Energy balance during recovery from malnutrition.

This report presents an account of energy balance of young Jamaican children recovering from protein-energy malnutrition (PEM). This was done in three steps. Initially the true gross energy of a formula used in the treatment of PEM was determined by bomb calorimetry. Then its metabolizable energy content was determined in a group of nine children recovering from PEM. In a similar but different group of eight children total daily metabolizable energy intake (EI), average rate of weight gain (g/kg/day) (WG), and total daily energy expenditure (TDEE) were determined. TDEE was determined by indire

Low respiratory quotient correlates with high mortality in patients undergoing mechanical ventilation.

OBJECTIVE: Oxygen consumption (VO2), carbon dioxide generation (VCO2), and respiratory quotient (RQ), which is the ratio of VO2 to VCO2, are critical indicators of human metabolism. To seek a link between the patient's metabolism and pathophysiology of critical illness, we investigated the correlation of these values with mortality in critical care patients. METHODS: This was a prospective, observational study conducted at a suburban, quaternary care teaching hospital. Age 18 years or older healthy volunteers and patients who underwent mechanical ventilation were enrolled. A high-fidelity aut

Energy expenditure profiles and the risk of early limiting toxicity in older patients with cancer: The ELCAPA-25 prospective cohort survey.

BACKGROUND & AIMS: Predicting the risk of early limiting toxicity (ELT) is major challenge for the clinician seeking an effective, safe treatment for older patients with cancer. The Cancer and Aging Research Group (CARG) and CRASH (Chemotherapy Risk Assessment Scale for High-Age Patients) toxicity scores were designed to predict chemotherapy-related toxicity. Elevated resting energy expenditure (REE) may predispose to cachexia and increase ELT and mortality in older patients with cancer. The primary objective was to assess the association between elevated REE and ELT in older patients with can

Effect of Chronic Kidney Disease on Metabolic Rate: Studies Using Doubly Labelled Water.

OBJECTIVE: The causes of protein malnutrition and body composition changes in chronic kidney disease (CKD) are poorly understood. Alterations to metabolic rate caused by CKD may be a contributor. Using the doubly labeled water technique and indirect calorimetry, we set out to determine whether reduced glomerular filtration rate is associated with alterations to total energy expenditure (TEE) and resting energy expenditure (REE). We also aimed to determine whether TEE in patients with CKD can be easily predicted from a physical activity questionnaire. METHODS: In a prospective, observational s

Basal metabolic rate in narcoleptic patients.

STUDY OBJECTIVES: We investigated basal metabolic rate (BMR) and energy expenditure (EE) in narcoleptic patients and in BMI- and age-matched controls in order to explore the hypothesis that a reduced BMR or EE plays a role in narcolepsy-associated obesity. DESIGN: Control group design with comparison of EE and BMR. EE was determined by indirect calorimetry using the Deltatrac Metabolic Monitor system. BMR was calculated from the oxygen consumption (VO2) and the carbon dioxide consumption (VCO2) measurements after 12 hours of fasting in the morning. PARTICIPANTS: 13 narcoleptic patients and 3

Energy metabolism in exertional heat stroke with acute renal failure.

BACKGROUND: Heat stroke is the clinical syndrome produced when the body overheats. It can develop in the army and in healthy civilian populations who physically exert themselves in a hot and humid environment during the summer, and may result in a significant number of heat-related deaths. Since strenuous exercise is one of the major exacerbating and precipitating factors, the incidence of exertional heat stroke (ExHS) is high among military personnel undergoing military training. Furthermore, acute renal failure (ARF) may occur in 25% of patients with ExHS and it can cause metabolic alteratio

Resting metabolic rate variability as influenced by mouthpiece and noseclip practice procedures.

Mouthpiece and noseclip introduction affect resting physiologic function (i.e., hyperventilation, anxiety), which can influence a resting metabolic rate measurement. It also is obvious that practice is required to minimize this influence when such apparatuses are used. However, the type of practice that best minimizes this influence has not been established. To determine the effect of acute and longer term practice on measurement variability of resting metabolic rate, 27 healthy, premenopausal women were randomly assigned to one of three groups: (1) a practice group, involving three 10-minute

Serum uric acid potentially links metabolic health to measures of fuel use in lean and obese individuals.

