the evolution of brain edema was investigated in an experimental model of perinatal hypoxic-ischemic brain damage. Seven-d postnatal rats were subjected to unilateral common carotid artery ligation followed by 3 h of hypoxia with 8% oxygen at 37 degrees C. This insult produces neuronal necrosis and/or infarction only in the cerebral hemisphere ipsilateral to the arterial occlusion in the majority of animals; hypoxia alone produces no damage. CBF, measured by the indicator diffusion technique using iodo[14C]-antipyrine, and tissue water con

Increased leukotriene C4 and vasogenic edema surrounding brain tumors in humans.

Leukotrienes are pharmacologically active compounds that promote vascular permeability. In this study we sought to determine whether tissue leukotriene-like immunoreactivity was increased in intracranial tumors associated with peritumoral edema. In 20 patients undergoing craniotomy tissue specimens were immediately frozen after removal and tissue leukotriene C4 levels were determined by radioimmunoassay. An index of peritumoral edema was estimated from preoperative contrast-enhanced computed tomographic scans. There was a significant correlation between brain edema and tissue leukotriene level