BACKGROUND AND AIMS: Uric acid (UA) is a byproduct of the high-energy purine metabolism and is conventionally regarded as a marker of cardio-metabolic impairment. Its potential relationship with energy homeostasis is unknown to date. METHODS AND RESULTS: In a cross-sectional study on 121 otherwise healthy obese and 99 sex- and-age-matched lean subjects, UA levels were analyzed in relation to metabolic health, inflammatory markers, respiratory quotient (RQ) and resting energy expenditure (REE) as assessed by indirect calorimetry, fat mass (%FM) and fat-free mass (FFM) as determined by bioimped

Cytochrome P450-2E1 promotes fast food-mediated hepatic fibrosis.

Cytochrome P450-2E1 (CYP2E1) increases oxidative stress. High hepatic cholesterol causes non-alcoholic steatohepatitis (NASH) and fibrosis. Thus, we aimed to study the role of CYP2E1 in promoting liver fibrosis by high cholesterol-containing fast-food (FF). Male wild-type (WT) and Cyp2e1-null mice were fed standard chow or FF for 2, 12, and 24 weeks. Various parameters of liver fibrosis and potential mechanisms such as oxidative and endoplasmic reticulum (ER) stress, inflammation, and insulin resistance (IR) were studied. Indirect calorimetry was also used to determine metabolic parameters. Li

Standard equations are not accurate in assessing resting energy expenditure in patients with amyotrophic lateral sclerosis.

OBJECTIVE: To assess the utility of standard equations for calculating caloric requirements in patients with amyotrophic lateral sclerosis (ALS). BACKGROUND: Malnutrition substantially increases the risk of death in ALS. Weight loss can be stabilized and survival prolonged with early gastrostomy feeding. However the use of standard nutrition equations has not been validated in this population. We therefore compared measured caloric expenditure to 2 predictive equations in patients with varying stages of ALS. METHODS: Thirty-four patients were studied. [Nutrition of the critically ill - is less more? How much energy for the ICU patient?].

Decreased nutritional intake or preexisting malnutrition is associated with increased morbidity and mortality during hospital stay. However nutritional support in particular for the ICU patient is not trivial. Hyperalimentation in the acute phase of critical illness but also hypoalimentation in the chronic and stable phase of illness has to be avoided. Ideally about 25 kcal/kg/d should be targeted over a few days during metabolic monitoring. Alternatively indirect calorimetry should be applied where available.

Effects of the Paleo diet on resting metabolic rate in handball players.

The Paleolithic diet (PD) has positive effects on health status and body composition. The Paleo diet has been also used to assess the effects on handball players performance but the association of adherence to PD and resting metabolic rate (RMR) has not been investigated especially in this group. This study aimed to impact of the moderate-carbohydrate diet on RMR among professional handball players. Twenty-five handball players were assigned into two groups: 14 in the experimental group (the Paleo diet; PD) and 11 in the control group (rational diet; CD) for 8-weeks of normoenergetic nutrition

Assessment of Physical Activity Intensity with Accelerometers and Oxygen Consumption.

The study compares objective methods of assessing the intensity of physical activity. The goal was to identify the differences in physical activity intensity measured through indirect calorimetry and triaxial accelerometry on the wrist and waist, using the counts cut-off points for the accelerometers and ventilatory threshold one (VT1) and two (VT2). This research is a cross-sectional analytical study of university women (n = 35, 22.4 ± 1.9 years old). Triaxial accelerometers and portable ergospirometry were used during an incremental standardized walking-jogging test in an oval track of 160 m

Predictive equations over estimating resting metabolic rate in individual with spinal cord injury requiring mechanical ventilation support - A case series.

Context: The impact of mechanical ventilation on energy expenditure after spinal cord injury (SCI) is sparse. The objective of this case-series is to measure 15-minutes resting metabolic rate (RMR) to determine 24-hours measured-RMR (m-RMR) using QUARK indirect calorimeter (IC) and; compare the m-RMR with estimated RMR (e-RMR) using four commonly used predictive equations (Harris-Benedict, Mifflin St-Jeor, Henry and Schofield).Findings: We measured the RMR of four patients with SCI (one male and three female; mean age: 58.3 years) all with complete tetraplegia (ISNCSCI A) twice in a one-week i

Increased energy expenditure in infants with cyanotic congenital heart disease.

Infants with cyanotic congenital heart disease (CCHD) often have reduced weight gain compared with infants in control groups. Our purpose was to conduct a longitudinal study of energy intake, resting energy expenditure (REE), and total energy expenditure (TEE) of a group of infants with CCHD. We hypothesized that increased REE and TEE and decreased energy intake in these infants would lead to reduced growth. Ten infants with uncorrected CCHD and 12 infants in a control group were studied at 2 weeks of age and again at 3 months. Indirect calorimetry was used to determine REE; the doubly labeled
